The vehicle was a 2004 Chevrolet Tahoe. The vehicle had a crash. Vehicle identification number (VIN) of the vehicle is 1GNEC13Z03_. I have been seing commercials about airbags not deploying on chevy vehicles. My daughter was killed in my 04 chevy Tahoe where the airbags did not deploy. She was thrown from the vehicle and also another person was in the car as well. Airbags on the drivers or passengers side did not deploy. The truck flipped several times. The drivers seat also lifted up and was split in to. We asked the investigators and our insurance company to check into why the airbags did not deploy and they said it was normal, but we knew otherwise. They did not take any further action. This problem occurred on Jun. 22, 2009.

The vehicle involved is a 2003 Chevrolet Impala. I was on my way home from work when my driverside head light went out so I stopped and bought a new bulb at the closest store when I replaced the bulb the light still didnt come back on then the dtr light also started having issues the only time you get any lights on the driver side is when you move the entire headlight assembly around then you get dim lighting so for the past couple months we havent been able to use our car at night. I keep checking for recalls but I havent seen any and we have always had problems with our dtr lighting including the ones that control our dash lights. Cause we have none they work during th eday when they want too but at night we have no lights in our dash. Also the drivers side heated seat has quit working. It all occured roughly around the same time. Nobodys been able to fix the issues. The failure date was Thursday, April 17, 2014.

The car is a 2008 Chrysler town and country. Was driving down the road when the alarm started beeping, lights on dashboard flickered, and wipers go on and off on their own. Has happened several times. Now the passenger back door automatic buttons do not work and the driver's seat will move forward and back when driving. This is a huge safety issue and the Chrysler dealerships are not helpful in finding a solution. Said it will take several weeks to try and diagnose what's wrong. I am alarmed that more people are having similar issues and a recall has not been made. It's hard to drive when your seat is moving. In addition, sometimes my transmission will not shift. Seems to be an electrical problem. Once I turn the vehicle off and restart it, seems to run fine. The failure date was Saturday, April 26, 2014.

2012 Chrysler Town & Country (rental). Consumer writes in regards to rental vehicle sliding door & passenger seats malfunction. The consumer stated the sliding doors just behind the front doors were electronically operated. There were 2 occasions where the consumer's husband and a teen-aged driver inadvertently had their hand on the car frame, and when the button was pushed to close the door, it did not stop, when it hit their hands. It kept moving and mashed their hands between the moving door and door frame. The injuries were not severe. However, a safety switch is needed to stop the door. A much bigger problem, however, was found in the second row of the seat, just behind the front seat. The consumer stated she and her husband were sitting in the seats, and decided to take a nap. When she went to re-position the seat, to its upright position, she turned sideways to pull the lever on the side, but when she pulled it, the entire back rest assembly slammed her forward with considerable impact. At the same time, the head rest atop the back rest slammed forward with much more impact and it hit her in the back of the head. It was Tuesday, April 1, 2014 when this seats issue happened.

The vehicle having this issue was a 2000 Dodge Intrepid. I was grocery shopping and when I got in the car to go home there was a clunk noise and I was reclining in the drivers seat. I could not upright the seat so it would stay in place. I put the seat belt on and held down the console and made it home scared but safe. I looked under the seat and found a bolt broken off. I did receive a recall notice for the bolts some time ago and took the car to the local Dodge dealer. I assumed the problem was fixed although I had not had a problem until today. Can I go back to Dodge to have it fixed? obviously it wad either not replaced or I got a faulty bolt. Please help. This issue happened on Fri., Apr. 18, 2014.

There was a recall under NHTSA campaign id number 13v081000(seats). The manufacturer stated that since vehicle is over five years old they will not repair under the recall. The approximate failure mileage 68,068. Rg. It was Sun., May. 18, 2014 when this seats issue occurred.

The vehicle involved was a 2014 GMC Terrain. I had this car as a rental. My wife was in the passenger seat and uses a cushion to sit on, because of back problems. We noticed that air bag "on" light was not lit. When she took away the seat cushion the airbag "on" was lit. I don't know if this is a problem, but there was no warning msg anywhere about using a seat cushion. There was no override switch to force the airbag "on" light either and activate the system. It was 04/16/14 when this seats issue occurred.

I bought this car new. Also the heating element went out in my heated seat on passenger side where it is rarely used. All repairs over $1100. This seats issue occurred Apr. 29, 2014.

The car involved was a 2013 Acura Rdx. The brand new car was on fire by itself on October 30, 2012. It was after the hurricane sandy. My family woke up because of smelling gas from downstairs which was the attached garage to the kitchen. When my husband opened the kitchen door to the garage, he couldn't see anything but smokes. We called 911 and found out there was fire inside of the car. The fire started internally and spread out to the garage. There was sign of water that got into the garage. However, when fire fighter opened the garage door, there was no water in the garage. The front of the car was fine. The driver's seat area and the navigator area was totally melted down. There was no power in our town since October 29, 2012. The car windows(side and the top) were open by itself. I do not understand how the car caused fire by itself. There were thousands of cars but I did not hear the flood caused the fire which was parked inside of the garage. The car was brand new and had a temperate plate. There is no smoker in my family. This seats issue occurred Oct. 30, 2012.

The vehicle was a 2013 Acura Rdx. The VIN of this vehicle is 5J8TB4H54D_. My new 2013 Rdx caught on fire at my garage in the day after hurricane sandy. Acura and auto insurance company(progressive) insists that the car caught on fire by flooding. However, car battery and engine are fine. Only front seat, dashboard, and navigation system were burned and the fire damaged my garage too. Furthermore it does not make sense that flooding caused fire at all. I showed my Rdx to Acura technicians, but silent. Isn't it a defect of new model?. It was Tuesday, October 30, 2012 when this seats issue occurred.

The vehicle is a 2009 Honda Fit. This is nothing to serious I however have noticed it and I fixed it myself after a few minutes of tinkering. On the passenger side seat the front bolts that hold the seat in placed became loose. It was only loose enough to create an extremely annoying rattling and clanking noise. I just had to pop off the plastic covers and hand tighten the bolts. Problem solved. It was May. 14, 2013 when this seats issue happened.

The vehicle was taken to the dealer for diagnostic test. The dealer stated the passenger seat sensor had failed and would need to be replaced. The manufacturer was made aware of the failure. The vehicle was not repaired. The failure mileage 153,000. The VIN was not available. Az. It was March 1, 2014 when this seats issue occurred.

The vehicle having this issue was a 2008 Hyundai Azera. The car had a crash. The VIN of this Azera is KMHFC46F38_. This accident happened in waco, texas, mclennan county. Chevrolet pickup truck was traveling west on fm 308 on the inside lane. Azera was traveling north on fm 933 on the outside lane. The driver of the Chevrolet pickup truck was not wearing his corrective lenses according to his dl restriction (restriction a) and disregarded a stop sign and flashing red light at the intersection. Due to this, the Chevrolet pickup truck struck the Azera fd to rbq. This collision spun the Azera clockwise. The Azera left the roadway to the east and went into a ditch and received fd damage. The Azera came to rest facing south in the ditch. The Chevrolet pickup truck came to rest facing north on fm 933 on the inside lane. All air bags deployed in the Azera except the drivers seat and drivers side curtain. Driver received blunt force damage to his left shoulder. Surgery was preformed on March 12, 2014. Azera was towed to "big boys wrecker service, 415 s. Robinson dr. , waco, texas 76706". This issue happened on 04/01/13.

As I sat at a stop light it turned green I accelerated and ad I did the bracket on my front driver seat seperated from the framework of the seat causing me to fall backwards unable to see the roadway mirrors or properly accelerate or brake as any driver should be able to causing me to lose temporary control of the vehicle. I however reganied control and was able to pull over to the shoulder but was unable to drive until a repair was done. This seats problem happened on Friday, May 9, 2014.

The warning light goes out and there by arming the passenger side air bag. If this light doesn't go out, the airbag system on the passenger side will not activate. I went to the kia dealer to confirm this is what would happen. He said if the light stays on, the system will not activate. He stated that it was becoming a problem in my model of kia. It is approximately $2,000. 00 to repair. To me this is a safety concern worthy of further action by your agency. This problem occurred on 04/01/2014.

The car involved is a 2008 Kia Motor Optima. The vehicle identification number (VIN) of this Kia Motor Optima is KNAGE12348_. The air bag light came on in dash. The car dealer said it came from the passenger side seat and that the air bag is defective and will not deploy in a crash. He said that it would be $1600. 00 to $1700. 00 to fix if I wanted it fixed. My wife just pulled into her mother's driveway when the light came on and it will not go out and right now is scared that the bag will go off with her mother who is 92 in the seat. I think the price is steep and if anyone should pay to have it fixed that it should be kia to fix it. It was Tuesday, Mar. 18, 2014 when this seats issue occurred.

The vehicle was a 2014 Land Rover Range Rover. Our 2014 Range Rover sport passenger side airbag indicator deactivates intermittently during multiple trips at highway speeds with an adult in the passenger seat. After 7-12 minutes the indicator light near the sunshade button will illuminate to note the passenger airbag has been deactivated or is not operational; even though the passenger is sitting correctly and maintaining constant weight on the seat. Moving around the seat or shifting weight from one side to the other will not extinguish the indicator light; however briefly unbuckling the seat belt while the vehicle is moving and then quickly buckling the seat belt will cause the indicator light to shut off for 7 to 12 minutes. After this unbuckling and buckling process has been completed the light will come on again. The Land Rover dealership inspected this vehicle and found no problems with the passenger air bag sensors. Land rover also never mentioned to me that this issues has occurred with other Range Rover vehicles. On another note, the air bag indicator light is severly underilluminated. Very difficult yo see during the day and there is no other visual or audio queue to the driver that the passenger air bag has been deactivaited. This is troubling because you may never notice the issue unless it occurs at night, but even then it is still difficult to see in the dark. Bc. This problem occurred on 02/07/2014.

The car is a 2005 Lexus Gx470. I saw jen strathmore's news, watch dog report on cracked/melting dashboards. I want to file a complaint that I too have this problem. The cracking started a few years ago and has slowing continued. My entire dash is literally cracking off in pieces and there is foam padding exposed. It is getting worse. I'm concerned about the safety with the air bags. I drive a Lexus SUV, an expensive car. I did bring it to jm Lexus, the customer service manager said there was nothing he could do about it, now. I didn't think I had a leg to stand on, until I saw the multiple complaints by others with the same issues on television. Please let me know what I should do next. Thank you, lisa bezick. The car was a 2010 Lexus Rx. My Lexus has a mold problem. They have been unable to remedy it, but will not replace the vehicle. They have tried to remedy the problem no less than 7 times, but cannot eliminate the mold. Employees of Lexus of greenwich have admitted to me that they know this is a recurring problem. Yet they continue to return the vehicle, with mold, to their customers. This issue happened on 11/01/11.

The vehicle involved is a 2004 Lincoln Ls. The VIN of the car is 1LNHM86S14_. While traveling at 40 mph on flat, smooth two lane roadway the driver side curtain and side seat airbags deployed striking driver on left shoulder and facial area. There was no side impact or rough road prior to deployment. Driver was able to keep control of vehicle and move to berm. Driver suffered soft tissue trauma to left shoulder, upper arm and face. Side curtain airbag damaged interior roof header upon deployment. Driver seat side airbag appears to have not fully deployed and expanded inside the seat ripping out a 3 foot area of stitching and tearing the leather seating material in the immediate area. Contacted Ford motor company and they refused responsibility. The failure date was Apr. 28, 2014.

The vehicle involved was a 2013 Lincoln Mks. Driver's seat folded in on driver while in motion. First incident was March 25 when returning from dinner as entering the driveway. March 30 the seat folded as we were driving 70 mph in the left lane on interstate 295 outside richmond, va. March 31, third incident occurred as leaving the driveway and on the street. April 1, 2014 car taken to williamsburg Ford where it was left two days. Customer invoice notes: notes: a: customer stated that while driving, the seat back will fold forward/inward towards steering wheel could not duplicate concern, no evident faults. B: perform multi-point inspection. 99p perform multi-point inspection c: quick alignment check 31 quick alignment check this is very scary when it happens especially when driving at high speed in traffic. Car needs to stop and seat needs to be readjusted. Some local roads in this area do not have safe places to pull off to adjust this seat. Have there been other reports of this problem???. This issue happened on Mar. 25, 2014.

The contact had noticed extensive spider webs within the interior of the vehicle. The vehicle was taken to an authorized dealer who cleaned the interior and the safety concern had recurred. The contact had noticed that there had still been numerous spider webs after the local dealer was to inspect the vehicle. The vehicle was taken back twice to have the vehicle inspected and to remove the spider infestation. The vehicle was cleaned. The manufacturer was notified of the safety concern. The approximate failure mileage was 4807. Dyd. This problem occurred on Tuesday, October 1, 2013.

The car involved was a 2001 Mercedes Benz C320. My 2001 mercedes-benz C320's battery was constantly going bad and having to be jumped after not starting it or driving it for approx. 24 hours. Finally, after a certified mercedes mechanic investigated the problem, he informed me that the problem originated from the driver's seat module, which was draining my battery. He informed me that if I unplugged it for the time being, my battery would no longer drain. Sure enough, after unplugging the seat module, I no longer had issues with my battery draining. The seat module controls seat adjustments as well as steering wheel adjustments, so this is very inconvenient, and unsafe, because there are times, when I have to plug the module in adjust my seat or steering wheel, and it does not work, and I'm stuck with the seats or steering wheel in an uncomfortable position. I have also had problems with brake (stop) lights, turn signal lights, etc. , in which the vehicle indicator states that particular lights are malfunctioning, and then after awhile it goes away, and then days later, the lights are inoperable again. I have spent quite a bit of money replacing bulbs. I originally purchased this vehicle in 2004 when it was still under warranty. The failure date was Apr. 18, 2008.

The vehicle having this issue was a 2006 Mercedes Benz Clk350. While parked I started the car and placed an item on the passengers seat. Immediately the srs light came on. The srs has detected something wrong with the airbag system and shut the airbags off on the car which is an extreme safety hazard. After taking the car to the mechanic I was told code 9223 and 9080 displayed. 9223 states that the car needs a contact reel at steering wheel (part $410/labor nearly $300) and 9080 states the passenger side seat bottom needs to be replaced (part $1690/labor nearly $300) . I believe this should be a recalled item because numerous Mercedes Benz have this issue and the repair cost is extremely high. Please investigate this safety issue. Due to the extreme cost for repair, I am driving an unsafe and hazardous vehicle. I am sure, I am not the only person in this situation. This seats problem happened on Tuesday, Jan. 14, 2014.

The vehicle involved was a 2011 Mitsubishi Lancer. The VIN of the car is 6547673/65_. Rented from hotwire at san juan airport. Car rental was hertz. While reversing from parking spot at hertz lot, roaches came out of dashboard. I stopped at the office to change car. Agents were not concerned. I asked for another car without the bugs and it took some time to get another car. I inspected it and didn't find any. I drove off. The next morning I drove to ponce and back that night. Drove up & down a steep mountain side. When I parked that night roaches came out of that car too. Called several numbers, was freaked out, and no one seemed concerned. My dad sprayed the car that night, cleaned it up and the next morning we drove back to hertz to get a new car. My concern was that if those bugs would have come out while I drove the mountainside, I probably would have killed myself if they did come out and crawled on me. Im familiar with the roads but for the tourist that rent cars there it is definitely a safety issue and something has to be done. This seats issue occurred 01/27/2012.

Everytime it rains, or moisture gets on my tan tweed seats it makes brown circles that are even hard removed at a recon. I have spent 2 recons and it comes back while putting groceries, or getting into the truck, I have had this truck for 7 months, called mitshubishi and they suggested a scotchguard. . Forever? that's kind of ridiculous. This problem occurred on Oct. 29, 2007.

I was driving my 6 month old 2014 Subaru Forester at about 50 mph at night and my driver's seat began reclining. I almost lost control of the car and had an accident because the mechanism to stop it was not working. It happened three more times after that occurrence. Also, the power lift gate was inoperable. I had to crawl into the back cargo area to try to open the rear lift gate. It would not open, even from the inside. It was reinitialized several times by kingery rye Subaru in rye, NY and they failed to correct the problem. Eventually they replaced the part but the operation of lift gate remains unpredictable. I need to be able to use the power lift gate at all times to put a wheelchair in the back. I was informed by Subaru of America (in writing) that some of the electrical problems that are occurring are because kingery rye Subaru installed an after market remote starter into my car without my knowledge or approval. I have written documentation from Subaru of America stating the dealers must install Subaru parts. I paid a lot of money for a Subaru remote starter. Rye Subaru put a cheap after market starter into my car. That is fraud. My car is starting randomly and without a key in the ignition. A CO-worker came into work and told me my car was locked and running in the parking lot. Recently I closed the lift gate, using the button on the lift gate and the car started itself as I was walking away from it. The car has started on its own many times. There are multiple dangerous electrical problems that are putting myself and my family's lives at risk. The side driver's and passenger's windows fog up when the heat is turned on. The DE-fogger does not remedy the problem at all. I am unable to see anything out the side windows which poses a big safety issue especially during the winter. I bought the car because it had a high safety rating. This car is not safe. This seats issue occurred 11/25/13.

The car was a 2008 Suzuki Forenza. Starting in January 2014 car had approximately 75,000 miles on it and began lurching into gear from stopped. When put into reverse, jumps into gear. When put into drive after backing out of driveway lurches forward starting in 3rd gear and continues during driving. Husband diagnosed after much online research and a few months later, even taking code into dealership for repair. Dealer insisted they couldn't replicate and cleared code without making note of problem. My husband argued that we wouldn't be picking car up until the problem was replicated and taken care of. After insisting they couldn't replicate, we drove back to dealership and made service manager ride with husband until car "performed" malfunction, literally a half of a block from dealership. It was obvious they were avoiding problem and hadn't driven car at all! code was noted as 0700, just as my husband had reported initially. Transmission sensor was replaced and after diagnostics were finally properly preformed, airbag light was also reset (has continued to come on almost entire time we've owned the car-were told our daughters long legs were kicking wires underneath drivers' side seat) and also made aware voltage is too high on drivers side but nothing was done to fix either. Dealership offered no solution. Spent $191. 65 to fix transmission sensor. Originally charging $260+ but husband demanded reduction in diagnostic price due to the fact we had to drive back into town to dealership to duplicate problem for incompetent/unwilling service department . Very unsatisfied and will be looking into options after seeing transmission range sensor seems to be ongoing epidemic for many 2008 Suzuki Forenza owners. Have also had ongoing problems with seat airbag sensor not working on front passenger's side since 2009. It was Fri., Jan. 10, 2014 when this seats issue happened.

The car was a 2009 Volkswagen Jetta. Crash was involved with this problem. Vehicle identification number (VIN): 3VWTL71K69_. Our car is driven by 2 drivers- I am 5 feet tall, my husband is 6'1". The car seat gets moved back and forth a lot as we both drive the car daily. In late November my husband moved the seat all the way back and it made a cracking noise. We later noticed the air bag light was on. He found the broken wire under the seat and repaired it, but the light was still on. A call to the dealership revealed they knew about the issue and the fix was to "not move the seat so much". The next week- before we had a chance to take the car in to get the light cleared- the car was involved in a collision. I rear ended a car in slow-and-go traffic when she switched lanes in front of me while I was accelerating. $15,000 in damage was done to the car but the air bags did not go off. A witness that stopped to help had to pry the door open to get me out. The fighter fighters all remarked that they could not believe the air bags didn't go off. No airbags could have been a huge issue if the accident was worse. It was Dec. 3, 2013 when this seats issue occurred.

He came within inches of side blinding me. I quickly jammed on my brakes, and beeped my car horn. He then swerved back into his lane. Fortunately for quick actions, this incident didn't result into an "accident. " but what really concerns me is the fact that both seats in the back fell forward exposing my trunk! if my five year old was with me, he would've been sitting in the back and could have possibly gotten injured, as everything from the trunk including the seats flew forward. Please check if there are other complaints regarding this problem. I purchased this specific vehicle because of its high safety rating. It was July 31, 2013 when this seats issue happened.

The vehicle involved was a 2006 BMW 3 Series. I received a recall notice from BMW in January 2014 (recall campaign number 13v-564: front passenger air bag seat occupancy sensor mat). The recall notice states that in the current condition, the vehicle is unsafe to drive with a passenger in the front passenger seat. The recall notice states that the parts needed to perform the repair are scheduled to be available in March 2014. It is now may 6, 2014 and BMW customer relations has indicated that the parts are still not available and they are unable to provide an estimate on when they will be. It has now been nearly 5 months since receiving the original recall notice. During this time I essentially have a vehicle that is unsafe to drive with a passenger in the front passenger seat. BMW does not appear to be taking this issue seriously and providing a prompt resolution to this problem. I have placed repeated calls into BMW customer relations and continue to be informed that nothing can be done until the required parts are made available. Any assistance you can provide is greatly appreciated. This issue happened on Nov. 13, 2013.
