I am reporting a recurring safety issue with the forward collision alert/automatic emergency braking system on my 2023 Chevrolet Blazer 2lt. The vehicle frequently activates the forward collision alert when there is no vehicle, pedestrian, or other obstruction in front of me. The red collision warning will illuminate and the system will beep even when the roadway is clear. More concerning, on three separate occasions the vehicle has unexpectedly applied the brakes extremely hard while I was driving in traffic despite there being no obstacle or imminent collision. The most recent incident occurred on August 11, 2026. I was making a turn through a busy intersection when the automatic emergency braking system suddenly activated and applied the brakes extremely hard. There was no vehicle, pedestrian, or obstruction in my path that would have required emergency braking. The sudden braking occurred while I was in the intersection and created a serious risk of being struck by surrounding traffic. The braking force was severe enough that my seat belt restrained me very hard and caused an injury to my chest and flew me forward into the steering wheel while also risking the vehicle behind me to almost slam into me. This was not an isolated occurrence. This is the third time the vehicle has unexpectedly applied the brakes hard while I was driving in traffic. In addition to these three braking incidents, I have experienced frequent false forward collision alerts where the red warning and audible alert activate when nothing is in front of the vehicle. I am concerned that the forward collision alert/automatic emergency braking system is malfunctioning and could cause a collision by unexpectedly stopping the vehicle in moving traffic. I am requesting that this recurring safety issue be documented and investigated.
On February 14, 2025, at approximately 0700 hours, I was traveling southbound on xxx, approaching a left turn (eastbound) onto xxx. I was the first vehicle in the turn lane with no other vehicles or pedestrians in front of me. The road conditions were clear, dry, and cold. Upon receiving a green light to proceed with my left turn, I began to accelerate without issue. However, approximately halfway through the turn, my vehicle's pedestrian collision avoidance system was unexpectedly triggered. The red windshield alert light began flashing, accompanied by an audible warning, followed by hard braking. My vehicle came to a stop in the middle of the turn, despite there being no vehicle, pedestrian, or any other object in my path. This sudden and unwarranted activation of the emergency braking system put me at great risk. Had a vehicle been traveling northbound on xxx, it would have collided with my vehicle. Similarly, if there had been a vehicle behind me, it could have caused a rear-end collision. The system’s phantom activation, with no clear reason, was extremely alarming and could have resulted in a serious accident. I have experienced sporadic false alerts in the past, including red windshield alerts with audible warnings, typically during heavy traffic in parallel lanes. However, this was the first time the system engaged the brakes. I immediately deactivated the feature after returning home. On February 25, 2025, I took my vehicle to xxx, for inspection. During my visit, the service department could not identify any issues with the system. They kept the vehicle for further diagnostics and performed a road test on February 26, 2025. However, no issue was found, and the emergency braking did not activate during the test. I was informed that there was nothing more they could do.
The driver-side half-shaft disengaged from the transmission. This caused a roll away of the vehicle when it is thought to be securely in "park" gear cuasing a frontal colisión.
I was turning left at an intersection on 5/5/23 and the brakes locked up for no reason. As I entered the intersection the vehicle flashed the automatic emergency braking lights and locked up the brakes as I turned left. The lane in front of me was perfectly clear and the sudden stop fortunately didn't cause the vehicle behind me to rear end me. My wife and I were totally shocked and confused why this happened when there was clearly no obstruction ahead of us or reason to lockup the brakes. This is an absolute flaw and could have caused an accident and/or injury. This adas system is very sensitive in my opinion and constantly goes off on parked cars when driving in my subdivision. This I guess is unavoidable but the incident that happened is not explainable or acceptable!.
