Seat Belt Problems of Chevrolet Lumina

Paint is peeling off off my Lumina car no under coat under paint. Rear seat belt keeps coming loose.

Driver's side and passenger's side seat belts twisted up in male portion area. Dealers have noted problem, cannot be corrected. Possible manufacturer's defect.
