This investigation was opened on Dec. 14, 2011. It was closed on Aug. 16, 2012. This investigation resulted in a vehicle safety recall.
On August 7, 2012 general motors (gm) notified the office of defects investigation (odi) that it will conduct a safety related recall for model year (my) 2003-2004 Chevrolet express and GMC savana vehicles.

2 Brake power assist defect investigations of Chevrolet Express vehicles

This investigation was opened on Apr. 30, 2004. It was closed on Feb. 28, 2005. This investigation resulted in a vehicle safety recall.
Gm is conducting a recall on all subject vehicles build between may 1999 and October 14, 1999. Significant changes were incorporated into production in early October 1999, and vehicles built after that date have lower warranty failure rates.

3 Seats defect investigations of Chevrolet Express vehicles

NHTSA's office of chief counsel received a report from a private attorney representing the estate of a child passenger in a 15 passenger van who died in a crash allegedly occurring when the van was being employed by a school district for student transportation.
