The contact attempted to depress the brake pedal, but applied the accelerator pedal in error. The incident led to a crash that resulted in one fatality. The police filed a report and confirmed that the vehicle had been secured in park. The vehicle was not diagnosed or repaired. The manufacturer was not notified. The failure mileage was 147,000.
