Hankook dynapro as lt245-75/16 tread separation on a 2007 Ford e350 superduty van. This was the 4th occurence of a problem with this brand / model / size tire on a single Ford e350 van. One tire failure each year on average. All failures occurred at 15,000-30,000 miles, 7/32 - 10/32 tread depth, while mounted on the rear axle.
Tire tread separated after only about 23,000 miles of use on a "van conversion" of a Ford e350. The maker of the van conversions is pleasure-way (not found in your list of vehicle mfg and models). Multiple owners of pleasure-way vehicles have reported similar tire problems.
