Sped up a bit today get in front of my husband and as I did I noticed that all of a sudden I lost power. I wasn't able to go faster than 40 mph, and I wasn't able to go into gear.

There is a small plate that has 2 allen head bolts holding it on the bottom of the steering column shaft . This plate puts the vehicle in park. The bolts came loose and the van rolled backwards into the garage door even though it was in park and the driver was standing outside,next to the van.

During normal operation of vehicle speedometer intermittently fails, and driver cannot determine the speed at which vehicle is being driven. Duration of failure will last for a few minutes or for longer than a week. Dealership examined vehicle at 34,000 miles when problem first appeared, and replaced a fuse which temporarily remedied the problem. However, with 43,000 miles on vehicle, problem has started to reoccur. Please provide any additional information / documentation.
