Middle driver's side seatbelts are not bolted to the floor there just in a plastic enclosure by the seat which is not being bolted, causing seat to become unrestrained, and child was thrown off the seat.

Passenger's seat belt becomes so tight that passenger cannot move. Dealer has attepted to correct the problem 2 times, however has been unsucessful.
