The contact stated while driving 60 mph, the contact noticed an object in the middle of the road. The contact lost control of the vehicle and crashed into a cement wall. The contact stated that the side curtain air bag deployed as designed but caused the headrest of the car seat to collapse onto the forearm of the child seated in the car seat. Medical attention was provided, and the child was taken to the local emergency room. The child suffered an arm injury. A police report was filed; however, the police report number was not provided to the contact. The manufacturer was notified of the failure. The contact stated that the cause of the failure was not yet determined; however, the car seat manufacturer had opened a case. The car seat was not replaced. The vehicle failure mileage was unknown. The contact owns a diono safety seat, model type: radian rxt model, model number us16900 and manufactured in 4/01/2013. The safety seat was being utilized in a 2013 Honda Odyssey. The contact stated that while attempting to release the crotch buckle, the buckle failed to release and required excessive pressure to release. The latch that held the seat in place also failed to secure the seat. The manufacturer was made aware of the failure and advised the contact to purchase replacement parts, but then stated that the manufacturer would replace the crotch buckle. The seat was not repaired. The VIN was not available.
My toddler travels with me up to 4 round trips a day. In the last two months, her carseat has been getting harder and harder to unclick. You have to push very hard to get the red button to click open. Sometimes one side would click open, then the next time both sides would stick. Today, we were on an outing (for mother's day), and we spent over 45 mins. Trying to unbuckle her from the seat. I tried very hard as well as a large gentleman nearby in the parking lot who happened to notice me in distress. My toddler became increasingly upset as it was getting scary for her. The man was also shocked to see that he wasn't getting it unbuckled with strong force. We debated cutting it apart; we also debated calling 911 to have the fire department unleash it. I ruled both out because we wanted to give it a few more minutes. So to get her out, we loosened up the straps as much as we could and squeezed her from the seat. We spent another ten minutes or so continually trying to unclick the belt. It's shocking to me - although this has happened before. It has never taken more than 3-5 minutes to unclick the harness. I've got no choice but to keep using the seat until we can get resolution from graco. I do plan on contacting them first thing in the morning (Monday morning 5/13/2013) with the hope that they will allow me to receive another model of carseat - and return this one to them. I have not kept the receipt from the original purchase from target, so I'm certain they cannot exchange it. But in researching recalls about this model of carseat, I'm shocked once more that this has not been addressed. Please issue a recall immediately. It's extremely unnerving to have this happen to your child. And even more, the worst case scenarios are already playing in my mind: what if we had a car fire or a car accident? if the buckle is a defect, what else is defective about this carseat? Safety concern that the manufacture did not address re; e-z on molded # 86y universal harness. My son is 9 and has cerebral palsy he weights 98 lbs and due to his condition he cannot sit on a booster and is large enough to sit in a regular seat. We are using this harness slog with the lap shoulder belt institution along with the locking clip (item g on the owners manual). However the locking clip in the illustration is next to the latch plate. Unfortunately, this configuration does not work in our Honda Odyssey - as well as most other cars the locking clip has to be placed on the opposite side as if you do not the shoulder belt will not be held in a secure position- when I asked them if this position had been crash tested - they told me yes but could provide no documentation. We love the harness but I was to make sure that it is safe for our son. I can send you picture if you like. Thanks for your help. Failure of the latch strap on both of my sunshine radiansl model 16500 car seats. I will find the buckle on the latch strap opens/loosens which leaves the seat unlocked and not secure. I suspect that my toddler is kicking it open although I have not witnessed this event. This event has occurred 3 time on one seat and once on another seat. I called the company and they have replaced the superlatch/lower anchor strap on each of the two car seats. I am concerned because I have seen several forum posts of other consumers having the same issue with their radians. (your website did not have a model 16500 option when entering information, therefore I used the 18500 option which is a similar seat. )

I own 5 sunshine kids radian xtsl seats. All of them have the same problem. The company has not addressed this issue through a recall or even a mention of it on their website. All of the carseats become loose after proper installation. The belt that is used to tighten the seat has a faulty mechanism that allows the carseat to become so loose that it falls to the side. I can't believe this car seat has not become recalled yet. There has been numerous complaints and even a safety campaign to recall these seats started by parents. My daughter was riding rear facing in her sunshine kids radian 80xtsl. When I looked back in the rear view mirror I noticed that she was wiggling her foot on the latch buckle, and asked he to stop. When we pulled into a parking space and I went to take her out of her seat I noticed that she had actually unlatched the seat simply by pushing on it with her feet! this is dangerous and completely unacceptable! Seat installed rearfacing with super latch loosened excessively and dramatically when pushed/pulled. Initially there was no slack over very little slack, and it became inches of slack as the entire seat rocked back and forth. Had the child in the seat not been belted in, he could have easily fallen out of the car. The seat was retightened and the same process was easily repeated. The seat was reinstalled forward-facing, diminishing the protection offered to the child (rearfacing is 5x safer), so that the child could be safely transported home. The latch belt of my radian xtsl is consistently loosening itself. It does not stay tight. The adjuster sits on the edge of the seat when installed rear facing in my '08 Odyssey, and the weight of my 34 pound daughter getting in and out of the seat causes the harness to loosen. This is for a sunshine kids radian xtsl car seat. I am very capable of installing car seats correctly so I know that the car seat was installed per the manufacturers instructions. The car seat was installed rear facing with latch with less than 1/2" of movement. It had been properly installed and tight for over a month. On this day when I went to buckle my child in I noticed that the latch strap had loosened considerably to the point where the car seat was leaning to one side and could move freely at the belt path. I re-tightened the latch strap and it again felt tight. This is the 3rd time this has happened to me with this car seat in my car. In my sunshine kids radian xtsl, when I install the seat rear facing with superlatch, the edge of the belt adjuster hits the side of the belt path. If you slide the seat side to side to check the install, it loosens the latch belt. The latch belt is also right were my daughter puts her feet, and twice, she has loosened the belt with her foot. Radian xtsl convertible child safety seat with superlatch has a latch adjuster that does not allow the installation to remain tight. Even when I have properly installed the seat (as verified by manuals and car seat techs), a simple tap on the tilting adjuster will completely loosen the latch straps, resulting in a seat that is no longer tightly and properly installed! these adjusters need to be recalled and replaced with the locking type that britax, for instance, uses. We have a radian xtsl child safety seat for our child. It is installed in the second row captain's chair of our 2007 Honda Odyssey. The seat is tightened appropriately and installed correctly. The latch belt loosens itself in daily use. We have noticed it a couple of different times and now check it routinely. It always loosens. I have a sunshine kids radian xtsl installed with latch rear facing in my Honda Odyssey. Due to the location of the release lever for the latch strap, my son is able to easily loosen the strap with his foot while we are driving. It also loosens if he accidentally hits it with his food when climbing into the seat. As a cpst, I feel that this is a major design flaw. While I am well aware that I can install the seat using the seat belt, superlatch is a major selling feature of the radian line, and it is defective in my car. The strap should not be so easy to loosen. I can do it with the light touch of just one finger. Please make sunshine kids address the dangerous and potentially fatal issue. The decedent is a 3 month old female infant who was a passenger in a 1999 Honda Odyssey van. The infant was secured in a baby trend flex loc infant car seat (manufactured in 062809 hh, model # cs31029) and the base of the car seat was secured to a seat belt. The rear of the van was impacted by another vehicle, which caused the carrier to separate from the base. The infant, who was secured to the carrier, was ejected from the vehicle as a result of the impact. After the accident, the infant was transported to the hospital, where she was subsequently pronounced dead in the emergency room on January 30, 2010 at 16:02 hours.

We have three dorel/safety 1st alpha omega convertible car seats (22469-bse). After trying numerous times to secure two of the seats in a rear facing position, my husband and I had to use the anchor strap to keep the seats secure. According to the book that comes with the seats, the anchor is not to be used in the rear facing position. The local fire department is certified in car seat installation. The fireman pointed out to us that the latch belt on the two seats will not stay tight. After the latches were secure and we tightened the straps, the firemen were checking to make sure the seats were secure and the belt would loosen with the slightest pressure on the seats. Therefore, these seats cannot be installed properly and are dangerous for my children to ride in as is. I contacted dorel this morning, and all they could offer was a new latch belt for the seats with problems. I was told it would take ten business days for me to receive the new latch belts. When I asked the representative what I should do in the mean time, I was told to just use the seat belts in the vehicle to secure the car seats. My question to dorel and to you is why would I want to just use the seat belt when the seat is not secure and should be used with a latch system. I just purchased a britax marathon carseat from babies r us in wappingers falls/poughkeepsie NY. When I got the seat home I opened the box and was getting ready to set it up to fit my 6 month old son and put it in my car. It is at this time that I noticed the "latch" belts were missing. I am not sure if it is just this seat, I plan to take it back to get another one Monday, or if all the ones at the store have this problem. I own another one of this exact seat for my 2. 5 year old and this is how I caught the problem. I have not heard of any recalls, but would like to make you aware of this. These straps are sewn onto the seat and therefore it is not just that someone removed them from the box. I hope that there are not other people out there using this seat without the proper latches. As I was looking for the information on the seat, I was unable to find the model number, and the exact date of manufacturing. It just say on the box (not the seat) August 2008. The booster seat should include a tether strap to keep the seat in place. Just using a seatbelt around the booster is not enough to hold it in place in an accident. The removable base section of a century Smart fit plus infant child restraint, ser. # 4529, manufactured 11-21-97, shows signs of collapse caused by normal installation. The extendable foot used to adjust the base angle has thin plastic guides, which the foot moves up and down along. These guides have bent toward the rear of the seat, and may collapse further, allowing the base to settle to an improper angle. Also, the blue plastic foot itself shows whitening of the plastic, indicating that the plastic has bent or deformed. It appears that the assembly collapses because of the downward pressure applied during normal, but tight, installation with the car safety belts. This defect was found by, and is reported here by, officer mark bergin, alexandria va police department, 703-848-4763, with the permission of the owner.
