Automatic Transmission problem

I had the brakes applied on my car, and put it in reverse in order to correctly position the car in a parking space. With my foot still on the brakes, because I was only moving the car a few feet, the tachometer took off like crazy, the engine reved up and the car starting inching backwards even though I still had my foot on the brakes. I almost was going to hit the parked car in back of me so I slammed the (automati) gear into park. Only then did the engine stop revving and the car stop. I turned the ignition off and came in to work and tried to calm down because I was very frightened. This also happened two weeks ago, while I was in drive, with my foot on the brake. The engine started revving and moving forward. At the time, I thought this was because I may have been on black ice.
