The contact stated that while driving approximately 65 mph, the hood of the vehicle suddenly became unsecured and flew open, striking the front windshield, and creating a visibility hazard for the driver. The contact indicated that the locking mechanism bolts had erroneously unscrewed from the hood causing the failure. The vehicle was taken to the local dealer who replaced the hood and locking mechanism and the failure was remedied. The manufacturer was notified of the failure. The failure mileage was 4,000.

The contact indicated that on several earlier occasions while driving the liftgate mechanism erroneously activated but the tailgate door did not open.
