The pre-collision system (pcs) activated twice in the last couple of weeks with absolutely no reason. Both times it activated with transmission in reverse and vehicle in process of backing up. But there was no vehicle or person within the vicinity. The sudden braking startled the driver and passengers giving concern about the viability of the vehicle operating in a safe manner for the rest of the trip. The problem has not been seen or inspected by the dealer or service personnel.

Lexus of sarasota FL confirmed a faulty brake actuators causing brake , skid control and abs light to come on which created an immediate and extremely dangerous driving situation as I was on an interstate highway driving at 70 mph. It’s been confirmed that multiply Lexus models and years have had numerous brake actuator . Multiple failures have been documented yet only a few were given recall status!!this is outrageous and an immediate review of this oversight needs to be addressed.

Describe: • rear camera froze • parking assist warning did not activate • auto-brake did not engage • accident occurred before recall update I backed into my neighbors car which was parked opposite my driveway. Searched and found Lexus safety recall notice 25la06.
I ran my car’s left front corner into a concrete pillar at about 5 mph, put it in reverse and then the car engine sounded as if it was accelerating and it moved backward at a rapid pace. I put my foot on the brake, no response, then put gear shifter into drive, no response, put shifter in neutral, no response, then rear left corner of my vehicle rammed into a concrete pillar/metal window frame. I then turned the vehicle off, restarted and drove away. I did not put my foot on accelerator to cause the rapid engine acceleration.

How was safety compromised for you or others? while driving on the highway, the vehicle experienced unintended braking, causing it to swerve and spin across three lanes before crashing into the median, putting both my safety and the safety of others at risk. Has the issue been verified or confirmed by a dealer or an independent service center? the vehicle has been at the dealership for manufacturer inspection for 8 weeks, but there has been no response regarding the inspection results, thus the issue has not been confirmed yet. Has the vehicle or component undergone inspection by the manufacturer, police, insurance representatives, or other parties? yes, the manufacturer has conducted an inspection of the vehicle. Were there any warning signs, messages, or symptoms of the problem before the failure, and when did they first manifest? no, there were no prior warning lamps, messages, or indications of any malfunction in the system before the accident. The issue first occurred at the time of the accident.

I was attempting to stop at an intersection. I pressed the brake pedal and an error message came up on the info screen and the car failed to stop. I ended up crashing into the rear of the vehicle in front of me.
The contact owns a 2019 Lexus Es 350. The contact stated that on three occasions while reversing into a parking space the brakes engaged the automatic braking system without warning. While driving 55 mph while applying the brake pedal the vehicle vibrated and made an abnormal bumping noise. The vehicle was taken to the local dealer where diagnosed the rotors needed to be shaved down and resurfaced. The vehicle was not repaired. The contact was provided a loaner vehicle. The manufacturer was not notified. The failure mileage was approximately 2,000. The contact stated the failure was due to the automatic braking system.
