While the vehicle was parked and no one was pressing the brake pedal, the abs actuator pump continued to run and made a constant buzzing/whirring noise. This suggests that the actuator is cycling excessively to maintain brake pressure. I am concerned this could eventually lead to loss of abs function or reduced brake assist, increasing the risk of an accident. No dashboard warning lights are currently illuminated, but the problem happens repeatedly and can be reproduced. The vehicle has not yet been inspected by the dealer, but I am aware other Toyota/Lexus hybrid models have had similar brake actuator issues in the past.

As a dominos delivery driver, I am constantly braking in neighborhoods, on main roads, highways, parking lots, you name it. It’s important for me as a safe driver to be able to rely on my vehicle to perform properly. My VIN number search on NHTSA. Gov shows no such recalls for my vehicle as "0 unrepaired recalls". I never got recall notification regarding this issue but Lexus says I should have been notified.

Brakes malfunctioned almost resulted in a life threatening car accident. Car is at the Lexus dealership for repair. The have confirmed problem. Yes there were warning lights moments before needing the brakes and it not working whn pushed.

There was an unknown brake indicator light displayed. The passenger occupant sustained extreme chest and neck pain. The driver and passenger were provided medical attention. Additionally, the two occupants of the Mazda sustained leg and neck pain but no medical attention was provided. The unknown occupant (s) of the Honda sustained back, feet, neck, and head injuries, and medical attention was provided. The passenger occupant confirmed that the driver had pumped the brake pedal. The vehicle was towed to a lot. The air bags deployed. There was no reported fire. The contact tested the brakes and showed resistance. The contact had an independent mechanic test the vehicle, and the obd reading showed no results. There was an indication of a low battery. A police report was filed. The police tested the vehicle and noted the brakes were functioning. The contact stated that a witness traveling behind the driver did not see any brake lights illuminate. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 30,000.

I was able to reproduce the issue. When accelerating from a stop, but not yet passing 7 mph (typically 3-5mph registering on the speedometer), then suddenly braking, the brakes do not immediately engage, but rather engage after several seconds have passed. The system/vehicle is available for inspection upon request. The vehicle did not stop (as the brakes weren't applied) causing a collision. I am able to reproduce the issue in the vehicle. No inspection has yet taken place. No warning lamps or other vehicle messages appeared prior to or following the braking failure and the resulting collision. This undocumented behavior was not noted prior to the failure resulting in a collision. Normal driving conditions would not expose the failure. Limited video is available upon request.

While driving on the highway electronic brake boost suddenly failed, leading to brake malfunction. This is common malfunction of this model. This leads to potentially hazardous situation when in emergency or even regular traffic breaking car would not break as expected.

No injuries were sustained. The vehicle was towed to the dealer where it was not diagnosed or repaired. The manufacturer was made aware of the failure and a case was opened. 2013 Lexus Es300h driving on [xxx] , I felt the brake didn't work during the street. It's hard to push the brake, and I almost hit the car in front. Omg, I was very scared. I saw an abs brake light, and I tried to turn into a parking lot and find the nearest pepboys in birmingham, alabama to inspect the system. I was informed of a problem with the brake booster assembly. I’m sorry I can’t afford to pay the labor, service and parts at this time. I was driving down the highway coming up on a traffic jam put on the brakes and several warning. Lights came on the dash and my power brakes failed. I was barely able to stop the car without hitting someone. From that point forward the brakes did not work properly, the regenerative braking system did not function. Also, the cruise control stopped functioning. I took the car to the Lexus dealer, they tested the car and stated it need a new brake booster brake assembly and that there was a safety warranty issue that was extended out 10 years and up to 150,000 miles and although my car only had 79,000 miles on it was three months past the warranty date and the refused. They reset the car brakes apparently and I was able to drive the car for several days while I spoke with Lexus, and then the car brakes failed again.

Were there any warning signs, messages, or symptoms of the problem before the failure, and when did they first manifest? no, there were no prior warning lamps, messages, or indications of any malfunction in the system before the accident. While driving 55 mph while applying the brake pedal the vehicle vibrated and made an abnormal bumping noise. The vehicle was taken to the local dealer where diagnosed the rotors needed to be shaved down and resurfaced. The vehicle was not repaired. The contact was provided a loaner vehicle. The manufacturer was not notified. The failure mileage was approximately 2,000. The contact stated the failure was due to the automatic braking system.
The pcs (pre collision system) warning icon flashed in the display indicating that the pcs was temporarily unavailable. The vehicle was being driven on a highway. Pulled over to look in the owners manual only to find that the manual told me to stop. We proceeded to return home without experiencing any adverse handling. I really don't feel safe driving the car not knowing if the pcs system will fail.
When you step on the break to start the car the brake pedal becomes very stiff in the car will not start. You have to push real hard on the pedal to try and start the car.
Vehicle seems to accelerate on its own while pressing brake, even moving forward after coming to complete stop.

I was parking my car on a commercial complex. I am pressing on the brake as the car slows down. When I further pressed on the brake, the car instead of taking a complete stop, accelerated. It hit the building in front of me. When trying to back up, it accelerated again in reverse. I pressed the brake really hard then it stopped.
I had a car accident approximately one month ago due to sudden unintended acceleration and loss of steering in a 2004 Lexus Es330. I am the only owner of the car and I had it serviced regularly over the past 13 years. The night of my accident I lost complete control of my car as I attempted to merge on to the local lanes of an interstate highway in northern, NJ. The car accelerated to the left and crossed sped across 8 lanes of the highway, including over a median before it spun around and jerked to a stop in the left lane of the west bound lanes facing east. I was hit head on by another driver. I was not injured, but the car was a total loss. Both of my parents also currently drive Lexus vehicles, and both models were noted to have similar issues. Furthermore, my mother recently received a letter indicating her car was being recalled due to the passenger air bag issues, the letter stated while Toyota would offer what they considered to be a suitable fix at no charge. The letter further explained that they did not currently possess the parts needed for the repairs and would be in touch in the future, no indication of the expected time frame was provided. Between my accident and knowing both of my parents are currently driving vehicles that are unsafe to drive, by Toyota's own admissions, I have been and continue to experience significant distress. I am aware of the significant fines and suits settled against Toyota in the past, but they continue to seem unfazed despite their cars putting all of our lives at risk daily. I am also incredibly disgusted by their history of victim blaming and would love to do whatever I can to bring attention to this issue. I feel it is only fair that Toyota accept responsibility for their seemingly never ending list of wrongdoing and take immediate steps to correct ongoing issues.

Driving out of my garage usually slowly and when I put my foot on the gas while in reverse to pull out the car just flew out into the street. The immediately tried to brake and couldn't work. When I hit a truck in the street behind me the car finally stopped. I had no control. This never happened before, a first time and a very frightening experience. The car is at the Lexus authorized body shop waiting for the Lexus inspector to go over the car. I would be afraid to drive this car in the future thinking this could happen again anytime. I'm the original owner and felt this is the best car I ever owned until now.
Vehicle due for regular, periodic service at Lexus of manhattan. Arrived @ 07:00. At 09:35 service manager informed us of serious problems with the rotors, brakes, and alignment. Said they had completed the alignment and cautioned the vehicle would have to subsequently have major braking system repairs. Vehicle was leased new in may 2011. Had never exhibited a single problem. Was driven ~98% highways. Always garaged. Never parked on a city street. This Lexus either is a "lemon" or the dealer is fabricating the truth. Problems reported to Lexus. Response: "the problems with the vehicle are not covered and must have resulted from your misusing the vehicle. " in effect, Lexus is taking their dealer's side in this situation leaving the door open to this complaint to the federal authorities. This situation is pure nonsense and either the vehicle is a "lemon" or the dealer was "up-selling" an unnecessary repair. Lexus has refused to authorize this vehicle be inspected by their own technicians. Thus Lexus customer satisfaction (the people with whom I was speaking at Lexus) is putting itself into the position of admitting there is something wrong with their vehicle but they will not even investigate their dealer's findings! something is (again) very wrong with the people from Toyota!.

I pressed on the brake as hard as possible and managed to slow the car but not stop it. I put it in park but the engine continued to roar. It finally went back to normal. Lexus checked the car but said they found nothing wrong. This model had been recalled for the same problem. Clearly the car may be dangerous. Lexus denies all responsibility. At about 2:00 pm on Dec 26, 2011, while parking in a public covered garage, I turned into the parking space, positioned my car, and was about to turn off the engine when the car lurched forward, hitting the concrete wall in front of me. The passenger in my car, my 21 year old daughter, also a licensed driver, felt the car lurch and accelerate forward. Since I had reached a good parking position, I am certain that I had my foot on the break and would not have been trying to accelerate. The injury to my foot, from all indications, shows that my foot was on the break. I had it x-rayed on Dec 27 at a patient first medical facility and also was seen on Dec 28, by my family physician. He examined my foot and indicated that my ankle sprain was injured as I was breaking; it would not have sustained that injury if my ankle would have been on the accelerator.
The brakes will fade/release after being stopped for 1-2 minutes with the foot on the brake pedal. It appears that this happens as the engine rpm decreases about 100-200 and the brake pressure ebbs. The car will sometimes move forward about 1-2 feet before it stops again.
