Service Brakes Problems of Lexus GS

Brake actuator or booster accumulator, always has been failing on Toyota/Lexus hybrids, they need to do another bulletin or tsd because the malfunction is unsafe and many drivers are reporting it.

The contact owns a 2011 Lexus Gs460. The contact stated that while operating the vehicle, the message "hydraulic brake sensor fault" remained displayed. Additionally, while depressing the brake pedal, the vehicle experienced severe shaking and vibration. The cause of the failure was not yet determined. The manufacturer and local dealer were not notified of the failure. The failure mileage was 149,000. The VIN was not available.

This was an unintended acceleration with non functional braking of a Lexus Gs 350. This occured on December 18 around 8:00 am. The driver noticed an orange triangular light in the dash while travelling slowly through a neighborhood less than a mile from the incident. While approaching a lighted intersection, the car suddenly accelerated when the brake pedal was depressed. The brake pedal was stiff and did not work. The driver tried to avoid another vehicle by veering right. The car hit a curb, travelled over a small hill and ended up in a parking lot after the ensuing damage disabled the car. The driver was able to escape from the drivers door before a fire erupted on the drivers side of the engine bay. The driver's safety and any other cars or persons in the vicinity were obviously put at grave risk.

On July 29th at approximately 8:30pm, I got into my car after exiting the meijer store and noticed the car making strange noises and was hard to get in gear. I park so I don’t have to back up, so I put the car in drive and it went fast in reverse, when it went forward, it went crazy and I ran into a tree island and busted 2 tires and finally stopped. Fortunately this happened in a mostly vacant parking lot. This car has a lot of horsepower and those horses went crazy. I answered a question on how fast I was going? I answered 50mph, I went from 0 to 50 in a parking lot with my foot on the brake!.

The contact owns a 1999 Lexus Gs400. The contact stated while driving at 1 mph, the vehicle accelerated independently. The brakes failed to engage while depressed on the brake pedal almost causing a collision. The contact informed the independent mechanic of the failure, but the vehicle was not diagnosed nor repaired. The manufacturer was not made aware of the failure. The failure mileage was 67,000.

Headlamp (afs) does not work properly. Right headlamp supposed to be auto leveling does not work properly and faces to the ground. Asked the dealership to fix it while it was under warranty and was told they did. (afs) has never worked and right headlamp falls and faces to the ground and was told by another dealership how to fix it my self. Headlamps also would turn off at night and not work and was on a trip and had to wait until they wanted to turn back on again. The headlamp still today does not face at normal degree and cannot see on the right side very well. Total cost to replace the entire head lamp is over $1,200 and was told that warranty do not cover this ever since I have complained. There is also "supposed" to be a safe guard warning the driver that the headlamps are not functioning correctly and it does not work either. No alarms go off, no lights either. Searched the internet and have found the same thing as well. Head lamp flickers and headlamp facing the ground. Steering wheel also is never level and I have had it "corrected" but im told that it is "just the road" it is never straight. Brakes have to be pushed to the floor the pump is very loud and also am told its normal. Suspension has a banging sound as well since I have had the vehicle too.
