Description of incident: this is a recurring safety defect that has occurred multiple times from August 2023 to February 2026. The following description refers to the most severe incidents: 1. Nature of defect: the vehicle experienced a sudden "limp mode" event where all dashboard warning lights illuminated simultaneously, followed by an immediate loss of propulsion while in motion. Subsequently, the vehicle suffered from a dangerous 3-second acceleration lag/delay. This 2023 Lexus Lx 600 creates a life-threatening risk on public highways, as the driver cannot reliably control the vehicle's speed or power to avoid collisions. 2. Manufacturer’s admission: the vehicle has been presented to authorized Lexus dealerships. While the dealership could not provide a permanent fix, the manufacturer’s own internal tracking system now explicitly confirms: "remedy: not available. " this is a formal admission by Lexus/Toyota that the vehicle has a catastrophic system failure with no known fix. 3. Requested action: Lexus/Toyota has refused to repurchase the vehicle, forcing me to drive a dangerous asset they admit they cannot repair. I request that NHTSA immediately initiate a safety investigation into this loss of propulsion and acceleration lag issue for the 2023 Lexus Lx 600, especially given the "no remedy" status. A mandatory recall is warranted.

2 Other Fuel System problem of the 2023 Lexus LX

Failure Date: 04/10/2025

My problem occurs intermittenly, it occurs when I am idling at a stop. When I have to accelerate from the stop the car will lurch forward and die for approximately 3 seconds. It will then restart and drive normally, however the stopping in the middle of oncoming traffic is potentially life threatening. It has occurred approx 8 times since I bought the car in June 2023. I had the eco recall done and it also occurred shortly after that. I have taken it back to the dealer and they could not recreate the problem. It occurs after idling and waiting and then accelerating quickly to merge.

3 Other Fuel System problem of the 2004 Lexus LX

Failure Date: 05/08/2022

While driving up i70 in colorado I experienced a fuel smell in the cab of the vehicle. When I stopped to see what the issue was I found fuel vapors coming out of the fuel door/gas cap. I replaced the gas cap and again experienced this issue. This leads me to believe the fuel evap system does not work correctly causing excessive fuel pressure in the gas tank, leading to pressurized fuel coming up out of the gas cap. I google this issue and found this seems to be a comen problem that Toyota / Lexus has not addressed. This is a giant safety concern as well as an environmental concern that should be addressed. Fuel vapor under pressure is a fire risk and can be very dangerous. I took the vehicle to the dealer to fix and they said they have no history of this being an issue and that I would need to bring the vehicle in during the issue. Based on the below google results this is a very known issue for Toyota / Lexus that they simply do not want to recognize. I am hoping filling out this form will bring this dangerous condition to light so the dealer can correct the issue. 4 Other Fuel System problem of the 2017 Lexus LX

Failure Date: 07/10/2020

Original owner of 2017 Lexus Lx 500 33,000 miles- 2 1/2 years old at the time. The car failed on the interstate in the mountains going up hill. It sputtered and then lost power. Barely able to get to the side of the road. Was almost ran into by a semi. Family of 6 in the car. Warning lights came check engine reduced engine power had car towed to denver dealership 100 miles away. Lexus dealership could not find problem. After days of inspections, emptied fuel tank and assumed it was contaminated fuel. We had fuel inspected by the state of CO and by a third party. The fuel was clean. Lexus was notified about power failure. Nothing was done on their part for us. Car has continued to sputter when temps are high and driving uphill. Car has been in multiple times to Lexus dealerships and problem has not been resolved.
