The contact owns a 2004 Mazda RX-8. The contact stated that the vehicle was repaired per NHTSA campaign number: 14v362000 (air bags). After the repair, the dashboard began to crack in the location where the passenger side air bags were replaced. The vehicle was taken to dealer where it was diagnosed that the dashboard failed and needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was approximately 80,000.

2 Side/window Air Bag problem of the 2004 Mazda RX8

Failure Date: 01/08/2016

Takata recall. Traffic came to a halt on the freeway. I stopped and was subsequently rear ended at high speed causing the driver side and passenger side air bags to deploy. Either the air bag itself or a projectile injured my eye. The skin on my eyelid was torn requiring 7 stitches. This was extremely traumatic and I wanted to be sure this incident was included in the investigation.

Tl- takata recall. The contact owns a 2004 Mazda RX-8. The contact stated that the front passenger side air bag area was deteriorating. The contact later received notification of NHTSA campaign numbers: 15v345000 (air bags) and 15v382000 (air bags) however the parts were not available for an extended period of time to receive the recall repairs. The manufacturer was contacted and was not able to supply a permanent remedy for the front passengers side air bag failure and they were unable to determine when the part might become available for the front drivers side. The approximate failure mileage was 65,000. Parts distribution disconnect. Djr.

Tl-the contact owns a 2007 Mazda RX-8. The contact stated that the passenger side air bag seams was fractured and the air bag was exposed in the dashboard. As a result, the contact was concerned that the air bag would inadvertently deploy. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure. 5 Side/window Air Bag problem of the 2007 Mazda RX8

Failure Date: 06/15/2015

Passenger side airbag area, cracks in dashboard, in a square like shape. Afraid it may deploy, without having a wreck. Has become a distinctive square, circle shape. This has happened over the summer of 2015. I got a recall letter for driver airbag, which will be taking care of that with Mazda in next few weeks, but more concerned about passenger airbag, because my 13 yr old daughter is always in passenger seat.

The passenger side air bag door, part number fey157k5x, has become brittle and is cracked where the airbag deploys. This creates a hazard as the brittle pieces of vinyl will become projectiles in the event of airbag deployment. This is clearly the fault of the manufacturer and should be remedied.

8 Side/window Air Bag problem of the 2006 Mazda RX8

Failure Date: 02/05/2014

The dash cover over the passenger side air bag is cracking a lot in all different locations. I talked to the dealer about this and they told me it is a common issue in the RX-8's and that the factory won't cover fixing it. This seems like a serious safety issue with the dash cracking apart right above the air bag. I'm not talking about some minor cracks either. I first noticed the cracks in the dash about a year and a half ago and now they grown and expanded substantially since then. It just really looks like a dangerous situation with it occurring right above the passenger air bag. I would really appreciate it if someone from NHTSA would look into this issue further. Thank you.

9 Side/window Air Bag problem of the 2007 Mazda RX8

Failure Date: 08/08/2013

The contact owns a 2007 Mazda RX-8. The contact stated that the dashboard over the passenger side air bag was fractured. The vehicle was taken to a dealer where it was confirmed that it was a standing issue; however, it could not be repaired per a recall. The vehicle was not repaired. The manufacturer was made aware of the issue and stated that the VIN was included in NHTSA campaign number: 16v354000 (air bags). The failure mileage was approximately 83,000.

10 Side/window Air Bag problem of the 2006 Mazda RX8

Failure Date: 05/01/2013

For almost a year, cracks have started to appear over the passenger side airbag cover. It started off as a 1 cm crack in a single location, but now has progressed to multiple cracks that are up to 8 cm in length. These cracks follow the portions of the passenger airbag cover that would split open during deployment. I have spoken with mechanics and they have advised me that replacement of the passenger side airbag cover requires replacement of the entire passenger airbag system. The dashboard cover and the airbag are constructed as a single unit. I'm concerned that these cracks are destabilizing the safety of the passenger airbag. I would appreciate any insight into the matter.

Failure Date: 03/14/2009

Had an incident today where my RX-8 had been t-boned at an intersection. My side curtain airbags did not deploy though. I hit my head against the interior doorwell and have a bit of whip-lash. They hit the side of the vehicle hard enough to make it so my door doesn't open any longer and in-fact it's pushed up a bit from where it is suppose to sit. The other vehicle was a 1996 Geo metro.

Airbags failed to deploy after front and side impact accident resulting in significant damage (vehicle might be totaled). Speed at impact was approximately 40 to 60 mpg resulting in injury to driver and passenger.
