Aftermarket vehicle equipment (rough country hard tri-fold flip up bed cover, sku: 49520551a) suffered a catastrophic structural hardware failure while traveling at highway speeds (approx. 70 mph) on the interstate. Both the primary latching clamps and the factory-installed secondary safety retention straps failed simultaneously under standard operating conditions. The entire cover detached from the vehicle and launched into the roadway, creating an immediate, severe collision hazard for trailing motorists. Physical inspection of the truck bed afterwards revealed the mounting rails remained securely attached to the truck, but the integrated screw-handle anchoring hardware on the cover itself had completely failed and sheared off.
