My 10 year old niece attempted to open the power sliding door on the right side of our 2004 Oldsmobile Silhouette outside the mall. The motor was already running, in park, as I had come to pick up my daughter and niece from the mall. The door appeared to be locked since it would not open, so I pushed unlock a few times as she and my grown daughter tried to open the door. I just heard a clicking sound of the unlock mechanism as if it was locking and unlocking. The door would not open, so my niece entered through the manual side door on the left side of the van. We drove about 3 blocks from the mall to the stoplight of a major city street. About half a block or less after taking a left at the intersection stoplight, the power sliding door on the right side of the van just opened. My niece loudly exclaimed that the door opened and tried to reach across from her buckled position on the left bucket seat. My daughter and I both shrieked and said for her not to get near it. My daughter was seated up front on the passenger seat and reached back to the window ledge of the open sliding door and tried to pull it shut. It shut most of the way, but not fully. I pulled into a parking lot of a business and had my niece use the switch located near the power side door to open it fully and then again to close it fully. We made the next 20 miles without incident. We just bought the vehicle less than a week ago, and we have our 3 younger boys ride in the back, so this incident has been very scary.

The contact owns a 2001 Oldsmobile Silhouette. The contact stated that while traveling 40 mph, the side sliding door opened independently. The contact took the vehicle to a mechanic where it was determined that the unlatch actuator assembly had failed. The contact referenced NHTSA recall campaign id number: 01v067 (latches/locks/linkages), but the manufacturer stated that the vehicle was not included in the recall campaign. The failure and the current mileages were 123,000.
Power sliding door latch actuator failure. Latch is stuck in open position, will not latch. Exact same problem as described in previous recalls NHTSA campaign id number: 01v067000 and NHTSA campaign id number: 01v264000. Oldsmobile states vehicle has been repaired under the recall already in 2002. I purchased used with low miles (around 70k) in 2009. Vehicle has been meticulously maintained and is in close to showroom condition as you could find for a 2000. Oldsmobile states there are not enough complaints about the repair to consider any ongoing recalls. Oldsmobile informs me that with a vehicle of this age, regardless of mileage or condition, mechanical and electrical parts should be expected to fail. Somehow, I don't consider my door flying open going down the road to be just a minor failure or inconvenience. It is a huge safety issue, one that has been documented enough that there was an initial recall, then a second recall to fix the first recall. Obviously, this was a band aid repair. I got most of the service records from the first two owners. This vehicle was used mostly by an older couple and the sliding door has much lower than average use for a 100,000 mile car. There are not that many of these gm minvans on the road, and as a result the absolute number of complaints might be low. It should be considered that gm did not perform a sufficient repair and did just enough to get by. Oldsmobile told me that the repair at this point is up to the customer. What will it take for this recall to be reviewed? fortunately no one was in my captain's chair when our door flew open. Will someone have to be killed or run over for this to be reviewed?.

Flomaton, ala. (ap) - a van crossed the median of a highway in southern alabama and collided with a pickup truck, killing six people and injuring three others in a crash local authorities described as one of the worst they had ever seen. The force of the collision ripped the van's transmission from the vehicle and scattered debris along more than 100 yards of highway, cpl. James patterson with the state patrol. ``it looks more like an airplane crash scene than a car crash,'' said jim johnson of the flomaton volunteer fire department. ``the van's front end just disintegrated. I've never seen so much debris. All the windows exploded on impact from the looks of it. '' ``this is one of the most severe crashes we've ever had in alabama,'' patterson said. He said evidence at the scene indicated three occupants in the van were ejected onto the side of the highway up to about 40 feet from where the van came to rest. A third vehicle struck the transmission that lay in the roadway, and personal items such as pieces of a beaded necklace, eyeglasses and cosmetics and women's dress shoes could be seen along the road. State troopers did not release the identities of the five women and one man who died. The six killed were riding in the van, a 2003 Oldsmobile Silhouette.

During wet road/weather conditions power assisted steering failed. Dealership installed a water deflector plate to prevent water from contacting power steering belt. However, while traveling at approximately 25 to 30 mph in a rain storm, power steering failed, and belt for alternator slipped which caused light for battery on the dash board to come on. Dealership has been contacted, and owner was informed that nothing could be done for this problem. The dealer indicated that the wet conditions caused a belt to slip from the alternator and disabled the alternator.

Vehicle was welded incorrectly from factory, and at about 35,000-40,000 miles it will start to pull apart, causing tires on rear axle to slant and eventually blowout. The transmission failed at 11000 miles, but the manufacturer replaced it. The tires were prematurly wearing out due to defective axle. The consumer states that 20 new tires were used in 39 months period.
