I was using enhanced autopilot on Tesla Model 3. It breaks automatically in middle of road for no reason even when there is no object in front of it. Its a life threatening situation because the vehicle behind my car will not have enough breaking distance and time at high speeds. It happens almost every month and specially under the reflection of bridges. I complained to service center and employees just ask exact time when it occurred and if exact time is not provided than Tesla employees does not do anything to resolve the issue. Looks like Tesla is putting many lives in danger.
Our Model 3 was purchased with Tesla’s “full self driving (fsd)” software installed in 2018. Throughout our entire ownership period, whenever fsd is active, there are specific locations known to us - mostly at certain points on freeways - where the car will “phantom brake,” in other words suddenly decelerate the vehicle very rapidly and require *immediate* driver intervention. For instance, there is a freeway interchange ramp that passes under a bridge and doglegs to the left. The car will always phantom brake about 200 feet before it reaches the bridge. This has been happening for years, and we’ve reported this defect and this specific location to Tesla many times. There are other locations known to us as consistent phantom brake triggers, but there are of course times where this occurs when we are not expecting it as well, and that’s when it’s most dangerous. We believe it may have something to do with the car’s sensors when they sense certain shapes or objects while the road is also curving slightly, as this is the most common phantom brake scenario for us. Again, this has been happening since we purchased the car in 2018. It occurs randomly at least a few times a month, and it *always* occurs in certain locations.
I was driving on a one way street when the car suddenly stopped. I was not at an or close to an intersection. There was a pedestrian waiting on the side of the road, standing between cars. He was not in my way. He was not even close to the vehicle. The car sounded audible warning and came to a complete stop. I believe this was the reason the vehicle stopped. This is the 2nd time since my 3 year ownership that this has happened. I provided Tesla with time stamps of the event during my service visit. They told me this is normal function of the vehicle. Just reporting this in case there is a more serious underlying issue that warrants additional investigation.
The automatic emergency braking system activated when there was no collision imminent. (7:13am, 5/22/22) neither cruise control nor autopilot was engaged at the time of incident. I was nearly hit from behind by a driver who did not expect me to stop as there was no hazard or other vehicle. Tesla inspected the vehicle and found no fault and claimed that I needed driver education. Also on a subsequent interstate trip with cruise control engaged, the automatic emergency braking system activated unnecessarily. It appears to be related to shadows on the road or pavement changes.

Multiple episodes of phantom breaking with the car on cruise control on the highway. The car was not on autopilot during the episodes. The car abruptly drops from highway speed with full braking until you can override it with the accelerator. This will cause accidents of someone is close behind you. As a result, I do not use cruise control.
I intermittently get an on-screen message “automatic emergency braking is disabled. ” adaptive cruise control and auto-steering are disabled and I am unable to engage either one. Randomly the problem will resolve itself with no intervention from me - usually a day or two later. I have taken the car to Tesla service twice and they are unable to resolve the issue. On the last visit the service technician said that it is a firmware issue.
Twice the car in autopilot braked for no apparent reason. First time on an interstate traveling at 65 mph in the center lane. As I was about to pass a truck located in the right lane that was going very slowly with its blinkers on, the Tesla abruptly decelerated from 65 mph to about 40 mph. Thankfully there were no cars behind me because I would have been rear ended. The second incident happened on a road inside my community that winds through a golf course. I put the car on autopilot at 30 mph speed limit because there is radar hiding every. Day. The car suddenly braked for no apparent reason . My hypothesis is there was a sign close to the road warning that golf carts cross here. There were no golf carts or people nearby and perhaps the sign was seen by the autopilot as an object to stop the car.
My 2018 Tesla Model 3 failed to engage its automatic emergency braking system - resulting in a collision due to the vehicle in front of me suddenly braking.
Sudden unintended acceleration: while pulling forward onto our driveway (with incline) at a very slow speed, the car suddenly accelerated and brake was not working. Automatic emergency braking also failed.
The car has repeatedly (but not often) initiated emergency braking while traveling on a freeway. It has done this in literally the same place on California highway 85 several times, and occasionally on other freeways. It has always been a clear, sunny day, and it seems to occur when the vehicle travels from deep shade into bright sun (I. E. As when going under an overpass). The road was straight with no vehicle directly ahead for several hundred feet. I have reported this to Tesla via the app. And through the service departments. I have included date and time as well as location on some occasions. The only comment I have received was that the logs showed no problems, and no further action was taken by Tesla. When the severe braking occurred, an alarm went off in the car (a beeping sound). At least once a following car nearly collided with me. I was probably traveling in the range of 68-73 mph. I do not have dates or mileage information, but Tesla may based on the occurrences I reported.
"phantom braking" when cruise control is on, usually when on the freeway. Sometimes it occurs next to other vehicles, sometimes there are no cars around. On a 600 mile trip it will often occur at least once and most of the time 2-3 times. If it was slippery, the sudden braking could cause loss of traction. If someone was too close behind me they could rear-end me. No one has inspected the issue. Has been occurring since I've owned the car. No warnings.
When driving on undivided highways, the car will phantom / panic brake due to oncoming cars approaching in the opposite lane. At times, the car phantom brakes for no apparent reason at all as there are no cars nearby. At night, it gets much worse, with the car phantom/panic braking at nearly every oncoming car. My 2018 Model 3 is equipped with radar-assisted autopilot, but newer Tesla vehicles don't have the radar hardware at all and instead rely on a vision-only system that uses the cameras alone. When I opted into Tesla's full self driving beta, the new software disabled my car's radar and my car became a "vision-only" vehicle, like newer Tesla vehicles. At this point, phantom / panic braking became significantly worse. When radar was used, the braking was rare but still unsettling. With vision-only autopilot, it's nearly unusable. Even with basic cruise control, phantom / panic brakes happen with the same frequency as if you're using autopilot. I basically can't use any cruise control or autopilot when driving on undivided highways. On interstates and in-city highways, phantom / panic braking does still happen, but it's less frequent.

When driving on autopilot on highways phantom breaking occurs for no reason. There are no warnings, the car just slams on the brakes. Elon musk has state they were removing radars from cars to eliminate this issue and disabling it in cars with that hardware, but since my car have gone to all vision it is much worse. While following vehicles should always follow at a safe distance to prevent collisions, the reality is most drivers follow too closely and this could very easily result in a rear end collision. It could also trigger road rage from following drivers assuming the driver is doing a brake check on them. This has occurred numerous times over various software versions, but its much worse with the pure vision version of autopilot. Current software is 2021. 44. 30. 5.
When using autopilot on the freeway there are multiple incidents where the brakes are aggressively applied for no apparent reason.
I have been plagued with Tesla’s phantom braking for *years*. For all that they do, this seems like a problem they are not willing to invest enough resources to combat the problem. Some releases have been better than others but for last batch of releases, it has gotten far worse than ever before. The car is afraid of bridges and slows down unreasonably. The car is afraid of overhead signs (carpool ones especially) the car is afraid of cars riding on or near the lines of an adjacent lane. The car is afraid of oncoming traffic on the left while driving. What is ironic is these events of aggressive braking is all for nothing. 90% of the time,. The car brakes and does drama without actually having enough time or space if what it was afraid of actually happened. The car has a rear facing camera - why can’t it adjust behavior based on tail gating? would the car rather have a rear end collision or be afraid of a bridge? it is straight dangerous. The fact there aren’t a lot more accidents is auto pilot/fsd drivers be a large majority pay attention. But, I have had close calls and *many* angry drivers around me get frustrated when it happens. It’s been happening for 4 years…. I realize navigating a complex intersection is a tough problem to solve. They have prioritized this over the fundamentals of driving in a sane way on the freeway in a straight line. It’s ridiculous and I for one am very tired of this problem lasting so long. What chides me is that, “car, why are you braking when a car right in front of you magically went through the bridge you are afraid of?” it is basic logic and simple/obvious clues to help avoid this. They choose not to use it. Also - being in technology myself, I have to rake elon over the coals. No one Smart chooses to not use available data. Elon calling ultrasonic data to be ‘noise’ to turn to vision only is a sign of great hubris over being pragmatic. His ego regarding lidar is a problem.

Car randomly brakes very hard when using driver assistance systems. This has happened on wide open roads on clear days, multiple times.
1) what component or system failed or malfunctioned, and is it available for inspection upon request? answers: the automatic emergency braking system was incorrectly activated on several occasions, yes 2) how was your safety or the safety of others put at risk? answer: my Tesla braked unexpectedly in the driveway of my home 3) has the problem been reproduced or confirmed by a dealer or independent service center? answer: no 4) has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? answer: no 5) were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? answer: no.
Car rear ended, brakes froze after reacting to a car in front of me. Behavior unexpected and Tesla should not have frozen break maybe abs kicked in.
When the vehicle is in autopilot, it sometimes (once every couple of months or so) will suddenly engage automatic braking as though there is an obstacle detected in front of the vehicle. In nearly every instance, this has happened at one particular location on an interstate where an overpass casts a shadow on the interstate on which the vehicle is located. The problem seems to be that the vehicle senses an object in front of it and the collision avoidance system is engaged; however, that "object" is only a shadow. This has happened in a few other locations (not simply this where this one overpass is).

On several occasions while driving with autopilot on the car has suddenly and inexplicably braked hard while at highway speed. Each time there was nothing in the road, no car, no obstruction, no anything, but it was as if the car saw something that wasn't there and jammed on the brakes. I quickly took back over and returned to highway speed. Luckily we were not in heavy traffic or I would have been concerned about being rear-ended by another vehicle. I don't do a lot of road trips with the car and have used autopilot less as a result, but I would guess it has happened 4-6 times since I have owned the vehicle. I just thought it was a glitch that they would work out but just saw reports of others still experiencing this issue so thought I should report it so you were aware of the breadth of the problem. The date below is a total guess as to the last occurrence.
While driving on a highway Tesla suddenly hard braked out of nowhere. My speed dropped from 70 to 30 in a couple of seconds. There was an 18 wheeler behind me and it could have been a deadly collision if I hadn't intervened. This is a known Tesla but that someday would kill someone.
While driving to work, in 2021, I had experienced sudden deceleration and disengagement of the autopilot. It happened a few times. I have ignored it thinking it to be an electronic glitch. I did not know how to report to Tesla. But since I was sharing my data with them, I thought they will pick up some issue and alert me if there is really something malfunctioning. But I just found out that other customers has had similar issues and that I could report it to NHTSA, therefore I am reporting it albeit delayed.

While driving major federal freeways (i65 & i85) between houston and n. C. , the car while on autopilot and traveling at freeway speed (70 mph) would emergency brake when approaching some (6-7 times) overpasses apparently thinking the overpass was crossing traffic or a forward obstruction. This also occurred (3-4 times) when it was simply a rise in the freeway ahead. The braking only lasted for maximum 1-2 seconds, but since the vehicles behind me would not be expecting me to brake, I consider this a serious safety issue.
Vehicle will emergency brake on highway without warning and no other cars around. Also unable to access charging port to charge vehicle.

The manufacturer was notified of the failure and informed the contact that they were not aware of the failure and the issue would be escalated. The vehicle was not diagnosed nor repaired. The failure mileage was approximately 20,000.
Since I purchased this car new in sept 2018, the car has had a "front left safety restraint system fault". It has been fixed over 3 times and it is going for the 4th time now. The car is going out of warranty this year and I am afraid I will be stuck with the bill if it happens again. Even though all the fixes have been free so far, it is an ongoing problem also, the known phantom breaking, just add to the numbers. It is more than common. I almost see it coming when going under overpasses because of the shadow. The computer must see it as an object. I have inquired about the lane departure warning and they keep saying it is just a software issue. If it is activated at any point during the drive, then it says that "lane departure warning" is not long available and may be restored on the next drive.
