36 Service Brakes problem

Failure Date: 03/16/2022

Autopilot break the car for no reason on highways and high speed. Always happen on I-5 at anaheim, CA to los angeles, CA area.

37 Service Brakes problem

Failure Date: 02/26/2022

On a cold day at 26f, the parking system will seized. When attempting to move, you will have to move the accelerator pedal nearly more than a quarter to break free. This is extremely dangerous if you are parking close to another vehicle ahead.

38 Service Brakes problem

Failure Date: 02/23/2022

While driving along the interstate on my commute at freeway speeds 60-70 miles per hour, the emergency braking system will suddenly engage, and the car will issue a collision warning. During these incidents there is no car at risk for striking my vehicle or slowing down in front of my car. My commute usually occurs in the evening and early in the morning. This typically happens at least once during my work week.

39 Service Brakes problem

Failure Date: 02/21/2022

When using cruise control the car will suddenly brake without warning and for no obvious reason. This happens with regular cruise control as well as adaptive cruise control. It happens at low speeds (in town driving) and at interstate speeds (65+mph). No warning lights or sounds. Most recently, today. Tesla has not been responsive to date. At higher speeds, there is a strong chance of either being rear ended or forcing the car behind me to take evasive action if they are too close which could result in an accident.

40 Service Brakes problem

Failure Date: 02/20/2022

Car has too many phantom braking issues while using autopilot and cruise.

41 Service Brakes problem

Failure Date: 02/13/2022

I start noticing when you stop at stop light or traffic light and soon you start drive and before it’s get speed it’s break for second you feel and notice something stop for second and this start happening since last 10 days.

42 Service Brakes problem

Failure Date: 02/12/2022

Several times while driving highway route 2 in massachusetts, westbound and eastbound, the adaptive cruise control system slammed on the brakes for absolutely no reason. There was nothing in front of the car for 100 yards, sometimes longer. It happened at least 10 times within an hour. This caused a dangerous situation for myself and the cars behind me who were not expecting my vehicle to brake suddenly and for no reason. There have been many other cases of this submitted to the NHTSA, it is not an isolated problem. There were no warnings displayed during the phantom hard braking incidents.

43 Service Brakes problem

Failure Date: 01/28/2022

Every time I am on a two lane road, and sometimes even on divided highways, I experience numerous instances where the car seems to think there is an imminent collision, where the brakes are immediately applied excessively and for seemingly no reason. This issue is present with either the fsd beta engaged or simply having cruise control engaged at all. The issue is far worse at night, and in a 1 hour drive it is likely to happen several times. In addition to the above, the car also forces the auto high beams back onto auto when re-engaging the system after I've had to take control to stop it from stopping or swerving unnecessarily. Since the auto high beams are really bad, the car often will flash oncoming drivers on its own, and again for seemingly no reason. I usually remember to disable it every time, but have also forgotten and flashed oncoming drivers due to this as well. In addition to the above, the fsd system will make swerves that make no sense at times as well. For instance, it turned into what it thought to be the turning lane a block early, but maintained speed for the actual intended turn. When it saw the lines ending, it abruptly made a 90 degree turn a block early as it didn't know what to do. While I feel competent enough to be more alert than an average driver should expect to be when driving, I feel it is a matter of time, if it hasn't already happened, where Teslas cause a rear end accident, injury, or even death, from their seemingly random and sudden movements by a driver that doesn't understand the limits of the cars. For the approximate date field below. . . . . . It happens every time I drive on a two lane road, and even more so at night. It's not a one time instance. It's every single drive. I'll put the date the last time I drove longer than a couple miles from home, but want to emphasize that everything described above can be easily reproduced on every single drive.

44 Service Brakes problem

Failure Date: 12/15/2021

I was driving on a straight local street going around 40mph when all of a sudden, the car baked hard without the presence of any other cars or obstacles. This may have happened several times already and I am now reading that other people had similar incidences.

45 Service Brakes problem

Failure Date: 10/17/2021

On 10/17/2021 my son was killed along with his passenger in this Tesla. The car was thought to be going over 100 miles an hour on a city road coconut point road in estero florida. The car was thought to have hit a tree with the rear tire and was sent flying through the air and hit another tree and split in half. My son knew this road well as it was close to his home and drove it on his way home from work. I feel strongly that this was an acceleration issue!! he has driven this car for 2 years, so knew the car well.

46 Service Brakes problem

Failure Date: 10/13/2021

Location: 5 fwy north (solana beach area) situation: 5pm rush hour traffic. Speed 30- 40mph incident: emergency braking system engaged for no reason resulting in all 4 wheels locking up. Scared the s#!t out of me but no injury or accident. No warning lamps or messages before the incident. Outcome: looked like an idiot to my fellow commuters. When I arrived home I turned off the feature and never used it again. No inspections.

47 Service Brakes problem

Failure Date: 11/21/2020

I, deputy a. Sabrsula #5090 with travis county sheriff's office highway enforcement unit responded to a collision on laguna cliff rd. A female backed out of her driveway, and upon placing her vehicle in "drive" mode, the Tesla "took off out of no where" and caused the vehicle to drive through a neighbors front gate/fence cluster and through their yard. The vehicle then went through another section of fence and came to a final stop in the middle of a field. I noted there was zero evidence of any type of brakes being applied (skid marks, grass ruts, etc). The female stated she attempted numerous times to apply the brake but the vehicle continued forward. I attempted to load my photos from the crash but the files are too large.

48 Service Brakes problem

Failure Date: 11/11/2020

Took my car five times for brake/suspension clicking, driver seat noise (bulletin already available), and steering column rattle ( advised by Tesla to move the steering a little because it touching the instrument panel ( bad design and shouldn't the customer change his comfort to comfort the bad design by Tesla. ), also has issue with camera moist that prevent blind spot and autopilot lane change works in moistur weather, seat belt rattle. All these issue with some has bulletin they said normal and charge me for checking it while my car under warranty. All issues are while driving except battery breather supercharging.

49 Service Brakes problem

Failure Date: 10/30/2020

1. Phantom braking while driving on autopilot: mostly on highways, when driving on autopilot, the car suddenly applies brakes for no reason and no obstacle or vehicle in front of the car. This may lead to someone hitting me from behind (rear ending) as that driver would not expect my car to slow down. 2. Homelink system closes garage door randomly: the homelink gives command to garage door to close when it is not supposed to. There were an incident when the car was under the garage door and one incident when the car was backing out. 3. Lane departure alert: on newly marked road, the car did not take any action when the lane was crossed without engaging turn signal. The lane departure alert system should have brought the car back in lane. Tesla's response is that the systems are "beta versions". I can understand a system not working (like lane departure alert). However, phantom, braking and homelink issues are out of drivers' control and there is no corrective action a driver can take. For example, a) "the car does not apply brakes if there is obstacle", driver can apply brake and mitigate the risk. B) "the car applies brakes when the road is clear". Driver cannot help as it will be few seconds before the driver realizes that the car applied brakes for no good reason and then he/she will have to cancel the autopilot system and take control. It may be too late. . . There were several incidents of phantom braking. There were 4 incidents related to homelink. Date below is the date of latest phantom braking incident.

50 Service Brakes problem

Failure Date: 10/06/2020

While driving in the left-most lane on mopac expressway in austin (although has happened other times and places as well) with "full self driving" autopilot engaged (and with the car having been in control for quite some time before this), the vehicle slammed on the brakes for no apparent reason. I immedately pressed the accelerator to override the breaking action and avoid being rear ended. To the best of my knowledge no accidents occurred as a result, however it did cause multiple cars behind me to have to brake. Similar issues incidents have happened before, however it seems to be doing it much more often now. I'm unable to upload it, but I also have a video from the vehicle itself showing the incident.

51 Service Brakes problem

Failure Date: 09/21/2020

Driving on highway 280, the auto-pilot stopped working and driving became erratic and unsafe with random accelerations/decelerations. I had no choice but to keep on driving. Tesla kept confusing me with their service arrangements while I needed to use the car every day. It took a week to arrange for service. The car sat for a week at the dealership with no communication from Tesla. Then they sent a laconic request to pay them $1,070 --- while I know my car, being 10 months old is under warranty. Mice ate a wire within the auto-pilot cables. Tesla claims this damage isn't covered by warranty! the damaged cables are in a compartment which is completely unprotected against mice or anything. It should have been protected with at least a metal mesh. I asked why isn't that compartment protected to prevent rats/mice from entering it; after all, 300+ million american garages have cars and mice CO-located. . . Their answer: 'that's the auto-industry standard'. My reaction: "I did not buy my car from the auto-industry standard; I bought it from Tesla (which completely deviates from the "industry standards!)" I am requesting Tesla to issue a 'recall' for installing a metal mesh around the sensitive auto-pilot cabling compartment. Also, I am requesting a full refund of my expenses, plus compensation for the frustration and for the various troubles I had to go through. The no. 1 problem with Tesla's supposed 'service' is that they obviously believe that Tesla cars do not need any service. Here is their email to me from 10/15/"your Tesla service invoice is ready for review. Please review and accept the Tesla service invoice for your visit. Review and accept ". I cannot access that contents anymore. The email simply said accept this charge of $1,070 without explaining anything about the reason for the damage. Thank you, yair barniv.

52 Service Brakes problem

Failure Date: 08/01/2020

My Model 3 Tesla while in cruise control initiated braking while on a highway approaching an overpass. There was no obstruction on the road. I believe the cameras deduced the shadow of the overpass as an object and tried to stop. This incident has been termed ghost braking.

53 Service Brakes problem

Failure Date: 02/19/2020

The vehicle experiences sudden braking while driving freeway spped around 65 to 70 mph in the freeway underneath overpass during sunny day using autopilot. Tesla autopilot consider shadow underneath the overpass as stand still vehicle and apply sudden brakes. This can be very dangerous. I have reported this problem 2 times already to Tesla support, but, nothing has been done.

54 Service Brakes problem

Failure Date: 02/06/2020

I previously submitted a concern with this car that occurred in December 2019. Tesla in milford, CT had the car in service for 23+ days because of errors with the car: alert messages included (but weren't limited to) power reduced, front motor disabled, etc. The car kept powering off while I was driving and the brakes and steering malfunctioned. Tesla ultimately diagnosed the problem as a "shorted penthouse harness. " two weeks after receiving the car back, the error messages reappeared. The car broke down again and had to be towed back to Tesla in milford, CT. While Tesla indicated that the prior issues were resolved, they obviously weren't. The car shut down again while I was driving on the interstate and also in the city. The brakes and steering didn't function properly. I received error messages indicating power reduced, brakes disabled, power steering disabled, front motor disabled, automatic emergency braking disabled, car may be free rolling--just to name a few. This car is unsafe and unreliable. As of today, the car has been in service for 28+ business days; however, Tesla told me they worked on the car over the weekend, which would indicate the car has been in service over 30 days.

55 Service Brakes problem

Failure Date: 11/04/2019

I turned into a perpendicular parking space in a home depot parking lot. I let up on the accelerator as the car inched closer to the curb. As I began to press the brake to bring the car to a complete stop, the car suddenly lurched forward, jumped the curb, crossed the grass median and come to rest on the hood of another car. I know my foot was on the brake at the time and contend that the incident was due to a vehicle malfunction.

56 Service Brakes problem

Failure Date: 07/14/2019

My Tesla Model 3 just suddenly accelerated as I was turning right trying to the lot to park the car. My car hit the wall and then the hazard light came on.

57 Service Brakes problem

Failure Date: 02/14/2019

This is related to the latest recall for Tesla unnecessary braking and acceleration on the highways . This happens to me a few times while I am on autopilot the car would suddenly break and accelerate right after .

58 Service Brakes problem

Failure Date: 01/02/2019

The cause of the "sudden unintended acceleration" in Tesla vehicles is a design flaw in which the gas and brake pedals are too close together and not offset as in other vehicles. While Tesla alleges it is user error, this error is caused by poor design. 1. As you begin to stop or pull into a parking spot (vehicle in motion traveling at a slow speed in a parking lot), it is quite easy to push the wrong pedal, or push both pedals, causing acceleration. This is especially true for drivers with large feet. 2. Occurance is relatively rare, and can cause property damage and minor injuries. 3. User remedies include not wearing shoes while driving.
