1 Automatic Emergency Braking problem

Failure Date: 10/24/2025

The vehicle’s brakes did not work, automatic emergency brakes and collision warning both did not work. I pressed on the brake for about 5 seconds but the brakes did not work, resulting in a collision with the car in front. Everything is available for inspection if needed. Both my safety and the driver of the vehicle in front of me was at risk. The problem has not been confirmed yet. The vehicle is currently at a shop, waiting inspection. There were no warning lamps, messages, or symptoms.

2 Automatic Emergency Braking problem

Failure Date: 08/14/2025

During driving, Tesla car spontaneously stated multiple active safety features and regenerative braking were no longer available and stopped working during driving with multiple warnings appearing. Driver safety at risk during this as well as others safety due to the lack of functioning. The car was taken to Tesla service center and was explained that front lower lateral suspension links were damaged by "impact" although the car was never in an impact or accident preventing proper alignment and functioning of the car. The part has been since repaired and currently undergoing further investigation for warranty dispute.

3 Automatic Emergency Braking problem

Failure Date: 08/07/2025

On August 7th, 2025 at around 8 am in the morning, my vehicles mobile phone car key was in-operable and I was unable to get in the car which led me to trouble shoot how to fix this issue with the car. Upon researching I was directed by Tesla to directly complete a soft reset of the vehicle holding down both buttons on the vehicle once I was inside, and pushing on the brake pedal. Upon completion, my vehicle booted up and I realized that all camera systems were down, my navigation systems were down, my safety features were down, my auto pilot, lane keeping assist and auto brake were down, sensors were down and I was no longer able to pre heat my battery to charge my vehicle due to issues with the navigation. I then took the vehicle to Tesla service center irvine where a service advisor quoted me 2900 for a new ecu and bolts and told me that I needed to replace my whole unit and that I was to cover the cost of the repair even though my vehicle is only 2 years of age and this failure is due to the quality control and software of the vehicle. I have been driving around a vehicle with absolutely no safety features whatsoever such as no cameras, sensors, auto braking, or full self driving due to this ecu failure and I fear for my safety and well being while driving this car.

4 Automatic Emergency Braking problem

Failure Date: 02/18/2025

Trunk struts snap/come off. Causing trunk to slam shut. Damaging body. Tesla states vehicle is safe to operate, however vehicle trunk does not latch down anymore and opens every time I hit a bump. The trunk allignment is also well off. The vibration of the trunk causes several electrical issues including automatic emergency braking. Original problem occured September 15th 2023. Issue was fixed and persists. Current issue has been going on since February 18th 2025. Issue is not yet resolved or fixed. Tesla is not taking any action to repair issue at all. Vehicle is unsafe to operate.

5 Automatic Emergency Braking problem

Failure Date: 11/09/2024

If a Tesla software update fails it causes an apb version mismatch causing all safety features to become disabled until you schedule service for repairs. It is not covered outside of warranty even though it’s an issue caused by their update. If you google apb version mismatch, a lot of people have experienced it. Out of warranty repair is $2-3k for a computer replacement even though it was working fine before the update.

6 Automatic Emergency Braking problem

Failure Date: 09/19/2024

The system failed to detect the driver having a seizure and had no way for the passenger to break the vehicle. People in the vehicle and others were put at the risk of a seizing person accelerating in a car that could not be prevented by the car or by the passenger. No dealer/service center has confirmed the issues. The vehicle is currently being inspected by police. No known warning signs/messages before the incident.

7 Automatic Emergency Braking problem

Failure Date: 08/21/2024

The contact owns a 2023 Tesla Model 3. The contact stated that while entering a parking lot, the brake pedal failed to respond, the driver was unable to control the steering wheel and crashed into a Ford vehicle and a tree. The air bags deployed, and the vehicle stopped shortly after. The forward collision emergency braking system failed to engage. There were no warning lights illuminated. The vehicle was towed to a tow lot. A police report was filed. There was no reported fire. The contact sustained injuries to the neck and shoulder and had bruises. Medical attention was provided. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was contacted, but no assistance was provided. The failure mileage was approximately 12,166.

8 Automatic Emergency Braking problem

Failure Date: 08/18/2024

While using standard cruise control (without autosteer), car sporadically & aggressively brakes for no reason with no obstruction or cars ahead and while at highway speeds. With cars close behind, a serious, life-threatening accident could've occurred. Can not determine how to recreate issue on demand. This is a known issue with Teslas that has not been addressed by Tesla. I have not reported this issue to Tesla.

9 Automatic Emergency Braking problem

Failure Date: 07/22/2024

On [xxx] around [xxx], my daughter drove her Tesla Model 3 on [xxx] from school. Two strange/scary accidents happened to two different Tesla cars (model 3 and y). It was raining with low/medium visibility. One of the cars was driven by my daughter, and luckily, both drivers were safe with very minor injuries. The cars had significant damage but were drivable, and there were no airbags out. These two accidents occurred within 3 to 5 minutes at the “same location. ” when I asked them what happened, my daughter and other model y drivers described a sudden loss of control, with their cars spinning and becoming unresponsive to steering and braking. They then hit the guardrail on the center line. I checked the video recordings of our car multiple times; no vehicle-to-vehicle collision was involved. I also checked the road to see if there was any oil spillage, but the lane was clean. She didn't use auto pilot, fsd, and cruse control. The office recorded it as hydro plane in his report, but my concern was incidents that happened only to Teslas. Also, when I posted this incident on a Tesla whatsapp group, multiple Tesla owners reported that they had a similar issue when it was raining. It seems like sensors malfunctioning in the rain. Also, my daughter was driving below the posted speed (68/70), and she gave a written note to the officer too, but the officer recorded it as 70/70. I need to check that in the wake county traffic court. Information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).

10 Automatic Emergency Braking problem

Failure Date: 05/02/2024

I was driving behind someone when a SUV came over from left lane into the right lane in front of the cars that were in front of me and made a sudden turn into a business. I was already applying the brake and was the 3 second rule behind the car in front of me, however the car did not stop as I slammed on the brakes as hard as I could. There have been other times that the car will automatically slammed brakes on because it feels that it need to…when it’s not necessary at all. But the other day when I needed the brakes to help me stop abruptly…they failed to do so. I hit the car in front of me and was cited as well. If I had been going faster. . This could have been way worse. This definitely needs to be evaluated on Teslas. I have a dahcam video that I’m unable to upload.

11 Automatic Emergency Braking problem

Failure Date: 03/07/2024

Driving normally down I-5, no other cars nearby and system suddenly phantom brakes. I am lucky no other cars were behind me as it may have well caused an accident. There were no other cars in front of me within at least 1 mile.

12 Automatic Emergency Braking problem

Failure Date: 03/01/2024

Software updates caused failure notifications (seat restraint system) and disabled cameras/emergency brakes, happened immediately right after software updates. All hardware performed and properly functioned before the software updates. 1. First failure noticed on the begin of March 2024, component affected : " rear center safety restraint system fault". Second failure noticed on 25 November 2024, component affected: " autopilot cameras unavailable", "automatic emergency braking is unavailable" 2. I feel extremely unsafe now when using the vehicle with failure notification of the car's components, especially while driving my children to school. I am unsure if the system failure that could causes accidents and put my kids at risk or danger. 3. First issue was notified Tesla representative immediately right after the software update (happened over night). They scheduled an appointment, but I saw a charge bill that I have to approve before taking the car in for an inspection. It happened when my car is just out of warranty. I called Tesla dealership in buena park, CA to discuss about my safety concerns and got told it might be just a software glitch and I should be ok to drive. (??). Second issue was also notified Tesla and got a schedule for an inspection/service on December 10th, 2024, and again with an approval of $650 that I might got charged for ahead of the service ???? (why do I get charged for something that wasn't my false until Tesla pushed a bad software to make us pay for their bad scripts?) 4. There was no failure notification "at all" before the last 2 or 3 software updates. Tesla offered some kind of 1 month free fsd then software updates to remove the fsd then everything started to fall apart. I am very concerned with Tesla software updates now and failures it might cause that put our family at risk because of their bad scripts or batches.

13 Automatic Emergency Braking problem

Failure Date: 01/01/2024

On Jan. 1, 2024 at around 2:19-2:20pm, the vehicle in front of me made a sudden brake causing my vehicle to trigger the automatic emergency brake. My foot was on the brake pedal as well, I even floored the pedal to make my car stop right away but it continued anyway, the pedal felt so loose/soft. My car almost made a full stop but when the vehicle in front of me started moving, the car started to somehow accelerate, I was confused as to why the car was moving when my foot was firmly pushing the brake. This unexpected move caused me to rear-end the vehicle in front when they abruptly stopped for the second time. My thought is since the car in front moved, the Tesla thought the vehicle in front was no longer a threat and stopped applying the brake to make a full stop. I watched a lot of aeb videos online. 1. Automatic emergency brake - malfunctioned, maybe, the car log has it but I took a photo that aeb was in progress during the incident 2. My experience is similar to a car without a brake - no matter how hard I pushed the brake pedal, the car didn't stop. It's frightening because I'm out of control over the situation. 3/4. My car was already repaired but I haven't taken it to a Tesla service center to inspect the system - the manual brake is working though 5. There were chimes and screen notifications that an automatic emergency brake was in progress. I can upload the dashcam recording if you need it.

14 Automatic Emergency Braking problem

Failure Date: 12/11/2023

Autosteer was engaged while approaching weatherford, ok on i40 - sporadically, the vehicle slammed on the brakes, even though nothing was in front of me, and nothing was behind. The vehicle dropped in speed by almost 20 mph.

15 Automatic Emergency Braking problem

Failure Date: 11/09/2023

Vehicle auto stopped in the middle of the parkway.

16 Automatic Emergency Braking problem

Failure Date: 11/05/2023

Driving north/east on highway 87 at 1215pm, kind of into the sun (sun was to the right of us), the car would not maintain cruise control without constantly applying the brakes forcefully. No cars in the immediate vicinity during the event, though there were other cars on the highway at the time. Returning south on the same road, about an hour later, no issues whatsoever.

17 Automatic Emergency Braking problem

Failure Date: 11/03/2023

As part of transitioning from radar to a camera based system (this affects model years '22 onward), Tesla, has "temporarily" disabled key features such as lane departure, blind spot alerts and emergency braking. Though disabled, these features are still selectable (not greyed out) when the user chooses safety options. These features, which are available on most modern cars, have been disabled for many months with no committed date to restore functionality . . . This poses a major safety risk. This should impact Tesla's NHTSA safety rating which will pressure the company to re-enable these basic safety features ASAP. Thank you.

18 Automatic Emergency Braking problem

Failure Date: 09/28/2023

While driving with cruse controll, auto pilot, or adaptive cruise control my vehicle brakes suddenly without warning and without any reason or danger. This has happened many times. Today on the i8 traveling from yuma az to surpirse az the car braked around 15-20 times with out reason. I held the Tesla car symbol on the screen to report the problem.

19 Automatic Emergency Braking problem

Failure Date: 09/24/2023

Vehicle suddenly brakes while in autopilot with no road obstructions. This has happened to me several times and I'm concerned that if someone is following me too close this may cause a collision.

20 Automatic Emergency Braking problem

Failure Date: 09/11/2023

I was driving on I-35 from kansas city to des moines on 9-11-23, and three times the car experienced phantom braking (I. E. The car suddenly braked for no apparent reason) while the adaptive cruise control was activated. During these instances the car would suddenly brake and slow down and on one occasion I had to accelerate immediately to avoid possibly being rear ended. On all occasions there were no obstacles on the road or cars within 1/4 of a mile ahead of me.

21 Automatic Emergency Braking problem

Failure Date: 09/10/2023

The car does phantom braking on the highway when using cruise control. It is usually caused by a truck in the neighboring lane applying it's brakes, but not entering my lane.

22 Automatic Emergency Braking problem

Failure Date: 08/13/2023

During my first highway trip from dallas to my home in alpine, TX, I experienced numerous instances where the car would suddenly decelerate as if I had applied the brakes. At first I thought it was because there was a lot of road glare because I was driving late in the day into the west sun, but the next day it happened early in the morning when the sun was behind me and the road was straight, relatively clean with clear markings. The ultimate instance was when I was driving on a newly reopened dark asphalt road with new markings, straight, no glare, sun behind me. Clearly there is a problem.

23 Automatic Emergency Braking problem

Failure Date: 08/07/2023

While traveling on a busy interstate highway, my Tesla 3 presented four errors on the control screen: automatic emergency braking disabled, power reduced, vehicle shutting down and unable to drive. The left hand side of the screen flashed red and instructed me to pull over safely. The car was immediately slowing down, and as I was in heavy traffic, I was unable to pull over to the shoulder safely. I turned on my hazard lights and performed a soft reset of the car - which caused my hazard lights to turn off, all while cars were honking and whizzing around me. After the car reset, I was able to drive a short distance, with enough speed, to cross several lanes of traffic and come to a stop on the shoulder emergency lane of an exit ramp. The car is only 6 months old, with 4000 miles, but had to be towed to the Tesla service center. The problem has been diagnosed as a problem with the rear drive unit inverter and will be replaced under warranty. While I appreciate the resolution and that it will be fixed, it could have been a very serious situation and caused an accident. Thankfully, I was able to get to safety and wait to be towed.

24 Automatic Emergency Braking problem

Failure Date: 07/27/2023

My vehicle constantly slows down and occasionally even slams on the brakes to slow down for nonexistent obstacles. I believe this is a serious safety concern if there is a vehicle behind mine, as the driver of that vehicle would not expect mine to rapidly slow down. The root cause seems to be that the camera vision system frequently mistakes certain signs and dark cars reflected in mirages as pedestrians.

25 Automatic Emergency Braking problem

Failure Date: 07/21/2023

Emergency braking applied when no other vehicles were in sight. Braking was hard enough to spill coffee. Seems to occur when adaptive cruise control is on. Happened about 10 times on recent road trip. Lucky no one was behind us! felt very unsafe.

26 Automatic Emergency Braking problem

Failure Date: 07/16/2023

The contact rented a 2023 Tesla Model 3. The contact stated that while driving at 55 mph in the rain, a vehicle in front of him abruptly slowed down. The contact depressed the brake pedal, but the vehicle jerked to the side. The contact stated that the emergency braking system unexpectedly activated ("slams on") while he was driving due to the automatic emergency braking (aeb) system falsely detecting a potential collision. There were large rocks in the roadway, and there was no traction, causing him to crash into an embankment. The driver's side airbag did not deploy. The other airbags in the vehicle had deployed. The contact sustained a fractured rib, and medical attention was provided at the hospital. The vehicle was towed to a towing company. There were no reported fires. A police report was filed. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The vehicle was taken to an independent mechanic, where an unknown diagnosis was completed. The vehicle was not repaired. The manufacturer was contacted, but no assistance was provided. The failure mileage was unknown.

27 Automatic Emergency Braking problem

Failure Date: 07/14/2023

Random phantom breaking when using cruise control at high speeds. Goes from 70-40 mph and always worried someone is going to rear end me when my car automatically rapidly slows down for no reason. Huge hazard and risk!.

28 Automatic Emergency Braking problem

Failure Date: 07/09/2023

My Tesla Model 3 has had 3 phantom braking episodes since I bought the car new in June 2023. All 3 episodes were when the cruise control was on. Without any warning, the car will violently slow from 70mph to about 25mph in a very short time. My guess, less than 2 seconds. The tires loose grip on the road surface and the car will come to a stop. In all 3 instances, I needed to apply the accelerator to get the car moving again. During all 3 phantom braking episodes, there were no vehicles behind me that could of rear ended me. If so, the vehicle(s) behind me would have no doubt rear ended me. All 3 instances occurred on clear warm days close to yermo CA on interstate 15. I reported this phantom brake issue with the local Tesla location and they reviewed my cars software and said nothing was wrong. The system is working as intended. Once my Tesla Model 3 phantom breaking started, I have researched the issue online and noted a tremendous amount of complaints and reports to your agency. This phantom braking needs to be fixed and until then disclosed when a customer buys a Model 3.

29 Automatic Emergency Braking problem

Failure Date: 07/09/2023

Phantom braking occurred 3 times over the span of several minutes while driving at highway speeds. The first two incidents occurred while utilizing autosteer. The third time occurred while only using cruise control.

30 Automatic Emergency Braking problem

Failure Date: 07/06/2023

On July 6th, at 6:42pm, while driving 70 mph on California highway 152, on a clear day (88 degrees f), with no traffic around our car, the car slammed on the brakes, taking the speed from 70 mph to 46 mph in a period of 5 seconds (with most braking in the first couple seconds). I was using the cars' built-in autopilot system. The car did the same thing about 2 minutes earlier, so we happened to have been recording a video of the cars operation when it happened this second time. The car did the same exact behavior on June 29th at 10:30am, on the same highway but different location. It was also a sunny day, and there were (thank goodness) no cars near us when it happened. That time, it braked so hard it triggered the abs system.

31 Automatic Emergency Braking problem

Failure Date: 07/05/2023

Tesla has serious safety issues automatic emergency, braking blindspot warning, lane, departure warning, and other basic safety features which are very common in the cars these days. I purchased Tesla for my teenage child. In past three months we have encountered multiple instances where Tesla was not able to perform as promised this morning I was driving Tesla loaner vehicle when I was heading to pick up my car and someone drifted into my lane without warning and Tesla did not apply emergency brakes or generated any warning signals. I have notified Tesla service about the incident and past incidents to Tesla service representative. Tesla representative at costa mesa office opened the service request to get the software diagnosis completed so that they can get to the bottom of it, but drew tucker from irvine office called me and he is in denial that it is a car issue and blame it on me that it is between the car and the car owners manual, I have requested an escalation from Tesla service center. In the meantime, I am filing this complaint so that NHTSA can start their investigation before. It is too late and some life is lost because of irresponsible behavior by Tesla and send in selling the features which do not work I have Lexus RX 350, 2017 model and all these features work seamlessly in the car , I am shocked Tesla is falsifying and and selling inaccurate features to the consumers which cannot be discovered until it is too late. Please start investigation investigation ASAP. Thank you.

32 Automatic Emergency Braking problem

Failure Date: 07/05/2023

I was driving on a divided highway in the right hand lane out of two lanes. To the right was a grassy berm. To the left of the road was a grassy median. I was driving with the adaptive cruise control enabled and set to about 70mph. Full self driving was not enabled. There were no vehicles or other obstacles visible ahead or to the sides, no vehicles following. Without any reason I could perceive, the vehicle abruptly decelerated rapidly. I pressed my foot on the accelerator pedal and the car stopped decelerating and resumed normal operation at roughly 40mph. If another vehicle had been following me, the unexpected rapid deceleration could have resulted in a collision. This vehicle is a rental car owned by hertz, and I have not had the vehicle inspected or diagnosed by any dealer, service center, the manufacturer, police, or insurance representatives. The alert log in the vehicle computer showed at 6:16pm, code epbl_a180: emergency brake applied (see attachment).

33 Automatic Emergency Braking problem

Failure Date: 07/02/2023

Multiple times the car while on cruise control has slammed on brakes due to seemingly nothing. It's without warning our alert and it is extremely violent. It happens at least once every week.

34 Automatic Emergency Braking problem

Failure Date: 06/14/2023

On several occasions ( 8 or 10 times) while driving at highway speeds, with little traffic, on an extended trip, the vehicle suddenly applied the brakes for no apparent reason. I have learned that this is called "phantom braking". On one such occasion, I narrowly avoided being struck from behind by another vehicle following me by applying the accelerator. I have not yet reported this problem to Tesla, but intend to.

35 Automatic Emergency Braking problem

Failure Date: 06/04/2023

While driving on 15 freeway between riverside CA to las vegas nv, I put the car in cruise control. While near barstow, the car hard brakes dropping speed quickly. Luckily no other car was near. I changed lanes to pull on the side as I wasn’t sure what was happened, as it actually felt like the car had locked up. I did take the car out of cruise control and that seemed to fix the problem. I’ve used cruise control before and the adaptive braking has been manageable, this was extreme. On 4 June 2023.
