As a result, the vehicle abruptly stopped and caused the contacts wife’s seatbelt to retract back tightly. The contact stated that he had to disengage the features including the adaptive cruise control to operate the vehicle. The vehicle was taken to the dealer where it remained. No warning lights were illuminated. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 50,000.

2 Forward Collision Avoidance problem of the 2019 Tesla Model 3

Failure Date: 02/02/2023

The contact owns a 2019 Tesla Model 3. The contact stated that on several occasions while driving the cruise control or auto pilot activated, the vehicle experienced phantom braking with a beeping sound coming from the vehicle with a manual control warning message appearing on the infotainment screen. Failure Date: 07/20/2022

The contact owns a 2022 Tesla Model 3. The contact stated while her husband was driving 50 mph on cruise control, the vehicle began to decelerate independently. The contact stated no warning light was illuminated. The contact stated they continue to experience the failure five to ten times on the first trip. The contact stated she also experienced the failure on 9/23/2022, while driving 80 mph on cruise control, the vehicle began to decelerate independently to 70 mph and deactivated the cruise control independently. The contact stated the force of the deceleration caused the seat belt to retract and caused her to have whiplash and headache left side of her head. The contact did not sought medical attention. 7 Forward Collision Avoidance problem of the 2021 Tesla Model 3

Failure Date: 06/03/2022

The contact owns a 2021 Tesla Model 3. The contact stated while using a self-driving mode at 50 mph, it would not stay in the lane and drive slowly into the next lane. The contact stated on April 9, 2023, while using a self-driving mode at 15 mph, a pedestrian walked into the lane and the vehicle did not detect the person and hit the pedestrian. No air bags deployed. The contact did not sustain any injuries. The pedestrian sustained an unknown fractured leg and a fractured arm and sought medical attention for his injuries. A police report was filed. The vehicle was not drivable. The vehicle was towed to a Tesla collision shop and had the front windshield replaced. 8 Forward Collision Avoidance problem of the 2021 Tesla Model 3

Failure Date: 04/30/2022

The contact owns a 2021 Tesla. The contact stated while driving approximately 8 mph the vehicle inadvertently accelerated and jumped the curb. The contact pressed the brakes pedal to the floor; however, the vehicle continued to accelerate and hit the wall. There was no warning light illuminated. The seat belt was on and the driver's air bag deployed, however, the contact head hit the windshield, and it shattered. The air bag was not covering the contact face completely. The police arrived on the scene and filed a report. There were injuries to the contact's left wrist, neck, hip, and back, and a fracture to the right foot. The contact started receiving medical treatment the same day. The vehicle was towed to the contact's residence. The damage was to the front of the vehicle. The contact stated her personal phone was connected to Tesla, and an inoperative alert message illuminated. The manufacturer notified the contact at the time of the accident. Failure Date: 04/19/2022

The contact owns a 2018 Tesla Model 3. The contact received recall notification of NHTSA campaign number: 22v702000 (visibility), however, the parts to do the recall repair were not yet available. The contact stated that the front driver's side and passenger's side windows failed to operate as needed intermittently. There was no warning lights illuminated. 12 Forward Collision Avoidance problem of the 2022 Tesla Model 3

Failure Date: 11/22/2021

The contact owns a 2022 Tesla Model 3. The contact stated that while driving at various speeds with the cruise control activated, the vehicle would independently brake with the forward collision avoidance warning chime being audible. The contact stated that the failure would occur when passing large vehicles or as another vehicle in the opposite lane would drive past the vehicle. The contact called a service center where an appointment was made; however, the contact received an email that informed him that the appointment was cancelled.
