My Tesla Model 3 was brought to Tesla princeton service center on February 27, 2025 for a 12v battery replacement. During the service visit, Tesla also replaced the rear glass, citing the rear defroster as the root cause of the battery drain — a repair that was added without prior discussion and increased the total invoice from an original estimate of $180 to a final charge of $1,730. Upon picking up the vehicle, the rear backup camera, blind spot cameras, navigation system, sentry mode, and windshield wiper sensor were all non-functional. None of these systems showed any malfunction prior to the service visit. Tesla princeton’s service advisor denied responsibility without performing diagnostics, and in a subsequent message admitted he could not determine the cause with certainty without diagnostics — which have still not been performed. The vehicle was discharged from a licensed Tesla service center in a condition non-compliant with fmvss 111. I have escalated this matter to Tesla executive leadership in writing and am filing this NHTSA complaint to create a federal record of this safety violation.

My 2022 Tesla Model 3 experienced a heat pump compressor failure at approximately 89,457 miles. Tesla service center diagnosed an internal failure in the heat pump system. The heat pump system controls cabin heating, windshield defrosting, and thermal management for the vehicle. When the system failed, the vehicle lost heating and proper defrost capability. This can impair windshield visibility during cold or foggy conditions, creating a potential safety hazard while driving. Tesla quoted approximately $3,000 to repair the compressor and declined warranty assistance despite the system being critical to safe vehicle operation.

While using Tesla's full self-driving (supervised) software on the pacific coast highway (pch) and other highways, the vehicle’s automated steering system repeatedly malfunctions by failing to maintain lane centering. Specifically, the car drifts or actively steers across double yellow lines into the lane of oncoming traffic. This issue is a persistent failure of the advanced driver assistance system (adas) and lane keeping assistance. The vehicle has attempted this maneuver multiple times at [approximate speed, e. G. , 40-60 mph], requiring immediate and forceful manual steering intervention to avoid head-on collisions with oncoming vehicles. The system provides no warning chimes, error messages, or "take over immediately" alerts prior to crossing the lines; it appears to perceive the oncoming lane as a valid path or fails to recognize the double yellow boundary entirely. This has occurred during [mention conditions: e. G. , bright daylight with high glare / night time]. The vehicle is equipped with hardware 3. 0 (hw3). I believe the 1. 2-megapixel camera suite or the processing power of the hw3 computer is insufficient to safely navigate the geometry of the pch or any roads within mountains/hills. I have attempted to "clear calibration" and performed system resets, but the life-threatening behavior persists. The vehicle and its internal data logs are available for inspection. I have also sent "bug reports" via voice command to the manufacturer immediately following these near-miss events.

After downloading Tesla’s update (no. 2025. 44. 25. 4) software my heater (with the window defrost functionality) in winter stopped operating. I made my update while at my nearest Tesla service compound (location verifiable per system location log) on 01/25/26 when that same day software began throwing hvac error codes with blowing cold air, when previously my heat ran well. This is not coincidental; this is system design failure.

5 Wiper problem of the 2022 Tesla Model 3

When cabin heat is enabled, visible vapor/smoke emits from the front trunk area under the windshield cowl and is pulled directly into the cabin through the hvac intake. The vapor has a chemical/sweet odor consistent with coolant or refrigerant. Smoke enters the passenger compartment, especially when stopped or idling. This occurs repeatedly in cold temperatures and stops immediately when hvac heat is turned off. No warning lights appear. This appears to be a heat pump or coolant system leak upstream of the cabin intake, allowing chemical fumes to enter the cabin air stream. Occupants are exposed to fumes while driving. This is a safety concern due to inhalation risk and lack of driver warning.

Hvac system went out at 7500ish miles. It was repaired under warranty, and a "fix" was administered through software by Tesla. My hvac has gone out again at 84k miles for the same reason. About 77k miles after it was just repaired. Quick check online. This is still a very common issue. No heat creates a significant safety hazard due to lack of the ability to defrost the vehicle or frost building up on cold days.

While driving, the center display intermittently goes completely black. When this occurs, I lose access to the speedometer and all driver information. The issue has been ongoing and progressively worsening. The vehicle was purchased new. The issue was reported during the warranty period and initially attributed to software updates. A recent service visit confirmed the vehicle computer is failing, and the issue immediately recurred after pickup. Loss of speed and driver information while driving creates a safety concern.

This is especially hazardous when using standard cruise control without autopilot, where full driver attention is required.

This car uses auto-dimming rear view and side mirrors. However, the rear view mirror is only able to be dimmed automatically, or be disabled in the settings. The issue is that the dimming is not based on a light sensor at all, it is merely timed to dim around 30 minutes after sunset and return to normal 30 minutes before sunrise. I have confirmed this timing on many drives and it is repeatable. The safety concern is that there is no way to manually dim the rear view mirror, which is a big issue around sunrise and sunset when cars headlights turn on before the mirrors dim. This is especially an issue with larger trucks behind a sedan like my Model 3. The only option I have to fight the blinding glare from headlights is to manually move my mirror so that I can no longer see out of the rear window, which makes lane changes much more dangerous.

I am submitting this complaint to report a safety issue with the turn-signal buttons on my steering wheel of the 2024–2025 Tesla Model 3 highland. The turn-signal buttons are located on the steering wheel surface instead of a traditional stalk. When the steering wheel is turned at any angle, the buttons are difficult to locate by feel, often requiring the driver to look down to activate them and take his eyes off the road. Trying to signal during turns, lane changes, or roundabouts is not intuitive, and I have nearly had several collisions. This design presents a potentially dangerous safety hazard and could increase the risk of collisions. Tesla should be required to recall and retrofit these vehicles with turn signal stalks on the steering wheel column.

All safety and driver-assistance systems on my 2022 Tesla Model 3 failed due to an internal short in the vehicle’s computer, as confirmed in writing by Tesla’s technician. All cameras (rear, side, front) are non-functional, navigation does not work, and all adas features (autopilot, lane-keeping, collision warning, blind-spot monitoring, emergency braking, etc. ) are disabled. The vehicle has no visibility when reversing and no active safety protections while driving. Tesla documented the cause as an internal computer failure, not related to damage or misuse. This creates a dangerous condition on public roads since the car loses all safety systems. Tesla quoted ~$2,900 for a new computer even though the failure involves critical safety components. This may indicate a broader safety defect.

This eliminated access to all vital driving information including the speedometer, gear selection, and safety indicators. Safety concern: without visible speed data and wipers the vehicle cannot be operated safely or legally. I attempted to reboot the system and power cycle the vehicle but the screen still failed.

My vehicle was working fine when Tesla issued an over-the-air software update recently. The next day, my backup camera was not working. The back up camera is required for this vehicle under fmvss 111 and is otherwise essential for this vehicle due to limited rear vision through the rear window. In addition, gps/navigation stopped working, the windshield wipers did not function on certain settings and the display was at times not legible, presumably because it believed it was a different time of day and was in the wrong mode. There are likely additional issues that have not revealed themselves yet. I took the vehicle to the local Tesla service center on July 25, 2025. They confirmed the reported issues and said it was a problem with the software update, which they would perform onsite while I waited. The service center personnel also stated this was not a firmware issue or anything to do with my car and that it was just the software update. After a few hours, the personnel reported they could not get the update to install and asked that I bring the vehicle back the following week for further diagnostics. Before I brought the vehicle back, I was sent an invoice for approximately $2,300 for replacement of on vehicle's computer. I brought the vehicle back today, July 29, 2025, and asked them to perform the onsite diagnostics they wanted me to bring the vehicle back in for and did not agree to pay for the computer replacement. I also informed the Tesla personnel that, at a minimum, the the back up camera failure is a safety related issued covered for this vehicle under fmvss 111 and that they were not permitted to charge me for fixing it. The vehicle warranty period recently expired but it is well within the 10-year date of manufacture. Other issues with the vehicle may also qualify as safety related, but the back up camera is clearly one - Tesla recently issued a recall for this very same problem impacting the Model 3, but the recall did not include my model year.

Software update was pushed onto my car, after the update the car autopilot ecu no longer works. When the ecu finally failed these features were stuck or are completely broken/not working: - auto dim rear view mirrors (these were stuck in dim mode, preventing me from seeing in rain or darkened environments such as at night or under shade, 0% visibility under these conditions when changing lanes or reversing) - rear view camera completely stopped functioning, as did all cameras on the car - forward collision warning no longer works - autopilot (adas features in general) no longer functioning including lane keep assist - on screen navigation is locked in place where the car failed (screen shows location where the car ecu presumably failed). I am further unsure if the following features function as required: - airbag deployment - aeb (automatic emergency braking).

I believe this is a stress or pressure crack, which compromises visibility and may reflect a structural flaw in the glass or how it was mounted. I submitted a warranty claim through Tesla’s app and preserved the vehicle in its exact state for inspection (no washing, no driving, no impact). Tesla denied the claim without conducting a physical inspection or providing any supporting evidence. This kind of uninspected warranty denial is unacceptable, especially for a crack that can impair driver visibility. I have found similar reports from other Tesla owners online, indicating a potential pattern. If this issue is systemic, it could pose a broader safety risk related to poor windshield durability or design tolerances. I am asking the NHTSA to investigate this issue and Tesla’s handling of it.

While backing up, the rearview camera feed occasionally displays a noticeable delay, which prevents objects and pedestrians from appearing in real time. This lag creates a significant safety risk, as the visual information on the screen does not accurately reflect the vehicle's current surroundings. Additionally, the display on the left side (vehicle proximity visualization) does not match the camera view or actual situation, further contributing to confusion and potential danger when reversing. This issue persists despite software updates and appears to be a systemic flaw that compromises driver awareness and vehicle safety. Cross-traffic alert appears impacted.

The front and rear windshields of my Tesla Model 3 developed long cracks requiring full replacement. For over six months, I have been unable to complete these replacements due to repeated failures in the replacement process. The vehicle and damaged glass are available for inspection upon request. My insurance claim was filed with esurance, and safelite autoglass, esurance’s designated auto glass provider, has handled all replacement attempts. During this period, three shipments of glass arrived damaged and two shipments were the incorrect glass for my vehicle, resulting in canceled or rescheduled appointments. Safelite has documentation of these failed attempts. Driving with cracked front and rear windshields puts my safety and the safety of others at risk. The damage impairs visibility, especially in rain or low-light conditions, and compromises windshield integrity, which is critical for vehicle structural support and proper airbag deployment. The rear windshield damage also raises concerns about sudden glass failure while driving. The issue has been reproduced and confirmed multiple times by safelite autoglass, an independent service center. While waiting for service, I personally spoke with several other Tesla Model 3 owners at safelite locations who reported similar experiences, including multiple failed appointments, incorrect glass, and in some cases incorrect installations requiring return visits. This suggests a broader, systemic issue rather than an isolated incident. The vehicle has been inspected multiple times by safelite autoglass, and the issue is documented through esurance claim records and follow-up complaints I submitted regarding the prolonged lack of resolution. Tesla has not directly inspected the vehicle. There were no warning lights or system alerts prior to the issue. The problem presented as visible cracks that appeared and progressively worsened, prompting replacement attempts beginning approximately six months ago.

I am reporting a spontaneous windshield crack that developed on my 2020 Tesla Model 3 performance under normal operating conditions. After exiting a routine drive-through car wash in las vegas on a hot day, I noticed a hairline crack in the windshield. It originates from the very bottom edge within the black ceramic frit area, curves upward, and spans across the glass. There are no impact points, chips, or pits of any kind. I brought the vehicle to Tesla service, where the service manager visually confirmed it is a stress crack not caused by external damage. Despite this, they stated that Tesla does not offer goodwill or warranty repairs for windshields, regardless of the nature of the failure. This raises a serious safety concern. A windshield is a structural component of the vehicle and critical for driver visibility. A spontaneous failure like this without external cause compromises safety, especially at highway speeds. There are numerous reports of similar spontaneous cracks in Tesla Model 3 vehicles, often originating from the same area. This suggests a potential systemic defect in the design, installation, or bonding of the windshield. I am requesting that the NHTSA investigate this issue for safety implications and possible structural design flaws.

The interior rearview mirror exhibits tiny bubbles around the edges and streaking or cloudiness within the layered structure of the auto-dimming (electrochromic) mirror. The problem has occurred in multiple potential replacement mirrors, indicating a design or manufacturing defect. The issue first appeared as small bubbles around the outer edge, then progressed to visible streaking and cloudiness within the mirror surface. These symptoms impair rear visibility, particularly at night or in bright light conditions, when glare and distortion are most severe. This creates a safety hazard 1) obstructing the driver’s clear view of approaching vehicles. 2. ) reducing reaction time increasing the risk of rear-end collisions or unsafe lane changes. 3. ) it compromises overall driver situational awareness, especially in high-speed or congested traffic environments. The defect is not isolated to a single part. Defective mirrors have been retained for documentation and are available for inspection. Mirror replaced with same part number under limited warranty, but the replacement option showed same failure, suggesting a broader defect not yet addressed by a permanent fix. Additional replacement parts offered show similar defects.

When driving vehicle over the past month, vehicle turn signals on the steering wheel fail to respond to commands and feel almost as if the controls are locked out by a steering wheel software fault. Upon further inspection, checking service mode, found the sccm service alert to correlate with the exact moment of turn signal unpressability directly. No debris or liquid has ever entered the area, and this issue has gotten so bad that it happens 3-4 times over a 45min drive I cannot activate the turn signals at all when the failure occurs. There is no alternative way to activate them. The steering wheel is the only way to activate the turn signals and the fault does not permit me to use either turn signal when attempting to change lanes on the highway. Tesla reports that it's a normal software bug and to "deal with it" because there's nothing they can do to fix it. This is directly indicative of a known defective part, and this is a safety-critical defect where a recall must be issued to resolve. Numerous units are impacted through research of same failures. I cant use my turn signals when attempting to change lanes, and Tesla does not want to fix/refuses to address this issue. Even through a phone call from paul from the service center in orchard lake, mi, reported that the condition is a "known defect with interal documents" that confirm it, but he cannot share the doc with me, and "that the idea the issue of getting pulled over for not being able to use your turn signal is an imaginative concern. " this, is inaccurate, as the inability to actuate turn signals is a safety-critical failure. The service center replaced my steering when and sccm module, however the issue persisted after the visit, and the service manager of the west bloomfield, mi service center called me 12 hours before my appointment to deter me from dropping the vehicle off, and then told me this would be a customer pay diagnostic for a warrantable safety failure.

Shortly after purchasing my 2022 Tesla Model 3 rwd in February 2025, I began noticing a persistent musty, mold-like odor coming from the hvac vents whenever the air conditioning or defrost was on. The odor worsens when driving with the windows down. I reported this issue to Tesla multiple times between may and October 2025. Tesla replaced the cabin filters but refused to perform a full evaporator or hvac disinfection, stating the odor was “normal. ” despite the filter changes, the smell always returns within a few weeks. Between these visits, I also tried to resolve the issue myself by replacing filters, using odor-eliminating products, and placing moisture absorbers inside the vehicle, but nothing has resolved the problem. This ongoing issue has led to repeated respiratory and eye irritation, including a documented pneumonia case in April 2025. The symptoms clearly worsen after time spent in the car. Tesla has not offered any permanent fix, warranty repair, or replacement. The problem appears to stem from mold or microbial buildup within the hvac evaporator and duct system, which could affect air quality and occupant health. This raises safety concerns regarding possible hvac design flaws or moisture retention issues that may affect other Tesla vehicles. The issue is ongoing as of November 2025 and has been reported to the florida department of agriculture and consumer services (fdacs case #2511-00831) for further review.

The defrost function on the computer went crazy and cracked the windshield in the morning, covered in snow. - it’s hard to see out of, and Tesla said it’s not covered even though it’s only 6 months old and to just drive it. - no, they want $1600 to look at it. I can’t get them to agree to look at it, but don’t give up. - no the defrost was slow and low, as winter is with no issues. So it snowed a lot in kc, mo. I keep my heat at 60 degrees and the fan on low. I unlocked my car with my phone to get some stuff out, but I started clearing off the back door stairs first to make a path. But only 10 minutes after unlocking it, I heard a loud pop. I thought it was electric, so I unplugged it and got in to leave and immediately was shocked to see and hear the heater on so high I didn’t know it went to that level. It had to have been 100 degrees in the car then before I even sat down… I saw the huge crack across the windshield from the bottom right passenger side, perfectly in the corner of the glass, going all the way to eye level in front of the driver. Diagonally, corner to corner. There isn’t a scratch or speck on this car. I hand wash it every week when it’s not freezing out. I keep the left scroll wheel programmed to control the fan speed, so I pushed it to lower the leaf blower setting it was on to find out it was still set to 60 and fan speed 2. I had to go up and down a few times to get it to go down to normal. I took a video and photos, and Tesla said repeatedly it’s going to cost $1600, even though I just bought it 6 months ago.

Dear NHTSA, I am filing a formal complaint regarding Tesla's refusal to properly address a critical safety recall on my vehicle (recall number 24v554). This recall involves a significant safety issue that Tesla is legally obligated to remedy free of charge under the national traffic and motor vehicle safety act. The required over-the-air software update associated with this recall repeatedly fails to install, getting stuck at 50% completion. I scheduled a service appointment and took the vehicle to Tesla for assistance. However, Tesla informed me that they will not diagnose the issue or investigate why their software update fails to install unless I agree to pay $255. 00 for a general diagnosis. Additionally, they require me to pre-approve a total of $2,658. 28, including the diagnostic fee, in case the computer needs to be replaced. This approach leaves me unable to proceed with a mandatory safety recall and directly contradicts Tesla’s legal obligation to repair safety-related defects at no cost to the consumer. I urge the NHTSA to investigate Tesla's practices concerning this recall and their apparent refusal to adhere to their obligations under the law. This situation raises concerns about Tesla’s commitment to addressing safety-critical issues in a manner that ensures public safety and compliance with federal regulations. We own a 2019 Tesla Model 3 which had a left front headlight stop working (headlight, turn signal, and daytime running light). As soon as the headlight went out the windshield wipers stopped working. The vehicle does not have fuses but the headlight and wipers share a wiring circuit. I thought it might be a coincidence but found several of the same issues on a google search. It is beyond ridiculous that lowing a headlight at night would cause your wipers to stop working. Its bad enough to have a reduction in visibility due to a light going out but to have the windshield wipers tied to the headlight causing them to stop working is nothing short of reckless. The warning stating the turn signal needed to be serviced and to make an appointment came on when the light stopped working. I own an automotive repair shop so I ordered the headlight assembly and installed it, the lights then worked and not surprisingly so did the windshield wipers.

This morning the Tesla mobile technician informed me that the defrost is disabled. He explained that is something that only Tesla can fix. When talking to Tesla service they quote me over 1k to fix. After reading about this particular issue, Tesla has been recalling the Model 3 ( 2021 -2023 ) for the exact same issue. Can someone look into this since it appears to be defective from Tesla sincerely.

Since I purchased my car in July 2023, I’ve had multiple false forward collision warning resulting in my insurance premium increasing by sizeable amounts and has caused me undue stress. I’ve reported this problem to Tesla but to no avail. Today 8/9/2024, between 5:17 and 5:21 pm, while driving my cars forward collision warning was activated while no vehicle was in front of my but instead to a parked car on the pavement. This has happened multiple times and Tesla denies being aware of any issues.

My back glass on my Tesla Model 3 started to crack very bad there is no sign of any impact to the glass Tesla even stated it’s a stress crack and stated many Teslas have this problem and continued to tell me even though it is a defect and a safety concern that the back glass can shatter and injury passengers in vehicle I would be responsible for paying to replace a defective back glass.

1) driving at highway speed on a highway, the windshield wipers stopped and would not restart in the middle of a pouring rainstorm. We could barely see, but we pulled to the roadside and stopped the car. We put the car in park. Once we put the car in drive again, the wipers would work again. We took the car to the Tesla service center and they replaced the wiper module. 2) when stopped at a stoplight the car would not drive forward when the light turned green. The car would not go again until I put it in park and then back into drive. When exiting a parking lot and pulling out into traffic, making a right turn, the car just stopped and would not go. The car would not go forward in drive and would not go backward in reverse, it just sat there. I put the car in park. Then I put the car in drive again and it went forward as usual. I took the car to the Tesla service center and here is their report: "repair notes: technician verified customer concern through review of vehicle alerts at customer timestamp and tow. Technician performed a very exhaustive circuit integrity check on the vehicle communication network and found no faults. Technician then investigated closer at the customer timestamp and found the vehicle car computer restarted itself due to an over memory condition upon wake, resulting in the experienced symptom. This is a know firmware characteristic that Tesla engineering is aware of and actively working on a permanent fix to be released in future firmware. Technician also removed and replaced the faulty wiper motor.
