The contact owns a 2015 Tesla Model S. The contact stated that while he, his wife, and his son were in the vehicle he had begun veering right pulling into the driveway, at approximately 5 mph, he had begun slightly releasing the brakes when the vehicle suddenly accelerated at accelerated high speeds. The contact immediately depressed the brake pedal however the vehicle speed continued to accelerate. As a response, the contact attempted to maneuver the vehicle to avoid crashing into the garage. The contact crashed into a nearby tree, jumped the curb, and became airborne crashing into the side of the home. The driver's side of the vehicle crashed into the side of the home where the laundry room was located ricocheting hitting the kitchen and bedroom windows and eventually coming to a stop. Due to the home being on a slope, the vehicle ended up falling into a nearby embankment after coming to a stop. All air bags deployed. The vehicle was towed. The contact was informed by insurance the vehicle would more than likely be totaled however this had not been confirmed. A police report was filed. The contact received emotional distress, cuts, and bruising to their hands. Arms and forehead. The contact also suffered from back pain. The contact's wife sat in the rear on the driver's side and received emotional distress, trouble breathing, bruising on their face, and bleeding in the mouth. Their wife also received neck, back, and head pain. The contact stated that their wife ended up falling to the floor of the vehicle as a result of the crash. The contact's son sat in the rear on the passenger's side and received emotional distress, and back and neck pain. All persons received medical attention on-site, such as cleaning the wounds and icing any bruising. They were advised to seek additional care, however, have not been able to do so as of yet. April 21, 2023 me and my daughter was driving from jacksonville beach to fernandina beach around 6pm. Exiting from 295 to i95, I removed my autopilot to reduce speed, I was very focus driving, not on my cellphone and even sleepy. Then I saw the car in front of me suddenly stopped, I know I have enough distance and confidently press the break hard enough to make it stop. But the Tesla, while started beeping loud as it sensored the sudden stop of the car , seems to just slowed down and not stopping even with the full hard press of the break, the Tesla just keep on moving slow until it just hit the car and we just heard a loud bang from the airbags that blew up, gas fumes all over inside the car, shocked with what happened, I immediately opened the car door due to the gas fumes. It was traumatizing. I have been driving for almost 20 years, a lot of experience from high way to city driving, years of experience with crazy manila traffic and highways, using different kind of cars, SUV, family van, even u-haul, I am very familiar with how breaks work and how breaks could save life and save from accidents even with a split seconds, but the experience with Tesla’s break system is different. With this crash with Tesla, I remember this was not the first time the break didn’t work as expected. Had the same instance happened before with Tesla, though we didn’t hit any car but almost – me and my daughter together with her classmate was driving to school. Suddenly the car in front stopped due to the traffic light turned red. Again the car started beeping loud, I know we have enough distance and confidently press the break, but to my surprise, and the kids started getting scared when the Tesla seemingly not stopping and seemingly gonna hit the car as it continuous to drive and the break not helping to stop quickly even when the break was fully pressed. I believe this is a serious issue. My car now assessed total loss and just paid for market value, lost $20k in a year span.
My 2015 Tesla Model S was driving with autopilot on 3-17-2023 at 5:45 pm pst near the intersection of lafayette st and memorex dr. In san jose California when I noticed it panicked and slammed on the brakes while traffic was flowing at about 40 mph. The day was clear no rain, fog, or obstructions. There was a Jeep approaching faster from my rear left in the next lane but it wasn't in front of me. No other cars were in front of me either. There was traffic directly behind me and the car suddenly braked nearly causing an accident from the rear. The car showed an icon of a person in red (an indication of what it was braking for). However, there was no person in the area. I immediately hit the acceleration pedal to avoid the accident from the rear and to keep the car moving. The car never showed a "person" indicator before in the 4 years I have owned it. I know the newer models show it but not this year. I immediately requested service through Tesla as the car has phantom braked for ghost cars in the past but never a ghost person and it almost caused a serious accident. I was told by Tesla that there is nothing they can do and that I needed to press the "push to talk" button in the car and say "report a bug". I told them in a moment like that I am more focused on preventing an accident and that is not a good solution. I did provide the time, day and conditions to them.
When driving with autopilot engaged, the vehicle sometimes gives alerts even when there is no vehicle or obstacle in its path. Immediately once the driver takes control, the autopilot does not disengage but continue as usual. The autopilot suddenly brakes or slows down even when there is no obstacle on a freeway.
Wheel speed sensor kept getting disconnected. When it does, systems like abs, traction control, and other anti-collisions systems are disabled immediately. This resulted in lost of vehicle control on wet pavement. The service center replaced the sensor twice but the problem continues to pop up randomly. They won’t look at the issue unless I pay for troubleshooting at $150/hr with no limit of how long or if they will find something.
On two or three occasions now my Model S will slow down for no apparent reason while autopilot is engaged. It's happened at speeds between 35 - 45 mph and highway speeds. Problem seems to have started after ota updates. Car never had this problem before prior to the most recent ota updates.
Brake and accelerator pedals are configured such that both are easily pressed together. Other unintended acceleration or loss of brakes complaints here may be the same issue. I was cornering and needed to slow. Pressing the brake pedal did not slow the vehicle. An error that both pedals are pressed was seen in the first occurrence. This time I only heard the chime. I couldn’t look to see the message. No crash, just a scary incident.
When using auto pilot on the freeway my vehicle sometimes comes to a halting stop for no reason putting myself and passengers in great danger.
After having my battery coolant heater replaced on February 22, 2021 I noticed problems with my car starting up slowly. Note error messages were noted on my vehicle however. On March 10 while driving home from work on a smooth straight freeway the entire electrical system in my car failed causing propulsion to go off line and the brake pedal to lock up and become non-functional until the system rebooted. For approximately 20 to 30 seconds it was impossible to depress the brake pedal. Fortunately I was on the freeway and pressing the brake pedal wasn't required. I am concerned about the safety of this vehicle now since I had mistakenly believed that there was redundancy in the systems that would preserve brake function in the midst of a power failure. There is no manual hand brake. I am extremely concerned that a system failure like this could happen again as I am approaching an intersection, a school zone or going down a steep decline. I have attached invoices from the original service with the battery coolant heater that was replaced as well as the subsequent service after the incident where the technicians identified that the drive unit was improperly connected.
The vehicle had computer issues during oct10-feb1. Loss of connectivity, reboots occur while driving. No speedometer. No turn signal indicators, no parking brake ability, I parked vehicle feb1 for safety reasons. Attempted to get service. Vehicle by April[15] had gone from intermittent to permanent. No visibility of computer screen. No functions. No high headlights, just low headlights, no crash imminent buzzer, no autobraking. Drive&reverse only, steering different, slightly stiffer, no hands free phone & no gps ability. Easy to get lost or confused as to where to turn or whether to turn or go forward. Arm out windows for turning, reduced speed controls. No lane departure buzzer. Changing lane refusal w adjacent car present in lane inoperative. Comp. Not covered by warranty, while under warranty, the repair was declined, stating 'it must be completely dead before it can be replaced under warranty' by apr15 comp. Was completely dead but I had declared warranty inadequate upon first refusal to cover it, July, no parts available, so many weeks of waiting. Dealer unable to find an upgrade part to sell to me. Waiting for a $2500 install of a new part that I am willing to pay for but cannot procure quickly enuf, no matter what. Driving without control of headlights or without turn signals has been curtailed but for emergency food procurement. Doctor visits, mask&glove replacements for the covid situation requires those. I have been lucky I guess that this has happened permanently at home & only a few sudden times temporarily on the road. The failure was intermittent, denied replacement requests & finally failrd entirely, now all must wait while unable to safely drive much of anywhere. Car not sale-able, nor does it cease dropping in price every month while I await a fix. Rental is expensive. Insurance gave me first 30 days, those over now. Susp. Insur. Thank you.
