1 Service Brakes problem

Failure Date: 02/16/2025

While driving on the highway at maximum speed limit, I had a series of error messages that covered the complete screen. At the same time, my mph dropped to zero although I was traveling probably 70 miles per hour. I had no control over the vehicle, no steering, I had no brakes. There was no way to control the car. Fortunately, I was traveling in a low populated area of the 15 freeway between los angeles and las vegas in the far right lane, as I was preparing to exit less than a mile to a super charger station. I was able to get off that exit but since the odometer had a 0 reading, I had no idea what speed I was still traveling at. When I reached the end of the freeway exit, I was unable to stop the vehicle at the stop sign, running it while attempting with all of my strength to veer to the right on to a desolate road. The only thing that saved the car from running the stop sign at what I can only guess would have been 60 mph was a slight incline in the off ramp which slowed the car to about half that when I managed to make the turn. From there, I coasted to a rolling stop about 200 ft from exit, half on shoulder, half stili on road. I was not able to put the car in park nor see what gear the car was in due to the flashing warnings blocking the display screen. I could not use the hazard lights, the gear shift, power off, open glove box, turn off display, open trunk, as if it was in d. Once exited, no way to get back in the car. Tesla er service only able to tow next a. M. To nearest s c, 85 miles at my expense. Sat in car as getting dark, cold along desolate road with no services, until my ride could get me over 4 hrs or stay in car overnight with temps of 20°. After 2 days at Tesla, told steering rack failed, $4,900. Any other time, I would have plowed into the car in front of me at 70mph with no way to stop or steer away. Only prevented by the empty freeway I just happened to be traveling. Tesla claims to have no idea why this part failed at this low mileage.

2 Service Brakes problem

Failure Date: 02/01/2025

The contact owns a 2016 Tesla Model S. The contact stated while attempting to park the vehicle, the brake pedal was depressed and failed to respond. There were no warning lights illuminated. The contact depressed the brake and the vehicle kept rolling forward. The contact opened the door for the vehicle to come to a stop. On another occasion, the contact stated that the adas feature engaged and while driving at various speeds, and the vehicle failed to recognize stop signs, traffic lights, or pedestrians. The contact stated that the function was a paid feature; however, the system failed to operate as designed. On another occasion, the contact stated that the steering will shift to one side while driving at various speeds, and the vehicle became difficult to steer. The vehicle was not taken to the dealer to be diagnosed or repaired. The dealer was made aware, and the contact had a pending appointment scheduled. The manufacturer was not made aware of the failure. The failure mileage was approximately 79,000.

3 Service Brakes problem

Failure Date: 12/15/2024

My car slams on the brakes consistently at green lights when fsd is activated. This is version 12. 5. 4. 2 of fsd. Tesla is not responding to my inquiries.

4 Service Brakes problem

Failure Date: 03/13/2024

Vehicle error: parking brake not applied/vehicle may free roll. This issue appeared to be covered by the attached recall, but the service center indicated that it did not apply to my vehicle. Therefore, they charged me for the repair. I do not understand this as the recalled vehicle criteria seems to match my vehicle information. Are you able to evaluate whether this recall should apply to my vehicle? thank you. Repair notes: installed vehicle set of wheels. Replaced rh park brake rear brake caliper. Replaced lh park brake rear brake caliper.

5 Service Brakes problem

Failure Date: 03/08/2024

Tesla has refused to fix safety recall items. They asked me to replace the cluster own my own so I can receive updates.

6 Service Brakes problem

Failure Date: 02/13/2024

1. The component that failed was the parking brake. This failure occurred immediately after leaving the ballard way Tesla supercharger. Tesla in lynnwood, wa, replaced one of the parts on February 12, 2024. I am unsure if the replaced part is available for inspection, as I did not retain it. The other brake caliper, which has not been replaced, has not shown issues but is available for inspection. 2. The safety risk was significant as the parking brake malfunction rendered my vehicle immovable, causing a traffic obstruction immediately after use and again on the road, potentially endangering myself, other motorists, and pedestrians due to the unexpected immobilization of my vehicle in traffic. 3. Yes, the problem was acknowledged by the Tesla service center in lynnwood, wa, which replaced one of the malfunctioning parking brake components on February 12, 2024, thereby confirming the malfunction. There was also confirmation of recall 17v-260 and that my vehicle was experiencing the same symptoms. 4. The vehicle was inspected and serviced by the manufacturer's service center (Tesla in lynnwood, wa). There were no inspections conducted by police, insurance representatives, or other parties as far as I am aware. 5. Yes, there were intermittent issues with the parking brake being unresponsive when attempting to park the car a few days prior to the incident at ballard way. Although the physical brake had not locked up until the incident occurred, the parking brake fault warning did appear intermittently, indicating a potential issue before the complete failure experienced on the day of the incident.

7 Service Brakes problem

Failure Date: 02/12/2024

Similar issue to NHTSA campaign number: 17v260000. Vehicle was in park. Shifted the vehicle into drive and the passenger side parking break seized and would not release.

8 Service Brakes problem

Failure Date: 01/03/2024

Driving 75mph on cruise control. Car sensed a vehicle in front of me (which there were no vehicles in sight) my Tesla started flashing the red car light and making the alert noise. My vehicle automatically slammed on the brakes dropping me to about 40mph instantly. My greatest fear continuing to drive this vehicle is if I use cruise control, and it senses a vehicle in front of me without their actually being a vehicle and slams on the brakes again myself or someone is going to get hurt. This has the potential to cause death or cause severe injury.

9 Service Brakes problem

Failure Date: 04/27/2023

The car was on enhanced auto pilot and was moving at about 50 mph, the traffic in front of me slowed down. The car failed to give an alert, it failed to slow down and stop. I applied the breaks, but by then the car crashed into the pick-up truck in front of me. The car came to a complete stop after the crash and the computer system in the car completely shut down. The car had to be towed to the garage. My safety and the safety of the driver in front of me were at risk due to malfunction of the enhanced auto pilot system we immediately filed a service ticket with Tesla and upon repeated requests we were told that car could not be inspected and the breaks or auto pilot system could not be tested as the car was not drivable. Police and car insurance company were notified. Tesla was able to provide us with a edr there were no warning signs or messages prior to the malfunction.

10 Service Brakes problem

Failure Date: 10/09/2022

We were entering our driveway to park the car at the garage as always, I drove towards the end of the driveway to reverse into the garage. I turned the wheel towards the right, pressed the break to stop the car to reverse, car lurched forward and went down the small hill (3-4 feet high) to the neighbors yard and when I pressed the break again the car stopped (after about 15 feet from the driveway). Damages : car’s bottom hit the ground and all plastic parts came out. Also, some kind of fluid (likely radiator ) started leaking. Also noticed some scratches/damages to the bumpers.

11 Service Brakes problem

Failure Date: 09/09/2022

1) repeated incidents of Tesla ghosting braking on the highway in autopilot. Sometimes with no cars in sight 3) braking from a high speed to 35 or 40 mph - would have definitely caused a crash if auto close behind me 3) all incidents have been reported to Teslas with time date stamps and they have never gotten back to me and ignored all my warnings that this will cause an accident ( which I now see it has ) I can get a date and time on some if needed. You can also search the Tesla support call log.

12 Service Brakes problem

Failure Date: 06/30/2022

Cpu compatibility to existing os and memory system causing multiple failures during driving with: 1. Instrument cluster reset or reboot (temporarily disables all controls related to buttons/knobs on steering wheel and possibly other safety functions), 2. Main nav/computer screen reset or reboot (temporarily disables all controls), 3. Auto braking activation (cannot be disabled by user) enabled at dangerous and unpredictable times of city traffic, 4. Backup camera picture not updating/frozen during backing uo vehicle, creating illusion a. Not moving when in reverse while driving, b. Not showing objects that may be directly behind vehicle when in reverse manufacturer (Tesla) service ticket opened and closed as “resolved with memory reset” without follow-up if problems still occur.

13 Service Brakes problem

Failure Date: 05/20/2022

Driving on highway in early rush hour at 80 mph on autopilot (6:35am may 20). Car just full braking as if something in road even though there were no objects on roadway. Truck behind me nearly crashed into the back of me before I could hit accelerator.

15 Service Brakes problem

Failure Date: 02/11/2022

A newer Tesla will come to a complete stop with the one pedal operation, however my older car does not. It will roll through a stop sign or red light. This has been fixed on full self driving cars but should be fixed on single pedal operation cars.

16 Service Brakes problem

Failure Date: 01/03/2022

While driving the vehicle car shutdown and went into park for unknown reasons. The steering wheel became almost impossible to turn. I also experienced the regenerative emergency braking and brakes malfunction. Car rolled off into the street nearly hitting another vehicled parked in front of it. The malfunction was initially reported to Tesla on January 5, 2022. Appointment was made for January 26, 2022, but was later rescheduled for a later date due to the service centers backlogged. This pushed my coverage outside the warranty periord rendering my warranty claim useless. Tesla refused to make repairs for free citing, "vehicle is out of warranty period".

17 Service Brakes problem

Failure Date: 08/19/2021

I was stopped at a red light, left signal on, when the car suddenly accelerated forward through the intersection in an aggressive 0-60 kind of start. I braked safely and was about to execute a u-turn a minute or two later when the car bolted forward again. There was no traffic other than the car behind me at the light, thankfully. There were no warnings or other signals and the car was not operating in autopilot. I reported the incident immediately to Tesla and the car is now out of state in their custody while an escalated investigation is conducted. They have, however, told us that the car is safe to drive and that it is not possible for this to have occurred. They also assert that all sudden acceleration cases to date have been related to autopilot and all have been driver error.

18 Service Brakes problem

Failure Date: 03/13/2021

Sudden unintended acceleration: when pressing the brake on uneven paved roads the vehicle will suddenly accelerate while my foot is on the brakes. This has been an ongoing issue since I received the car and I was told this has to do with regenerative braking but it is definitely a safety issue. There are two commonly visited places on the freeway where this happens to me so I can definitely demonstrate the problem if needed.

19 Service Brakes problem

Failure Date: 01/06/2021

Brakes became 95% unresponsive after a car wash and near 0 degree f weather. This caused a low speed rear-end collision. Some ice build up on calipers, rotors were clean. Unsure of exact cause. Vehicle will be inspected ASAP, but I'm betting we find nothing after the ice dissipates. No warning lights. Contacted Tesla and remote diagnostics revealed no issues.

20 Service Brakes problem

Failure Date: 06/26/2020

Tesla mcu (media control unit) has well known and documented emmc chip that will fail on all Model S and x cars from 2012 to summer 2018. This was a result of Tesla engineering writing their logs to the emmc which has limited number of write cycles, Tesla has since resolved where logs are written but the damage has been done. Tesla refuses to accept this mistake and will charge users $1300+ to replace. When this issue occurs the following safety issues happen blinkers do not work. Drive after a 30 second timeout no aeb (advanced emergency braking) no controlling suspension height no manual control of headlight lights no blinker sounds steering mode defaults to some setting (e. G. If I have it set to sport, during reboot of mcu is changes to standard or comfort, not sure but I can feel the steering wheel take less effort to turn than my normal sport mode) no hvac (heat, cool, vent) no defrosters no seat heaters no steering wheel heaters no radio/streaming no app connectivity to control the car no ability to see the car charge status from app trip a/b profiles user settings homelink bluetooth pairings no rear view camera this issues happening to all models world wide reference this post to see the hundreds of users that are affected by this and force to seek alternative methods of repair that is not Tesla approved. At which at any point Tesla could remove features from cars for not using one of their approved service centers. Which appointments are over one month out, forcing most users to drive their car in unsafe manors.

21 Service Brakes problem

Failure Date: 03/03/2020

The main computer panel failed, giving no access to controls, camera, heat and air, auto braking. After the 3rd time of failure the main computer panel stopped working completely. Leaving it unsafe to drive. The vehicle in the first 2 instances failed while driving and a hard reset of the computer fixed the problem. The 3rd time it stopped working upon startup where I had a 100 mile drive home and did have to drive without the use of the main computer screen. I took the vehicle to Tesla and had to have the entire module replaced for over $2000. 00.

22 Service Brakes problem

Failure Date: 08/22/2019

Please see the attached report I filed on safer car. Gove and the letter I wrote to Tesla. I have not got response to any of those. . . This problem happened after I had successfully turned the car 90 degree to park headon. The car had to be at a very low speed at that time. Then when I as just a couple of feet from the curb, the car accelerated out of control. If you get the report from Tesla, you will find that car could not have accelerated this fast even if I had pressed the accelerator pedal by mistake. Many other drivers have experienced the same problem. It is a serious problem and please don't take it lightly. I asked Tesla to give me the report of that time and they did not give. We all feel that Tesla has become so mighty that it doesn't care. We depend upon you for our safety. So please do full enquiry.

23 Service Brakes problem

Failure Date: 08/22/2019

As usual, I drove Tesla to work. My parking spot is a head-in parking spot. To park my car I have to make a 90-degree turn, so I pressed the breaks to slow the car. Just as the car completed the 90-degree turn and I was about to stop the car to complete parking, it accelerated out of control. It jumped the curb in the front and hit bushes about 5-6 feet ahead. The car suffered extensive damage that is estimated to cost about $20,000. I also trust that Tesla is committed to making Tesla 100% safe to drive. Therefore, it is in both parties' interest to find what exactly happened. Fortunately, Tesla keeps track of every movement that every Tesla makes. The record can prove beyond doubt what exactly happened. I, therefore, request a complete record of what happened at the time of the incident. This incidence happened between 9:50 am and 10 am est on August 22, 2019. As I was making a 90-degree turn to park, your record would certainly show that I pressed the breaks during the turn. The only way I could have put the car in the high acceleration - that my wife and I noticed - would be that I lifted my feet from the break and instantly pressed very hard on the accelerated paddle. I can't understand how could that be feasible. If the record does not show this precise pattern, than it points to a potential software flaw. Please provide me and the national highway traffic safety administration (NHTSA) with the complete record of every event that happened with my car on August 22, 2019 between around 9:50 am and 10 am. Some of the most important parameters that are crucial for the analysis of the situation are: what was the lowest speed of the car, at what time that happened, what was the highest acceleration of the car and the time interval between the lowest speed and highest acceleration.

24 Service Brakes problem

Failure Date: 08/20/2019

Tesla is offering this VIN for sale on its website, but is in non-compliance with 573. 15 (b) 2 as there is no hyperlink on the manufacturer's main united states website. In order to access VIN recall lookups, the user must click 4 levels deep (main -> privacy and legal -> recall -> click here to check if your car is affected by a recall).

25 Service Brakes problem

Failure Date: 04/20/2019

There is a pending recall for this VIN. I was delivered this used vehicle by Tesla in Apr 2019. I asked them to complete the recall at that time. I was told the part is backordered. Several reminders and 9 months later it seems Tesla in not interested in addressing this recall.

26 Service Brakes problem

Failure Date: 11/29/2018

Takata recall thank you for taking the time to review this complaint. I am sending it due to two main reasons: 1 - this is the second recall I've received for the same car; 2 - I am having safety issues with my Tesla since November 2018. After several e-mails and visiting a center in person, my issue remains unresolved and I am rarely getting any responses. The last time Tesla got back to me was in February. ----- core problem: I am having recurring issues with the electrical systems of the car, and they remain after taking it to a service center. The automatic emergency brakes for the car did not work when we had an accident, even though the warning went off. I've checked and the option was turned on. The main issue is that I've been paying for a car/feature that does not work as intended, putting my family's safety at risk. More importantly, I am receiving zero support from Tesla. Thanks for the help, and please reach out if you need any additional context.

27 Service Brakes problem

Failure Date: 10/19/2018

Failure of vehicle to engage automatic emergency braking or apply adaptive cruise control braking in repsonse to a vehicle abruptly stopping ahead. Vehicle had adaptive cruise engaged at 57 mph on interstate highway and an acc sensitivity of 2. Vehicle forward collision warning did alert. This failure resulted in a rear-end collision with the car ahead.

28 Service Brakes problem

Failure Date: 10/19/2018

Reference complaint id #11142878 for more detail. This is additional information regarding the complaint id#11142878. I believe that my vehcile failed to stop in a reasonable distance as found in several drive tests of Model S by automotive review sites. Many sites list the braking distance from 60-0mph as about 110 feet. I am certain I was well beyond this distance when I applied my brakes at 57mph, yet the car was unable to stop in time and in fact the antilock brakes kicked in for a large portion of the stop. The driving conditions were dry and temperate at the time of this crash. I have a video available of the incident.

29 Service Brakes problem

Failure Date: 06/12/2018

While in reverse backing up to a charge station in a covered parking lot, the car went into sudden acceleration and failed to stop and collided with the charge station and the wall next to it. None of the warning lights or sounds went off either. Although the car has, on several occasions demonstrated its ability to stop short of colliding with obstacles, it failed to react in this instance. Tesla has ignored our repeated requests for providing an explanation or any evaluation of the logs associated with the car, and has blamed the matter on driver error.

30 Service Brakes problem

Failure Date: 02/20/2018

On2/20/18 I was driving up my driveway. Upon reaching the top of the driveway, the car suddenly accelerated by itself resulting in the front of the car hitting the concrete wall in front. I informed Tesla and they claim that the computer showed that I pressed on the accelerator instead of the break. My answer is that I did not pressed on the accelerator. It just accelerated by itself. Aside from me, I have 4 passengers that can attest to this. I have lived in this place since 1986 and this has never happened. Since I have been driving up my driveway for 32 years, it is so routine for me that it is already ritual for me at almost the same speed and when to pressed on the gas and brake pedals the same way every time. This has not happened to my other cars and as my car insurance will show I have not been involved in car accidents for many years. I have not been shown any evidence except what Tesla told me. It is their words versus mine and my 4 other passengers who experienced what actually happened. Case number [xxx] thank you. ('parts of this document have been redacted to protect personally identifiable information pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6). ').

31 Service Brakes problem

Failure Date: 01/28/2018

My husband was driving my car, we arrived at the parking lot and as he was pulling into a parking space and coming to a stop, the car suddenly accelerated at high speed, went over the cement bumper for the space, over the one for the space in front and hit a parked Saturn in front of us. He is certain that he was braking at the time. We took the car in to Tesla who ran a diagnostic. They advised that the throttle went suddenly from 2% to 97% and then the brake was applied, but when I asked for more details about the approach to the parking space, for a copy of the report they said they would not provide it without a subpoena.
