I’m reaching out to document and summarize an incident that occurred in sanford, florida, and to ask for the appropriate next steps and any documentation you may need from me. Incident summary (sanford, FL – appaloosa court – Dec 2, 2025): in the early morning on Tuesday, December 2, 2025 (around 5:00–5:15 a. M. ), a fire occurred at a residence located on [xxx] . A Tesla located in the garage was reported as being involved, and the fire spread to the garage that totaled the house the vehicle was reportedly charging in the garage when smoke/fire was noticed. Emergency responders (sanford fire department and sanford police) responded to the scene. The incident resulted in property damage and at least one injury reported as non-life-threatening (per local reporting). Fire forensics stated that the fire started and caused by the Tesla around the charging plug area by the rear left passenger door. [xxx] information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).
On December 2, 2025, my Tesla was charging in the garage of my residence in [xxx] , when a fire appears to have originated in or around the vehicle area. Before the fire, I smelled smoke/burning and also heard popping sounds. The fire spread rapidly and resulted in the destruction of my home, creating an immediate and severe risk to the safety of all occupants and anyone nearby. This incident exposed those present to danger from flames, smoke inhalation, toxic fumes, and structural collapse, and it caused emergency evacuation and displacement. The fire department identified the garage or vehicle area as the origin. The exact component or system that failed is unknown at this time, but the circumstances indicate a possible failure involving the vehicle, charging system, battery system, or related electrical components. I am not aware of any prior battery, charging, or electrical warning messages before the incident. Emergency responders and insurance representatives were involved after the fire. I previously attempted to notify Tesla of this incident, but I did not receive a response. Because this event involved a catastrophic fire and total residential loss, I believe it should be reviewed as a serious vehicle safety matter and that all relevant vehicle data, charging data, components, and related evidence should be preserved for inspection. Information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).
Center console usb component part#1522264-00-b, available for inspection. Usb-c charging port that had been plugged in for more than a year, started smoking and wire melted while vehicle was parked. If this would have happened while my wife was on the highway on the way to work and smoke suddenly appeared, it would have cause her to be startled and degrade her ability on a busy highway. This would have most likely have created a traffic incident in memphis on highway 240. I asked the dealer if there were any reports of defective part and I was told no. I brought the components in to show but the parts manager or service staff were not interested in the details or my concerns that smoke had been detected and that it melded the metal piece of usb-c charger to that Tesla part. I bought another replacement part from Tesla, but I am not utilizing the charging port for fear of smoke/fire. There were no warning lights or indication prior to the day of event where my wife observed burnt plastics and visible smoke. On November 26th called me in a panic that the car was smoking. She went out to our vehicle during her break and she smelt burnt plastic and observed smoke from the middle console. Shortly after she called me at my work, and after making sense of what she was telling me, I told her to unplug the usb-c but the parts manager melted of and the metal piece was stuck in the charging port. I left my work and after 30 minutes got to the vehicle, the burnt plastic was still evident but smoke no longer visible. I detached the panel with the part unplugged from power source, thank you youtube for the diy video. Pictures will be included with this report, but format doesn’t allow for video upload. While the car was in operation a message popped up with flashing red light and said drive unit issue car powering down. It went into neutral. Turns out the inverter and rear drive unit had to be replaced.

I am writing to formally raise a complaint regarding a serious malfunction that occurred in my Tesla Model Y (2024) following a recent software update pushed by Tesla. A couple of days ago, my vehicle received a new over-the-air software update. After installation, my car began displaying two error messages, and key safety features, including detection of nearby vehicles and warning systems, ceased functioning. This creates a significant safety risk when driving, as the car no longer alerts me to surrounding traffic or visualizes other vehicles on screen. Upon contacting Tesla customer service, I was instructed to schedule a service appointment, which I did for may 21, 2025, at 2:15 pm. During the appointment, the Tesla service advisor confirmed that the issue began only after the software update. However, I was told I would need to pay $3,000 out-of-pocket to repair the issue because my car is no longer under warranty. I raised the point that the malfunction was caused directly by Tesla’s software update. Despite this, I was informed that Tesla refuses to repair it at their cost, even after acknowledging the connection between the update and the malfunction. Moreover, the technician informed me that this issue could escalate and create safety hazards in 4–6 months, yet no resolution or recall has been offered. Given the acknowledgment that: the problem was introduced by Tesla's software, the malfunction affects critical safety systems, and there is imminent risk to safety, I believe Tesla should bear responsibility for the repair and ensure the vehicle is safe to operate. This situation warrants urgent attention, both from a consumer protection and vehicle safety standpoint. I request that Tesla take full accountability and cover the repair costs immediately, or offer a rollback/fix to the software that caused the issue. Otherwise, I will be escalating this matter to the national highway traffic safety administration (NHTSA) and consumer protection agencie.
The computer suddenly failed on [xxx] due to a short circuit caused by a manufacturing defect in certain 2024-25 Model Y vehicles. This causes nearly all advanced functionality and safety features of the car to fail completely, including all cameras, parking assist, full self-driving, autopilot, autosteer, autopark, cruise control, lane keep, lane departure warning, blind spot monitoring, auto windshield wipers, map, gps navigation and more. Also, the car does not sleep, resulting in excessive battery drain. The problem is widespread. It's this problem: [xxx] as a result the vehicle is significantly less safe to drive. Tesla continually delays service appointments due to unavailability of parts. Information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).
On 12/16/2024 my vehicle received software update 2024. 39. 15, after installing the update I went to the grocery store and noticed nothing was working in the car except basic driving functionality. Cameras were showing up as a black screen. Tacc, aurosteer and fsd supervised were no longer working as well as aeb, fcw, bsm, ldw, lkas and the gps had me in the middle of the ocean, headlights on during broad daylight. I informed a service center by my home and they simply didn’t care, I took it to another service center 50 minutes away and they took two days to do repairs and diagnostics, I was told the computer had shorted out and the technician replaced the car computer and ai4 (hw4) computer system which runs safety features. A software update physically pushed from Tesla should not be shorting out computers on its vehicles, I heavily rely on the cameras and safety features since living in a state where nobody knows how to drive (lol). I hope my vehicle is fixed and this issue doesn’t persist anymore.
I was told by Tesla that a computer system update installed by Tesla burned out the ap4 computer on the car. This affects all cameras, navigation, automatic lights and much more.
Around Dec 16 all cameras in my 2024 Model Y Tesla stopped working and it has not worked since that date. This affects many systems, the most important of which is that I now have no rear backup camera. I believe vehicle manufacturers are required to have functioning back up cameras. I notified Tesla service on 12/16 and was told there was a known computer hardware problem which would cause my incident. Tesla service told me they were unable to obtain the needed computer replacement for a month at least. They informed me there was no safety risk, but I disagree since I don’t have a functioning back up camera, nor lane change camera assistance. Also the display panel is very dark and impossible to read most of the time, posing another safety concern. Why has Tesla not issued a public statement about this problem, and why has NHTSA not mandated a formal recall?.
Vehicle information: • make: Tesla • model: Model Y • year: 2024 description of the problem: after a software update pushed by Tesla, the ap4 computer in my 2024 Tesla Model Y shorted out, causing a complete failure of all systems controlled by this computer. The following safety-critical and driver-assist features have become non-functional: 1. Cruise control (adaptive or otherwise) 2. Forward collision warning 3. Lane departure warning 4. Lane keeping assistance 5. Blind spot warning 6. Parking collision warning 7. Full self-driving and auto park features 8. Cameras required for autopilot functionality additionally, because the gps system has failed, the vehicle's navigation system is inoperable. This impacts Tesla's unique route planning feature that sets up supercharger stops during long journeys. Without a functioning navigation system, I cannot properly plan my travel or locate charging stations, which severely affects the usability of the vehicle. Safety concerns: the failure of these systems, especially safety features like forward collision warning, lane keeping assistance, and blind spot monitoring, puts both the driver and others on the road at risk. Losing access to safety-critical functions after an update is unacceptable and poses a serious danger to vehicle occupants and surrounding traffic. Incident details: • the issue occurred immediately following a software update. • Tesla pushed the update remotely without providing any warning or indication of potential risks. • the failure appears to be related to a hardware short in the ap4 computer, directly caused or exacerbated by the update. Desired outcome: • a thorough investigation into Tesla's software update process and its effect on vehicle hardware and safety systems. • a resolution that prevents similar failures in other vehicles and ensures system updates do not compromise vehicle safety or critical functionality.
Bought my Tesla Model Y on [xxx]. Exactly one month later on [xxx] I start my car to find that none of the cameras are working. This means autopilot, cruise control, rear camera, blind spot monitoring and many more safety features are completely down. In addition the gps does not function meaning my car is not trackable and maps don't work, another safety concern and main reason we bought the car. This is a widespread issue that Tesla service centers have been informed about from Tesla that it is a bad computer that needs to be replaced - in a 1 month old car. My only option is to wait for a computer to be assigned to me which is at earliest on January 21st of 2025, almost 6 weeks without a fully functioning brand new $46,000 car with only 1400 miles on it. Tesla are also unwilling to make any kind of restitution such as a loaner car, or a refund for the full self drive and connectivity packages I just purchased but cannot use as my car is broken. Here is a link to an article showing this - [xxx] information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).

The autopilot computer failed. This has rendered all safety systems and camera on the vehicle inactive, including the rear view camera, all forward and side cameras so no active safety systems are functioning. This includes crash avoidance, lane keeping, blind spot warning, cruise control, and rear camera during reverse, the vehicle is nearly impossible to reverse as the view is terrible with no cameras.
Vehicle cameras stopped working. Navigation stopped working. Screen also stopped working for a time. Was able to get it back on but it went off multiple times since. If I can't see the screen, I can't tell how fast I'm going to do anything else. Cameras control many of not all of the safety systems. So those are all inoperable. Issue is being reported across multiple forums all over the internet as "short in ap4 module. ".
Ap4 computer failure has led to navigation, cameras, blind spot monitoring, lane departure and other systems being inoperable. Tesla has stated they don’t have the parts to fix and have pushed out my service repair time twice already. Issue reported on December 4th, since then no time table for repair has been given. Tesla keeps stating the car is operable, leaving out the facts it’s dangerous to drive due to small rear windows and blind spots based on the cars design to work with operable cameras that give the drive clearer angles to view.
The computer is not working which connects to the forward collision warning system, navigation, and all safety when it comes to driving. I am still waiting to hear back from Tesla about the problem and haven't had a resolution.
Tesla has told me the ap4 computer has failed in my car which has resulted in them having to replace it. I had this issue start right around thanksgiving. I scheduled the soonest appointment available for 12/16 however Tesla contacted me and moved my appointment because they stated that the part was unavailable at this time and should be delivered by 01/20/25. Not offering me a rental or anything on my brand new '24 Tesla. It has just over 1,000 miles on it and is ~3 months old now. This issue impacts: emergency braking, blind spot detection, lane keep/centering, assisting with parking, automatic high beams and wipers, cruise control/ adaptive cruise control, vehicles built in gps to be stuck at one location every time, has made my touchscreen not automatically brighten or dim, my auto dimming mirrors be to dim in most driving conditions, constantly fail to install system improvements and safety updates, and because my vehicle keeps trying to do this, it has caused increased battery drain as well. This has increased the safety risk of my vehicle tremendously. . . Tesla has not seemed to publicly acknowledge the issue yet either.
Components failing - reported as central computer. This made the vehicle turn signals and backup cameras fail as critical components. These two elements were/are completely non-functional. Additional less critical safety functions that have been disabled include blind spot monitoring and cameras, both forward and reverse emergency braking, and pedestrian alerts. My safety was put at-risk due to camera failures while driving and backing out of parking spaces. These vehicles are designed with poor and minimal visibility due to their cameras compensating for visibility shortcomings. You cannot adequately see proximity to other vehicles or pedestrians when cameras are not active. This particular impacts reversing when backing in and out of parking spots since the rear windshield is so small and poorly angled. Drivers have reported that the cameras for them are failing mid-drive leading them to not know where other vehicles were next to them and making unsafe lane changes thinking their warning systems were active. The vehicle component has not been inspected by anyone as there was thankfully no actually accident or incident due to the failure. Tesla has not inspected the vehicle due to no availability for 3+ weeks and they will not consider human contact to expedite review. There are now 100s of community reports of the same defect on online communities all showing failures on same-day or within 24 hours of each other. There were no warnings prior to the failure. The symptoms simply stopped working. It required a phone call to Tesla support to even receive an accurate error code. Tesla is claiming a "short" in the main computer system of the vehicle. This occurred for myself and dozens of others all within several hours of each other in the early morning hours of Friday November 29th 2024. Suspicion is that that this was due to a software update from Tesla.
Cameras not working with latest software update on November 26th. Without even inspecting the car in-person, Tesla service agent told need new ap4 computer on 2024 Model Y which is 1 month old . I think Tesla knows about this issue and there is lot of wait time for getting the ap4 computer. Tesla service advisor states - car is totally safe to drive with just mirrors till appointment in few weeks. As per NHTSA, if a car is not having a backup camera, that is a safety violation and there are lots of Tesla vehicles driving without backup cameras. No backup camera, navigation, battery life dying faster. It seems like a liability to me to drive this way and very negligent to me.
On Nov 23, 2023, I was reversing car from my driveway in the morning. Car started giving warnings with issue on all its cameras. While reversing car, it will give error that rear view camera not available. While giving left/ right turn signal, it is showing blank screen for that view. All other safety features like blind spot detection, lane assist are not working at all. I also started getting error message that emergency brake is not available. I have been contacting them for immediate repair/ provide loaner car from same date (11/23/2023). Earliest appointment date given is Dec 11, 2023. This is not acceptable at all. It is very dangerous to drive the car without those safety futures. I am also concerned on any other issues which it has not reported. I don't feel safe to drive the car.
One day the cameras, gps, and sensors just stopped working and haven’t worked since. Been 4 weeks.
My Tesla Model Y experienced a complete computer failure, rendering all active safety systems inoperable. This included backup cameras, speedometer display brightness, and collision avoidance system. After receiving assistance from Tesla service, the issue was diagnosed as a shorted computer. A computer replacement was performed, resolving the problem. I am submitting this case as a reference for potential investigations.
I had my computer replaced (ap4) on the Model Y. After a software update around Nov 13, my computer glitched. The car was drivable, but no functionality (cameras, navigation. Etc). After the computer was replaced it seems fine. Just wanted to report after the fact. Thanks.
All safety features and cameras on this vehicle are currently disabled. Specifically, the following functions are not operational: forward and parking collision warnings, blind spot warning, lane departure and lane-keeping warnings, rear cross-traffic warning, self-driving capability, adaptive cruise control, navigation system, location tracking, sentry mode, premium connectivity features, reverse camera, front and side cameras. Additionally: the headlights remain on continuously during daylight hours. The interior control panel screen and the side and rear mirror displays are stuck in night mode.
