4 Vehicle Speed Control problem

Failure Date: 12/16/2024

Around Dec 16 all cameras in my 2024 Model Y Tesla stopped working and it has not worked since that date. This affects many systems, the most important of which is that I now have no rear backup camera. I believe vehicle manufacturers are required to have functioning back up cameras. I notified Tesla service on 12/16 and was told there was a known computer hardware problem which would cause my incident. Tesla service told me they were unable to obtain the needed computer replacement for a month at least. They informed me there was no safety risk, but I disagree since I don’t have a functioning back up camera, nor lane change camera assistance. Also the display panel is very dark and impossible to read most of the time, posing another safety concern. Why has Tesla not issued a public statement about this problem, and why has NHTSA not mandated a formal recall?.

5 Vehicle Speed Control problem

Failure Date: 12/15/2024

Vehicle information: • make: Tesla • model: Model Y • year: 2024 description of the problem: after a software update pushed by Tesla, the ap4 computer in my 2024 Tesla Model Y shorted out, causing a complete failure of all systems controlled by this computer. The following safety-critical and driver-assist features have become non-functional: 1. Cruise control (adaptive or otherwise) 2. Forward collision warning 3. Lane departure warning 4. Lane keeping assistance 5. Blind spot warning 6. Parking collision warning 7. Full self-driving and auto park features 8. Cameras required for autopilot functionality additionally, because the gps system has failed, the vehicle's navigation system is inoperable. This impacts Tesla's unique route planning feature that sets up supercharger stops during long journeys. Without a functioning navigation system, I cannot properly plan my travel or locate charging stations, which severely affects the usability of the vehicle. Safety concerns: the failure of these systems, especially safety features like forward collision warning, lane keeping assistance, and blind spot monitoring, puts both the driver and others on the road at risk. Losing access to safety-critical functions after an update is unacceptable and poses a serious danger to vehicle occupants and surrounding traffic. Incident details: • the issue occurred immediately following a software update. • Tesla pushed the update remotely without providing any warning or indication of potential risks. • the failure appears to be related to a hardware short in the ap4 computer, directly caused or exacerbated by the update. Desired outcome: • a thorough investigation into Tesla's software update process and its effect on vehicle hardware and safety systems. • a resolution that prevents similar failures in other vehicles and ensures system updates do not compromise vehicle safety or critical functionality.

6 Vehicle Speed Control problem

Failure Date: 12/14/2024

After a recent software update - all cameras/sensors/cruise control/adas systems are no longer functioning.

7 Vehicle Speed Control problem

8 Vehicle Speed Control problem

Failure Date: 12/04/2024

Starting on 12/4/2024, my car's camera stopped working, no blind spot warning, no navigation, no rear view camera and no cruise control. Reported to Tesla on 12/8 and they said it is a known problem nationwide and some autopilot computers are getting shut or something. They have given me an appointment for 1/4/2025. The vehicle is really unsafe to drive without these components working and the vehicle draws on heavy amount of battery usage without these controls and for my commute on highway, I am always nervous about if there is going to be enough for me to reach the next charging station. I would expect this falls into safety recall and Tesla should at least send out a service bulletin as well as provide alternate vehicles for the owners to use until this can be fixed.

9 Vehicle Speed Control problem

Failure Date: 11/29/2024

Tesla has told me the ap4 computer has failed in my car which has resulted in them having to replace it. I had this issue start right around thanksgiving. I scheduled the soonest appointment available for 01/03. Not offering me a rental or anything on my brand new '24 Tesla. It has just over 1,000 miles on it and is ~1 months old now. This issue impacts: emergency braking, blind spot detection, lane keep/centering, assisting with parking, automatic high beams and wipers, cruise control/ adaptive cruise control, back camera, vehicles built in gps to be stuck at one location every time, has made my touchscreen not automatically brighten or dim, my auto dimming mirrors be too dim in most driving conditions, constantly fail to install system improvements and safety updates, and because my vehicle keeps trying to do this, it has caused increased battery drain as well. This has increased the safety risk of my vehicle tremendously. . . Tesla has not seemed to publicly acknowledge the issue yet either.

10 Vehicle Speed Control problem

Failure Date: 11/01/2024

All of the car's cameras have gone dead since November 2024. The car has been collecting water in the truck area and inside panel of the car by the door frame.

11 Vehicle Speed Control problem

Failure Date: 10/21/2024

#issue description I experienced an issue with Tesla’s full self-driving (fsd) system when a trailer hitch was attached to my vehicle. From what I observed, the fsd system appeared to detect the trailer hitch as an object in close proximity to the vehicle. This caused the vehicle to accelerate rapidly to the fsd maximum speed of 85 mph, regardless of the road conditions or posted speed limits. This issue occurred when the lane markings were visible, but the hitch obstructed the rear camera’s view. On the fsd display, this obstruction was represented as a persistent “cloud” following the vehicle. I attempted to adjust the maximum speed limit manually to mitigate the acceleration, but this did not prevent the behavior. Each time I engaged fsd, the car would accelerate rapidly, even in situations where such high speeds were not appropriate for the road conditions. # steps taken to resolve the issue I covered the rear camera, which allowed fsd to function normally and prevented the rapid acceleration. The “cloud” visualization on the fsd display also disappeared once the camera was covered. # trailer mode Tesla offers a “trailer mode,” but I did not enable it since I was only using a hitch without towing a trailer. My understanding is that fsd does not operate in trailer mode. # reference: this issue has been reported by other Tesla owners, as discussed here: [xxx] information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).

12 Vehicle Speed Control problem

Failure Date: 10/07/2024

I was in a parking lot on [xxx] in mamaroneck, new york waiting in line to get out at approximately [xxx]. Many cars were ahead of me due to the school drop-off rush, with parents and kids lined up in the parking lot waiting to leave. While I was waiting, the Tesla unexpectedly accelerated on its own, but I managed to hit the brakes just in time to avoid colliding with the car in front of me. A few seconds later, my car suddenly accelerated again without any input from me. I tried to steer away from the cars in front while repeatedly pressing the brakes, but they weren’t working. All I could think about was avoiding hitting another car or person, so I swerved the car toward the building in the parking lot. The impact was hard, and the car finally stopped, but the airbags didn’t deploy. I’m deeply shaken by the incident; I could have seriously injured someone or even worse. When I got home, I discovered that this issue is a known problem with Tesla, yet they’re placing the blame on drivers. Having never been in a car accident before, this experience has been overwhelming. As a mother and attorney, I believe it’s crucial to examine this situation more closely. Tesla controls the data, and it’s clear that responsible drivers wouldn’t suddenly veer into a building in a crowded parking lot without cause. I urge you to act on this life threatening defective car and investigate this problem before it is too late. Information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).

13 Vehicle Speed Control problem

Failure Date: 09/14/2024

While using adaptive cruise control the vehicle regularly decelerates for no reason and without warning "phantom braking". I experience this about once a week, but experienced it multiple times on a longer drive this weekend. The unexplained abrupt braking could result in being rear-ended by another vehicle.

14 Vehicle Speed Control problem

Failure Date: 09/03/2024

The contact owns a 2024 Tesla Model Y. The contact stated that while driving approximately 15 mph the vehicle suddenly experienced unintended acceleration causing the driver to loose control of the steering and crash into a second vehicle. During the crash the frontend of the vehicle was severely damaged but the air bag did not deploy. The contact sustained a right hand injury that did not require medical treatment. Three occupants from the second vehicle sustained undisclosed injuries and were transported to the hospital. A police report was taken at the scene and the vehicle was towed away. The cause of the failure was not yet determined. The manufacturer and local dealer were notified of the failure. The failure mileage was 7,608.

15 Vehicle Speed Control problem

Failure Date: 08/25/2024

I was parking the Tesla Model Y at very low speed, when there was a sudden unintended acceleration. The car banged against the wall of a building and was severely damaged. My family - wife and two children were in the car.

16 Vehicle Speed Control problem

Failure Date: 07/27/2024

The vehicle was under full self-driving mode and approaching a shopping center. There was a crossroad with signs on the corner. The vehicle was supposed to turn left but kept slowly approaching the signs on the corner. I was ready to step on the brake to stop the full self-driving mode while expecting the vehicle to turn left or step on the brake. Instead, the vehicle accelerated toward the signs, so I had to steer the wheel to the left and apply the brake immediately. The vehicle jumped a bit as it ran over the corner of the crossroad and stopped after about 10 feet. I was fortunate because I took action immediately, avoiding hitting other people or property. However, human reactions seem not to be quick enough to prevent some accidents as in this case. During this incident, the right corner of the front bumper of the vehicle hit the signs, and the top right corner of the vehicle (right above the front window) was also slightly damaged.

17 Vehicle Speed Control problem

Failure Date: 07/18/2024

I was on [xxx] heading towards chicago and following the traffic. I started changing the line to pass the vehicle in front of me and suddenly my Tesla completely lost control and started drifting to the sides. I released the accelerator right away but it made it even worse. It seems that acceleration continues to happen that’s what made car to drift. Eventually car slowed down and I was able to make it to my line. I don’t understand why line assistance didn’t work to keep the vehicle stable, why after releasing accelerator the vehicle continued to gain the speed. It was sunny day and dry road. No foreign objects on the highway. Information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).

18 Vehicle Speed Control problem

Failure Date: 04/30/2024

I was driving down the highway with my wife and baby at about 65 mph. I was using regular cruise control with no one in front of me when all of a sudden, the car braked extremely hard. Luckily, the car behind me was far enough back to switch lanes and pass me. If it was closer, this could have resulted in a highway accident. The car stopped braking maybe around 30 or 40 mph. I may have it recorded in the Tesla camera system since I pressed the save button approx 4 min after it happened. I also scheduled a service call with Tesla that will hopefully happen on may 7th.

19 Vehicle Speed Control problem

Failure Date: 04/26/2024

My wife and I purchased a Model Y 2024 just a few weeks ago. We have had multiple experiences when using cruise control where the car inexplicably slows down rapidly when there are no vehicles in front of us decelerating.

20 Vehicle Speed Control problem

Failure Date: 04/12/2024

While driving to work, using fsd, the autopilot steered me into the path of an overtaking vehicle moving at about 20 mph faster than my vehicle. By quickly taking over steering and speed control, I was able to avoid a collision. A couple miles further down the road, the car could not decide on which exit lane to use, and weaved between two possibilities, probably alarming and angering drivers behind me. Again, I had to take over control. Taking over speed and steering control was necessary two more times in my 25 minute trip, most of it on well-marked multi-lane highways. Of the four incidents, only one would have resulted in a serious high speed collision, had I not taken over very quickly. Having coached robotics competitions, and piloted aircraft with autopilot, it is my opinion that the fsd (supervised) software is at alpha stage in development (not yet up to beta stage, at which it could be safely released only to select skilled drivers, with suitable disclaimers and warnings) . At delivery, no introduction to the car's controls is provided, so customers are on their own regarding finding out, by trial and error and the occasional potentially lethal collision, how to safely drive the vehicle with fsd engaged. The manufacturer should be required to instruct all customers on the very high level of vigilance required to operate with fsd engaged.

21 Vehicle Speed Control problem

Failure Date: 04/09/2024

While using autopilot or beta self-driving - when a minimum speed limit sign is detected on I-75 in georgia, the system usually reduces to the speed of the vehicle to posted minimum speed (40 miles an hour) instead of maintaining current speed limit.

22 Vehicle Speed Control problem

Failure Date: 02/18/2024

Car decelerated drastically to a low speed when driving on the highway using cruise control.

23 Vehicle Speed Control problem

Failure Date: 02/10/2024
