Front passenger safety restraint system fault alarms comes intermittently.
We were in a head-on collision (not our fault) in April. The police determined the combined speed of the crash impact to be 60-65 miles per hour. The 2020 Tesla y did not deploy its airbags. And as far as we can determine, the accident was not automatically recorded by the Tesla as it should have been. Because the airbags did not deploy, I sustained a fracture to my sternum that traverses over to my collarbone. we both have tbis and other injuries. With that kind of an impact, one would expect an airbag to protect oneself, yet it did not. There were no warning lights/messages/etc from our vehicle. We are asking that you please look into this safety matter. (especially since you are part of the push to have us own electric cars, they need to be safe for us). I do not know how much longer the Tesla will be available to inspect.
Tesla suddenly accelerated forward from a complete stop at very rapid pace and hit a wall.
What component or system failed or malfunctioned, and is it available for inspection upon request? >>> the passenger detection hardware underneath the front passenger seat had faulty wiring. The repaired hardware completed by Tesla this week is available for inspection upon request. I did take amateur photos of my seat hardware before-and-after repair. How was your safety or the safety of others put at risk? >>> my passenger's airbag could randomly fail in the middle of a drive. Anywhere from 10 seconds to 10 minutes while driving with a damp passenger in the front passenger seat (e. G. , a 100 lb. Person after swim practice), a red airbag indicator will assert with error code rcm2_a636 ("front passenger safety restraint system fault"). The indicator would stay asserted for days, then start toggling on-and-off randomly. According to Tesla Model Y documentation, "if the airbag system detects a fault in any of the previously mentioned components, the airbag warning indicator stays on. In this case contact Tesla service immediately. Do not drive the vehicle until the airbag system is inspected by Tesla. . Read more...
(1). Electrical wire related to passenger airbag function is allowed for rodent chewing. I was told by technician that if this wire chewed by rodent or broken, passenger air bag will not function at all when there is an impact or an accident. I'm not really sure whether the driver will be even alerted whenever this wire is broken. This is simply a design fault because such an important wire should not be placed in a vulnerable location where rodent can chew it. I have never come across this in any car in my life so far. (2). If an airbag does not function, a passenger could get hurt or die (3). I asked the service team about the design fault and filled their service visit survey as well, but no one answered me so far. (4). Service technicians inspected the broken electrical wire, repaired it, but there is no guarantee that rodent can not chew it again because repaired wire is at the same place as before. (5). There is no prior warning for me. Service technicians came over to replace the passenger seat. Once they replaced it, the senior technician tried to check all the system using his laptop and it appeared that he noticed a fault on his laptop screen. So, he opened the frunk and found the broken wire. (6). Instead of replacing this part with oem part he repaired it and even charged me for the cost of repair though my car is under warranty. This is the design fault and this wire can be chewed again and manufacture can keep charging the customers till the life of the car.
I’ve received repeated errors of front right safety restraint system needs service with an indication of the airbags being disabled. Tesla has attempted to fix this on two occasions by replacing various wiring and seat belt parts, but the problem returns each time.
Vechile started skidding and didn't respond to steering wheel control, ultimately failed to a make a turn and fell off road into a ditch. Also emergency braking wasn't triggered to prevent vechile from falling off road and furthermore upon impact airbags weren't deployed. The driver sustained grevious injuries and was transported away by ems.
The front passenger safety alert system fault warning appears occupant classification system (OCS) of front passenger seat is faulty and Tesla said it’s not covered under warranty.

Front passenger safety restraint system fault. This is 3rd time this system warning has come up. The previous 2 fixes were under warrantee (the last one was Feb 2023), the new one is not and estimated at $1100 fix.
Airbag did not deploy / system failed at the time of the car accident happens. My safety was at risked during the time of accident. There were no indications, warning, messages or symptoms of the problems when the accident occurred.
Front passenger safety restraint system fault?, error message appeared. Tesla is asking for ~$1500 to fix a sensor that would prevent air bag from deploying. Seems like a safety issue that should be addressed. The message appears whenever there is someone/some thing in front passenger seat. Tesla has inspected. I believe it happened after a minor accident in a parking lot, but Tesla and the repair shop can not prove that it did, so insurance will not correlate to the accident.
My vehicle encountered a severe loss of control, resulting in a collision with a concrete wall on I-90 west towards seattle at 19,877 miles. This critical event, leading to non-deployment of airbags and subsequent hospitalization, occurred shortly after two prior services addressing a rear drive motor unit malfunction, first at 14,768 miles and again at 14,820 miles. These incidents underscore a troubling pattern of safety concerns. Inspection details: front suspension: replacement of stabilizer bar link (1188391-00-b). Steering gear: replacement of steering gear assembly (1044831-00-h), essential for control. Sub-frame & steering components: reinstallation of the sub-frame and replacement of the tie rod end (1044841-00-e), indicating significant structural and steering issues. Safety risks: the sequence of malfunctions, particularly the recurring issues with the rear drive motor unit followed by a severe loss of control and airbag system failure, presents an extreme safety risk. The absence of warnings prior to these failures compounds the danger, suggesting systemic safety and reliability flaws. Conclusions & reporting: the vehicle's history of critical malfunctions within a short mileage interval demands an in-depth examination by the manufacturer and safety regulatory authorities. The objective is to uncover root causes, evaluate if these issues are widespread among similar models, and implement corrective actions promptly. This report aims to highlight the critical nature of the encountered safety issues, advocating for immediate interventions to safeguard vehicle occupants and the public.

Every few months, my car receives a warning stating "front passenger safety restraint system fault". This error noted that there are issues related to checking the front passenger seatbelt/airbag status, and that they may not work as expected. Tesla has tried to repair this 4 times on my car already, but the error keeps coming back after a few months. This is now the 5th time this is happening in my car over the 3 years I've owned it. The date/mileage listed is when this issue first started occuring.
Tesla Model Y purchased on 06/23/2023 from springfield NJ center. I had 2 accidents on the same day. I having been driving in the us from more than 5 years and never had a scratch on my vehicle nor even a parking ticket. I drove approx 600 miles in 2 weeks. So I was very comfortable in driving the Tesla Model Y. Date of incidents - 07/07/2023. At tinton falls NJ. I was trying to take a k turn on a flat road at around 9:30 a. M. I put the car in reverse. When I pressed the accelerator pedal the car went forward and hit an electric pole. I then pressed the brake pedal . Made sure the car is on reverse gear . On leaving the break pedal the car was automatically moving forward towards the pole. This should never had happen. I put the car in park mode and waited for 5-10 mins approx. Then after some time when I put the reverse gear and left the brake pedal the car didnt move and on pressing acceleration it reversed. I parked my car at the site where I had my work. Police were called and they gave me a ticket. I had a sprain in my neck. Later by 2 pm I drove with the same car home at edison NJ. (1 hr drive) I called Tesla customer service and they advised me to take the car to springfield NJ dealership as it could be a software glitch. I decided to drive the Tesla with my family in other car to the dealership incase they decide to keep the car. I was about to enter into fadem rd when lifted my feet off the acceleration pedal to slow down. The car all off sudden accelerated with no steering control. I noticed lots of car on the street and a lady exiting out of near by staples drive way. I grabbed the steering and rotated it all the way to the right to avoid any of them, hit a pole on the passenger side , pressing the brakes really hard and hitting a bush. The car stopped, no air bags deployed , no warning signs nothing. I was in a shock and pain . Emts took me to er , I had severe muscle sprain and still recovering.
Front passenger safety restraint system fault intermittently appears and disappears. Might be defect of manufacture of harness squezed. Lots of users are complaining about said and manufacture wants $500 to repair what seems to be a recall. Https://teslamotorsclub. The car suddenly crossed the double yellow line on a local road and hit another car coming from the opposite direction on a local road (adams lane, north brunswick, NJ). The wheel joint was faulty that broke (see attached pictures) and pull the car to the other side of the road without warning. Even the airbag in this car is not deployed. The vehicle was towed by police to a garage (coppa's service center & towing, 1 moore road, north brunswick, NJ 08902. Ph. 732-297-7530). The vehicle has been declared as a total loss bt the insurance company. Because my car crossed the yellow line and hit the other car, I got tickets (crashdocs. Org; polish report # 23034105; date: 06/20/2023). I and the lone person the other care are lucky to survive this accident. I feel that such a fault might put other Tesla Model Y drivers/passengers and others in danger on the road. Please investigate to keep people safe. Kalyan das 908-720-8099.
The contact rented from turo a 2022 Tesla Model Y. The contact stated that while his daughter was driving at approximately 65-70 mph in rainy weather, the contact's daughter released her foot from the accelerator pedal to decelerate; however, she lost control of the vehicle. The vehicle crashed into a guardrail and went into a ditch. The vehicle then inadvertently reversed onto the highway. The contact's daughter slammed her foot on the brake pedal, and the vehicle stopped immediately. No warning light was illuminated. No air bags were deployed. A police report was filed. The contact's daughter suffered body aches, and medical attention was received. The vehicle was towed to an unknown location. The contact was unaware if the vehicle was deemed totaled by the insurance company. The manufacturer was not made aware of the failure. The failure mileage was unknown. The VIN was unavailable.
My car met a major accident and the impact was so severe that it was total loss. The car was badly damaged from drive side and it skid at least 100 meters and it turn around and I am aware that Tesla has side airbags also but none of airbag were deployed I tried to contact Tesla support and report this issue but no one from Tesla gave proper response . If you want I can share the pics of my car and claim details.
I was going to park in front of the store at the shopping mall. Before I reached the parking spot car was not slowing down automatically after releasing the acceleration pedal. I decided to use the brake (which I rarely use because the car stops automatically when you release acceleration pedal) and I really don't remember how everything happened and I might accidentally step on the acceleration pedal instead of brake, but the car was not able to detect any obstacles or objects and I ended up braking heavy metal bench in front of the store and broke into the store. I ended up smashing entire front window and drove into the store as deep as 1. 5 feet breaking the liquor boxes. After this heavy impact the airbags were not deployed. My concern is into two safety issue: airbags not deploying and car safety sensors not functioning.
Air bags malfunctioning, if fact, not even working on this 2023 Tesla Model Y.
3 days after purchase, headlights would not work, car towed to service center, both front headlight assemblies were replaced. 3 weeks after purchase, fault detected on passenger restraint, service center states passenger seat needs to be replaced to make airbag operational. I live 90 miles from service center, making these repairs quite difficult.
The contact owns a 2023 Tesla Model Y. The contact stated while attempting to make a left turn at approximately 5 mph, another vehicle crashed into the rear passenger-side end of the vehicle. The air bag warning light was illuminated. There was property damage and the vehicle came to a stop in the middle of the road. The vehicle was towed to a dealer and was not given a diagnosis. The vehicle was repaired however, the failure still occurred; the contact was informed the headliner was damaged due to the repairs done along with several other failures. The manufacturer was notified of this failure and referred the contact to the dealer. The failure mileage was 7,000. .
I am writing to file a complaint against Tesla, inc. For refusing to repair recurring suspension issues that were supposedly addressed through previous recalls. As an owner of a 2020 Tesla Model Y (VIN: 5yjygdee1lf040216), I have experienced numerous problems related to the vehicle's suspension system, steering, and safety restraints, which I believe pose serious safety risks. My vehicle has undergone multiple repairs at Tesla service centers for issues such as squeaking noises when turning, vehicle shutdowns while driving, steering difficulties, and excessive tire wear. Many of these problems are related to the vehicle's suspension system, which has been the subject of several recalls by Tesla in recent years, including recalls 20v709000, 21v835000, 21v912000, and 21v389000. Despite these recalls supposedly addressing the suspension issues, my vehicle continues to experience problems. Most recently, on January 24, 2024, I brought my vehicle to a Tesla service center for a loud and distinct sound coming from the steering system. The service center diagnosed the issue and recommended repairs to the front half shaft assembly, front damper assembly, and front lower lateral links, with a total estimated cost of $3,670. These issues should be covered under warranty and are directly related to the previous suspension recalls. However, Tesla has refused to cover the repairs under warranty. Furthermore, I have experienced recurring issues with the front passenger safety restraint system, which required a retrofit of the occupant classification system (OCS) on two separate occasions. This suggests that the initial repair was ineffective and that the system may have an underlying design flaw. I request that the NHTSA investigate these issues and take appropriate action to ensure Tesla addresses these safety concerns and honors its warranty obligations. I have attached documentation detailing the repair history, invoices, recall notices, and correspondence with Tesla.
The contact owns a 2020 Tesla Model Y. The contact stated that while driving at an undisclosed speed, the air bag warning light illuminated, indicating that the passenger’s side air bag was disabled. The manufacturer was made aware of the failure and informed the contact that the vehicle was out of warranty; however, the manufacturer sent a mechanic to the residence, who replaced the sensor; however, the failure recurred. The mechanic came back a second time and performed a bypass on the sensor; however, the failure recurred. The manufacturer informed the contact that the passenger's seat needed to be replaced. The contact related the failure to the code rcm2_a636 (front passenger safety restraint system fault). The failure mileage was approximately 90,000.
Was hit by a f-250 running a stop sign, estimated speed of f-250 was 45 mph. The airbags failed to deploy in my vehicle.
The contact owns a 2023 Tesla Model Y. The contact stated that the message “passenger air bag off” would illuminate while his wife was occupying the front passenger seat with the seat belt buckled up. The vehicle was taken to the Tesla service center, where an unknown software was recalibrated. The service center then informed the contact that the front passenger seat would need to be replaced in the event the failure persisted. The contact stated that the failure reoccurred approximately three weeks later. The manufacturer was notified of the failure and informed the contact that the vehicle was operating as designed and the passenger air bag would not activate unless the passenger weighed approximately 130 pounds. However, the contact stated that the vehicle manual indicated that the air bag would be activated if the passenger weighed 100 pounds. The contact stated that his wife weighed 126 pounds and was 5. 1 ft. Tall. The vehicle was not repaired. The failure mileage was approximately 15.
The front passenger seat airbag does not properly arm when occupied by an adult female weighing between 120-125 pounds. According to the vehicle's owners manual, it should arm at any weight in excess of 100 pounds. There is an indicator on the main display that clearly displays when the passenger airbag is armed and when it is unarmed. This is an intermittent issue, occurring more than 50% of the time the seat is occupied. The best we can do to characterize the behavior is as follows: 1) the passenger airbag is more likely to arm correctly if the passenger gets into the car at the same time as the driver. 2) it is less likely to arm correctly if the passenger gets in after the driver has started the car and/or driven it a bit. Two examples: driver gets in to set the temp or navigation before a trip and passenger gets in a few minutes later, or driver retrieves car from a parking place and then drives a short distance to pick up the passenger. In both cases, the airbag is unlikely to arm correctly. 3) even after a successful drive (in which the airbag armed), say to the grocery store, it is unlikely to arm when the driver and passenger get back in together a few minutes later. In all cases above, the longer it is driven in a single trip, the more likely it is to randomly arm/disarm itself multiple times during that trip. By failing to correctly and consistently arm when occupied by a 120-pound adult passenger, it leaves said passenger unprotected in case of an accident. Tesla service performed static, calibrated-weight tests, and confirmed that it takes greater than 125 pounds to consistently arm the front passenger airbag. They have replaced the passenger seat twice so far, and claimed that the most recent seat installed passed the 100-pound test, but the issue persists. The car was serviced for this issue on November 28th, and again from November 23 to December 7. This issue remains to this date (December 19).
The contact owns a 2022 Tesla Model Y. The contact stated that after he made a right turn, another vehicle crashed into the front passenger side door to the rear passenger side door. The air bags failed to deploy. No warning lights was illuminated. The contact sustained soreness to the shoulder, neck and back. No medical attention was required. A police report was filed. The vehicle was towed to a tow lot. The dealer nor the manufacturer was notified of the failure. The failure mileage was 8,607.
The contact owns a 2020 Tesla Model Y. The contact stated that after an update was made to the vehicle's auto pilot feature, the phantom braking feature would intermittently activate without incident. The contact stated that while his wife was driving in autopilot at 55 mph, the phantom brakes independently activated which caused the vehicle to hydroplane. The vehicle initially veered to the left and then to the right, hitting a guardrail. The air bags did not deploy upon impact. The guardrail was damaged as a result of the incident. The contact's wife did not notice any warning lights during the failure. His wife did not suffer any injuries during the crash and a police report was not filed. The contact's wife managed to drive the vehicle home after the incident. The service center nor the manufacturer had yet to be notified of the failure. The vehicle had yet to be repaired. The failure mileage was approximately 30,000.
