Second time !!!! during ownership of car that brakes fail. Same problem as before brake actuator/booster failures: multiple reports indicate issues with the abs actuator or brake booster pump, leading to warning lights ("braking power low", "abs system malfunction").

On [xxx] at [xxx] I was backing out of a driveway when my 2016 Prius v5 accelerated on its own. When I applied the brakes, nothing happened. I hit a curb, which flattened the front passenger tire. In order to get the car to stop, I took it out of gear and put it in neutral. The car finally came to a stop. My speed while backing out of the driveway was less than 5 mph. It accelerated to about 15 mph. I was told that because the tire flattened due to the force of impact with the curb, Toyota considered it an accident. Toyota had me complete an "unintended acceleration" form so they could investigate the situation. A case number ([xxx]) was assigned. I was also told that I needed to contact Toyota's brand engagement center at (800) 331-4331. Toyota assigned engineering analysis associates (eaa) to perform a thorough inspection and read out the event data recorder (edr) of my vehicle at findlay Toyota. Anne chapman with eaa did the inspection and sent a copy of the report to me on 09/15/25, the day of the inspection. She suggested that I keep the copy because I may need it if I needed to contact an attorney. My insurance company, usaa, sent an adjuster to examine the extent of the damage. Findlay collision center completed the repairs on 09/26/25, which cost $3,433. 92. The accelerator nor the brakes were addressed. On 10/09/25, Toyota provided a response to the incident: they were not at fault. They claimed operator error. Unintended acceleration and unresponsive brakes have been an issue with Toyota's in the past. At that time, instead of fixing the problem, they told dealerships to educate their buyers on how to take the car out of gear. Approximately one week after picking up my car from the repair shop, the vehicle accelerated on its own again. Fortunately, the brakes worked this time. Because I felt very unsafe, I got rid of the car shortly after that incident. Information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).

Car was driving fine one day and then while driving down the interstate I had the pre collision system malfunction error, brake light came on, cruise will not operate, abs light is on. After much research it seems Prius v has a common issue with brake booster faulting.

We were driving in san diego and had driven oven a bit pothole. About 50 miles after driving over the pothole, the brakes stopped working. We were pressing down hard on the brake, but the brake was soft and did not do its job. We almost rear ended the car in front of us. We brought the car right away to a close by auto shop and they said there were no leaks as the brake fluid was still close to the max and they did not find any indication of a brake fluid leak and sent us on our way. We are still unsure as to why the brake from this vehicle stopped.

While driving vehicle on the freeway it stated braking power low, stop in a safe place, antilock brake system malfunction, pre-collision system malfunction, lane departure alert malfunction. I was almost in a major scary accident.

When putting the car in park and then engaging the foot parking brake on a slope, such as my driveway, and then turning off the car, the car rolls back significantly. Then, while exiting the car, the rear brakes make loud grinding and creaky noises as the car settles downwards slightly. Then, when entering the car, the brakes make the same creaky and grinding sounds. When holding the brake pedal to power on the car, then disengaging the foot parking brake, then shifting the car from park to reverse or drive, the car jerks abruptly and at the same time makes a loud and stuck disengagement sound, as if the car was being held on the hill/driveway solely by the parking pawl. It seems like the parking brake is not holding the car, and instead, the parking pawl is.

When I am in the process of braking and hit a rough spot in the road such as a pothole, I essentially lose control of the car for a split second. It feels jerky like it lurches forward or shakes side to side. Its not very pronounced but enough to make me feel unsafe. This happens every time I am braking and run over potholes, so its not an isolated incident that only happened on one day, and it doesn't matter how hard I'm braking or how fast I'm going unless I'm barely moving (then it doesn't jerk).

While parking my vehicle and slowing down to enter a parking space, I eased off the brake to pull into the space when I heard the engine rev loudly and then the car accelerated and continued to accelerate despite attempts to pump the brakes, release the brakes without touching any pedal and also trying to put the transmission into park. The vehicle would not stop at all. I was afraid for my life and tried everything to avoid other vehicles and pedestrians as the car accelerated greatly. I steered the car to try and avoid a slight embankment and curve but with the rapid speed of the car, it became hard to maneuver the car. The vehicle jumped the curb striking a tree and then a concrete post. The car came to rest on its side after impact. The car was a total loss.

9 Service Brakes problem

Failure Date: 06/05/2018

When the car is in drive and stationary on a hill facing up wards (like in a driveway) the car still sends electrical power to the electric motor and to the wheels even when the brakes are applied. This only happens when facing up a hill. All electrical power is suppose to turn off when the brake is pressed. This started happening two weeks ago and the dealership said that this is normal but it is not normal as it never did this before. It does not happen when on flat ground or when facing down hill. Dealership claimed that this is the hill assist feature holding the car but that's a lie as the hill assist utilizes the brakes and not the motor. Dealership could not find any error codes and refused to dig further into the issue and made up lies about it.

When breaking while traveling over bumps/ripples on the road or over rough road conditions such as potholes or roadwork, the vehicle feels as though the breaks are skipping or failing. This has occurred multiple times under the same or similar conditions. There is a sensation while attempting to stop, that the car is not going to stop in time or that you have lost control of the vehicle's ability to stop safely.

While driving the vehicle started to shake from underneath fiercely, I called my mother who came and picked up the car she turned around and went back to my residence, while attempting to park in the garage the car started to roll down the driveway and the brakes wouldn't work, the car landed on the street. Nhtsa recall no. 16v672 and 16v741 were not completed. Called the authorized Toyota dealer, stevens creek Toyota, 380 kiely blvd, san jose, CA 95129, on 01/20/2017 for remedy to be performed. I was told that they will call me back when teh parts are available. Remedy not comleted yet.

Toyota of surprise is saying that they can't fix our Prius recall for the emergency brake until January 2018.

16v741000 was not performed on the car even though it was purchased 11/17 , and the recall began 11/10. Dealer lee's Toyota located at 139-65 queens blvd, jamaica NY 11435.

Vehicle has clicking noise when we push the break paddle. This is the second vehicle that I got from Toyota which has the same problem. The company kept the first vehicle and then give me this one after the trade in.

When gently applying brakes for a stop light, and the right wheel hits a 1 and 1/8 inch deep depression in the road, the brakes feel like they momentarily release. The depression is for a 9 inch diameter city water cover, and the overall diameter of the depression is 12 inches, and it is 1 and 1/8 inches deep. This can be replicated at this location and we have also noticed the same thing happening elsewhere in town.

As I took a highway exit, following behind other cars doing the same, my Prius refused to brake efficiently when another car abruptly invaded our line. It felt as if the brakes made a slight release or couldn't reach a full stop. I had enough space to stop, and I pressed the brakes on time but there was almost no brake reaction and I crashed into the truck in front of me. I was probably going 20-30 mph, somewhere in the middle. . . So not even that fast. Best part is that the airbags didn't inflate, even though the car was almost totaled. This feels explainable to me. . . I had it for only two months thus far. I do not recommend this car to others.

Tl- the contact owns a 2016 Toyota Prius. The contact stated that while backing up and pressing the break pedal, the vehicle was jerking while coming to a stop without warning. As a result the contact had to release the breaks to get vehicle to stop on its own. The vehicle was taken to city Toyota located at (255 san pedro rd, daly city, CA 94014 (650) 204-8135) where it was diagnosed that the breake activator needed to be replaced. The vehicle was repaired. The dealer repaired failed to provide a permanent fixed and the failure continued. The dealer stated that the vehicle needed to be diagnosed again to receive the repair. The manufacturer was made aware of the failure. The failure mileage was approximately 17,000. As.
