Electronic control module failed due to the cup holder failing. It is available for inspection. Safety was put at risk because the vehicle randomly slammed itself on the brakes, when in drive the vehicle was moving backwards then abruptly braking and moving forward. The problem was confirmed by the dealer. There were warning lamps for the following: drive start control malfunction, parking brake unable to disengage, hybrid system failure, check engine, parking support brake malfunction. The vehicle was showing as being in both park and drive at the same time. The dealer determined the cupholder allows water condensation to leak into the center console compartment where exposed high and low voltage wiring is contained as part of electronic modules causing the vehicle to malfunction. The vehicle was put in gear while throwing the malfunctions, however, the electronic components should not be exposed and stored within a compartment where liquids are known to be stored (cup holder) in a way that allows water to leak into them. This could happen while the vehicle is being driven and cause serious issues. This same issue happened once before several months back before the cup holder started to fail, so regardless if the cupholder is working properly, it seems water can penetrate this area of the vehicle and cause issues because there is no barrier between the electronic modules/wiring and the cup holder drip tray. The service department actually stated to me that they see this issue all the time on the Sequoias and tundras.

On three separate occasions we have lost braking power preventing instantaneous stopping, and control of the vehicle. They partially work when your foot is pushing on the pedal all the way to the floor. This occurs autonomously without warning and the following occur: lose breaking power, inability to brake and stop at safe distances instantaneously and momentarily, brake abs alarm, parking support brake malfunction ("visit your dealer"), check engine, collision assistance fails, and traction control fails. The first occurrence my spouse was driving the vehicle on a 65 mph three lane highway that at the time was evening rush hour. She had to decrease her speed to 30 mph in a 65 mph speed zone in order to slow the vehicle enough at a safe distance from the vehicle in front of her, then continue at a safe distance as traffic ahead of her excelled. Had this happened traveling the speed limit, and had to stop abruptly, this could have either caused severe bodily injury and/or death to her and the vehicle behind and in front of her, and other adjacent lanes. The second occurrence after the dealer looked at it (as covered under warranty), we received the vehicle back. The next day the brakes went out and alarms appeared once again as she pulled out of the driveway. I pulled the vehicle in the driveway for her and couldn't stop and almost drove through my garage at only idling speed. The third occurrence was most recently after an oil change. I drove the vehicle around the block and again, the brakes went out and alarms appeared once again. We have owned this vehicle since may 10th and it has been at dealerships for repair for at least 35 days minimum. On the first occurence they had a regional tech/engineer inspect the vehicle. Nothing was found as problematic and a factory reset was performed - nothing replaced. The second occasion - same outcome. . . Nothing fixed or replaced. The vehicle is in the shop again for the same problem.

My vehicle has randomly been flashing vbs/abs/traction control lights. Sometimes nothing happens other than the flashing lights. Other times I’ll be driving & when going around a curve my car makes a beeping noise (sounds like the abs noise activated) & I’ll feel a pulsing under the brake & hear a grinding sound. It doesn’t allow me to brake effectively. Another time I was coming to a stop behind a car at a red light. It was clear as day, roads weren’t wet & I couldn’t have been going over 10 mph. When hitting my brakes, my steering wheel kind of glided back & forth for a sec and it felt like my tires “floated”in the same motion as wheel until it stopped.

It has happened 3 times, every time I have been driving and an alert with flash on the dash that there is an object in the road and it will brake check me, but there isn't anything infront of the vehicle. One time it happened getting off the highway and 2 other times it has happened when I was in a residential area. 2 out of 3 times, a vehicle behind me had to swerve to the shoulder of the road, so they didn't rear end me. Second issue, the hybrid system is not working. The vehicle was parked and when I went to put it in drive, the alert popped up and I had to turn the car off and restart. Third issue, the lane assistance. While driving in the middle lane and going around a 20 or 30 degree right turn, the vehicle pulled me into the left lane, almost side swiping the vehicle next to me.
After several years of fighting problems with, after braking to a brief stop, the loss of power/abs grinding when accelerating and turning right, now the abs is completely dysfunctional. Both trc lights will turn on and usually the parking brake, and when braking at speeds less than 10mph the abs will activate and prevent bring the truck to a stop without pumping them, on dry pavement. When the abs light turns on, then braking is possible, but all pressure is gone from brake booster and brakes sound like they're grinding, not like the sound of abs engaging. Very dangerous to drive in snow as front brakes work but drive axle doesn't disengage when the brake pedal is depressed. Regular mechanic won't engage with diagnosing issues as he feels the problem is partly computer malfunction and Toyota won't allow for used part mismatching. Abs/ecu is failed.

In the past few months the automatic braking feature was activated without any reason. No obstruction in front, no unusual weather or driving conditions. Fortunately nobody was close behind and no accident occurred. Today the blind spot and cross traffic indicators stopped working. I have not taken it into the dealer yet. I will do so soon.
