Multiple times in my almost 3 year ownership I have experienced a “dead pedal” while pulling out into traffic from a stop. It has almost caused a t bone accident on a few occasions. It’s been to the dealer and had a software update TSB 0111-22. After the update I didn’t have the issue for a little while, but it has come back and it now happens almost every time I drive it. Online search shows that there are multiple people having this problem. Toyota needs to come up with a fix for this before it causes a bad accident.
The issue is a dangerous hesitation during low-speed rolling acceleration. In real-world driving, when I slow down but do not come to a full stop — such as when merging, changing lanes, entering traffic, crossing an intersection, or rolling through a turn — the gasoline engine can shut off while the truck is still moving. When I then press the accelerator to re-enter traffic or move quickly out of the way, there is a noticeable delay before meaningful propulsion is delivered. This hesitation is not a minor comfort issue. It creates a real safety risk. In a small hybrid passenger car, electric-only rolling behavior may be acceptable because the electric motor can often move the vehicle adequately on its own. But this is a full-size half-ton pickup. In my experience, the electric motor alone does not always provide sufficient immediate response for the weight and use case of the vehicle. The result is a dangerous dead zone where the driver is asking for acceleration, the vehicle is rolling, traffic conditions are changing, and the truck does not respond quickly enough. If a Toyota hybrid half-ton pickup is moving, the gasoline engine should not shut down in a way that prevents immediate acceleration when the driver presses the accelerator. This behavior has created multiple close calls for me. The most concerning scenario is a rolling slowdown followed by the need for immediate acceleration — for example, pulling into traffic, changing lanes, or clearing an intersection. In those moments, a 2–4 second delay is not merely annoying. It can be the difference between safely completing a maneuver and being exposed to a collision.
When at proceeding from a stop or a "rolling stop", the vehicle has delayed throttle response or lag. Causes big problem when trying to get into traffic. This has been ever since I have owned this vehicle.
While at a 2-way stop sign, when attempting to accelerate while turning right, the truck hesitated and did not move. Pressing the accelerator further down did nothing for what felt like 1-3 seconds and then all of a sudden it rushed forward. This is incredibly dangerous as there were vehicles heading towards me. This is not the first time this has happened. It happens nearly daily, while the engine is warm, while the engine is cold, in eco, normal and sport modes. The only time I had zero issues was when the computer was reset after having service done to the truck. It ran incredibly well and shifted and ran as expected.
This was just the latest with this transmission. It happens 2 to 3 times per week. This time it was an intersection with a slight down hill. I stopped, then let off the brake as a car past in front of me. Since it was down hill, the truck started to roll forward. I then pressed on the gas pedal to pull out into traffic. The truck failed to responds for 1-2 seconds. There was plenty of space for a normal vehicle to make it, but with this delay, I barely made it. If you are not at a complete stop for at least 1 second, then accelerate, this thing fails to respond. It has been in many times for this, had the software updated, downgraded, generated reports for Toyota's engineering group. Still no fix. You never know how it is going to respond, but basically if you need it to accelerate, when you request it through the gas pedal, it will fail to do so.

I have throttle lag or hesitation when pulling away from a stop sign or atop light. I have almost been hit several times from it. Something needs to be done.
Severe throttle lag and hard shifting when accelerating from a stop. This has caused several near misses when the vehicle will “stall” in the line of traffic causing lack of acceleration. Often happens when stopping at a four way stop or other stop sign followed by immediate acceleration. Was able to replicate it several times and happens on its own once a week. A widely known issue for the newest generation of Toyota Tundra. There are no safety lights, notifications or error codes. The vehicle did this both before a complete engine replacement and after.
Have severe throttle lag and/or hesitation when giving gas from a stop. It causes a unsafe condition when you give it gas to go and it hesitates so bad I have almost gotten into accidents. This has been happening since new. Currently 30k miles on the truck.
Pickup has regular but not every time throttle delay. For 1-3ish seconds and then it charges forward. Making pulling onto a busy road a bit of a gamble. Seems very very unsafe. I’ve had the new engine put in and still the same issue.
The accelerator pedal is unresponsive at times when attempting to accelerate from a complete stop, or a rolling stop, while traveling below 5 mph. This presents itself as a "dead pedal" type situation where no matter the accelerator angle given by the driver, zero power output at the engine is given. This can be logged via electronic data logger, and clearly the throttle angle does not change when this scenario presents itself. This can present a serious safety concern as the truck becomes a sitting duck, creeping out into a intersection with no more than idle power.
Engine hesitation from a stop. Engine takes 1-2 seconds to respond to throttle pedal input. Creates timing safety hazard when attempting to cross oncoming lane to enter roadway. Puts vehicle at risk of not clearing the oncoming lane in time to avoid collision. Intermittent and haven’t been able to reproduce with dealer.
In certain situations when stopping briefly and then accelerating the pedal is dead. For a short amount of time the accelerator does not work and then it launches the truck forward aggressively. There has been a few instances when I am turning left and the truck does not move and I am stuck in the middle of oncoming traffic before the truck suddenly lurches forward.
There is a well-known issue with these new model Toyota Tundras where there are problems with acceleration. When at a stop and pulling out from the stop, the engine and acceleration hesitate, and there is a flag of between 2 to 5 seconds before the vehicle starts moving. The gas pedal is depressed and there is a hesitation or lag of several seconds before forward movement progresses. There was a Toyota safety bulletin update that was supposed to fix the problem and it did not. Many users are still reporting the same problem and it’s just as bad as it was before. This creates a massive safety issue when pulling into traffic or trying to increase speed to keep up with the flow of traffic when initially pulling out. My truck specifically is experiencing this anywhere from one to three times a week.
Truck lunges forward while driving ar highway speeds as if you have been lightly bumped in the bumper by someone. You need to give at least a 6 car car distance between your truck and car in front to prevent a crash. Truck has also been shutting off while exiting the highway. Will get "put into "p" start engine" as well as, "stearing pressure low" alert on dash in instrument cluster information cluster. Also at low speeds in bumper to bumper traffic is shifting hard from 2nd to 1st gear. Truck was drop off to dealer on 12/18/25 after asking them to take it due to feeling unsafe driving it for 3 days.
When at a stop, pushing the gas pedal to go, the truck hesitates. Almost like the truck dies then all of the sudden takes off. I have had multiple instances were I was almostnt-boned. Dealer said that is normal for a vehicle with turbos. We don't experience this with my wife's kia. No warning lights appear. Just to be clear the truck hesitates at almost every stop when trying to accelerate.

Have notice a lot of throttle hesitation when taken off from a stop or after letting off the gas pedal and then hitting it again. A couple times it has made me feel un safe as there was on coming traffic and the truck took a second or to accelerate.
Attempted to shift out 4-wheel high back into 2-wheel drive. On the dashboard, both 4 wheel high and 4-wheel low were flashing yellow/orange. Put truck into park to clear, nothing continued to flash. Put truck into neutral, nothing continued to flash 4 high and 4 low. Turned the truck engine completely off and started again. Both 4 high and 4 low continued flashy. Put truck into drive, 4 low and 4 high now became a solid orange/yellow, no flashing. Drove truck a mile and a half down the road and lost complete control of the steering. Ended up in ditch with several thousand dollars of damage. Contacted dealer where purchased. Explained what happened to the manager of Toyota service department. He claims it has happened on other Tundra's, says it happened on his truck. Offered no explanation or fix to the problem.
When accelerating from a stop the truck stumbles and does not take off. This is dangerous as people may run into you. I have asked many times at dealership if there is a fix yet and the answer is no.

When completing a slow down, when going to accelerate there is a noticeable delay in the engine responding and before actual acceleration. This typically results in the need to press the pedal hard and suddenly it finally accelerates and at times spins the tires, almost uncontrollably. This has caused many near misses when trying to cross oncoming traffic, or entering traffic of a busy road or hwy. There have been no warning lights or indicators of any issues. This can be repeated on demand and even the dealership service tech has felt and acknowledged it. It is a common issue across the gen 3 Tundras (2022+) models as found across many forums and owners.

Transmission overheating while driving uphill in 4x4.
I have experienced consistent vehicle jerking since the time of purchase, despite acquiring the vehicle brand new. This jerking typically manifests when maintaining a constant speed between 35-40 mph. Additionally, the vehicle occasionally exhibits a complete lack of acceleration. A particularly concerning incident occurred when I was attempting a left turn from a side road onto a main street with a 40 mph speed limit. Despite assessing sufficient time for the maneuver, the vehicle stalled for approximately one second upon acceleration, leaving me in the middle of the lane with approaching traffic. Similar instances of delayed acceleration have been observed when attempting sudden acceleration after a rolling stop.
Lag when pressing on gas pedal may cause an accident.
On may 20th, 2025 I was traveling in my 2022 trd pro Tundra 5. 5 hv. I was taking a left turn at a green light, when all of the sudden lights appeared on my dash saying “hybrid system malfunction, visit your dealer”. The car continued to function for approximately 1 minute, until the next light where it turned off. I tried to turn the vehicle back on, but it started to shake when it tried to start. At this point I was stuck at the light with traffic behind me, the engine had failed and the hybrid system had failed as well. I had to call 911 and request an officer to come assist, because I was blocking traffic. The vehicle had to pulled to a lot where I could unhook my boat that was in tow, and tow the truck to the beaver Toyota dealership. After arriving at the dealer and being there for over a week, I have been informed that they will be preforming a short block rebuild, because my truck is not listed in the 2022-2023 Toyota Tundra recall. Even though my truck experienced the same engine failure due to debris in the engine as all the 2022-2023 Tundras. My specific truck was not included in this recall because Toyota says the hybrid system would be able to move my car to a safe position if there was an engine failure. This was in fact not true because my hybrid system malfunction at the moment of the engine failure resulting in me stuck in the middle of the road blocking traffic having to call the police. Toyota needs to include trd pro hybrid owners in the 2022-2024 full block recall or face extensive legal action.
Transmission or engine has a lag, or dead spot from a complete stop to acceleration, such as leaving a stop light. At times barely noticable, other times it seems the engine is going to die. Dangerous when pulling onto a busy street from a side street. Dealership says "unable to replicate" which is understandable, as it's somewhat intermittent. This appears to be with all model year 2022 and newer Toyota Tundras.
Occasionally, upon coming to a stop or from the stopped position the vehicle briefly hesitates when trying to accelerate cause the vehicle to aggressively jerk once the vehicle begins to accelerate, sometimes leaving you in the middle of an intersection or in a dangerous situation in traffic due to unresponsive acceleration inputs.
Vehicle has lag when accelerating from a stop as well as when in motion.
When I step on the accelerator the engine will sometimes have a very long lag which has almost caused me to get in a wreck when crossing traffic.
