September 18, 2020, driving on quiet residential street, instrument panel lit up with message "hybrid system failure. Pull over immediately". Before we could find a spot at the side of the road the engine shut off and wouldn't turn back on, blocking traffic. Vehicle towed to local Toyota dealer. Engine turned on at dealership. Dealer ran diagnostic tests and said nothing was wrong with car. Sent us home saying car was fine. November 24, 2020 received notice from Toyota about customer support program for failure of "intelligent power module (ipm) located inside the inverter assembly of the hybrid system". Condition described as "hybrid system warning" showing on instrument panel and vehicle going into "fail-safe driving mode". Called phone number on the notice and was told the problem we had with the car was the condition described in notice, but nothing could be done until it fails again and the diagnostic tests are positive. If they know this is a safety problem, and it already happened once in our car, they should replace the faulty parts, not wait for us to get into a wreck on the freeway.

I was leaving work and as I got on to the freeway, I noticed that when I pressed the acceleration, the car would rev and jerk. Didn't think much of it thinking I would make it home. By time I made it to the freeway, my car began to rev loudly and I noticed I was loosing acceleration. Around this time I was going almost 65mph. So I pulled off the freeway and inspected the engine, didn't see anything out of the ordinary so tried to crank it up again. Car crunk, ready light came on and so did electric vehicle mode. As the car went start the engine, the loud rattle and shaking of the car could be hear and felt. I was able to drive another half a mile before the car just shut down. They car would not drive over 5 miles per hour. When it shut down the engine began to smoke so I knew that could not be good. I tried to turn it back on and at the point I noticed I could not put the car into reverse nor drive. The ready light did not come on and I no longer had power steering. The following day I got it towed to the mechanic and was informed that I had threw a rod. I looked and saw that other owners had submitted a complaint with very similar issues. Thought that submitting this would be effective as if this issue persist. There may not be other owners who are lucky enough to make it off the freeway without having any acceleration power.

Today, 9/11/2019. I was driving on highway 405, my car was going at highway speed around 65. Suddenly there was a beep sound and my engine light turn on. My vehicle went into the "limp home" mode, the car would not accelerate. Then the car red light was on, and the speedometer went off and the engine shuts off. Luckily I was driving on the slow lane as I was planning to exit the highway in a mile distance. My vehicle would not respond to the acceleration pedal at all, so I put on the emergency flashing light up and slowly row out toward the exit. This is the third time happened to me, within the last 30 days. Two incident happened on highway and one happened on local road. The first time was also on highway with similar experience. I am lucky to be alive. I have brought my vehicle in to the Toyota dealer maintenance 5 days ago, 9/13/2019, where they claimed they have checked everything and told me the vehicle was fine after charging me $150 analyze fee. The vehicle is obviously not fine and I could have been killed on highway. The picture provided was the day before I brought my car in to the Toyota dealer.
The contact owns a 2018 Toyota Prius. While driving 20 mph, the electrical system failed and everything in the vehicle shut down. The vehicle decelerated and lost power. While the vehicle coasted, the engine restarted on its own. The vehicle was driven to Toyota vallejo (201 auto mall parkway, vallejo, CA 94591, 707-552-4545) to be diagnosed, but the failure could not be replicated. The vehicle was not repaired. The manufacturer was not made aware of the failure. The failure mileage was 8,600.

Driving with cruse control set at 72 mph, driving on 4 lane interstate. Master warning light came on along with the following warnings "check hybrid system" & "check pcs system", all dash lights lit up. Engine stalled unexpectedly and resulted in no power when pushing gas pedal. Steering was not compromised. Was able to cross 3 lanes on the interstate to the shoulder with no power and the car decelerating rapidly, again with no prior warning of any issues. Turned car off and restarted, was able to drive approx. 25 feet before the engine shut off unexpectedly again leading to a complete loss of power. Breaking and steering were affected after restarting the car.

My Toyota Prius loses power randomly after having the recall fix j0v completed updating the software in inverter. Vehicle stalled 3 times on freeway, highway and off ramp. Dash board lights light up with all warning lights. Vehicle loses power. . . . Can drive a mile or so with almost no power. . . Then vehicle shuts down. Dash panel reads check hybrid system conditions are exactly the same as the ze3 warranty enhancement program describes. However the dtc code is slightly different so dealer and Toyota will not fix inverter. The car is dangerous to drive as the symptoms appear randomly, usually after an acceleration of the vehicle puts load on engine. Pls help me.

The car starts as normal and drives normally but once you start to come to a stop the car shuts off and will only restart when you depress the gas pedal but shuts down again when you release the gas pedal, normal city streets and highway.
While the vehicle was in motion, the engine started shaking, the steering and brakes locked up and the engine ceased up as the car shut off. Got error code p3190 for poor engine power, and then a day later everything was normal. This is the 2nd time this has happened while driving, the first time was in a 2002 Prius.
Check hybrid system car shuts off and can not restart, tried system reset per manual but would not clear hybrid. Towed to dealer, dtc code p3004-800, tech said this was the only code, had recall eoe reflash 04-14-2014 ipm extended 15 years. Car was in motion per wife. (it had been raining) the dtc p3004-800 power cable malfunction.
I was driving 50mph when the car lost power. The car suddenly had no power brakes, no ability to easily steer, no engine power and many warning lights came on including an warning that the main hybrid battery was not charging. The car would not respond to acceleration. The car came to a stop. I held the start button the car shut off. The car restarted with no problems. All the warning lights were off. I put the car in drive and started to accelerate. All the warning lights come on just as before and the car suffers the same issues again. I went through the same process of turning the car off and turning it back on. The car started up again with no problems again, all the lights were off. It failed on more time on the trip home. I was able to restart the car and put my emergency lights on. I never drove over 25mph on the trip back to the house. My family took the car to wilson Toyota in bellingham wa. They examined the car and said the 12v battery had failed. They told my family that the main 12v battery runs all the electrical to get the car started and running. We told the dealer the car still had electrical power. They still replaced the 12v battery. The following day my family went to look at the car and a service person at the dealer, who said he was new to this location, said he had noticed that the 12v battery had seemed to be replaced recently. They said they would keep the car to keep looking over its systems. On 4/16/2015 my family called Toyota to discuss our safety concerns with regard to how the car failed. We explained we also have a 2007 Prius that had the main hybrid battery fail and that all the warning lights that came on were similar in both cars. The dealer has said there are no codes that said the main hybrid battery is failing. I am worried that I or my family could be seriously injured if the car failed in this manner again while on a busy highway or intersection.
Driving on sb hwy 35 at 74mph, master warning light came on and vehicle shut down, no electrical or engine control. Pulled safely to shoulder of road. Car would not restart. Got out of car, locked car, unlocked car (Toyota technician instructions), then car restarted. Got car off the road, contacted state farm for tow to autonation Toyota south austin. Toyota technician removed a third party telematic device (in-drive) from obd port. Vehicle diagnostic scan showed two communication errors u0100 and u0111. Third party telematic device was not reinstalled. Vehicle was test driven, held overnight, test driven again and the errors or shutdown did not repeat. Toyota service manager said they have seen third party devices connected to odb port cause errors and have to be removed or replaced. We have three hybrid vehicles in our household each with a third party telematic device from in-drive. All three have been removed and returned to in-drive with supplied packaging and return instructions.
At approximately 65mph a red triangle warning light appeared on the dash panel accompanied by a yellow circle. The parking button was flashing and a pink cartoon car appeared on the display. The car was running fine so I continued till I reached my freeway exit. At the bottom of the exit I had to stop for a red light. The exit is dark and has a sharp curve. Upon stopping the car the transmission locked and the engine shut down. I was unable to start the engine and and the car could not be pushed out of the way due to the transmission lock. Frantically I had to get a flashlight and wave off oncoming traffic. It was very dangerous. After a few minutes I retried the engine and it started allowing me to get off the ramp and get home. The warning lights stayed on. I took the car to the dealer the next day and after they tested everything they said that there was nothing wrong. Next time I might not be so lucky.

About a mile after having entered the freeway as I was merging towards the carpool lane the car shut off in the number three lane. The dashboard displayed the message, "check hybrid system", and would not respond. My speed was approximately 65 mph when this occurred. I immediately pressed on the break but the car was not slowing down as it normally does. When I finally got over to the number 1 lane, I entered the emergency lane but was still having trouble coming to a stop. I brushed up against the freeway wall in order to come to a complete stop. After switching the car off and attempting to restart it, I would get no respond and all the dashboard indicators would remain on with the "check hybrid system" displayed. This incident was terrifying as I was driving with my two year old daughter and two other occupants. The car should have displayed a warning and not simply stalled out. If this incident had taken place a few minutes later, I would have been in the hov lane and could have resulted in a devastating accident.
Driving 70 mph car began to go into high rpm, then accelerated to 74 from 70 mph. I tried to decrease speed then vehicle shut off got a long beep and message on display "check hybrid system, pull over immediately" all lights on dash were indicating failures. Coasted to safety with 5 passengers in vehicle across traffic at 9:30 pm to shoulder. Tried to restart several times then vehicle went into high rpm then engine knocked loudly and was completely disabled. Lost brakes, pedal was hard, lost power steering, difficult to maneuver. I still had 2 bars indicating plenty of fuel in car. Prior fillup was 7. 6 gallons at pump, usually get over 500 miles per tank, trip meter only showed under 400 miles since fill up so had plenty of fuel and there was no low fuel indicator. Reported all problems to dealer. They only noted the car shut down but told me they confirmed everything I said and that Toyota engineers are trying to resolve this since they are aware of this type of problem. So far dealer and Toyota have not addressed the high rpm, the acceleration, loss of electronics and hybrid system failure and told me they havenever heard of any Toyota ever having a report of a high rpm or acceleration without intent to accelerate form the driver. Vehicle is unsafe, has 4944 miles and this problem occurred about a month after purchase. Toyota still does not know what the problems are and in possession of the vehicle.

Hybrid engine suddenly shut down without warning while vehicle was traveling 65 mph on a busy freeway, causing vehicle to come an unsafe stop. Depressing accelerator had no effect. Onboard computer gave error message "check hybrid system. Stop the vehicle in a safe place. " restarting vehicle produced the same error message and still unable to shift vehicle into drive or reverse. Fuel tank full at time of incident. Vehicle purchased new 5 months prior from Toyota dealership. No aftermarket parts or modifications present. ## VIN passed ## Toyota plugin Prius 2012 ##.
I have a 2010 Prius VIN:jtdkn3du3ao216287 with 37,000 miles. The car experiences random losses of power during which the engine shuts down and does not activate no matter how hard I pump the accellerator. Once I turn off and push again the start button, the engine starts up without any problem and I can continue on my way. I have experienced this problem 5 times since September 2012. I have taken the car in to the dealer (caldwell Toyota, NJ) where it was examined and they told me the onboard computer did not record any malfunction. I am getting worried since some of the power losses happened when I was on highways at speeds over 60mph and I had to coast to a stop in high traffic areas. I would like to know whether other Prius owners have reported similar problems and whether you have suggestions on how to proceed thank you for your attention, stefano crema.
Engine shut down while entering highway. Charged for ecm update when problem has been known. Cnn reported, "a software problem is causing some Toyota Prius gas-electric hybrid cars to stall or shut down while driving at highway speeds. " a Toyota press release states:
"Toyota motor sales (tms), u. S. A. , inc. , will launch a special service campaign involving certain 2004 and early 2005 model year Prius sold in the u. S. On the involved vehicles, an issue with the program logic in the electronic control module (ecm) system may cause some or all of the following warning lights to illuminate: master, hybrid system, engine, vehicle stability control, and brake. If this occurs, the vehicle will enter a "fail-safe" mode which allows limited operation via the electric motor. However, the electrically-powered brake and steering operation will continue to function normally if this condition occurs. The electric motor will also provide enough power to allow the driver to pull the vehicle over and away from traffic. Once the vehicle is stopped, the driver may be able to restart the gasoline engine in the vehicle by pushing the start button. Approximately 75,000 2004 and 2005 model year Prius vehicles may be affected by this condition. Toyota is proactively conducting a campaign in the interest of its customers. ".

I purchased a new 2009 Toyota Prius hybrid (gas/electric) on March 31, 2009. The car worked great, but with only 490 miles on the odometer, I did a routine shut down of the car on may 20, 2009 which turned out to be anything but routine. During this event, I shut the car off using the procedure taught to me by the dealership salesman, which was the same procedure outlined in the user manual. However, at this shut down, a very dramatic power surge went through the system, jolting the engines in their motor mounts (I heard an extremely loud squeal when the engines, both gas and electric, jolted to a stop, which I assume was the motor mounts taking the force of the power surge), and moving the car violently forward stopping with the transmission lock and brake which I had set). This was a very unpleasant and un-natural event. Following this event, the car did start and operate through its functions, but the driving characteristics changed dramatically. What had been a smooth operating synergy drive system was now slightly grabby both in acceleration and deceleration, and I experienced hard clunk shifts when driving. Approximately every 15th gas engine shut down, the gas engine would diesel and have a disturbing long cycling shut down vibrating and jolting the car in an alarming fashion which is not safe. On June 2, 2009, I brought the car into my Toyota dealer service department, was told the car "can't do that" and was told the computer showed nothing wrong. On July 8, 2009, I went to another Toyota dealer service department, they said the car was rough, and tried to re-seat the engines in the mounts, which temporarily corrected the problem until a major gas engine jolting shut down occurred again. I took it in again on 24 September, 2009, and asked to have the engine mounts replaced. They tried to reseat the engines again, with no success, and the random jolting gas shutdowns on the hybrid system continue, which are a safety hazard during operation.
The engine shut down without warning in the middle of a busy street, creating an unsafe situation with cars coming up behind in a single lane.
2006 Toyota Prius gasoline engine shut off. The following light illuminated on the dash: master warning light, hybrid system warning light, brake system warning light, vehicle stability control system warning light and malfunction indicator warning light. The vehicle continued to operate at a low speed on the electric engine and the consumer was able pulled onto the side of the road. The consumer shut the vehicle off then after a few minutes started it back up. The vehicle operated normally but all the light were still illuminated. The vehicle was parked until the next day. When the vehicle was started the lights were no longer on and the vehicle operated normally. The vehicle was taken to the dealer who could not find anything wrong with the vehicle.

Reference investigation # pe05029:
my Toyota Prius 2003 (VIN # jt2bk18u330072386) gasoline/ electric hybrid vehicles that can be operated in gasoline or electric mode. On 4 separate occasions, the gasoline engine shut down suddenly without warning. Each time I was driving with the speed of highway traffic. The three previous times the vehicle continued to operate using the electric mode for a period of time after the gas engine stalled. This last and 4th time the vehicle shut down at the same time that they felt the gas engine stalled. I was able to restart the vehicle after 3 immediate attempts to turn the car on. I was able to slowly drive the car off the highways and to park. As with the last 3 times, I again had the vehicle towed. Each of the 4 stalling incidents, I was alerted by an audible sound and various warning lights. After the car stalled the brake and steering operation continued to function as the car drifted to a stop. Each time the reason was for the failure to operate was caused by something different, as I am told by the service center. This last time, (4th) the dealership said the car is "fine" asked me to pay the tow and is sending me on my way. Nothing was done. I drive about 90 miles a day with my 8 yr old daughter as my primary passenger. This is a very dangerous situation that has not been corrected by Toyota. My dealership (kings Toyota cincinnati ohio).

My 2005 Toyota Prius failed with engine shutdown at 65 mph on florida turnpike. After restarting on shoulder, vehicle would only achieve 52 mph, then failed again.
I have a 2004 Toyota Prius (purchased Dec. 2003). Approximately 10 March 2005 I was driving at city speeds (<40 mph) when the gasoline engine shut down. I was able to proceed about a mile on the electric engine until the battery became drained. I called for a tow under warranty service. While waiting, I was able to restart the gasoline engine after about an hour, but had the car towed into the dealership for investigation of the failure and repair. After a couple of days of investigation, they claimed that they could find no problem and Toyota had not issued any service bulletins dealing with such a problem. As recently as last week, when I had the car serviced, the dealership said that they were unaware of any similar reports and Toyota had not issued any service bulletins. I gave them a copy of an article from a recent wall street journal details reports of these problems around the country. I have no further problems of a similar nature, but I am somewhat apprehensive of recurrence. Otherwise the car is wonderful. I get ~50mpg in the summer and it handles well.

I own a 2004 Prius and also experienced a complete system shutdown while driving on the highway about 3 months ago. The entire gas engine shut down and all warning lights came on. The electric motor remained operational and allowed me to get off the road and into a parking lot safely. The dealer (cabe Toyota - long beach, CA) gave me a brief and unsatisfactory explaination. . Brushing this off as a minor problem similar to a computer needing to be restarted. I assume they upgraded my system but still fear a recurrence in a less convenient location. Please let me know if you would like any additional information on this. Vehicle VIN = jtdkb20u540074988.
My 2004 Prius' combustion engine shut down on the freeway in Sep'04. The car switched to battery mode only and rolled on until the battery was completely drained and then completely stalled. I got stuck in lanes, created a rush-hour traffic jam, and had to be pushed to a shoulder by a police car. The car was towed to a dealer who said it was a software glitch which they proceeded to fix and the car has been working well since then.
Our 2004 Prius on four separate occasions had a complete engine shutdown with lots of indicator lights lit on the dashboard. This happened twice while we were traveling on interstate highways in traffic at highway speeds, once in a Toyota dealership while the vehicle was being serviced, and once as we pulled into a toyoto dealership for service. We were not aware we could send a complaint to NHTSA until we say today's msnbc article. The first engine shutdown occurred in the hyannis Toyota dealership in massachusetts. They took quite a while to fix the vehicle and made several phone calls to Toyota engineers for help. They wrote on our receipt: "needs trans replacement for permanent repair. " after we left massachusetts we drove across country toward our home in California. The other three engine shutdowns happened during this trip. We had to be towed (at Toyota's expense) twice as a result of the interstate engine shutdowns and ended up at a Toyota dealership in jackson, mississippi. We had to stay several days at our own expense while they repaired the vehicle, again consulting several times with Toyota engineers during the repair. After we reached California we took the vehicle to south bay Toyota and requested the "permanent" repair described on the hyannis Toyota receipt. We were told they would do nothing because there was no indicator showing any problems at that time. We made several phone calls to Toyota but were unable to get them to do anything further to our car.
While driving vehicle shut down and would not restart. All 3 times that this happened consumer was on the freeway. Consumer owned the car for 1 year before it started stalling, and it has stalled two more times since then. Each time the car was taken to Toyota, and each they time they were unable to figure out what the cause of the problem was.
