Contining with issue referenced in 11701484. This morning (1/28/26) when leaving my home, I backed out of my driveway. Once I put the car into drive, the forward collision warning went off and locked up my car. The brakes would not release and I could not move out of the way of oncoming traffic. I do not know what allowed the brakes to finally release. I changed over to 4x4, put it into park and back to drive. My car finally was then allowed to move.

This forward collision system will engage at inappropriate times when there is no crash concern. This vehicle has routinely engaged in a highway setting locking up the brakes and forcing the vehicle to a full stop on the highway with vehicles at speeds of 60+mph causing a grave safety risk and an immanent risk of serious injury or death from a serious highspeed crash. This software simply does not work and is not designed and programmed properly. It should not be on the road and shouldnt be used in a public setting. Each instance of my safety concern there was not a vehicle withing the proper distance that a crash was going to occur.
The front camera lens cover — located in the grille below the chevy bowtie logo — has developed a spiderwebbed/shattered crack pattern. There is no impact damage to the surrounding grille or bodywork. The vehicle was garaged and not in use when the damage was discovered. The camera appears to still be working and parts of the image are crystal clear but the cracks in the cover distort the picture and make it impossible to use reliably. This is consistent with a material defect, not road damage. I have opened a case with gm customer care regarding this defect. My case number xxx. Multiple safety-critical malfunctions have occurred on this vehicle involving unintended braking, unintended acceleration, and false activation of driver-assist systems. These events did not happen at the same time and occurred under normal driving conditions. At various points, the vehicle has suddenly applied the brakes without driver input, including hard braking events that felt like an automatic emergency braking activation when no hazard was present. In separate incidents, the parking brake has engaged while the vehicle was in motion, causing abrupt deceleration. There have also been instances of unintended acceleration where the vehicle surged forward briefly without additional throttle input. These malfunctions appear to be related to electrical or adas system faults affecting braking logic and propulsion control. No warning lamps or messages reliably appeared before these events. The issues pose a serious risk to the safety of the driver and others because the vehicle’s behavior is unpredictable and can interfere with safe control. The dealership was unable to reproduce the conditions during inspection, but the incidents have occurred multiple times in real-world use.

Windshield has terrible glare and reflects the dashboard as to limit visibility. Front collision and auto braking including rear collision and braking are a safety hazard as they create hard braking events for no reason. Car will stop sudden for no reason. These are both serious safety issues. These are problems we have had with the car since purchase and have had the dealership shop work on the problem and were told just to leave it turned off. This is not the correct way to fix a problem with a vehicle.
The first incident started on 08/16/2024, 2 days after buying the vehicle used from dealership. I started getting notifications stating "service-front camera not available" and sure enough, I no longer had a front camera, which then means I no longer have front collision alerts. Then I started getting the random notifications to drive carefully because drivers assistance has been reduced, and now my vehicle's fan will randomly run for up to 30 minutes after it has been shut off and even after a less than 2 mile drive to gas station and back home, it does this. Last item is the vehicle pulls to the right, and I have not hit anything nor has it ever been in an accident prior to my purchase, and my brakes squeal horribly, which shouldn't be happening condidering I haven't even driven the vehicle 5,000 miles in the year since I bought it !.

On the evening of 6-12-24, I was making a left hand turn onto m-59 from crescent lake rd. I was the second car in line on the left turn green arrow. The first car went to the right lane. I went into the left lane. My car immediately flashed the light in the front window, the alarm went off, and slammed the brakes as if something was in front of me. Nothing was in front of my car in the left lane. Luckily there wasn't a car on my rear bumper otherwise I would have been rear-ended. This is the 2nd time at the same intersection making the same turn that my Blazer has done that.
The radio and all the sound notifications such as turn signals, emergency flashers, crash avoidance, cross traffic, lane departure, and the use of the emergency onstar system do not work. This has been addressed in gm service information #pit5971b: loss of audio and onstar led is red - (Apr 3, 2023) gm appears to be addressing this as an inconvenience of not having the entertainment of the radio and not the safety features associated with it. This also includes the use of hands-free cell phone.
