1 Equipment problem

Failure Date: 04/24/2026

Pulling into our parking lot of apartments. After picking car up from dealership for a different problem. The “service high voltage” light came on. Turned car off restarted. Same issue.

2 Equipment problem

Failure Date: 04/17/2026

Sometimes when I am backing up, the breaks will slam on. Back in 2025 first I noticed, but thought it was human error, just wasn’t sure. After it happened more than a couple of times in 2026, I realized it wasn’t me and must the safety warning system? I googled after it happened this month and (phantom breaking) popped up for the chevy Equinox. I have an appt. With Chevrolet to check it out in 2 weeks. Scary when it happens. I’ve only noticed it so far when backing up. If it were to happen other than in a parking lot, that could be dangerous, especially if I am going faster.

3 Equipment problem

Failure Date: 03/25/2026

The navigation system and the one-pedal driving system started malfunctioning. The navigation system began displaying a different location during remote driving, causing it to send me down a street against traffic. This issue has never been resolved and continues unabated. Then, while on the highway, when I needed to slow down, the one-pedal driving system deactivated, causing me to almost collide with another vehicle. This has happened more than three times this month. Gm and Chevrolet just keep saying they'll call me back to resolve the issue, but they never do, and they refuse to fix it.

4 Equipment problem

Failure Date: 03/22/2026

Notice the windshield was cracked after leaving a restaurant!.

5 Equipment problem

Failure Date: 03/01/2026

Driving down the road with my mom and my panoramic sunroof randomly exploded sending glass through the car. This was a very dangerous situation. Is the glass defective ? this should not happen and is a safety concern.

6 Equipment problem

Failure Date: 02/20/2026

The telematic control module drains the battery. I bought the car in February 14, 2026 and it was towed to the dealer on February 20. Today is March 17 and I have had no results. I opened a case with gm directly but I receive nothing but canned automatic emails. The dealer isn't taking my calls. Please help.

7 Equipment problem

Failure Date: 02/11/2026

While driving, a burning odor developed followed by visible smoke entering the passenger cabin from the dashboard/hud area. We pulled the vehicle over and had it towed due to safety concerns. Dealer required dashboard disassembly to investigate potential electrical/fire condition but was unable to replicate the issue.

8 Equipment problem

Failure Date: 01/17/2026

Onstar module failure. This did not trigger any type of notification. I had to make several phone calls to onstar to determine I would need to schedule a service appointment for a technician to look at the vehicle. After inspection it was noted that the onstar module is not working and will need to be replaced. Upon ordering the part I was informed it is on backorder with no delivery time noted. If I were to get into a crash onstar would not have triggered a call to first responders for my safety - which is a service that I pay for. No warning lights appeared and the the onstar button still shows "green" as in active service.

9 Equipment problem

Failure Date: 01/16/2026

In the Equinox lt, VIN [xxx] , the telemetry module has an issue per the dealership. The vehicle loses power due to the telemetry module issue. The part is on order and as of January 16, 2026. It will take 4 to 6 weeks to arrive. In the mean time I have had to call roadside assistance twice in two weeks, and the vehicle has been jumped 3 times in the past two weeks. However, the vehicle has been to the dealership 4 times. Once prior to the diagnosis, and three times since the diagnosis. The vehicle has approximately 16, 600 miles, and is driven to and from work. The telemetry module is the cause of the over all issue, per the dealership. The battery continues to go dead on vehicle [xxx] , 2025 Chevrolet Equinox. Information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).

10 Equipment problem

Failure Date: 01/14/2026

Just after thanksgiving, my vehicle displayed a “service emergency calling” message. I brought it to the dealership and they documented the concern, referencing gm bulletin pip6077b, but no repairs were performed. On December 30th, while parking, I shifted into reverse and the vehicle would not reverse and instead moved forward. A “service transmission” warning appeared and the vehicle was towed. The dealer cleared the codes and released the vehicle without performing any diagnostics. About two weeks later, when starting the vehicle after work, several warnings appeared, including “service safety restraint system,” “forward collision system unavailable,” and a rear-seat alert despite no passengers. While driving, lane keep assist, lane departure warning, and blind zone steering assist were not functioning. The next morning, the vehicle suffered a complete electrical failure. It would not start, the screens stayed off, and the doors would not lock or unlock. The vehicle was towed again. The dealer replaced the battery, claimed it had simply died, and attempted to return the vehicle to me. I refused because none of the prior warnings were diagnosed, and I was told the codes “erase when the battery is changed. ” after I demanded full electrical and transmission diagnostics, including a parasitic draw test, the dealership informed me the vehicle was unsafe to drive. They stated the onstar/telematics module was draining the battery and the replacement part was backordered with no estimated arrival date. I was told multiple customers are waiting for the same part, which means this is not an isolated event and needs way more attention than it's being given. This defect results in loss of essential safety systems and complete vehicle shutdown, creating a significant safety risk.

11 Equipment problem

Failure Date: 01/13/2026

After hitting a deer, the body shop fixed the damage to the front part of the vehicle. They realized the battery kept being drained even after purchasing a new one. It’s now at a dealership and they are staying it needs a new onstar module, not sure when it will come in either. I’m paying for a rent car that I can’t afford to pay for anymore. I wanted them to just do away with the onstar but they said it’s not possible.

12 Equipment problem

Failure Date: 01/07/2026

On star module failure with severe backorder on parts.

13 Equipment problem

Failure Date: 01/06/2026

The onstar telematics module has stopped functioning. That means I am not receiving any systems updates, cannot use onstar, and cannot use the my Chevrolet app for remote start or remote unlock. I took it to the dealer 2 weeks ago and they said the telematics module is backordered. They did do a number of system updates. After those updates my adaptive cruise control has completely stopped working. When I called about that, the service department said that one module could have caused the other module to fail and that they won’t try to look at my cruise control until after they can replace the telematics. There is currently no eta to receive the part.

14 Equipment problem

Failure Date: 01/05/2026

My vehicle has had recurring interior water intrusion and related safety concerns. The most serious symptom was interior window fogging on the inside of the vehicle for multiple days during or following rainy, snowy, and cold conditions, which significantly reduced visibility while driving and made it difficult to see out of the windshield and side windows. To clear the fogging quickly, I have had to turn off the heated seats and defrosters and open the windows; otherwise, I have needed to pull over and wait until I could safely see through the windshield again. A strong mildew odor would also develop and could last for days after rain. Sometimes the seats felt damp and the odor became worse when the heated seats were turned on. The first repair visit for this issue was January 13 to 19, 2026 and during that visit, the dealer was unable to recreate the issue. The problem continued, and after my navigation/infotainment screen began having issues, I returned the vehicle to the dealer again on March 7, 2026. I was then informed there was water intrusion and water-damaged carpeting, and I was told there was a known issue involving a faulty or broken seal on this model and there was a recall, but I have not been able to confirm that recall by VIN online. It remains unclear whether the leak is the full cause of the repeated interior fogging condition. In addition to the water intrusion issue, the vehicle has also had other electrical and safety-related concerns, including the infotainment screen turning white for weeks, abnormal turn-signal behavior after completing turns, lane-assist steering irregularities that pulled the vehicle toward the lane line, and braking irregularities, including the brakes locking up while backing into a parking space or during slow stop-and-go traffic. It is currently unknown whether moisture intrusion contributed to any of these other issues. My vehicle is still with the dealer for repairs until around April 15.

15 Equipment problem

Failure Date: 01/01/2026

2025 Chevrolet Equinox had, on numrous occasions, complete power failure. The vehicle had to be jump started more than once. The vehicle was brought in to the dealership, by the owner, with power failure concerns. The vehicle was towed to the dealership due to complete power failure concerns. The vehicle had brake pedal issue when power failure occurred. Meaning the brake pedal was not able to be pressed. Display signals: erroneous date/time, abs, check engine, vehicle drive mode unavailable your vehicle may need service. Furthermore, on one ocassion a power surge was detected. The vehicle was diagnosed and an advisor stated the telemetry module is going out. However, the part that was replaced in the 2025 Chevrolet Equinox, several weeks later, was actually the telematic control module. The vehicle displays drive mode unavailable your vehicle may need service, even after telematic control module replacement beginning the next day after part replacement. The message displayed by the 2025 Equinox occurs on a daily basis. Issues are October 2025 to present day.

16 Equipment problem

Failure Date: 12/20/2025

The vehicle became completely disabled due to a sudden battery drain at approximately 5482 miles. I purchased the car with 2553 on it in may. The vehicle would not start a few times and had to be towed to the dealership on Friday Dec. 26, 2025. The dealer confirmed that the onstar/telematics module is malfunctioning and continuously draining the battery. The defective module is on backorder with no estimated arrival date, leaving the vehicle unusable. They also do not know if they have a loaner for me. This creates a safety risk because the vehicle can become inoperable without warning, potentially stranding the driver. The issue has been reproduced and confirmed by the dealership. Prior to the failure, the onstar system stopped functioning, but no warning lights appeared on the dashboard. My chevy app stopped working 26 days ago as well. I now have a loan payment to make for a car that isn't even working and brand new. Dealership doesn't know a timeframe either.

17 Equipment problem

Failure Date: 11/27/2025

My Equinox ev telematics module does not work. Prevents the use of navigation features and onstar navigation features.

18 Equipment problem

Failure Date: 11/06/2025

When I started my car there was a very loud vibratory sound that caused my car to shake. When I accelarted the noise quite down but it was still there. When I stopped in traffic the sound reappeared. This occured for 2 hours ths length of my commute. When I made it to my destination. I lifted the hood. Idk what I was looking for. This morning the trembling is now light but the noise was not.

19 Equipment problem

Failure Date: 11/02/2025

Returning home on I-40 on Sunday, the engine just quit working. Fortunately it was Sunday and the traffic was light. Had there been more traffic, someone probably would have hit me from the rear. We managed to get it almost all of the way off the road before it stopped. According to the dealership that worked on it, it was the fuel supply service module, $46. 09 part. The service technician said they had been having a lot of problems like this and knew what to do. All of the code were lost because to tow truck driving had to take the leads off of the battery. That was the only way he could get the car out of park in order to tow the vehicle.

20 Equipment problem

Failure Date: 11/01/2025

The vehicle’s display screen intermittently goes black, rendering the rearview camera inoperative. This failure significantly impairs rear visibility and creates a safety hazard while reversing.

21 Equipment problem

Failure Date: 11/01/2025

The vehicle’s heating system operates intermittently or takes an excessive amount of time to produce heat, reducing cabin visibility and comfort in cold conditions and creating a potential safety concern.

22 Equipment problem

Failure Date: 10/30/2025

Our 2025 Equinox ev was leased new in March of 2025. Unknowingly, it had a water intrusion in the front passenger floor due to a manufacturing defect that gm is aware of. This is related to TSB 25-na-149. The water intrusion was covered up and went unnoticed for some time due to the factory gm all-weather floor liners. Once discovered due to the musky smell in the vehicle, black mold was found after pulling up the floor liner. The vehicle was inspected by a Chevrolet dealer, who confirmed it was due to a poor body weld that has now been resealed. Replacement carpet is on backorder due to many experiencing this defect. Our safety and the safety of others is put at risk since gm is not recalling the vehicle for this defect. We have been unknowingly driving around breathing in black mold. As someone who is immunocompromised with an autoimmune disease, this is especially troubling to discover. My autoimmune disease was sucessfully managed with medication since 2017, however, shortly after leasing the vehicle in March I began to experience flare ups. I have been unable to determine why, but this discovery makes me believe it was due to the mold in the vehicle. From discussing with my doctor, mold exposure is a potential trigger of my condition.

23 Equipment problem

Failure Date: 10/01/2025

Unknown. The roof is separating along the groove line causing rust on my 2025 chevy Equinox.

24 Equipment problem

Failure Date: 10/01/2025

Onstar module isnt working and was told to report it to you guys.

25 Equipment problem

Failure Date: 09/26/2025

Over-torqued lugs, broken wheel studs, safety issue.

26 Equipment problem

Failure Date: 08/20/2025

They have the gear shift stick near the wheel like older models. You have to brake to move the gear shift and push a button in on the gear shift to park. However, when driving you can move the gear shift, which I have done accidentally. The gear shift does not lock while driving. Twice the gear switched to l2, which I do not know what that is but it causes my engine to over work. This is dangerous especially trying to switch the gear back to d when at a speed of over 50mph.

27 Equipment problem

Failure Date: 08/18/2025

I leased this vehicle brand new. At around 500 miles, the vehicle had occasionally a humming noice, at 1900 miles the humming noice is loud with vibration. My suspicion is a structural defect, my fear is to end up with half a vehicle on one lane, the other half in the other lane at the expressway. I have recorded several of the occurrences, provided a gm dealership with the recordings, contacted gm executive offices, and have not been able to resolve or advance, meanwhile my household is without a vehicle. Neither gm nor the dealership is willing to provide a courtesy vehicle. The dealership was unable to duplicate my complaint, gm executive office agent recommended me to take the vehicle to a different dealership for a "second opinion". This is depriving me of work, transportation and giving me additional work.

28 Equipment problem

Failure Date: 08/08/2025

At certain times of the day and depending on the lighting, the dashboard of my vehicle, has a very significant reflection that interferes with my ability to see through the windshield clearly. This puts myself and those in my path at risk of injury or death. I have contacted the person who sold me the car but he has not responded. I have read numerous online accounts of the same concern. Gm is aware of the issue and chooses to do nothing.

29 Equipment problem

Failure Date: 08/01/2025

Telematics module stopped working out of nowhere and drains my battery to my vehicle. Had to have the vehicle towed to nearby dealship. Huge safety hazard as it is essentially the brains to my computer. App doesn't work without it, battery doesn't work and can't even get inside vehicle with key fob. Please issue recall.

30 Equipment problem

Failure Date: 07/29/2025

What component or system failed or malfunctioned, and is it available for inspection upon request? the air conditioning (ac) condensate drain system is either blocked, misrouted, or improperly sealed, causing ac condensation to pool and leak into the front passenger-side footwell through the factory floor sealant. The vehicle is available for inspection upon request. How was your safety or the safety of others put at risk? the continuous leak has caused significant mold growth on the interior insulation behind the passenger footwell, creating a potential health hazard for occupants, especially those with respiratory conditions or allergies. Prolonged exposure to mold inside a sealed cabin environment poses a serious risk to occupant health. Has the problem been reproduced or confirmed by a dealer or independent service center? the vehicle was brought to a dealership for this issue, but the problem persists. I later reproduced the issue myself by running the ac for 30 seconds, after which water visibly seeped through the floor seam. Mold growth was also confirmed behind the interior trim. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? the dealership previously inspected the vehicle but failed to resolve the issue. No other parties (e. G. , manufacturer representatives, police, or insurance) have inspected the vehicle at this time. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? there were no warning lights or messages. The only symptoms were damp carpet in the front passenger footwell and a persistent musty odor, which appeared prior to discovering the mold and leak source.

31 Equipment problem

Failure Date: 07/22/2025

Discovered water underneath passenger footwell carpet, causing a musty and mildewy smell. Many reports of similar discoveries online prompted the initial search. Safety concern is mold growth and rust. Dealer was unable to replicate issue after two attempts (ran through car wash, hose over windshield, ran a/c for extended period, checked ac condensate drainage). Dealer dried up water and carpet and told owner to monitor. Owner pulled up carpet to do own investigation after the second failed attempt by the dealer. Confirmed dry before starting investigation on warm sunny day (no rain). Owner found leak in passenger footwell underneath black access plate that only appeared with a/c usage and during/after driving, not while car was stationary. Owner brought vehicle back to dealer. Dealer removed black access plate and confirmed leak was coming from a compromised body seam weld. Dealer repaired the failed body seam and no water intrusion has been seen by owner since repair was completed. Owner still has concerns about underlying cause of where water was coming from - theory is that condensation is forming on the the a/c lines and is pooling somewhere in the body then sloshing around while the car is moving, causing further potential for water related issues, including rust.

32 Equipment problem

Failure Date: 07/13/2025

Telecommunications module is out. No timeline on parts. National issues. All safety features malfunction.

33 Equipment problem

Failure Date: 07/06/2025

Test drove and bought the car on an overcast day. Next day is bright and sunny - the dashboard glares like a mother. What moron designed this picasso? did gm r&d even drive the car? the safety hazard is you can’t see the road through the dashboard glare. I’d say that puts myself and others at risk. Should’ve known not to buy american.

34 Equipment problem

Failure Date: 07/01/2025

The glare on the windshield and side mirrors from the dashboard/vents is unsafe. When driving during the day, I’m unable to clearly see out of windshield and side mirrors, views heavily obstructed by the glare off the interior surfaces of my car. Tinting the windows hasn’t helped the situation at all. Driving with the windows down, helps with the wide mirrors but the windshield glare is so fucked. I wouldn’t have bought this damn car if I test drive it during the day. How does this pass inspection? these cars shouldn’t be on the road in their current state.

35 Equipment problem

Failure Date: 07/01/2025

Nhtsa safety complaint (loss of emergency connectivity) subject: 2025 Chevrolet Equinox ev — onstar/telematics failure disables emergency services & automatic crash response; part on national backorder VIN: [xxx] make/model/year: Chevrolet Equinox ev, 2025, rs trim mileage at first occurrence: 4000 date problem first occurred: [xxx] frequency: continuous (services remain unavailable) injuries/crashes: none to date component(s) affected: electrical system — telematics/communication (onstar); emergency calling/sos; crash notification (acr). Description of the safety defect: the vehicle’s onstar/connected services display “services unavailable. ” the dealer diagnosed a failed telematics module. Replacement is on national backorder with no estimated timeframe. While in this condition, emergency services and automatic crash response are disabled—the sos button does not connect, and the vehicle will not automatically contact onstar after a crash. Parts availability (scope/delay): my dealership and multiple gm/Chevrolet and onstar owner forums indicate 3+ month waits for the telematics module with no eta, leaving vehicles without emergency connectivity for extended periods. What has been done so far: dealer confirmed module failure and opened a parts order; advised no eta due to national backorder. Manufacturer has not provided an interim fix or loaner vehicle. Issue remains unresolved as of Aug 11, 2025. Why this is a safety concern: loss of emergency calling and crash notification defeats safety features marketed as critical for post-crash response. Owners cannot rely on the vehicle to summon help after a collision or medical emergency. Requested NHTSA action : track this failure and parts unavailability, assess scope, and determine whether a safety investigation or recall is warranted. Information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).
