On may 9, 2026, while pulling into a parking garage parking space at low speed (below 5 mph), the brake pedal was pressed but failed to engage or slow the vehicle. The pedal was pressed completely to the floor with no resistance, and the vehicle continued moving forward until it struck a wall, causing damage to the vehicle. Earlier the same day, the brakes produced an abnormal hissing sound while pulling out of a parking space. No other symptoms or abnormalities were present prior to failure. The vehicle may also have recently received a software update, although it is unknown whether that is related to the incident. The failure appears to involve the brake system and the driver safety assistance systems equipped on the vehicle like forward collision alert, automatic emergency braking, and dynamic brake support systems. The exact failed component is currently unknown and remains under inspection by a Chevrolet dealership. No warning lights, messages, or audible alerts appeared before, during, or after the incident. The automatic braking/collision avoidance system also did not activate. Because the vehicle had no braking capability at the time of the incident, the occupants and nearby pedestrians were placed at risk of serious injury. On may 10, 2026, the vehicle was taken to a Chevrolet dealership for inspection. The dealership has been unable to reproduce the issue, and no mechanical defects or diagnostic trouble codes related to the incident were identified. On may 13, 2026, the issue was escalated to the gm technical assistance center (tac) for further review, and the vehicle remains at the dealership awaiting a response to the tac case. The dealership acknowledges similar brake-loss concerns referenced in gm service bulletins involving the 2025 chevy Equinox and other models, including bulletin n24471431, although they stated the bulletin does not apply to this specific VIN.

Sometimes when I am backing up, the breaks will slam on. Back in 2025 first I noticed, but thought it was human error, just wasn’t sure. After it happened more than a couple of times in 2026, I realized it wasn’t me and must the safety warning system? I googled after it happened this month and (phantom breaking) popped up for the chevy Equinox. I have an appt. With Chevrolet to check it out in 2 weeks. Scary when it happens. I’ve only noticed it so far when backing up. If it were to happen other than in a parking lot, that could be dangerous, especially if I am going faster.

I bought this vehicle brand new on January 18th 2025 and it has been into the dealership 3 times now for a backup camera malfunction. My rear traffic alert or any of my other safety features when backing up are not working. On several occasions the entire instrument display including my speedometer and everything has completely blacked out.
Infotainment center is not operable. Had a diagnostic test completed by certified dealership and it is found that the infotainment center and radio is not operable. Contacted general motors corporate office. $47 was offered to go towards the $950 to fix it it. They did not offer any reimbursement for the $204 paid out of pocket for diagnostics fee that I they advised me to get after filing complaint. Due to the malfunction of the infotainment center and radio other safety issues have stopped working. Such as the onstar safety feature, the phone in case of emergency, the turn signals and also the safety features on the rear and side view mirrors, car length warning, and back up camera. Was advised from the dealership that all of these functions have ceased working due to the failure of the infotainment center and the radio. Unable to hear any warnings. Made a complaint with ohio attorney general's office and general motors' response was only to provide me 5%, which is the $47 towards the dealership invoice. There reason was that due to the mileage they cannot assist further than that. Its not about entertainment of the radio. I feel this is a safety hazard on so many levels, and it is not caused by simple wear and tear or any fault of the driver. This is a safety issue that I want addressed. This should not be a financial burden that the consumer should carry by themselves.
Vehicle was being parked in garage when it accelerated in reverse, on it's own, very quickly. Fortunately I was able to avoid colliding with any thing behind me by braking in time. I then shifted into drive when vehicle accelerated forward, on it's own again, very quickly. I was not able to brake before vehicle crashed into concrete garage wall, severely damaging front end. Vehicle is still under warranty, and I intend to have dealership haul it in for repairs. I have found other drivers complain on the internet of experiencing the exact same issue as I described above. This model, Chevrolet, Equinox, 2025 is not safe to drive!.
The infotainment center failed completely and does not power on. Because of this failure, I cannot see the front, rear, or side cameras, and there are no audible warning sounds for parking, lane safety, or seat belt alerts. Essentially, none of the safety features connected to the infotainment system are functioning. This failure puts my safety and the safety of others at risk because I cannot see camera views when backing up or parking, and I receive no audio alerts or warnings for seat belts, obstacles, or safety features.

The park assist light keeps coming on and should have been a recall for this item I was told by the dealership last year and they supposedly ordered part. I never received a call and now light is still coming on due to not showing any open recalls and item was never repaired.
I have noticed my car does not signal to me when I'm getting to close to something in my backup camera. In other words I backed into someone's garbage bin while reversing. I was going very slow. This is a problem for me as I don't feel the camera is very accurate. I was watching the camera and listening for a beep with no music or air going because I had noticed I hadn't heard the thing beep before. I was only in the driveway but this could have caused an issue if I was parallel parking on a side street in town.
