At a stoplight the fcw ( forward, collision), warning dash light went on and engaged the brakes, thankfully, no movement was taking place. I had to put the car in park and then drive to be able to resume. This indicates that it could have occurred at high speeds, potentially causing a collision.

Component failute: first the cruse control would stop working while driving. Then the engine would rive way up while trying to accelerate and not gain much speed, gas mpg dropped. When breaking to stop I had to push the break pedal harder. When the cruse had stopped and I shut the engine off, it was hard to start, and would bump and shake as if it was hitting on 2 cylinders, cruse still would not work. After I made it home, the next time I started it, it was ok, then the last time I parked it the brake will not depress and it will not start. Battery has 12 volts, jumped it anyway break still want depress. Took battery cables off shorted them together , did not help. Break pedal still will not depress.

I was recently on a trip and I was traveling i39 in WI at 70mph there was a semi next to me on the right side and another semi in front of me approximately 5 car lengths when all of a sudden the the brakes applied with the dash lighting up saying brake in big red letters this braking action happened for about 2 seconds this was a very dangerous situation as there were vehicles behind me.
Parksense unavailable. While reversing we backed into another vehicle. Normally, the parking brakes if we get to close or at least a warning that we are approaching a potential hazard.

1. This vehicle was recalled for a software update due to the rear camera failing to operate at times. The software was updated by the dealer. However, this did not completely fix the problem. While the camera fails less frequently, it still does not energize at times. I took the vehicle back to the dealer. I was then informed that there is water intrusion into the rear lift gate, which causes the electrical to short out. I was told that I would have to replace the wiring harness which energizes the camera as well as the rear vehicle lights. I have not noticed any problem with the lighting as of yet. However, I have been told that this is a problem with the Pacifica in general, as well as the durango. I was told that replacing the harness would not guarantee that the problem would be solved as there would still be water intrusion, which could cause the problem again, even with replacing the harness. This is obviously a safety issue, as the camera fails to operate at times, and there is a potential of losing lighting on the back of the vehicle. This is also a known defect by Chrysler. 2. When using the adaptive cruise control on streets and highways with multiple lanes traveling the same direction, the vehicle will brake suddenly and hard when the vehicle in front of me changes lanes. This occurs after the vehicle in front of me has completely changed lanes and is not impeding my forward progress whatsoever. My vehicle speeds up to the set speed and then violently brakes as if it is about to crash into the vehicle ahead of me, even thought the lane is completely clear. The vehicle has been inspected for this scenario. I was told that it didn't perform as described above when the technician drove it. This is a safety issue as the vehicle suddenly and violently brakes at highway speeds, with traffic behind me.

In may 2025 shortly after purchasing this vehicle I had complete brake failure while driving down the road. I was driving approximately 65 mph when the forward collision warning, abs warning, brake warning, auto emergency brake warning lights came on and when I pushed on the brake pedal it was hard as a rock and wouldn't stop the vehicle. It was returned to the purchasing dealership, and they replaced the brake booster. On November 27th of 2025 I was driving down the road and the exact same problem occurred with a complete loss of brake function. It went to the local Chrysler dealership; they replaced the master cylinder. I picked it up 12/10 drove it 5 miles home and parked it. I got in the vehicle the next morning to go to work and on my way to work all the same dash lights came on and I had a complete loss of brake function. It was towed back to the dealership again; this time they replaced the abs module. I picked it up on 12/30 drove 5 miles home, parked it, tried to leave 3 hours later and couldn't push the brake pedal far enough to get the van started. When I did finally get it started all the same dash lights were on and the brakes were not functional. It was towed back to the dealership again and this time they replaced the wiring harness. I picked it up 1/16. Drove it minimally over the weekend, on 1/19 the front-end collision warning lights came on and the check engine light came on while driving 70 mph. The brakes didn't lose function this time, however. The following day I was sitting in line at an elementary school waiting to pick up my daughter when all the same dash lights came on again and I had a complete loss of brakes, it was towed back to the dealership, where it still is. There was contact made with Chrysler engineering during the 2nd repair. The dealership has been able to recreate the problem more than once since they have had it. This put myself and my children a significant risk of injury, and other cars if there was an accident.
At random, the dashboard will light up with a red brake light, the check engine light, front end collision warning, and the abs warning light. It gives me several messages as well. Service electronic stability control, auto emergency braking active braking off, auto emergency braking unavailable service required. When this happens the brake pedal is impossible to push and it takes every ounce of power to get the vehicle to stop. This is no warning that this is coming and it happens most often when you are driving. This puts myself and my family at risk as well as everyone that is driving near me when this happens. It has been repaired 6 times by 2 different dealerships, and it still isn't fixed.
[xxx]- bought new 2025 Chrysler Pacifica w/ 29 mi at troncalli Chrysler Dodge Jeep (tcdj). [xxx]- 1st time -I was driving the car when it made a “clunk” sound when turning left. All the lights on the dashboard came on and the abs system turned off. The car safety features disabled and it did not recognize we were in the seats (seat belt lights came on). I was able to drive into a parking lot. Turned the car off and then restarted it after a few min and the lights on the dash were gone. Decided to cont driving, figured it was a computer error. Went 2 mi and had to turn left again. All dash lights came on including engine, abs. Pulled over and lights turned off after car was turned off/on again. Tried to get the car home. Turned it on and no alerts. Drove <1mi and the car began to stutter and dash lit up. Lost the ability to accelerate, and the steering seemed stiff and hard to control. Lost abs system. Pulled over into parking lot barely moving. Had to leave the car in the lot and have it towed to tcdj 3/29- towed to tcdj service depart. . Total miles on car:283. 3/31- tcdj states it's the batteries that caused the malfxn. Explained I did't think the batteries were the issue. They replaced both batteries. [xxx]-serious safety issue. Picked up car @ tcdj. We did not make it home from tcdj. Trip from tcdj to home is ~27 mi. At mile 22, turned left and ~0. 5 mi later, while driving ~ 15-20mph, the car shifted on its own, throwing itself into park at a major intersection. All of the dash lights came on. The emergency vas, abs system and shifter disabled displayed on the dash. The windshield wipers turned on at high speed. The head and tail lights began to flash. The car shifted so abruptly w/o warning we were almost hit from behind. It did not slow to a stop or brake it just threw itself into park. Police were called. Car wouldn't turn off or move. Could not put in neutral manually. Still @ tcdj now >30days. Alternator and abs replaced. Continues to break and need a tow when tcdj test drive it. Information redacted pursuant to the freedom.

My Pacifica completely shut down. All the warning lights came on such as crash detected, abs light, and the windshield wipers came on. The vehicle was towed to the dealership that I bought the vehicle from. They completed a hard reset of the vehicle, and I took it home. Two days later it did the same thing all the lights came on, but it allowed me to drive it to the dealership. Still waiting to see what they think the problem is this time.
During 3 separate drives (october11, 2024; December 30, 2024, and January 2, 2025) the vehicle shut off while going uphill on an interstate while the engine was at 3,000 rpm. When I pulled over to the side of the highway and stopped the car, the emergency parking break automatically came on. I had to restart the vehicle 2 or more times in each instance. On January second, the vehicle shut down 5 separate times, some on interstate 70 and some on us route 40, all while going uphill between 2000 and 3000rpm. I took it to a local Chrysler dealer January 8th, but they could not recreate the problem or explain why the vehicle was shutting off with no warning.
Hello. Writing to you about a serious safety concern on my 2023 Chrysler Pacifica hybrid. Overnight, I had it parked in my driveway, and this morning, I found it had rolled down the driveway. In addition, because it was plugged in charging overnight, the movement of my van rolling, caused it to pull on the cord and ultimately snapping my charging port. Fortunately,no one was walking at that time as you could understand the concern this would be if this had been the case and my van would’ve harmed someone. I checked my cameras, and it shows the van beginning to slowly roll on its own around 9:40pm until it reaches the end of my driveway. I believe the only reason it did not go any further is because our sidewalk is a bit raised where it meets the end of the driveway, which stopped it from sliding even further than it did. I would like to see if this is a known issue and if not make it aware as this is a serious concern to the public. Also what steps can I take to Chrysler to fix this issue, I don’t feel comfortable now with the car sitting idle in the driveway with kids playing. Also corrected steps to repair the damage to my car. I contacted the dealer and have an appointment scheduled for Friday 12/27/2024. I just purchased my 2024 Chrysler pinnacle hybrid on may 8 on Monday the 13th I was driving and the lane sense and service lane sense went off telling me to keep both hands on the wheel they were it pulled the car to the left and then stopped it was scary, yesterday while driving with cruise control, nothing around us coming back from an appointment on with granddaughter all the sudden cruise control stopped working and the symbol aeb turned off, also service automatic emergency braking needed servicing, adaptive cruise control (acc) fault I do not have pictures of the first incident as I was driving I had my granddaughter take the picture of the second incident.

It’s the same issue that the hybrid Pacificas were recalled for. It pops up the random stop safely on the dash & then you’re halting to a dead stop & the vehicle shuts off. This has happened several times.
My vehicle now 3 times just stops on me luckily 2 of the 3 times I was going maybe 10 mph. Only once I was going 35 and it just slams shut I am being told they are no active recalls however this is a problem and going to cause an accident when this happens my drive light on the dash is flashing as well as on the knob to put it in to drive. This is just one of the many problems with the vehicle the transmission constantly jerks you around. The sliding doors have a mind of their own. I am a single mom of 4 and I purchased this car almost bran new and I only have 3000 left to pay on it and I can barely drive it I dont even like going around my neighborhood in the vehicle for fear it will just stop. It has also left me stranded when everything just stopped working in the car and that turned out to be a recall that I was notified about til a few weeks after it left me and my kids on the causeway bridge. And before that alot of time my car wouldnt even start it was something with the battery for starting the vehicle. Chrysler should be sued for making this vehicle it is horrible I got it to have a reliable car for me and my kids and now im paying almost 500 a month for my car to sit in my driveway because no one wants to ride in it.
The uconnect 4 infotainment system is consistently rebooting every 2-3 minutes, leaving several safety measures for the vehicle from being used during those times. This leaves risk that the safety feature is not usable when needed for example rear warning for small child behind vehicle is not operable. As well the navigation system is not working appropriately and has the vehicle always located at a street that I am no where near. This leaves risk that it will be a distraction to driver to locate their whereabouts in unfamiliar areas. Also not able to use hands free system for mobile device when the system is continuously rebooting as such and therefore would not be able to make call for emergency assistance when needed. Fca us llc has been provided many complaints about this yet continues to ignore that this is a wide spread issue and is nationwide. Therefore they fail to extend the warranty related to the uconnect 4 infotainment system and render thousands of people having to cover the cost of their faulty uconnect product.
Week of 6/18/23 vehicle began flashing codes- service electronic stability control, lanes sense unavailable service required, awd system unavailable and several warning lights appearing on dash. Took to dealership replaced star connector. Still same issues. Took to another dealership replaced abs module. Still same issues. Chrysler engineers are guiding dealership on how to proceed but neither the dealership or Chrysler engineers know what is wrong with the car or how to fix it. Has been at 2 dealerships since June 23, 2023 and still no fix. The only repair documentation I have is from the first dealership. The dealership it has been at since June 29,23 said they cannot provide me with a document of the work they have done until the service repair order is closed. That won’t be closed until the car is fixed. But they can’t figure out the issue.
Driving the car and the engine shut off on me. Brakes didn’t work. Steering didn’t work. Just coasted on a busy highway to safety. I’m lucky I’m not dead. No warning it would happen.
Exiting I-75, vehicle initially slowed but then accelerated while braking. Result was a collision with vehicle on exit ramp. Vehicle is still at collision shop. There was no forward collision warning. Air bags did not deploy despite impact that resulted in radiator being punctured by hitch of pick-up truck.
My 2021 Pacifica phev has 16,200 miles on it at time of incident on 10/31/2022. I had just pulled off the highway to get gas when the issue began. I received a “service electrical system” on the dash and a check engine light and I had to turn off and back on to get it to restart. The check engine light stayed on and upon continuing my drive home another warning light came on telling me that the emergency braking was no longer operating. The warning light stayed on for a few miles and then went off. I took snapshots I will include. I also noticed that the vehicle was recording erroneous data such as suddenly having an electrical charge of a large percent when I had zero electrical charge. I got the vehicle to the dealership for repair. On 11/1. No loaner or rental reimbursement was available from Chrysler so I was without a vehicle during repair. I received the vehicle back on 11/3/22 and I was informed that the vehicle had error codes poe 15-50 hybrid/ev battery pack coolant heater a control and p1649-00 (? not sure I wrote the number correctly) hcp int… code. The paper also showed poe 15-00 hvac blocker power but he only mentioned the first 2. The service technician indicated that the vehicle had a z76 recall (I was not aware of a recall) and that they had done a software update to fix it and the vehicle should note be fixed. Upon reading other online complaints I see multiple reports of the same thing happening repeatedly to other 2021 phev Pacifica’s with low mileage while driving at high speeds including within 10 miles of having the same repair done. I am now scared to drive this vehicle on the highway after reading of multiple other 2021 phevs no longer operating on multiple vehicles while at highway speeds, despite multiple attempts at repair. I am afraid for my safety and for anyone else in my vehicle and I I want Chrysler to pay off my note immediately and take back possession of the vehicle. I no longer feel safe driving it.
I was on the highway traveling at 60mph, with the adaptive cruise control on. The semi in front of me began to brake, I tapped off the cruise control and began braking. The semi came to a complete stop and I pushed my brake pedal to the floor to stop my vehicle. It did not stop. My pinnacle has emergency braking and forward collision warning. My driver's instrument panel did not give a forward collision warning indication in a timely manner and my emergency braking did not stop my vehicle. At the time of the accident vehicle had less than 10,000 miles, dry weather conditions, and vehicle was used for leisure - in town driving. A month previously, I took my pinnacle into the dealership because I didn't feel my brakes were working properly. They felt mushy, little resistance, and were making a noise. An inspection was performed by the dealership but were unable to detect a noise or visual issue. My safety was put at risk when a system I depended upon, that was advertised as, "full speed forward collision warning with active braking assists you by providing both visual and audible warnings as well as applying emergency braking to help bring the vehicle to a full stop, if needed" (Chrysler. Com/pacifica/safey-security. Html) failed. The brakes didn't engage properly to stop us before hitting the vehicle in front of us. The forward collision warning indication didn't happen in a timely manner, it was delayed causing us to lose precious seconds. If this had been a passenger vehicle in front of us we could have injured the occupants and we ourselves could have been gravely injured had we been traveling at a higher rate of speed. The problem has not been confirmed by a dealership, but was taken there prior to the incident less than a month before. I reported the problem to Chrysler the day of the accident. The vehicle is pending inspection by insurance and I have asked that the electronic data recorder be removed and the information on it to be evaluated.
On more than one occasion when I was on a four-lane highway with no vehicle in front of me in my lane, while I was passing a slower moving vehicle in the outer lane, my vehicle slammed on the aes brakes and would have absolutely caused a crash if there would have been a vehicle behind me. That person could have no possibly reacted fast enough to prevent his vehicle from rear-ending my vehicle. Again, there was no one in front of me, only the vehicle in the outer-lane. As far as the rear-cross-traffic problem, my vehicle is one that is missing Chrysler's sphere and rear-parking-assist because of the chip shortage. There aren't any sensors in the back bumper at all and I'm not sure what triggers the rear-cross-traffic alert, but it does not alert until I've backed up so far from my parking space that if there was a car coming quickly it would surely hit me. This system does not work any where as quickly as any other car I've had it on in the past.

While reversing out of driveway with 4. 3 deg. To 5. 5 deg. Down slope, car stopped with rear in street while displaying an apparent rear collision symbol. There was no cross traffic and a car parked along the curb on passenger side was not encroaching on driveway. Car then refused to move either further back to complete the street entry maneuver or forward to re-enter the driveway. Car was left in a dangerous position part way into the street and blocking sidewalk. Car could not be moved until ignition was turned off for 10 seconds, then turned back on. At that point car was first pulled fully forward into driveway, then successfully backed out into the street and driven off.
22' Pacifica has advanced front brake assist . It does not work. I have tried and it only sets off a audible warning noise and dash says brake. No braking assist like it was stated it would have. I took it back to dealership 3 times now and have been told every time it has no codes. This safety braking does not work. The settings are all on . But there is no assist braking at all.
The braking system does not respond in time - there is a delay which almost caused an accident once and did end up causing an accident in Dec of 2021. My daughter was in the car, and my husband was driving. His back was injured, and thank goodness my daughter was not hurt. I tried to contact Chrysler three times (2 times via filling their customer service form and once over the phone), and they said there is nothing they could do and finally gave this form to complete if I was concerned hence writing today. The delay in the brake system is a significant issue, and our local dealership and repair stated there is nothing they could do. Contact customer service of Chrysler and Chrysler service representative said there is no recall, so he cannot do anything about this critical safety matter. After a lengthy conversation, he referred me to you. We bought this van for our family, including young children (purchased days before the birth of my son) and elderly parents, and it cost us over $57,000. I am very disappointed in Chrysler for not taking this braking delay issue seriously. We even tried to sell our vehicle, but the price is $13000 less than what we paid in just about a year, which we cannot afford anymore. Besides this vital safety issue, we are very much satisfied with our vehicle and are concerned that others might be in a similar situation to us while Chrysler is not even listening to this vital safety concern; hence I am writing.
We’ve written the NHTSA in the past and have not received a response. We understand covid was a problem that messed a lot of things up and we’ve been patient until now and we’re writing again hoping for a long awaited response. We purchased new (on 03/10/17) a 2017 Chrysler Pacifica with the factory installed tow package, which dealer and Chrysler corp claimed it could tow 3600 pounds, so we can purchase and tow a 3500 pound RV but since the van purchase we have learned that we need trailer brakes on any trailer weighing more than 2500 pounds so the van and trailer could stop safely. After much investigation and many calls with our dealer, Chrysler corporation and many RV dealers, it turns out the 2017 model year that Chrysler corporation claimed it could tow up to 3600 pounds, can not in fact safely tow over 2500 pounds without a brake controller/trailer brakes and our van, which was purchased with the factory tow package, was not designed or wired to accommodate the necessary brake controller to tow over 2500 pounds and it can not be retrofitted with a necessary brake controller. The dealer advised taking the matter up with Chrysler corporation and Chrysler advised taking the matter up with the dealer, and neither will resolve the safety issue. Can you please tell us if this is a safety issue or if in your opinion we can safely tow a 3500 pound RV without a brake controller and trailer brakes or do you recommend we seek legal counsel.
