The contact stated that the ongoing pain was so severe. The vehicle was taken to a local tow yard. The vehicle was condemned as a total loss. A police report was filed. In addition, the contact referenced an unknown seat belt recall that was initiated a while ago. The dealer was contacted; however, the contact was informed at that time that the contact's VIN was not associated with the unknown recall. The manufacturer was not made aware of the failure. The failure mileage was approximately 165,000.

I was in an accident. Where the van rolled 17 times. On or about the 12th roll. The retaining lock on the shoulder strap released. This allowed me to slide out of the seatbelt. This caused me more complicated injuries. The vehicle is currently at copart impound lot in oklahoma city, ok. Will be available for inspection upon your request. The oklahoma highway patrol report indicated. I was wearing my seatbelt but the retaining lock on shoulder strap had broken. The oklahoma highway patrol report # ohp25-024333. There were no warning lights or messages to indicate that there was a problem.

The vehicle was sold with a “minor accident” reported, but the condition suggests the damage may have been more significant. After purchase, I discovered the rear seatbelt did not work and required interior panels to be removed and replaced. A non-working seatbelt points to possible prior impact or structural damage affecting the restraint system. The vehicle also has an active safety recall for the side curtain airbags. The combination of a recall and a failed seatbelt raises concern that the vehicle may have been in a more severe collision than reported or repaired incorrectly. Driver side seat belt locks when I first put it on but as I'm driving down the road, sometimes it will come unlocked by itself.
The middle row passenger seat belt will no longer latch. There were no warnings. The seatbelt became unlatched on it's own while I was driving my children to school on 3/15/2024. I had to immediately pull over and call for help. None of my children are old enough to ride in the front seat, so this was quite scary. The seat belt has not been able to latch since.

In the past 6 months, we have noticed 2 seat belts with tears in them. One of the seat belts was not even being used as a car seat using the latch system had been in that seat. The first was noticed August 2023 and we just noticed another tear affecting a different seat belt January 2024. We replaced the first and are scheduling the 2nd for replacement.
My 2017 Chrysler Pacifica touring minivan has a steering issue. Within the last several months, when driving at a variety of speeds, the steering wheel “sticks” in turns. It has occurred on regular side roads, secondary roads, and on the highway. It generally happens when turning the car slightly. Examples include: (1. ) during a slight turn in the road in a yield situation, (2. ) during a slight turn when there is a bend in the road, (3. ) when making a slight turn while on the on-ramp on the highway, (4. ) occasionally when switching lanes on the highway. During these situations, the steering wheel sticks and does not return to center, instead, it has to be forced out of the momentary stuck position causing unnatural/unpredictable realignment of the car (swerve, jerking the steering wheel, causing panic) . There are no associated lights or warnings. This problem first lead us to stop driving the car out of fear of getting into an accident (safety for ourselves and others). Then, upon research for similar problems on the internet, we see that there has been a recall for 2017-2019 Chrysler Pacificas for steering issues, but our VIN is not included. I’ve had mechanics to look at it, one kept it several weeks trying to find the issue, done research online, the only answers are there’s a problem but no one has a solution. All guess work. The occupant classification module also has created problems with transmission failure. To be able to drive the van, you have to disconnect the passenger seat sensor. Driving down the road with the ocm connected it will suddenly lose power, transmission won’t shift, which can cause a wreck not being able to get out of the way or completing a maneuver. Anytime there’s a steering problem, random transmission problem, or loss of power in traffic, there’s a safety problem.

The vehicle was taken to the dealer where it was diagnosed that the driver's side rear seat belt assembly needed to be replaced. The vehicle was not repaired. The seatbelt looks as though it has been cut partially through by the fixed seatbelt guide on the middle row of seats. Those seatbelts hold in place two car seats for a young child and an infant. The seatbelts rarely get moved because we keep the car seats secured to the two middle row seats. The seat belts were briefly inspected by a service advisor at the Chrysler dealership, taris, who stated that it looks like it could have been done by the seatbelt guide. The first time I noticed it was just on the driver side back in 2022. When I noticed an identical cut on the passenger side about a week to two weeks ago, I knew that there was a problem, and I also noticed that there is a secondary cut starting on driver's side because in March we had another baby and placed a new car seat base on the driver side seat. The driver side seat belt is now locked into place and cannot be viewed unless the side interior panel is removed. That has not been done at this time.
My wife was stationary at a red light with my daughter in the second row. A car approached the light and slammed into the back of her car ii don't think they were slowing down). She then hit the vehicle in front of her. The air bags did not deploy and the seatbelt did not fully engage. As a result, she hit the steering wheel causing fractures, a concussion and a minor brain bleed.

The third row bench seems easily capable of popping off its attachment to the floor with a bit of pressure. When we installed our baby car seat in the third row for the first time, we locked it in place with the seat belt and then tightend the seat into place by using the electric recline to squeeze some extra room and make sure there was no give. At some point we tightened the recline too much and the base of the whole third row popped off of the floor. Everything seemed fine once we started over and reset the seat in place. I also disconnected the power recline button in that row so that our son wouldn't be able to re-create that issue. However, a few weeks ago my wife was putting some large items in the car and she was trying to put down the third row passenger seat using the recline buttons in the trunk. She accidentally pushed the recline button for the half of the row the baby car seat is in instead of the side of the row with an empty seat. Again, the whole third row popped apart from the floor at its base. To fix it I uninstalled the car seat and then reinstalled the third row bench and then the car seat again. This second incident got me wondering how much pressure it takes for the third row to detach from the floor? I'm concerned that in an accident the whole back of the car would become very dangerous and seat belts and car seats would do little to protect anyone from a floating bench. By putting the car seat in place and tightening the recline we must be putting a lot of pressure on the floor attachments and any added pressure potentially pulls it right off. That seems like a huge design flaw. In the manual all it says in a collision, you or others in your vehicle could be injured if seats are not properly latched to their floor attachments. Always be sure the seats are fully latched.

Tl the contact rented a 2019 Chrysler Pacifica. While the contact's eight year old daughter was seat belted in the rear driver's side seat, the seat belt seized into the lock. The contact had to use scissors to cut the seat belt. Also, the rear passenger side seat belt failed to engage when the brake pedal was depressed abruptly. The contact was able to stand with the seat belt operating as intended. The vehicle was not diagnosed or repaired. The vehicle was returned to hertz (located at the orlando International airport, 1 airport blvd, orlando, FL 32812, (407) 859-8400) and the contact was informed that the seat belt would be replaced. The manufacturer was informed of the failure. The approximate failure mileage was 12,196. The VIN was not available.
During the severe braking the belts did not block. My son hit his head very hard in the front seat. It was very dangerous, he could break his neck.
The safety pull on the seat belt did not let up when unbuckling. It forced my daughter into almost suffocation and ultimately needed to be cut (back row, passenger side). I've now noticed that the middle row, middle seat (8 seater) has a similar issue. It continues to tighten and does not let up on, what seemingly should be a safety feature. See attached pictures.
The seat belt on the passenger side in the second row seat fails to latch when you try to buckle it. It was working properly and has suddenly failed to latch on its own. The vehicle has 37k miles.
Passenger side middle row seat belt fails to fully latch, unless you move seat forward as to stow it and slam back. . . . This works most times but sometimes it clicks like latch but comes undone when you yank on it. I have videos of this concurring and have reported to Chrysler who states this is not a problem.
Seat belt locked in open so you cannot lock in seat belt. (male part of seat belt does not stay in female buckle). 1st time it did not stay connected, keept trying and then I got it to connect. The next time I went to use it it will not stay connected performed the t54 232t54182 safety recall on 12/26/17. Now it is the middle row outside seat buckle that is not working! the vehicle was stationary when not working.
I purchased a 2017 Pacifica and was told after I purchased it to bring it back for a seatbelt recall. I had an appointment and when I called to confirm I was told it was fixed before I purchased it ?? a week later I had a van full of children and my daughter was in the 2nd row , my husband came to a stop and the buckle broke and my daughter fell forward. My husband and I are very unhappy with this van and after hearing recall after recall we want out of this lease!! I called Chrysler customer care and put in a formal complaint and was told, " their hands are tied ".

I test drove one of these vans at a Dodge dealership in sacramento. When I sat in the third row middle seat, the seatbelt went over by neck rather than over my clavicle. As a board-certified emergency physician, I am well aware of the potential risk to life that this poses, and it was one of my reasons for not buying the vehicle. To be clear, I do not own this vehicle nor was I injured by it. However I am 5'9" tall and I think anyone less than 6 feet tall is likely to be injured by the seatbelt since it will not fit them correctly and there is no way to adjust it. Obviously the person stuck in the third row middle seat is likely to be a small person. This is a faulty design that needs to be re-engineered to avoid injuring consumers. In order to fill out this form online, I had to submit a VIN. I selected a VIN I found on the internet that matches the make and model. I did not actually drive that vehicle. The one I drove has a VIN that is not available to me. It should not matter as this is a standard feature of the car.
This vehicle has had one major issue with the fuel system. When the fuel gauge gets to half way, the gauge drops to e and does not want to accept gas. It acts like it's full and you can barely put any gas in. When this happens the vehicle acts like it's out of gas even when it's not causing me to run out of gas while driving down the interstate and sometimes the car won't even start when it drops below half way. Also, my odometer reads gascap like there's a leak but it's a brand new gas cap. I've read days worth of information on the world wide web and this is the most common issue but noone has done anything about it. It is not fair to the consumer. I know I will never buy another Chrysler and I will warn others not to also. Not only that but my seatbelts do not work anymore, two of them anyway. My kids have to sit in the far back to buckle up. I bought this vehicle used. I've had the fuel pumps replaced, the vapor canister and vapor filter replaced, gas cap, ect. Nothing repairs the issue. I know now why the previous owner gave it up, if I didn't have to pay a down payment, I would have already let it go back.

Seat belt / restraint connector lock failed to properly function. This was communicated by children complaining that the seat belt not working. While attempting to make another seat in the vehicle available and verifying that seat belt was functioning properly, it was notice I notice could here loose part in the internal locking mechanism connector harness. I used a flash light to see the part. I could only detect/see a small broken black plastic item. This is a high concern and is directly related to safety.

When I initially purchased my vehicle, 2005 Chrysler Pacifica, from genesis auto sales located at 1005 springdale rd in austin, TX. 78721 there was noise by the front left tire. After bringing this to the companies attention they agreed to fix it. The kept the vehicle about a week. The day I picked it up my father looked it over and found duct tape on the part, cv joint. I took the vehicle back and also mentioned the horn also went out. The company agreed to fix this again as well as perform my inspection as the sticker had expired. The car was kept another 2 weeks. When the vehicle was picked up for a second time the car began overheating. At this time my father and mechanic looked it over and found a busted hose, but they noted that the cv joint was duct taped againg this time covered in grease. He also noted missing lug nuts on all tires. I brought this up to the company again who refused to listen. I filed a complaint with dmv, bbb, oag, ftc and dps. Dps sent out an investigator who took pics as well and also noted that the tint on the vehicle was illegal and that not only were lug nuts missing but the lug bolts were broken off. All this as well as the motor mount have since been replaced. The bolts holding the cv joint and motor mount were also found not to be the proper size. On 12/24/11 as I was driving my seat belt became unbolted from the vehicle. I was able to place it back in where it came from but it is not secure. The company refuses to help with these problems and I will be pursuing legal action as all problems with the vehicle have been safety hazards and I am now unable to drive the vehicle for fear myself or my family will be injured as im not sure all problems have been fixed as different noises still occur.
Instruction manual for dorel gb1b3, infant carrier says to use lap belt across and then shoulder belt around the back of seat. The belt is not long enough for shoulder portion to go around back of seat. Installation done in 2008 Chrysler Pacifica in middle and outboard positions.

I own a 2006 Chrysler Pacifica and the seatbelts do not lock in when tightened. Example when hooked in a childs car seat. The date below is just a date, it has been this way since I bought the car. I was told that it was normal for the seat belts not to lock in.
