The contact owns an evenflo maestro booster seat, model number: 31021433. The seat was manufactured on may 15, 2014. The safety seat was being utilized in a 2010 Chrysler town and country. The contact stated that the child safety seat was forward facing with the latch system or the safety belt used to secure the child. While removing the cover from the seat to wash it, a piece of padding detached from the interior of the child safety seat. The contact stated that the padding could not be placed back on the seat. The failure occurred outside of the vehicle. The child was not in the safety seat at the time of the failure. The manufacturer was not made aware of the failure.
