While driving in damp wheather applied brakes to avoid hitting another vehicle, seat belt bolts popped out, and consumer went into steering wheel. Consumer felt that bolt with lock tight was not screwed in.

3 Seat Belt problem

Adjustable seat belt anchor on the left side of vehicle pulled out of the assembly, causing seat belt to become inoperable. Please give any further details.
