1 Seats problem

Failure Date: 08/20/2025

The part that failed on the 2015 Dodge Grand Caravan was the active head restraint (ahr) on the passenger seat. It suddenly deployed while driving smoothly. The vehicle was going about 15 mph and there was no sudden impacts or movement that would have caused the ahr to deploy. It is available for inspection upon request. The safety of the passenger was put at risk by the potential impact of the headrest unexpectedly hitting their head that could cause physical harm. The deployment also caused a loud noise that acted as a distraction to the driver. The problem has not been reproduced as far as I know. I have seen others on multiple websites reporting the same failure of the ahr. The headrest has not been inspected as of the day of this incident report. There were no warnings or symptoms that would have led me to believe something was potentially wrong with the headrest.

The passenger headrest deployed when traveling down the road without being in a collision. The passenger has hit in the back on head with headrest going down the road. I took the vehicle to my closest dealership and they advise me to call stellantis customer care and report it. I has informed that the x87 warranty extension recently expired. I was told it would have to be an out of pocket expense even though it is a safety risk.

The driver's headrest deployed without collision. The incident occurred at an auto service center and I noticed when the vehicle was returned to me. The mechanic inspected the headrest and stated a part in the headrest broke causing it to deploy. He advised to call the dealer for repair. The incident has been reported to the dealer and I am waiting for a return call. There were no warnings for seat malfunction or symptoms prior to deployment. Headrest is available for inspection. This defect puts the driver at risk for concussion, head and neck injuries that could affect the ability to control the vehicle.

The front passenger headrest deployed for no reason. We had just parked the van and my 13 year old daughter was behind the passenger seat when the headrest deployed and something hit her in the face. We were in no accident nor rear-ended.

The active head restraint deployed on it's own. I was stopped at a light and the headrest slammed into the back of my head. I was alone in the vehicle, and nothing struck the seat or my vehicle. Upon inspection, the plastic clips that hold the metal bar in place have broken. It caused a headache and neck pain, as it jarred me unexpectedly. This affected my safety, and had I been moving at the time it could have caused an accident, putting others at risk as well. I called the dealer I purchased the vehicle from and was quoted over $600 for the part (replacement headrest) and labor, but have not taken it in for them to inspect it. There were no claims filed, so no inspections have occurred. There were no warnings, neither lights or messages, or any other problems with any part of the driver's seat. It just exploded.

Drivers side heat rest deployed sitting still. Plastic holding metal strike pin broke. Luckily no one was in the drivers seat. I just got out of it a few minutes before it happened.

My van was parked in my driveway overnight and both of the headrests deployed. I am grateful I was not driving it. I couldn’t get them to close they are stuck open. When called dealer was told was not covered even though I have mopar coverage. Since it’s so much to fix it out of pocket the safety feature is virtually useless because I can’t have it fixed.

While driving to work, the driver side head rest deployed. I was not involved in an accident, just idling at a stoplight. Thankfully, I did not sustain any injuries. I called the dealership I purchased from and the service department said that the repair cost would be out of my pocket. How is this ok when it clearly has to be a manufacturer malfunction. This seriously needs some attention and a recall issued.

On Thursday June 4, 2020 I was operating a 2015 Dodge caravan owned by the danville public schools transportation division. While stopped at a traffic light I raised the height of the head rest one click to place the head rest higher up the back of my head. When the light changed to green I proceeded to the next intersection and had to stop for a second red light. As I pulled away from the second light (after a green indicator) the head rest suddenly popped open making impact with the rear of my head. The vehicle was in motion when the head rest deployed. I mentioned the head rest to our shop technician who began to make inquiry into the possible cause. The shop technician spoke to the local Dodge dealership about the issue and was advised there were no recalls or safety bulletins on the issue and that new replacements were available for around $600 and that our best option would be to retrieve one from a junk yard. A closer look on line indicated issues with Dodge ahr head rest. The locking pin should be released by a solenoid powered latch that is triggered by the sensor system in the event of a crash. The plastic retainers that hold the locking pin failed and broke away causing the front section of the head rest to deploy without triggering the solenoid. Please see the attached photos of the damaged retainer points and the locking pin. This 2015 Dodge van has 58,976 original miles.

Failure Date: 10/01/2019

Tl- the contact owns a 2012 Dodge Grand Caravan. The contact stated that while driving approximately 5 mph the front passenger side head rest erroneously activated. Upon inspection it was determined that the internal bracket had broke causing the head rest to deploy. During the incident no passenger was seated in the front seat. The cause of the failure was not determined. The local dealer(unknown) was notified of the failure but no assistance was offered. The manufacturer was not notified. The failure mileage was 70,000. Jft.
