A massive amount of people in forums have encountered the same problem. When you start the vehicle it will idle pretty rough with black smoke. If and when you press the accelerator pedal it will continue to throttle up on its own. In park or driving! forum post: #1. While driving my 04 Dodge 3500 cummins down the street, at a speed of around 45mph, all of a sudden it went to wide-open throttle. It completely caught me off guard and the first thing I did was hit the brake. It had very little effect on what was happening. In less than 1 block I was well over 70mph with the brake on in a 45mph zone. #2. I had the same exact thing happen, I was at 90 mph on a 40 mph road when I finally had to put it in neutral, the pedal had no effect on rpm as I coasted to a stop the engine sat pegged at the rev limiter, and stayed there until I shut the motor down. I did not have cruise control on or showing as ready to go into the cruise, ie cruise light on in the instrument panel. I had some codes, but not sure when. Fuel pressure too high and another I forget. It was scary with the family in the truck. Waited a few minutes trying to see that all connections were tight and they were, restarted and all was back to normal. 2 complaints of 1000's. I've been in the mechanical world for 50 years. Everything from motorcycles to heavy equipment and this one took me over a year to figure out. Generally, there are no codes thrown as in mine. The fuel pressure regulator malfunctions and forces pressure on the fuel rail. This feeds the injectors too much fuel and the computer will not default and shut down. I was traveling at 55mph with a 45-foot king pin trailer on my truck. Went to accelerate a bit and took off. At 75 mph, loaded (45000 lbs) I turned the ignition off. How many people has this killed and the possible reason was driver error?.

While driving on a busy interstate highway recentally and pulling a car trailer, I was returning home from a vacation when the throttle position sensor (tps) failed. This results in a "dead pedal" situation, where the engine falls to idle, and there is no 'drive' to the wheels. This situation is very serious as sometimes there is no where to pull off the interstate, due to traffic lane narrowing. My personal situation was that I was able to coast to the side of the road. To re-set the tps, you must stop, put in park, turn the ignition off, wait approx. 10-15 seconds, and then re-start. It should re-set and you can drive away, although a 'check engine' light appears. In my case, the tps failed a second time, a short distance away, and I had to repeat the steps. The second time it "worked" and I was able to drive home. This failure of the tps had a profound effect on me personally. There is no warning that this is about to happen. When it does happen, you have no throttle input and you must get off the road. All I could think about during those moments of panic, was being smashed into from the rear! I am aware this is an issue with my type of vehicle as I have done some follow up research on the internet following my incident. I want to inform you, that I purchased the vehicle new in 2004. This vehicle is immaculate, and all maintenance is performed at the local dealer. In car guys terms, this vehicle would be considered a "garage queen" and there would be no contributing factors such as lack of maintaince, dirt, road grime, or abuse. I am convinced this situation will lead to a serious accident, if not to me, then to someone else, and I want to inform you of this potential serious issue. I have spoken to the dealer/mechanic and the solution is a tps replacement, but in my opinion, this should never happen in the first place.

Truck died in middle of road, truck would not start . Son towed truck out of road to parking lot. Truck, started again ran 3 days. Pulled out of parking lot , the truck accelerated out of controlled hit brakes a few times and truck died again in middle of road. Started again , truck moved 10 feet and died . Tipm (totally integrated power module ) has failed. Unexpected acceleration scared me considerably. Less experienced driver or my wife would have got scared and crashed.

Engine revving to 3000 rpm and not idle down. Had to ride brakes put trans in neutral or park to stop. Breaks would not stop truck only slow it. Shop found cod p0606 ecm/pcm communication failure. Why did this not put the truck in limp mode or prevent it from starting. Replaced ecm/pcm control module truck runs fine. Very expensive 3400 dollars.
Runs out of fuel at 1/4 tank,breaks act like front breaks or on ice when coming to stop dry road , cruse control dont work with vacuum leak.

Ecm/ecu cummings diesel engine mount problems . Tps error throttle erratic speeds up /down . No response throttle - merging in traffic freeway big problem. Speeds up - almost drive through. Shifting transmission force down/up. In house known engineer ing compatibility cummings /Dodge controller . Not public info. See youtube ecm/ecu problems. Changed out 3 tps s & 1 ecm u not compt. Accident looking for a place to happen. Death.
While leaving the lot I go to accelrate and the truck slowly increases speed and then starts to move. This happens all the time and I took the truck into the dealership and they stated this is chryslers torque management system. This system has almost caused me to get into a wreck twice. I know of one friend who has gotten into a wreck. I researched this issue and sure enough there are hundreds of members on Dodge and cummins forums complaining of this issue and Chrysler will not do anything about it. There is an extreme hesitation between the time the throttle pedal is applied to when the vehicle begins moving. This issues seems to be most noticeable when the throttle petal is pressed slowly and also when the throttle pedal is pressed hard, released, and pressed again. I have had many close calls when merging into traffic or when turning at intersections.

My truck has been plagued with a terrible delay in throttle response. When trying to merge into traffic there is a severe delay between imputing throttle and the truck actually accelerating. It's almost like the truck takes 3-6 seconds to realize throttle input. This has become very dangerous when attempting to merge or turn across traffic. I have taken my truck into the dealership but there is nothing available to rectify this. I am clearly not alone on this, many of us having been having lengthy discussions on the forums in hopes of a fix.
Randomly, while attempting to avoid hazards and/or collisions and while merging onto motorway, gas pedal becomes unresponsive for up to 5 seconds. This usually happens from a dead stop; however, has occurred while traveling at highway speeds also.
While at a complete stop, with foot on the brake, there was tremendous amount of power being transferred through the transmission to make the truck surge forward. I had to put both feet on the brake pedal and pull on the steering wheel to stop the vehicle. At one point, sitting at a four way intersection I released the brake pedal and I immediately slid sideways uncontrollably across the intersection. Thankfully there were no other vehicles at the intersection otherwise there would have been a guaranteed collision. Every stoplight or stop sign was like a hole shot with a transbrake.

The vehicle was taken to the dealer for repairs. The contact stated he had experienced the failure previously and had an exhaust flush performed on the vehicle to correct the failure but to no avail. The dealer advised the contact not to allow the vehicle to idle to prevent the failure. On a separate occasion, the vehicle abnormally accelerated causing the contact to crash into a barbed wire fence and into a ditch. The vehicle was left in possession of the dealer for repairs. While shifting down to 2nd gear, let out clutch and rpms went up to 300 without touching gas pedal. Vehicle lurched forward. Consumer went into guardrail. Been to a couple of Dodge dealers, said this was normal, and Dodge company knew about this. There just have not been enough complaints.
