1 Equipment problem

Failure Date: 10/19/2025

Description of the incident: while driving our RV on the expressway, the slide-out unexpectedly extended on its own without anyone pressing the control button or getting up from their seat. The slide-out extended several feet outward while the vehicle was moving at highway speed. Component/system that failed: the slide-out mechanism/control system malfunctioned. It appears to have activated spontaneously. The component is available for inspection upon request. Safety risk: this malfunction created an extreme safety hazard. The slide-out expanded into the next lane of traffic, putting our passengers, specifically our children seated at the dinette, at immediate risk of being struck or crushed by a passing tractor-trailer. A collision could have resulted in serious injury or fatality. Reproduction/confirmation: the problem has not been reproduced or confirmed by a dealer or independent service center. Inspection: the RV or the slide-out component has not been inspected by the manufacturer, police, insurance, or other representatives at this time. Warning lamps or prior symptoms: there were no warning lights, error messages, or symptoms prior to the failure. The malfunction occurred suddenly and without any prior indication.

The contact owns a 2018 jayco greyhawk built on a 2017 Ford E-450 chassis. The contact stated that the hydraulic tank was leaking fluid onto the ground. The RV was taken to the dealer where it was diagnosed that there was a hole in the hydraulic tank and the hydraulic leveling system had detached. The dealer patched the hole in the hydraulic tank; however, the failure recurred. The RV was not repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 17,000.
