The contact stated that there was no warning light illuminated. The contact stated he experienced the failure on three separate occasions. The vehicle was taken to the local dealer and the dealer was made aware of the failure. The contact stated he also experienced the vehicle overheating and engine failure. The vehicle was not diagnosed or repaired. Upon investigation, the contact associated the failure with NHTSA campaign number: 15v614000 (forward collision avoidance) however, the VIN was not included. The manufacturer had been informed of the failure. The failure mileage was approximately 50,000.

2 Forward Collision Avoidance problem

Failure Date: 07/24/2019

The contact owns a 2015 Ford F-150. The contact stated that while driving at 70 mph with the adaptive cruise control engaged and traveling under an overpass the brakes would engage slowing the vehicle down abruptly. The contact stated that after passing the overpass the set speed would resume. The contact stated that the forward collision warning audio and visual would illuminate. The vehicle was not taken to the dealer or independent mechanic for diagnosis or repairs. The contact stated that he disengaged the adaptive cruise control feature. The contact researched the failure and found NHTSA campaign number: 15v614000 (forward collision avoidance) and was relating it to the failure. The contact stated that his vehicle was not included in the recall as it was manufactured in September of 2015 which after the recall manufactured date of March 18, 2014, to August 6, 2015 for affected vehicles. The manufacturer was notified of the failure and a case was opened. The contact stated that Ford informed him that he would receive a recall notice if the recall was extended and the VIN was included. The approximate failure mileage was 50,000.

3 Forward Collision Avoidance problem

Failure Date: 09/19/2018

Tl the contact owns a 2015 Ford F-150. While passing an 18-wheeler on the right side of the road at highway speeds, the adaptive cruise control activated and the collision warning with brake support feature engaged without warning. The failure occurred twice and caused the vehicle to come to a complete stop. The contact felt that the sensors or an electrical fault within the vehicle could be affecting the adaptive cruise control and collision warning with brake support, which could cause a crash. The dealer (autonation Ford frisco, 6850 TX-121, frisco, TX 75034, 1-(972) 763-5523) was notified and stated that there was no way to replicate the failure without a diagnostic trouble code. The dealer did not test drive the vehicle. The vehicle was not repaired. The manufacturer was notified and advised the contact to call a dealer. The failure mileage was not available.

4 Forward Collision Avoidance problem

Failure Date: 06/10/2018

The contact owns a 2015 Ford F-150. The contact stated that while driving on the highway and approaching a semi-truck, the forward collision avoidance system applied the brakes with the warning light illuminated. 5 Forward Collision Avoidance problem

6 Forward Collision Avoidance problem

Failure Date: 09/05/2016

The contact owns a 2015 Ford F-150. While driving approximately 70 mph and attempting to overtake a trailer, the forward collision warning indicator illuminated.
