On 5/8/2026 at 1555 hours, while driving at normal freeway speed under normal driving conditions, vehicle began an exhaust system regeneration cycle. During the course of the region cycle, vehicle began to smoke excessively and realizing this wan not normal, I was able to yield to the right shoulder of the roadway. Almost immediately the vehicle ignited on fire and completely burned to the ground. The fire appeared to have commenced in the engine compartment, driver side near the firewall. The location of which appears to be consistent with reports of fuel filter failures I have read about in the past. The resulting fire ignited nearby brush which burned several hundred feet of brush and resulted in the spillage of diesel fuel and coolant which combined from the resulting melting of the coolant system and fuel tank and lines. This incident put both me and surrounding communities at risk, particularly from the resulting fire and hazmat spill. The vehicle was in good running condition prior to this incident. The incident was investigated by the templeton California highway patrol.
I am reporting a serious safety issue involving my 2017 Ford F-250 diesel truck. While towing a 34-foot fifth wheel trailer and traveling from [xxx] to [xxx] , the vehicle experienced a sudden and dangerous failure near [xxx] . The engine warning light appeared, the vehicle went into low power mode, and then completely shut down on the highway. This created a serious safety hazard, particularly while towing a large trailer at highway speeds. The vehicle was towed to a repair facility where it was determined that the high-pressure fuel pump had catastrophically failed. The pump had developed a significant crack and released metal debris throughout the entire fuel system. This required extensive repairs, including: - replacement of the high-pressure fuel pump - replacement of fuel lines - full fuel system cleaning, including the fuel tank the total repair cost exceeded $14,000 usd (near $20,000 cad). The mechanic indicated that this type of failure should not occur under normal operating conditions. Additionally, the Ford parts dealer advised that the failed fuel pump could be returned for credit, suggesting the failure may not be consistent with normal wear and tear. This failure occurred suddenly, without warning, and resulted in a complete loss of power while driving, which I believe represents a significant safety concern. I respectfully request that NHTSA investigate this issue as a potential safety-related defect involving high-pressure fuel pumps in diesel vehicles. Information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).
2 different issues- 1st issue was Ford factory fuel filter leaked at a seam. Resulting in fuel loss all over engine! 2nd issue is fuel filter inlet line retaining clip comes unclipped on its own resulting in fuel loss all over the engine. Both incidents happened during long distance travel which also resulted in several gallons of fuel drenching whole undercarriage of vehicle.
This past year I experienced catastrophic failure with my bosch, cp4 fuel pump in my 2017 Ford f250 with a 6. 7 l diesel. Recalls have been published by Ford, but not beyond a 2018 vehicle. After seeing metal fragments in my fuel water separator, I was forced to flush my entire fuel system, replace the cp4 fuel pump with a cdr high pressure pump (replacement), flush the entire fuel rail system and replace my fuel injectors. This repair cost me approximately $13,000. I find it difficult to understand why my 2017 vehicle was not included in the recall when it had the defective item as the later models had. The truck has 129,000 miles on it currently. Certainly, these are not excessive miles given the year of the vehicle. I would like to know what can be done for older models(the bosch cp4 fuel pump goes all the way back to 2011). Thank you for your consideration and attention to this.
We get a power reduced code and the truck shuts down while driving. We have to pull over and put it in park and restart it. When we are on the freeway we can’t get over we put it in neutral and turn off the truck and restart it. It is very scary and dangerous! this has been happening since June of 2025.
While pulling a 38ft 5th wheel camper on an incline with the cruise control set at random the vehicle completely dies. It experiences a total shutdown of the engine, which in return causes a complete loss of power steering, brakes etc. The truck will not restart until you push the off button and open the drivers door to shut the entire system down.
Ford has a problem where the 34 gallon gas tank only reads as a 20-25 gallon gas tank. They have a service bulletin on it and the number for that is 17-0046. After calling multiple dealerships I was told thousands would have to come out of my pocket because of their factory defect. The fix includes replacing the whole fuel tank and also replacing the fuel level sender. My vehicle being within the dates are clearly affected by this defect and I don’t understand why it should be coming out of my pocket. Thank you for your help!.
A message was displayed to reduce the truck speed. Then the speed started to reduce itself, I wasn't able to increase the speed. I was on a hwy and the speed limit was 65 mph and on a slight hill the trucks speed was only 39 mph. I put the flashers on hoping to alert the cars passing me that I had a problem. Then the engine died so I steered it over to the side of the road. I tried to re-start it and the truck shook before it died again. I had it towed to a Ford dealer in boerne, TX. They called me the following day and said the high-pressure fuel pump came apart. They said they saw metal parts in the fuel and their estimate to fit the truck was $18,000! I had it towed to another automotive place, I just needed another opinion. Then I heard about 2 recalls Ford had this year, 1 in January and another just ramping up. The description of the problem sounds exactly like my issue however it didn't cover the 2017 models! I let chandler automotive, in castroville, TX know and they said they would look in to it.
Fuel pump loses prime at the fuel rail, causing 6. 2 gas engines to crank for a long time. Faulty check valve. It seems Ford made a revision on the pump and never did a recall.
I own a 2017 Ford F-250 6. 7l power stroke diesel with a cp4 fuel pump that recently failed. Ford has already recalled this exact part for 2020-2022 models (recall 24v-841) due to known defects, but they refuse to assist owners of earlier models like mine, even though the issue is the same. The cp4 failure can cause sudden fuel system failure, which is dangerous while driving. Ford should extend the recall to cover all affected vehicles before more owners are left with costly repairs or safety risks.
On Tuesday, [xxx], at approximately [xxx] pm, I was driving southbound on [xxx] near coalinga, CA, when my truck suddenly experienced a failure. I noticed smoke in the side mirror and asked my passenger to confirm. Moments later, a "low fuel pressure" message appeared on the dashboard, and smoke began coming from the hood. I immediately pulled over and attempted to pop the hood, at which point flames erupted from the engine bay. The fire quickly spread, fully engulfing the vehicle within minutes. Emergency responders, including cal-fire and chp, arrived on the scene. The truck was towed by cal-state towing service in huron, CA. The fire captain on site provided an incident card, but the full fire report is expected to take at least a week to obtain. There were no prior indications of mechanical issues before this incident, and no recent repairs or modifications to the fuel system or engine. However, I recently became aware of an ongoing NHTSA investigation into fuel leaks and fire risks in Ford super duty trucks equipped with the 6. 7l diesel engine (tfl truck report). The investigation highlights a potential issue with fractures in the secondary fuel filter housing, which could lead to fuel leaks and fire hazards. Given that my truck displayed a low fuel pressure warning immediately before catching fire, I strongly suspect this failure may be related to the issue under investigation. The truck is currently in tow yard storage and is available for inspection upon request. This incident posed a serious safety risk to myself, my father and others on the roadway, and I believe it warrants further examination by NHTSA. Information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).
The fuel pump failed , yes it is available for inspection upon request. Truck cut off in middle of busy road way , unable to move to shoulder creating a high risk to be hit or someone to be hit . Independent service center diagnosed was made aware this is a common problem . No warning lights or previous symptoms before engine cut off . We have been informed this is a recurring issue with Ford f250 creating a dangerous enviorment when driving for the driver and all other vehicles on the road .
1 . Driver's side door will not close in cold weather due to frozen actuator. TSB has been issued on this problem; Ford will not repair due to vehicle being out of warranty. 2. Backup/vehicle camera's do not always work when truck is in reverse. TSB has been issued on this problem; Ford will not repair due to vehicle being out of warranty. 3. Ford inadvertently installed a substantially smaller fuel tank on my specific vehicle. TSB has been issued on this problem; Ford will not repair due to vehicle being out of warranty.
The cp4 fuel pump began failing with the truck just over 100k miles. When taken in for routine service there were traces of metal shavings in my fuel. This is a widely known problem with the cp4 and is still being used in Ford trucks. The cost of repairs is substantial. The best case is you catch it early and it only costs between $3-6,000. If I had not caught this and the entire fuel system had been damaged, my local Ford dealership informed me it could cost close to $16,000. This is a devastating amount of money to pay for a well known issue in Ford trucks.
The hose connecting to the fuel filter in the engine compartment came off and dump fuel all over the engine. Luckily we turned it off before a fire started. This has happened 3 times now.
This is the third time I have had to tow this vehicle in less than a year and half for a long crank no start. The fixed a melted wiring harness the first time. Second time couldn't tell me what the problem was but got it running and then now its back at the dealership again for the same problem.
The contact owns a 2017 Ford F-250. The contact stated he was driving 60 mph when the vehicle shut down. The reduce engine power light was illuminated. The vehicle was towed to a dealer service center. The vehicle was diagnosed stating the fuel injection needed to be replaced. The vehicle was not repaired. The manufacturer was made aware of the failure and referred the contact to NHTSA hotline. The approximate failure mileage was 140,000.
The engine light activated on the dash randomly while driving. My truck was experiencing loss of power and for the next few days and reduced fuel mileage. I checked the engine code and learned it is a common error with 2017 Ford super dutys with the 6. 7l diesel engine. The code is “dtc p203b”, sometimes indicated as p203b00 is an obd diagnostic trouble code indicating a manufacturer-specific fault with the vehicle powertrain (I. E. Engine and gearbox). This code identifies an error with the air/fuel mixture causing reductant level sensor circuit range/performance. I believe this error in manufacturing is a safety risk because it is common within these trucks and can leave motorists stranded or potentially disable the vehicle in the roadway causing a traffic hazard.
I have been made aware of the probe involving 2015-2021 Ford super duty pickups where the fuel filter housing and fuel lines have cracked. I wanted to make you aware that in November 2023, my vehicle suffered the same part failure. Park Ford in tallmadge ohio did the repair and I paid for it out of pocket. It was significant and I had diesel fluid leaking profusely. My vehicle is a 2017 Ford F-250 with the diesel engine and at that time only had approximately 50,000 miles on it. I thought it was odd that this part failed. I see it may not have been an isolated occurrence. I wanted to add my complaint to those being investigated by your agency.
While responding my personal vehicle to a motor vehicle collision (volunteer first responder) my 2017 f250 lost all power at 70+ miles an hour on a highway. Power steering and power brakes were not functioning and caused a delay in service to the community. Mechanic diagnosed it as a contaminated fuel system from the problematic bosch cp4 pump, which Ford is well aware of. They even have a “kit” to fix this issue costing well over $6,000. This is a major safety issue when all power is lost especially if towing trailers.
Vehicle looses power and shuts down while driving. . . . . Fuel injection kit is not available for purchase. Part number is hc3z9b246b to correct problem is on back order and no estimate on when part will become available. The truck has been at the dealership for 5 weeks. A hugh company like Ford this is totally unacceptable and dangerous say the least.
There is a pin hole in the center of plastic housing for the upper fuel filter on my 2017 f250 diesel. Shoots fuel out when driving. Very dangerous. Appears to be similar to the 2022 Ford diesel fuel filter recall.
Engine light keep coming on and I was losing power. I had to spend $10,000. 00 getting it fuel filter system had metal savings. I need to be reimbursed for this problem, when I took it in the shop to get it fixed. They stated 2017’s had this problem come on Ford I love my truck and very sad that I had to spend this amount of money. Please contact me with your help (904)412-7500 dcally1@yahoo. After driving for over an hour at highway speed, I smelled something but could not determine what it was. The next day I drove around town and once when I pulled up to a traffic signal, I noticed the smell again and some light smoke coming from the front of the truck. When I got home, I opened the hood and diesel fuel was dripping off the hood insulator and running all over the engine and around the seals around the hood. I cranked the engine again to see if I could determine where it was coming from and a saw a stream shooting straight up from the center of the engine. I took it to a local Ford dealership, and they repaired it. The top of the fuel filter mounted in the top center of the engine ruptured and sprayed diesel all over the engine. I left the part at the dealership after the repair. There were no dashboard warnings and there was no change in vehicle responsiveness in pressing the accelerator. When I returned from the dealership, the diesel that had spilled reached the exhaust system and smoke came out from under the center of the truck. It half-emptied the fuel tank in 3 days. I know diesel in not as volatile as gasoline, but this was still a dangerous situation.
Fuel filter cracks on the top and squirts diesel fuel all over engine bay. Happened once at 35k miles and then happened again at 44k miles. Very dangerous considering it can start a fire and no lights turned on.
Ford 2017 f250 - fuel system completely collapsed, with no warning truck completely went powerless doing 70 mph on the highway with my son in the back of the truck. Luckily I had the mind to pull over safely. What if I was towing a load or similar on mountain roads. Ford said they found rust and debris in the fuel system, they would not cover it under premium Ford warranty. Safeco/liberty mutual stated they tested the fuel and was fine. No coverage either on auto insurance. Total cost was $13,500 to repair the system on which I had full warranty and full insurance coverage. Ford did help with a $4000 customer retention help fund. I am reporting in hopes to save lives, if I I wasn't aware at the time of the complete power failure things could have been significantly worse. I was completely unimpressed with both Ford/liberty mutual on the handling of the scary situation. I send a personal letter to the Ford ceo and got nothing. I believe Ford is hiding on this issue with bad fuel pumps (cp4 version) and trying to blame owners or bad fuel systems.
Truck has less than 60,000 miles. Driving along the hiway with grand daughter, going to dance , truck dinged said reduced power, and started losing power started to pull over and engine died, battery light came on, went to get out out some one driving by said get out your truck is on fire, as I got out could see fire smoke, coming from area under engine on left hand side, fire was dripping out . Popped hood release, but now less than a 2 minutes later from reduced power had fire boiling out under the hood. One person had a fire ext and we sprayed in the crack of the hood, as too hot to get hood open, paint was turning colors, by time fire dept got there, the truck was engulfed in fire, loud explosions going on, windows has already blow out left hand side tire on fire. By the time port lavaca texas 77979 fire dept got the fire out, nothing left of the truck. But rear bed. Truck had bee going in regeneration a lot lately , while waiting for grand daughter to come out of school in 10 min, it went in and out of rengen several times with weird, rpms rise and drops. Drove fine, when we left school. Was app. 18 miles from home when the truck said reduced power and started to slow down and died. Called Ford customer care, got passed on twice to different parts, then told office of general counsel would call. After a week I called back could not get a phone number to call. Only a fax. When office of general counsel did call about 11 days later, the guy would not give me his name, a phone number, a email address, told him video on youtube of 2016 Ford test truck in cal. On side of road, 6. 7 diesel I had seen same thing started under the engine compartment and burnt to ground. See several more post about them catching on fire, all around same age and all 6. 7 diesels . Then he said just tell your insurance sub ligation, and let them handle it since I had insurance. Calhoun county texas sheriffs office and port lavaca texas fire dept has the reports,.
Connectors on upper fuel filter cracked and diesel was spraying all over the engine compartment.
Vehicle has experienced multiple catastrophic upper oil pan failures, lower oil pan failure, coolant leak, fuel line dry rotted, and other issues at less than 138,000 miles. Resulting in potential loss of control of vehicle due to potential tire and brake exposure to engine oil and fluids. Potential of fire and other catastrophic loss.
Diesel leaking from front right. Found diesel fuel pooled under truck in the driveway while it was parked.
As I was driving down the road, I started to smell diesel fuel. No warning lights were illuminated on the dash. I pulled over and found that diesel fuel was leaking under the truck from under the engine. When I checked under the hood, I found that the secondary diesel filter had a stream of diesel coming out of it. I was able to safely get the vehicle home and started researching online, I found that this was a common problem but was not issued as part of a recall from Ford. I did find that the part was simple to replace so I performed the replacement myself. Unfortunately, I discarded the faulty part so it is not available for inspection but this is a common issue which many have experienced and I believe that finding a faulty filter will be easy. I am in contact with someone that has one on hand.
Has happened three times. While driving truck will buck then lose total power and will not restart. Always happens at highway speeds.
Today, I experienced a fuel leak while driving down the road that emptied about 10+ gallons of diesel fuel all over the top of my engine and on the roads. The truck has been dropped off at the Ford dealer (koons Ford, falls church va) on 6 March 2020 for investigation. Truck leaked fuel over hot engine parts and all over the roads making the roads a dangerous place for other drivers.
Tl- the contact owns a 2017 Ford F-250. The contact stated that upon entering his vehicle, he could smell diesel gas emitting from the vehicle. The contact pulled the vehicle over and could see the fuel leaking from the vehicle. Upon investigation, the contact discovered NHTSA campaign number:16v778000(fuel system, diesel). The contact called five star Ford 6618 ne loop 820, north richland hills, TX 76180 (817) 500-0096 where he was informed that they were unaware of a recall. The manufacturer was notified and informed him that they were aware of the recall but that his vehicle was not included. The vehicle had not been repaired. The failure mileage was 81,200. Jo.
While driving on interstate 20, eastbound, my low fuel pressure indicator light came on. I started losing power and we pulled off at the exit that was there. As soon as we stopped, the engine bay was on fire. It gave no indication of odd smells or problems. It seemed to burn the engine compartment completely, all the electrical systems, interior, possibly firewall, etc. It seems this is possibly a known problem.
