Safety hazard notification: intermittent ignition switch failure & strand risk I am filing a formal safety complaint regarding a known manufacturing defect in the ignition start/stop switch of my 2017 Honda Hr-v. This component is suffering from an intermittent failure that creates an unpredictable and immediate strand risk, compromising driver safety by leaving the vehicle unable to start or power down correctly in potentially hazardous or isolated environments. Manufacturer american Honda motor CO. Is fully aware of this systemic defect, having issued technical service bulletin TSB 20-116 (diagnostic trouble code dtc b12c5) specifically addressing ignition start/stop switch failure. Despite this being a documented engineering flaw admitted by the manufacturer, american Honda case management (case 16436837) is actively enforcing predatory financial barriers to prevent remediation. They are refusing to issue a conditional diagnostic waiver to verify the code, choosing instead to force an upfront out-of-pocket dealer diagnostic fee onto the consumer with no guarantee of goodwill coverage. Furthermore, corporate representatives handling this file on July 1, 2026, actively refused to provide their names or employee identification numbers when requested. I request that NHTSA review this case file and investigate american Honda’s bad-faith deployment of cost barriers to intentionally depress the reporting and remediation of a documented, safety-critical manufacturing defect.

Sunroof caused a leak in the vehicle cabin, under the front driver rug. So the water was coming up from the floor. This was after a heavy rain. I was told by Honda the drains were clogged and paid $1k for repair 5 months ago. I just recently noticed more water pooling on the rug after another heavy rain and now they say the seals on the sunroof are failing. I rarely use the sunroof. I purchased the car new. This is another $1k repair. It seems as if this is a manufacturer defect.

I got in my car and started driving home from work then I heard something just shatter and when I looked back my rear windshield was completely shattered. I pick up my nieces and nephews and drive other peoples children to their appointments. This could have caused a serious injury should I have experienced it during a time I have little kids in my backseat. I am taking it to gardena Honda first thing in the morning for an inspection. I have seen other people report this issue on 2023-2025 Hondas and after comparing my windshield to theirs its literally the same. There were no warning signs that flashed it was so crazy.

On 5/232026 . . There was a severe thunderstorm . No hail . No signs of cracked window prior or as I approached my Honda 2023 hrv . I got in car and noticed windshield were foggy so I turned on both defroster. 10 mins later I heard a loud “boom “ in the back of my car. . I was starteled and immediately turned around and noticed the rear windshield was shattered and had a circular shaped broken glass hole. I never rolled anything on the street and no none was in sight . . I approached my home and parked the car and immediately checked the back to see if a rock or some sort was in the trunk however there was no evidence of that . Researching I notice many others have experienced this random spontaneous shatttering related to manufacturing error of window/defroster glue on defroster coiling . This was a huge danger for anyone in the back including myself the driver . . I’m very upset that Honda has not set a full recall on this other customers . I could have or another person sustained a serious injury. Now I’m waiting to get this repair which is a huge inconvience because all weekend it has stormed and having a garbage bag as a cover is so embarrassing . , I’m very displeased with Honda right now .

I am submitting this complaint regarding a catastrophic cvt transmission failure in my 2019 Honda Hr-v at approximately 97,500 miles. In may 2026, while driving in king of prussia, pennsylvania, my vehicle suddenly experienced a complete loss of propulsion. Multiple warning lights illuminated on the dashboard simultaneously. Although the engine would rev and the vehicle could be shifted into gear, it would not move properly and became unsafe to operate. The vehicle left me stranded away from home and required towing. I incurred significant expenses for towing, diagnostic fees, transportation, lodging, and related costs due to the breakdown. After diagnosis, I was informed that the cvt transmission had failed and required replacement at an estimated cost of approximately $10,000. I later learned that Honda had issued service bulletins 21-046 and 21-047 concerning cvt belt deterioration and transmission issues in 2016-2020 Honda Hr-v vehicles. These bulletins acknowledge that an incorrect pcm value could cause excessive pressure on the cvt belt, potentially leading to partial or complete belt breakage and sudden loss of acceleration. Honda also extended warranty coverage on affected vehicles due to premature cvt belt deterioration. My vehicle falls within the affected model years and experienced symptoms consistent with those described in Honda’s service information. Despite this, Honda denied my request for goodwill assistance, even though the failure occurred at relatively low mileage for a modern transmission and only shortly after the warranty extension period expired. I am concerned that this transmission defect creates a safety risk because it can result in sudden loss of acceleration or vehicle propulsion while driving, potentially placing drivers and passengers in dangerous situations. I respectfully request that NHTSA investigate the cvt transmission failures affecting Honda Hr-v vehicles and determine whether additional action is warranted.

My husband and I were driving down the road on the way home from dinner in the rain when the car suddenly started braking very hard but very quickly. I thought at first that my husband had braked a little too hard all of a sudden, but he said he did not. Before he could tell me that though the dash started lighting up. Brake system failure, electronic parking brake failure, hill start assist problem, power-steering problem, and many others - pretty much every safety sensor that could come on. I then worried we would not be able to brake when needed, but luckily we made it home. As soon as we were able to this morning, my husband brought it to the Honda service to be checked out. We have to heard from them yet other that $230 to diagnose the problem. We were very lucky there were not others close to us when it happened. This could have been a very serious wreck if we were in traffic or going an even higher speed on the interstate(which we had been doing earlier).

All of my braking/abs lights intermittently go on and off. My 2019 Honda hrv runs fine but when dealer looked at it they said it was the vsa and to replace would be over $4k. Dealer said eventually my vehicle may shut down if not taken care of. I am wondering if this is a common problem and if will be a recall?.
My drivers side front window shattered while I was driving on xxx since it’s shattered, the window itself isn’t available for inspection. There were no prior cracks to the window and nothing hit my window when the it shattered. I was going 75 mph on the highway and had two passengers in my car. I ended up with glass all over me. Luckily, I was wearing sunglasses, so none it went in my eyes. I didn’t swerve or lose control of the vehicle, but myself and the two passengers could have been seriously injured as a result of this defect. I am unaware of whether this problem has been confirmed by a dealer or independent safety center in other vehicles besides my own. I did take my car to the Honda dealership in [xxx] where I bought it from, and they will look at it tomorrow during business hours. The vehicle has not been inspected by the manufacturer, police, or insurance representatives. There was no warning. It happened spontaneously. Full catastrophic failure of the cvt belt in the transmission, a known issue that Honda had an extended warranty for for up to 150k miles and or 7 years from purchase (that letter was never mailed to my house, it was found after the fact while looking for the title at a family memeber's house. The cvt belt failed at 189,837 miles at highway speed of 80 mph as posted on texas hwy130 which posed a huge safety risk due to this hwy's speed, and heavy tractor trailer routes with no warning, this was not due to wear and tear, the 2016 Honda hrv is known for this issue. Took it to Honda in round rock, TX, where they confirmed via diagnostic testing it was in fact the defcet known, but Honda corp refused any type of goodwill assistance given its "too far outside the warranty" (the whole point of a goodwill exception is being outside the parameters) my family has owned Hondas for over 25 years, and they disregarded the brand loyalty completly.

About 3 hours into a 4 hour drive (while the car was only months old, with just 1800 miles), the sound switched from playing the radio at a normal volume to suddenly extraordinarily loud crackle, static, popping noises. The noise continued at this indescribably high volume for about one minute while I tried anything to help -- I tried adjusting the volume knob, unplugging my phone (which was plugged in just for directions), switching inputs, switching from fm to am, pushing the volume knob in to turn the whole thing off, etc. Nothing stopped the noise. After about 90 seconds, it stopped, but I still do not know why. After that, I could not get any sound to play. My ears rang the entire rest of the drive (another 1 hour). The noise was so loud and so sudden that it made me very upset and scared at what could have happened that luckily didn't - I could have easily been so startled to swerve into another lane, and if my 4 year old had been in the back, I am really scared to think of how traumatized he would be, how damaged his ears would be, and what else would have happened. My ears are still ringing today, a full day later.
I was driving on the highway when all of a sudden, no warning signs, no sounds/car shaking, anything, my car started lighting up with break system, engine light, parking sign, etc. I didn't know what was going on so I pulled over. When I pulled over, I checked the parking break to see if I might have hit it and I didn't. Then the car wouldn't go. The check engine light and all the break system lights were on and my car wouldn't move. I was stuck on the side of the highway for an hour waiting for my vehicle to be towed to dover Honda, where I always get my work completed on the car and I complete all my inspections there as well. They advised the ctv belt snapped and when I asked how this could have happened, because I was just there a few months prior and nothing was said they advised "this just happens" and "there was no way to know". This vehicle has been inspected, multiple times over the past five years that I have owned the car and they never mentioned this could happen without warning.
Honda hrv 2025 malfunctioning when brake hold system is activated. Brake hold system lets individuals use the brake whenever needed without your foot having to be on the pedal the entire time. You can come to a red light, step on the pedal and take your foot off the pedal and the car will remain stopped until you press on the gas. Over the course of leasing this vehicle sometimes the brake system problem message would show but it would disappear right away. I let the dealership know of this issue when I got my first oil change but since the message wasn’t currently displayed on the dashboard they did not attend to it. Earlier this week the car suddenly glitches when brake hold is activated and deactivates itself followed by all kinds of messages on the dash. I will upload them here to see. From brake system, tpms, hill start assist all malfunction and when you turn the car off it doesn’t start back up automatically. This is a serious safety concern as the brake system malfunction and car not turning on can happen suddenly when driving causing injury to myself and those around me. A simple google search shows that this is common for this vehicle make, model and year however no recall has been filled.
2020 Honda Hr-v — obd monitors fail to reset after battery replacement. Dealer refused help. Cannot pass MA state inspection. Following a routine battery replacement, my 2020 Honda Hr-v obd monitors will not reset, making it impossible to pass massachusetts state inspection. I have followed Honda’s recommended drive cycle exactly — driven 562 miles, completed 142 warm-up cycles — and the system still shows “not ready. ” I have made multiple trips to inspection stations, waited in long lines each time, and been turned away every single time. I have lost significant time, wasted fuel, and face real risk of registration penalties — for a car that runs perfectly. When I contacted Honda of newton corner — my exclusive service dealership — they told me they could not help me. No escalation. No fix. No support. I am not a mechanic. I am a customer who followed every instruction, trusted the brand, trusted the dealer, and has been left completely alone to deal with this! this is a safety concern. If Honda’s obd system cannot confirm its own monitors are functioning after 562 miles and 142 cycles, there is no assurance the vehicle’s emissions and safety systems are working properly. This issue is not isolated — multiple Hr-v owners across 2019–2022 model years report identical problems. Honda has issued no software fix and no dealer protocol to resolve it. I am requesting NHTSA investigate Honda’s obd monitor reset failure across Hr-v models and require Honda to issue a remedy — including dealer-authorized in-shop resets and a software update. Honda created this problem and has abandoned its customers entirely. I should not have to choose between driving an uninspected vehicle and being stranded. This is a defect in Honda’s drive cycle reset logic. Honda needs to issue a software update and instruct dealers to perform monitor resets!a dealership that exclusively services your vehicle should have all the necessary tools and the obligation to help. Deeply disappointed.

Dear american Honda motor CO. , inc. ,I am writing to formally document my experience with a premature failure of the continuously variable transmission (cvt) in my Honda Hr-v. The vehicle was covered under the extended warranty program Honda implemented specifically for the known cvt issues affecting 2016–2020 model years. In on 3/10/26 the cvt failed at approximately 96,000 miles. The vehicle was towed to an authorized Honda dealership, where technicians confirmed premature belt deterioration and breakage. Prior to the failure, I had the vehicle serviced at the dealership for the powertrain control module (pcm) software update outlined in Honda service bulletin 21-047. During that visit, I reported a noticeable burning smell, which I now believe originated from the failing transmission belt. The dealership assured me there were no issues and stated that the software update had adjusted the gear ratio to reduce strain on the transmission. The vehicle was returned to me with no further action recommended. Subsequent communication with american Honda revealed that the company had been aware of widespread cvt belt issues in these model years. Rather than issue a full recall, Honda extended the powertrain warranty to 7 years or 150,000 miles (whichever occurs first). My repeated attempts to escalate the matter with Honda customer service, including multiple requests for contact information for a regional manager, were unsuccessful. After several weeks of delays—during which my vehicle remained at the dealership for over 20 +days—Honda corporate initially offered to cover a remanufactured cvt replacement at a cost of $4,500 to me. The dealership ultimately assisted by reducing my out-of-pocket expense to $2,900. I find it unacceptable that a vehicle with only 96,000 miles required a major transmission replacement costing nearly $3,000, especially after Honda had already acknowledged the defect through a technical service bulletin and an extended warranty. The dealership’s.
Vehicle information year: 2018 make: Honda model: Hr-v VIN: [xxx] mileage at time of incident: 96,993 incident date: 03/10/2026 I am filing this complaint regarding the premature and complete failure of the cvt (continuously variable transmission) on my 2018 Honda Hr-v with approximately 96,993 miles. Description of failure: the transmission on my 2018 Honda Hr-v failed without prior warning. Symptoms included sudden loss of acceleration, vehicle unable to move under its own power, etc. The vehicle was taken to page Honda of bloomfield where it was diagnosed with a complete cvt transmission failure. The repair cost totaled $8,873. Safety concern: this failure poses a serious safety risk. A sudden loss of transmission function while driving — particularly at highway speeds resulted in loss of vehicle control and created dangerous conditions. This is not a minor mechanical inconvenience; it is a critical safety defect. Honda's response: I opened a case with Honda customer service; case # xxx requesting goodwill assistance reimbursement for the repair costs. Honda denied my claim. After submitting a formal written demand, Honda agreed to reconsider, but has yet to provide full reimbursement. Pattern of failure: I believe this is not an isolated incident. I am aware of other 2018 Honda Hr-v owners who have reported the same cvt transmission failure pattern. I urge NHTSA to investigate this and help me with resolving this case. I am happy to provide additional documentation, repair invoices, and correspondence with Honda upon request. Information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).
Dashboard is blank. Where the miles, gas measure , etc usually shows up. Very concerning as you don’t see how much gas the car has, and the mileage information is not displayed.
My transmission went before the car hit 100 thousand miles. It put me at risk because I was in the middle of the road and my car wouldn’t move. The problem was confirmed by the Honda dealer. There were zero warning signs before this happened.

Hello, I am reaching out with extreme dissatisfaction of my recently purchased 2016 Honda Hr-v. This car was part of the extended warranty via service bulletin 21-046 warranty extension: 2016-20 Hr-v cvt premature belt deterioration. My car has been cared for extremely closely, and we planned to be a Honda customer for life up until this past week. On [xxx], my wife and I were driving on the highway when all of a sudden the entire dashboard lit up with warning signals and went into "limp" mode. We were able to get off the highway, although it induced great stress as we did so. Upon taking it to the Honda dealership (fred beans of abington), we were told the cvt has failed and would cost an estimated $8,400 to repair. The car is 9 years and 3 months old, with 117,000 miles. We submitted a goodwill request through Honda americas where the Honda team came back and stated that the car is outside of the timeline and denied our claim. I spoke to jocelyn and she was pleasant while delivering extremely upsetting news. I am not disputing the fact that the extended warranty clearly states 7 years or 150,000 miles, whichever comes first. I am disputing the fact that Honda has basically admitted that the cvt will fail early, and if you don't drive as much as Honda deems "average", then you are [xxx]. The extended warranty should be up to 150,000 miles, period. We are a one car household and are now basically stuck between a rock and a hard place trying to decide what to do. This car is effectively totaled even though we conducted regular maintenance. I am appealing to your better judgement to re-open our case and get somebody else to re-evaluate the circumstances. Information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).
There is premature belt deterioration that ultimately caused the transmission to be a complete failure. It has been confirmed by the dealership. My parents were driving down the i95 when this randomly occurred. There was no warning message. TSB 21-047 warranty extension: 2016-20 Hr-v cvt premature belt deterioration.
My car randomly lost power on the road and I rear ended some one, my car lost power 3 times on the road.
I've been the only owner to this 22' Honda hrv and I have serviced it at the dealer 90% of the time. All oil changes, coolant flush, transmission services etc. Done on time. I recently got it diagnosed at my local Honda dealer because it was starting to make a noise when you stepped on the gas. The noise started at about 95k miles and becomes louder as you increase the speed of the vehicle. The dealer diagnosed it with an internal transmission failure. There were no warning lights or messages ever. No prior check engine lights on or mechanical issues prior to this. I am very disappointed with this vehicle. Hopefully this issue is looked into. I do not hope for this to happen to anyone with this vehicle.
On Sunday, 02/08/2026, I parked briefly to drop something off. When I tried to restart the car, it was clicking but would not start. An engine light came on, and a flashing message appeared saying “park. ” I initially thought it was a battery issue, but the warning light remained on. The next morning, the car started, but the engine was locked and the vehicle would not move forward or backward. I called the Honda dealer, and the technician advised me to tow and bring the vehicle in as soon as possible. I restarted the vehicle, and the message on the screen cleared and the vehicle was engaged. I then dropped the vehicle off at the dealership. After inspection, they told me the front passenger seat weight sensor needed to be replaced, which is very expensive ($1,300) when I researched the, Honda has already recalled their vehicles for this same issue, and I am not sure I need to pay all this money for something that is caused by an identified defect of the vehicle system. Safety recall: 2020–22 multi-model seat weight sensor campaign notice replaces service bulletin 24-042 safety recall: 2020–22 multi-model seat weight sensor stop sale, dated February 6, 2024.
We experienced two problems with our Honda hrv, which was purchased in June 2025 (less than a year ago). The first issue is that the engine started displaying warning lights and would not turn on. This was due to rodents chewing on the wires, which were apparently soy-based plastic. The second issue is that the rear windshield shattered for no reason the other day. This is apparently a known issue with 2023 and 2024 hrv vehicles that led to a recall, but it also appears to be occurring with the 2025 hrv.
The instrument cluster within the dashboard cuts out in cold temperature preventing you from seeing your fuel, mileage, as well as oil life. It was inspected by 2 different dealerships and was told its a common problem with the hrv's this is a common occurrence with this vehicle.
Driving on freeway interchange ramp and sunroof exploded. Left a rectangular section broken in center of sunroof. Not following a truck, no rocks flew up, no land on either side of ramp, no overpass above.
When I turned my car on, the airbag system (srs) problem light was on. I was worried if it was an airbag problem, or sensor problem, so I brought the vehicle in for inspection at two locations. The first location was an independent service center, in which I was told it was a sensor problem. They weren't available to repair or needed an appointment to thoroughly check the problem. Therefore, I made an appointment for the next day at a Honda dealership. I thought that my vehicle may have been a part of the safety recall from the service bulletin 24-036. Upon inspection and a partial diagnosis (they didn't took apart my vehicle near the wheel or touch screen/audio for the sensor underneath), I was told I had a sensor problem under the front passenger seat with the code error u3000-49. However, I was told that my vehicle was not covered under the safety recall. I'm afraid of whether my airbags will be working properly if the sensor isn't working. There was no warning lights, symptoms of problems, or messages prior to the sensor light turning on.
I had just arrived home from a short 2 mile drive and was sitting in my car for a few minutes with the vehicle running when the rear glass suddenly shattered. Confused and startled, I got out of the car to inspect what happened and noticed the the entire glass was shattered and left and right edges broken more than the rest. While taking photos, the middle of the glass caved in. This is a known issue, and the manufacturer issued a recall regarding this. The rear windshield was replaced last December by the dealer.
The car after a while drives sluggish & brakes very badly even after replacing the brake pads & the roater. I have replacement everything that could affect any of these issues.
My back windshield shattered and when I went to the dealership they said there was a recall on the back windshield but I never received any notification about this recall.
Rear view window shattered while driving.
