I am submitting this complaint to report a serious safety defect involving my Honda Odyssey equipped with Honda sensing technology. During normal operation and without any accident, impact, windshield damage, repair, or environmental event, all Honda sensing safety systems suddenly and simultaneously stopped working. The disabled systems include collision mitigation braking system, adaptive cruise control, lane keeping assist, road departure mitigation, forward collision warning, and related camera- and radar-based features. Multiple warning messages appeared, and the vehicle effectively lost all advanced driver-assistance and collision-avoidance capabilities at once. This occurred without warning and without any action on my part. This failure is extremely concerning because Honda sensing features are marketed and relied upon as critical active safety systems intended to help prevent or mitigate crashes. A sudden, total loss of these systems materially reduces vehicle safety and places occupants and other road users at increased risk. A safety system that can fail entirely and unexpectedly—absent any triggering event—indicates a potential systemic defect, not routine maintenance or wear. I am concerned this issue may affect other Honda vehicles equipped with Honda sensing and may be related to a common failure in cameras, radar units, software, or vehicle electronics. I respectfully request that NHTSA review and log this complaint, investigate this failure pattern, and determine whether a defect investigation, service bulletin, or recall is warranted in the interest of public safety. 7 Adaptive Cruise Control problem

Went to pull into parking lot, adas system starting going crazy. Throws transmission malfunction error. Headlights won’t work - they turn on and off or go extremely dim. Van keeps jerking and losing power. Code reader showed u3000-49 electronic failure. Van drove normal, no issues until this code popped up. Issue started 01/08/2026.

Several errors needed to be cleared every time the vehicle was started. Our Honda dealership determined that it was a failed multipurpose camera (code u3000-49), and that the entire camera unit needed to be replaced and re-calibrated.

In December, out of the blue suddenly had rotating list of warning on dash panel (lane keeping assist problem, collision mitigation system, departure mitigation system problem, auto high-beam problem, adaptive cruise control problem) plus brake warning system light is on constantly and brake hold not working. At first this was intermittent but now on constantly when starting and driving car. Only 55k miles on my Odyssey. Dealer said he had 3 of these in the last three months. Said it was multi purpose front camera failure. Cost over $1500. Since huge safety issue putting not only my family but other cars/people at risk, we are paying to repair, however we just found hundreds of people complaining about this same issue. This should be a Honda recall. We did nothing wrong to cause this safety issue and this part should not fail in 5 years. I don't have the final invoice yet but I can upload later if required.

The multipurpose camera unit has failed resulting in loss of adaptive cruise control, lane keep assist, automatic braking and other features related to the mcu. There are approximately 6 warnings when starting the car of features not working, and 3 lights that remain lit on the dash including "brake system". This is the second time this issue has arise. The first time was replaced by the dealer under warranty.

Components have not been evaluated by anyone.

On Sunday 9/28/2025 I started up my vehicle and received numerous error messages: collision mitigation braking system (cmbs) road departure mitigation system (rdm) lane keeping assist system (lkas) adaptive cruise control (acc) brake system light I contacted the dealership in which I purchased the vehicle and they did diagnostics. It came back that the front facing camera needs to be replaced. Which is a $2000 repair. After researching online this seems to be a common issue with years 2018-2020 Odysseys. This needs to be investigated and a recall issued from Honda USA for remedy as these error messages are concerning when they pop up repeatedly.

The forward facing camera failed on my 2020 Odyssey with only 90,000 miles. Because of this failure, I am loosing all my forward facing safety features. This seems to be a common issue across many Honda vehicles and most users report being told the issue is being caused by overheating. Since users are being told this is a cause and effect issue, the manufacturer should address this at their expense. I first noticed this issue in mid September 2025.

For the last few days I turn on my car and get alert lights that I have problems with: the acc, lkas, collision mitigation, road departure mitigation systems and auto high beams. From online Honda forums I can see that many people have had these problems and it’s likely due to needing to have the windshield camera replaced. This is a 2020 model but I bought it new in 2021, so it is less than 5 years old and I’ve got 94k miles on it. Honda markets itself on its safety features, but nearly all of my safety features are not operating properly less than 5 years from the date of purchase, it’s a widespread issue, and Honda still has not issued a recall.

The multipurpose camera in mounted underneath the rearview mirror stopped working. No apparent reason for the malfunction to shut the vehicle off while filling gas and then five different warning lights occurred, including issues with lane keep assist, adaptive, cruise control, automatic, breaking, and several other safety features no explanation for the failure when that vehicle was taken to the dealer and currently being charged $2500 for a replacement of the camera and labor associated with the repair. My belief is that the system is malfunctioning due to overheating, which is a design issue associated with the vehicle and isn’t something that I should be responsible for.

Internal failure of the multipurpose camera/millimeter wave radar unit with code u300-49; available for inspection upon request. This problem has been confirmed at the local Honda dealership in addition to the numerous forum posts made by like Honda Odyssey owners with this same reoccurring issue. There were no warning symptoms or messages prior to failure.

The camera sensor is malfunctioning. It is affecting the collision sensor and lane assist sensor. Also adaptive cruise control, road mitigation and auto high beams. My vehicle is scheduled for diagnostics with a dealer, but seeing the many, many cases and videos online of the same problem, I am certain of the problem since it has been confirmed by others who have already paid for this costly repair. My warning lamps come on every time I start my vehicle. This is a definite safety issue since the issue is with the safety sensors.

With all these issues, vehicle safety devices don’t provide any safety. These could result in bodily injury and even death.

For the past couple of weeks, every time I drive the car, I get a number of error messages regarding problems with the advanced driver assistance features. A diagnostic check showed error u3000-49, namely "control module internal electronic failure". I took it to a shop, they ran diagnostics and told me I needed to replace the multifunction camera in the windshield. It seems to be a common problem and is very expensive to fix, at more than $2000.

Talking to friends and neighbors they have had the same issues, it is about a $2000 fix to replace and recalibrate the camera link to user forum [xxx] most of the reported issues including mine are related to heat, the camera is in an enclosure located at the top center of the windshield. We just had heat indexes above 100 degrees. The camera is always hot to the touch. I also had this issue in the past put happened whole under warranty. I read online that a lot of Honda Odyssey owners are experiencing same problem. It could be the front camera sensor. I have always had electrical problems with car. There crackling sounds when the radio or music is playing on the stereos.

Upon start up my 2020 Honda Odyssey ex-l throws a slew of pop-up’s on the dash (collision mitigation problem, auto high beam problem, adaptive cruise control problem, lane keep assist problem, road departure mitigation problem). It's been doing this for months, but when it started, I had just gotten an oil change 2 weeks prior and knew my mechanic would've caught this if it was something major. Last week I took it back in for another oil change and asked them to check these errors. They checked the battery and it’s fine (under 2 years old), but they confirmed it's the adaptive cruise control camera. They said not many places work on those cameras and I'll have to have the dealer do the repair. Well upon searching, it seems pretty consistent that this repair is costing owners $1700 because the camera has to be recalibrated with the software after it's replaced. I'm also seeing this problem appears to be widespread for Honda owners. Honda needs to do something to make this right for their customers. A $1700 repair for a camera that randomly dies by no fault of the owner is insane, and considering how many driver assistance systems to improve safety are affected, Honda should be held accountable. The front multipurpose camera has failed “due to internal failure” according to the service tech at local Honda dealer. The camera functions - lane keeping assist, adaptive cruise control, front collision warning, and auto high beam lights, have not functioned for the past month. The windshield was replaced more than a year ago and no other incidents caused the failure, so it seems the camera itself is the issue. My van only has 63,868 miles on it.

This created a serious safety concern because the vehicle unexpectedly lost its lane-keeping, adaptive cruise control, and collision mitigation capabilities while in motion. It increased the risk of a crash, especially during highway driving or when relying on these systems for lane and speed control. Has the problem been reproduced or confirmed by a dealer or independent service center? yes. The Honda dealership confirmed the issue after diagnostic testing and stated that the front camera module had failed and must be replaced. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? the vehicle has been inspected by the authorized Honda dealership. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? yes. The dashboard displayed multiple warning messages, including: acc, lkas, collision mitigation, lane departure mitigation these warnings appeared suddenly at approximately 40,000 miles and have persisted since. The dealer quoted $2500 also I found numerous reports from other Honda owners experiencing the same camera-system failure at low millages.

Online forums show this is a pervasive problem with 2020-2021 Odysseys at around 50,000 miles. Took it dealer for diagnostic, paid $229, and told me it would cost $2,000+ to fix.
