My 2025 Honda Pilot’s collision mitigation braking system and backup sensors have failed three times at low speed, resulting in collisions with objects directly behind or in front of the vehicle. These include a front collision with a tree, running over a small tricycle, and backing into a tree — all under 5 mph with sensors fully enabled. The system failed to detect or respond in any instance. This is a critical safety failure.

It is constantly breaking when cars are coming the other way. It almost caused a vehicle to almost hit me. Also the auto shut off has almost caused accidents because it causes my car to stall and I wish I could keep it off and not have to push the button off every time.

Vehicle has many fault codes that will not clear. Took to dealership and the state they believe its a wheel speed sensor and vehicle is safe to drive. Flashing warnings include awd system problem, electric parking brake problem, brake system problem, brake hold system problem, electric power steering problem, antilock brake system problem, vehicle stability assist problem, hill start assist problem, trailer stability assist problem, hill descent control system problem, adaptive cruise control problem, lane keep assist problem, collision mitigation system problem, road departure mitigation system problem, low speed braking problem, auto high beam problem, parking sensor problem, antilock brake system problem, as well as drive mode change unavailable, rear hatch will not open, drivers seat will not automatically move into position once vehicle is started, vehicle will not shut off without having to hit the off button numerous times, and vehicle is not auto shutting off after the 10-15 min timeframe. Dealer states they have ordered the part and is currently on back order. Was told Honda does not provide loaner vehicles for warranty work. Seems strange that a wheel sensor could cause so many errors and shut off all safety features, brakes and drivetrain of the vehicle.

Vehicle collision system for warning distance was placed to low setting. Driving about 30mph with no cars in front but a large dually on my left lane and a truck on my right lane, similar speed or slightly less, all of us 3 approaching an under construction intersection. Neither car came in my lane yet my system warned me to break and then activated the brakes forcefully. Rear passenger witnessed also and agreed they saw no cause to trigger sudden breaking. No car behind me or else we would have been rear ended. Tried turning system completely off but it resets every time at car startup.
I was travelling northbound in the right lane, of two lanes, on hwy 55 in st. Louis Tuesday around 0715 when my Honda Pilot suddenly braked very hard; the collision mitigation braking system (cmbs) had gone off without reason. I was in a construction zone and I was probably traveling about 50 miles per hour. There were no barriers immediately in front of me, as in a hard curve left or right and definitely no vehicle in front of me and thankfully no vehicle behind me, considering the normal bumper-to-bumper morning traffic. I did not come to a complete stop, well I don't think I did, but slowed greatly enough that all vehicles on my left driving past me fairly fast. I was in shock and confused unable to figure out happened. After a brief assessment of my surrounding I accelerated and continued on my way to work. If someone has been following me I have no doubt they would have collided with me, worse it could have been a tractor trailer! I got to work and was mentally shot for the rest of the day. The dealer was friendly, somewhat empathetic and I scheduled a maintenance check up. I have owned the car less than one month of this this report. I am already looking for a new vehicle and will never own a Honda again. Driving this vehicle with that cmbs that can go off randomly, a/k/a "phantom brake" is nerve raking. I do my best to remember disable the system before driving. Sadly, a system I would ordinarily enjoy having. Also, I now fear following other Honda drivers because if their cmbs fail I might crash into them! the idea Honda has not repaired this or a recalled their vehicles is mind bending; this could cause fatalities! frankly, their complacency is nothing short of apathy for human life. I am going to lose thousands of dollars getting out of this vehicle, which I find unacceptable and cruel. I am happy to testify to this account and am telling you this is nothing short of my sworn statement.
All safety/collision prevention systems related to camera have become inoperative. Vehicle will not lane keep assist, emergency brake, or notify of possible collisions. Problem occurs every time vehicle is turned on.

The automatic emergency braking system in my vehicle has repeatedly activated without any visible reason. The brakes engage suddenly even when there are no cars, curves, reflective surfaces, or obstacles nearby. This has occurred on straight, empty roads and in clear conditions. We have previously taken the car to the dealership for issues related to sensors and wiring, and were even told at one point to turn off certain safety features. Most recently, we went through the dealership and escalated the issue to Honda corporate, but they closed our case and claimed there is nothing wrong with the car. I am very concerned that this unresolved issue could lead to the brakes locking up in a dangerous situation, potentially causing an accident.
Automatic braking system (abs) automatically applied brakes when I was 2 car lengths behind a vehicle that was turning and the sensor detected the vehicles brake lights. There was enough space between my vehicle and the one in front of me that was turning for me to stop if needed.

None of the safety sensors were working.
This vehicle continues to have issues related to a problem that was issued for the fuel pump earlier this year. My fuel pump was replaced, but the vehicle continues to do the same thing. It stalls and jerks while I am driving and does not start sometimes after getting gas. When I am trying to accelerate the vehicle does not go at full power. One time I was going out into traffic and had to reverse because I knew that I would not make it and get hit. In addition to this, the vehicle has an issue with the braking system. Other years have been recalled for what they call ghost breaking, and although this one does this, it has not been recalled. I will be driving and the car will all of a sudden just brake on its own as if there is a vehicle/object in front of me. Sometimes this happens in the same location. Honda is supposed to be a reliable car, but it seems they have failed with this year/model of the Pilot because it has been big issue after big issue.
I have a 2023 Honda Pilot elite. I have noticed a few times that the driver assist system doesn't work as intended during driving in snow or just rain. I will get an error message on my dash, "some driver assist systems cannot operate, clean area shown. " Honda wants the driver to pull over and clean the snow off of the h in front of the SUV. This also happens when it's raining and no snow. The driver assist sensors must be located there. What happens if I should get in an accident when this malfunction goes off and I get hurt or even kill someone? I see this complaint all over the internet from us owners on different forums. The dealerships are doing nothing. I paid a lot of money for this SUV. I expect it to work at all times, especially the safety features. .
Will engage brakes when nothing is present, when going around a curve, or certain times when the sun shines just right. The other day it did so causing a jarring pain to now be present in my back.
While driving the cmbs (crash mitigation braking system) turns on at random timed intervals causing sudden breaking and jerking of the steering wheel towards center. This causes severe safety issues including increasing the chances of a wreck due to wheel jerk movements during turning and sudden breaking. The cmbs error message pops up on the touchscreen and during the episodes of sudden onset the entire digital display both on the gage cluster and entertainment center go black and an audible clicking is heard in the steering column.
An internal failure on the front windshield camera assembly part #36160-tg7-a63 had caused the advanced driver assistance systems to fail. Which in turn has caused the following systems to fail: adaptive cruise control, automatic emergency braking, blind spot warning, forward collision warning, lane departure warning, lane keeping assistance, parking collision warning, and rear cross traffic warning. Leaving these systems unrepaired due to a known and common hardware failure issue leads to this specific vehicle and the ones similar unsafe for the driver and occupants due to the safety systems being inoperable. The problem has been confirmed on two separate occasions by my local Honda dealer. The known issue with this specific camera assembly leads to a multitude of warning lights being displayed including: adaptive cruise control warning, lane keep assist, collision mitigation system problem, and auto high beam mitigation.

Automatic emergency breaking applied at 50mph on a perfectly clear road with not a single car around. Caused panic in the vehicle and would have absolutely caused severe collision if there were any cars behind. Dealer said cannot do anything without reproducible issue. Police was not called. Thanks god nobody got hurt. Not the first time this happens, it happened crossing railroads before. But this time there was absolutely nothing on the road.
Vehicle struggles and jerks to keep speed between 1st and 2nd and 3rd gears and jerks trying to accelerate up until around 50 mph check engine light came on and off a few times and information center on car reports exhaust system problem and to take to a dealer, took to autozone to get codes read replaced part it said to replace but problem persisted light came back on took back to autozone same code showing different bay so I took vehicle to Honda dealer and had them do a diagnostic on it which they could not complete without me paying an extra 180. 00 for a more in depth diagnostic I paid and they found the fuel injectors not functioning properly and said I need to replace them I declined Honda to do this and took it to a different shop myself to have it done where I trusted the mechanic had all the injectors replaced and same problem is still happening. The breaking assist comes on randomly while driving and hits breaks and shakes steering wheel while flashing (break) but there is nothing in front of me.
While driving on an open road, two lanes traveling the same direction the collision mitigation system was activated. The only thing I can think is that it was due to a bicycle that was traveling the same direction at a slower speed than I was driving but I had plenty of room to go around and was not even close to hitting them. The brakes were activated which was not only scary but dangerous. Had a car been behind me there is a strong chance I would have been rear-ended because nobody in their right mind would expect me to hit my brakes in this situation. This prompted me to look at the manual where I saw Honda has a list of reasons the emergency braking could activate despite "little chance of collision". The list is extensive with many reasons the system could activate incorrectly causing an accident, not preventing one. The unexpected braking itself is terrifying but how difficult Honda makes it to "opt out" of the system that obviously has many flaws is even more concerning. I tried to turn off this system only to find out that it is a multi step process that can only be done when stopped so if you forget to turn it off you are stuck using it even if you don't trust the system. Every other safety setting can be turned off while driving except this one. Why make it so difficult for the driver to choose to have complete control of their car, especially when their system is admittedly flawed. In addition, the emergency braking system is the only safety feature that resets every time you shut off the car. So every single time you start the engine you have to go through multiple steps to choose to have control of your own vehicle. It seems the system that is the most flawed is the only one Honda is trying to force drivers to use. This is not safe for my family or those driving around me. I called Honda and they told me there is nothing that can be done to disable it and have the setting hold even after turning off the car like all the other safety setting.
Car slammed on breaks for no reason. Good thing I was paying attention and was able to keep my vehicle under control. My neck hurt for the rest of the day and is sore today. Now my cars system says the system isn’t working correctly after my car slammed on the breaks for basically no reason. I know this is an ongoing problem certain Honda models already. I believe every Honda with this system should be recalled.
My brand new 2025 Honda Pilot frequently (several times per week) will deploy the cmbs (collision mitigation braking system) causing the brakes to be applied harshly and bringing the vehicle to a near stop, despite there being nothing in the road ahead, behind, or to the sides of the vehicle. This happened the first time with only 100 miles on the vehicle. My concern for safety is that sudden unexpected braking could cause an accident and lead to potential injury/death. I've had the vehicle to the dealership 5 times and they've had a handful of excuses. First the alignment of the car was off. Next was a replacement of the radar unit, which they believed to be faulty. The next visits resulted in my Honda tech telling me that a "large radio tower" near where I live are causing interference with the system. However when I received the paper work it says there are large power lines causing interference. I live in a small town and there are no large power lines or large radio towers visible from where I live. I believe Honda is making excuses to get me to stop bothering them with this issue. After all of these trips, Honda is refusing to look at the vehicle again, despite it's behavior putting myself and my family at risk. There were no warning lamps, messages, or anything other symptoms prior to this happening.
Cmbs activates for no reason whatsoever. I can't even count how many times this vehicle has locked the brakes up and no cars anywhere near me at all. Like miles down the road. Honda said they can't replicate the issue. Well, it's going to be hard to replicate something that happens 4 or 5 times in a week by driving it 10 min down the road. This vehicle is going to get my family killed.
When we are driving the vehicle brake light lights up on dash and car breaks for a second or two. There are no cars around and no railroad tracks. It just does it a random times. We aren’t speeding. Just driving down road.
My car started stating that 6 systems were having problems. The adaptive cruise control system, automatic high beam system, blind spot information system, road departure mitigation system, collision mitigation system, and lane keep assist system. I took the car to the Honda dealer it was purchased from and was informed that the technician found that the multipurpose camera was not working and needed to be replaced. The component that failed is the multipurpose camera. My safety and the safety of others was put at risk because the following 6 safety systems stopped working. Adaptive cruise control system, automatic high beam system, blind spot information system, road departure mitigation system, collision mitigation system, and lane keep assist system. The problem has been diagnosed by the dealer. I don't know if this component has been inspected. There were no warning lights, messages, or symptoms prior to the failure. The car was just at the dealership the previous month for the annual state inspection and nothing was noted by the dealership. I was driving when suddenly the component failed and I was notified that the 6 safety systems were no longer working.
Car in front of me stopped. My vehicle did not causing a rear end collision. I do not recall the brake warning flashing to alert me to brake. The automatic braking system did not seem to engage. Even though I pressed the brake pedal the vehicle did not seem to want to stop. It reminded me of a vehicle without power brakes. Nor did the wheels lock up or leave any skid marks from hard braking.
Upon turning on my vehicle all of the emergency warnings came on. I did a diagnostic and was shown that nothing was wrong with the vehicle but the warnings will not reset.
Vehicle brakes or collision warning comes on when roads are curved and traffic in oncoming lanes.
For the third time in less than 4 months, I have taken my car to the dealer because all of the warnings light come on and the car sputters and dies in traffic.

The forward collision warning and automatic braking system are terrible and have been terrible since we purchased our Pilot new from a dealer. The warning lights and beep go off frequently when driving on highways and surface streets, in fact it’s nearly impossible to have an incident-free trip longer than 20-30 minutes. The automatic braking system has engaged many times, causing unexpected and sudden slowing of the vehicle, which has the potential to cause confusion, loss of control, or even a vehicular accident. Making matters worse, one cannot simply disengage the system permanently; it must be turned off each time the engine is started and requires holding a button down for several seconds.
My cmbs stopped my car in the middle of the road coming home one day. There were no other cars around me (in front of me, behind me, or coming towards me). The word “brake” flashed across my dash and then it beeped at me and just stopped in the middle of the road going 55 mph. My kids and elderly mother in law were in the car with me and we were all shaken up and also very glad there were no cars behind us, or else we could have been injured in a rear-end collision. The dealer has had my car for 21 days now and cannot reproduce it, but they asked if I was in the road that it occurred on, because they have had similar complaints from owners. I do not understand why they don’t drive my car down that same road to be able to reproduce the incident. My below email was sent to the dealer on June 18, because the car keeps locking me out with the key fob inside and the dash resets itself, but won’t turn back on, it is just in the constant spinning reset wheel. My attached photos are of the car refusing to lock because it says a door is open or a window is down, when none of that is happening. (the doors are closed and the windows are up. ). The cmbs incident occurred on June 24th, I drove home and had Honda tow my car to the dealership. They have had it ever since.
The front camera has gone out on the Honda Pilot, but inconsistently works. It will turn on while driving and turn off while the adaptive cruise control is on. The vehicle does not have a way to bypass the camera, so then there is consistently lights and alerts on. With not being able to bypass the camera features they will automatically come on or off while driving while the camera is not working properly. This vehicle was purchased in October of 2019, and has had camera issues since January 2024. The vehicle is not even 5 years old and has consistently had problems. The fuel injectors went out at 3 years, which I would not expect to go out, and from discussion with dealership there was an extended warranty for the same year Honda odyessy, which was not extended for the Honda Pilot, but both vehicles have the same fuel injector system.
Sudden automatic brake was applied for no apparent reason. There was no collision situation at all. Car suddenly applied strong brake when we were driving at 40 mph. The car came to a complete stop on a busy road, luckily for us the vehicle behind was bit far back. It was so scary.
Vehicle would brake suddenly. Then shortly thereafter, the collision mitigation system error appeared on the dash. Honda dealership says the camera no longer works and the entire system needs to be replaced. The fact that the camera stopped working spontaneously is concerning.
The vehicle emergency braking kicks in for no reason, there were no other cars in sight and I was driving, in my lane when the vehicle flashed brake and slammed on brakes for me. That is going to cause me to get into an accident when I am cruising and in my lane, no other vehicles around and it slams on brakes, very startling. Also, if it is raining to hard all safety features stop functioning and it says I have to clean the emblem, the emblem is clean it is just raining to hard and the “safety” features can’t function.
Radar and multi purpose camera simultaneously failed. Dash lights came on indicating that auto high beams, lane keep assist, forward collision, adaptive cruise control, and braking system where not working properly. I think my safety and the safety of others where compromised with these systems not working.
Vehicle braking randomly. Cars can be nowhere in sight and this vehicle will slam the brakes. I have almost been rear-ended several times and almost ended up in a road rage incident due to this vehicle slamming the brakes.
