When base of child safety seat was initially installed, infants head fell forward when the brakes were applied. Consumer was forced to manually hold infant's head until she was able to completely remove the base. Last incident involved the stroller/carrier part of the system. Had carrier locked in place on the stroller. The securing bar and carrier fell out head first, causing child to land upside down.
