On April 1, 2026, while pulling into a parking space at our church, I was applying the brake when the vehicle suddenly and forcefully surged forward without warning. The unintended acceleration was intense and unexpected, occurring despite continuous braking input. This resulted in the vehicle striking the building and causing significant damage to both the vehicle and the structure. This incident has occurred twice within a 10 day period, both times during low-speed parking/stopping maneuvers. There were no warning lights, alerts, no automatic emergency braking engagement, no air bag deployment, or prior indications of malfunction. I did report this to the police and they filed an incident report. Lexus was notified and an independent analysis engineer examined the data from the vehicle's cdr system. This report was sent to me and to Lexus.

I was just driving regular and the Rx350 started to jerk and apply the breaks by itself and slip and go side ways and the car behind me almost hit me because my Lexus started to stop hard and fast . Im pressing the gas but it’s not accelerating. The engine wanted to stop and very dangerous I’m scared to even drive the vehicle. I am the 3rd owner and now I know why the otheer people don’t want it . There is a recall for that problem but im new to the car recalls so im not to shure what to do to get the car repaired . On Feb. 10th 2026 I was driving north i405 (los angeles). The car suddenly applied emergency breaks in the middle of the freeway, while there was no car in front of me, at least for 100m. I was lucky that the car behind me didn't hit me. It could be a serious multi-car collision.

I am submitting this correspondence to formally report a serious safety incident involving my 2020 Lexus Rx 350 and to request a thorough investigation. On Saturday, February 7, while attempting to park the vehicle, I experienced a sudden loss of control, resulting in the vehicle accelerating forward and colliding with a metal gate. The exact cause of this event is unknown. During the incident, the vehicle’s pre-collision and automatic braking system failed to engage despite an obstruction being present. Additionally, the airbags did not deploy at the time of impact. A minor child was present in the vehicle, properly secured in a child's safety seat. While no injuries occurred, the vehicle sustained extensive damage and was rendered inoperable. I would also like to note that this vehicle was subject to an airbag recall on November 29, 2025, which was serviced prior to this incident. Despite completing the recall service, the airbags failed to deploy during the accident. After the vehicle was towed, the airbags unexpectedly deployed while it was being lifted onto the tow truck using a crane, further raising concerns regarding the reliability and safety of the airbag system.

My Lexus 2019 Rx450h is making a squishing squeaking brake actuator noise when pressing the brake pedal. I located well publicized TSB’s documenting the awareness by Toyota/Lexus of the problem. (TSB: mc-10245447-9999) brake squawk/knock noise. I checked with my local Lexus dealership to see if my vehicle had been repaired. It had not!. Temecula valley Lexus, 42081 dealer drive, temecula, CA. 92591. (951)365-6834. I scheduled an appointment for repair on 1/26/26. They rejected making the repair because my vehicle did not have any codes in the computer or any lights on the dash. They did in fact, confirm the brake actuator noise. I reject that concept because who could know ahead of time if a part experiences a (1st time) total failure without throwing codes and turning on dashboard lights? as I read the symptoms of severe problems experienced in a higher level of failure by brake actuators, I would not want to have to experience a crash accident in which the actuator would operate correctly in a hard stop condition or have abs failure and brake lockup occur. Personal harm and property damage should not have to be an indicator if I can have the manufacturer cover a repair. Please advise what I should do to increase the response in the direction of getting this known problem repaired. I was dropping my husband off at cardiac rehab. I was stopped and had my foot on the brake suddenly the car launched forward into the back of a car parked in front of me. The other driver got out of the car and said he wasn't hurt. We were able to drive the car home stopping 3 times to pull pieces of the car off that were dragging on the road. We took the car to a mechanic at the end of the road. He scanned the codes for us. Pre-collision 2 clall ; intuitive park assist clall front sensor comm. Circuit ; front recognition camera clall ; lane keeping assist clall front radar sensor incorrect axial gap ; bcm b124b light source sensor open/short. The insurance adjustor totaled the car and they picked it up today. No warning lights showed up on the dash until after the accident.
I went to reverse the car. Suddenly, the car's automatic emergency breaking system engaged. This occurred on 2 separate occasions. The first time, was in a parking lot during a sunny day. There was definitely nothing behind me, not even a person, but the camera saw a leaf or something small, and engaged the automatic emergency brake. The second time, it was raining in the evening, the sky was dark. I went to reverse (to complete a u-turn in an empty lot). Suddenly the automatic emergency braking engaged. In both instances, if I was reversing (as part of a u-turn on a busy street, this would have been very dangerous. Please investigate this. It was very scary.
I was at the gas station, in line waiting for one of the three gas pumps to be available. I put my car in reverse to back up and pull around another car in order to go to the open pump in front. When my car was in reverse motion, I stepped on the brake pedal, but the car did not stop. I stepped on the brake pedal numerous times, but the brake did not function. My car backed into the front of the car in line behind me damaging both vehicles. The automatic emergency braking system did not activate, and there was no rear collision warning sound or message. I switched my car from reverse into park to stop its movement, but it was too late to avoid the collision. I had a passenger in my car who corroborates this description. Fortunately there were no pedestrians behind my car, so the only damage was to the vehicles. My vehicle has not been inspected by anyone since the accident, but will be soon. I have owned this car since April 2021 and previously had a few similar situations when moving in the forward direction. Previously, the brake problem occurred while moving forward at very low speed pulling into parking places. The brake failed to activate, and I had to throw the car into park to stop it, but this was the first time I had experienced this problem traveling in reverse. When this situation occurs, I am pressing on the brake pedal, but the car is not stopping and the brake pedal seems to go to the floor. On March 5, 2025 I took the car to a Lexus dealer and reported the brake failure problems. They inspected, road tested, and checked for codes, but could not identify the problem. I believe that the collision back up system should have prevented this accident, but it did not function. I suspect that there may be some kind of computer issue that prevents the brake from actuating. The car will be going in for repair soon, and I have requested that a complete diagnostic review be performed of this brake problem.

I was driving the vehicle up a road I drive every day, when suddenly the instrument cluster showed a picture of a red pedestrian icon, and slammed on the breaks. It threw me forward, but quickly proceeded on like normal. It detected a pedestrian that was not in front of the vehicle. This was only 20 days after I got the car. It proceeded to do this three more times, with similar severity, but at different speeds and different locations. The fourth instance, I was driving home and the vehicle had the same pedestrian detection of an imaginary person, this time breaking me from 40 mph to 12 mph with a car behind me. Thank the lord I was not hit. I made a service appointment ASAP, and it was there for 8 days. I had it happen to me 5 times in 3300 miles and 2. 5 months, and they drove it 12 miles and said they couldn’t duplicate it. Cooperate told me to send it up to another store which I did. It has now been there for 16 days, and they can’t find an issue either. I am not comfortable driving the car because it could have a breaking incident at any time, and is putting me in danger. Cooperate has told me to file a lemon case or quote “wait for it to happen again and try and document it in better detail. ” I have given them locations, time and date estimates and weather estimates for every instance, and I’m still getting put back in a car that someone at hendrick Lexus north considered a “liability. ” I am beyond disappointed in Lexus for going against their word, is making me the guinea pig in a effort to try to replicate the issue. This is my third Lexus, and my image of the brand is ruined.
Unintended acceleration causing accident March 2, 2023. Low speed frontal collision avoidance assist did not engage causing accident June 11, 2024. August 2023 I agreed to video and Lexus is not providing video of accident June 11th, 2024! recall on fuel pump and software replaced but still bad. Radio off. Heard cybercrimes of communications systems for video game data by meta.
I’ve approximately the last 45 thousand miles, Lexus dealers have replaced the brake actuator and other components of the brake system on this vehicle at least 6 times due to noise issues. Lexus has stated that this is not a safety issue, I have an open ticket with Lexus, but yesterday I had to slam on the brakes and did not get nearly the response that should have come right away. There are no warning lights, as far as I can tell the traction control didn’t engage, and eventually the car suddenly started breaking much harder, but it was very delayed l, and disproportionate with any additional force applied. Lexus knows the brake system is an issue in this model to the point where they are covering the system for 10 years or 150,000 miles, but to say that it isn’t a safety issue seems short-sighted. This system works in conjunction with the radar sensors adaptive cruise control, and I’ve had. This car has also had a number of seeming electrical issues, with every door lock actuator replaced at least once, some of them more than once, and the infotainment system shuts down and reboots randomly or freezes.
Automatic emergency braking 6 times on a new car has violently and suddenly engaged at low speeds. Twice in the middle of the street with no obstacles near by.
While driving car came to an abrupt stop and stalled. (abs) had to engage brake to restart car. Noticed abnormal shifting while driving in nc mountains.

I was sitting at a complete stop for more than 45 minutes behind a fatality accident on the highway they had closed while occasionally removing foot from brake to inch forward a few feet a best and then stop again never touching the accelerator when I realized at one point traffic beside me was moving, I thought, it was my Rx 350 while pressing brake all the way to the floor with no resistance I attempted to put the car in neutral to see if it was slow me down as an upcoming crash was unavoidable. No brake assist, precollison warning or assist, no emergency braking, no resistance from brake pedal and ** warning lights came on at all prior to impact with the back of a stopped semi truck** and during the entire incident the speedometer remained at zero or moved a tiny bit off of zero as if it was pulsing somehow and the accelerator was never touched while car felt like I was being pushed from behind, randomly accelerating on its own before impact.

On 6/03/23 traveling nb on bannock st during a green light to make a left turn wb on w littleton blvd. The defendant ran a red light and broadsided my 2001 Rx300 Lexus and was cited. The airbags did not go off, and my head hit the windshield. The defendant was cited. I filed a claim with Toyota , case no 230621002014 who inspected the totaled car at the yard tow. They stated they were unable to read the black box. They declined. There were side airbags and front air bags which should have deployed and did not. The accident happened on 12/28/22. At 8:50 am, I was picking up a friend and was leaving the parking lot. My friend got in and and then opened the door to lock it again . My foot was on the gas pedal trying to leave the parking lot. The car jumped and went to the opposite side. I turned the steering wheel to keep it from hitting an object but could not get any control on the car. The car kept accelerating and I had to put the car back into park to stop the car. The acceleration was so much that the car jumped the curb and stopped by a wall. The air bags opened up.
On December 28th , 2022, I was giving a ride to a collegue of mine. She opened the door and got in . I proceeded to get the car out of park and suddenly the car accelerated and I could not get the car under control. I turned the car so it would not hit the structure in front of me. I turned but still could not control the car and the only way was to put the car in to park . It jumped the curb and stopped. The left side door opened in the process and was open as this happened.
The pcs detected another car in front and my car went into park, but kept rolling and the engine revved to 7500 rpm.
The sole occupant driver drove into a slightly elevated driveway and stopped. The driveway is maybe 5 degree incline. The shift select lever was placed in park and the driver visually confirmed it was in park. Driver sat in the car for 12 seconds before exiting the vehicle with the engine running and ac on. The driver exited and was standing outside of the vehicle with both feet on the ground for 6 seconds reaching back inside at which time the vehicle began to suddenly roll down the driveway causing injury to the driver. This seems to be a failure of the parking system of the vehicle causing serious injury to the driver. The vehicle has not been inspected yet and investigation is ongoing. It is unknown if there were any warning lamps, messages or other symptoms of the problem prior to the failure, but there was no prior service work performed on the near new vehicle. There is a video of the incident.
On July 15, 2022, my 2022 Rx450hl drove itself across approximately 150 feet of rugged desert terrain and ended upside down in my neighbor's driveway. No one was in the car and no one was in the driveway where it landed. I had parked the car on the side of my house, after several seconds the car took off across the desert to self-destruct. I contacted Lexus, someone from their legal dept. Contacted me and they sent a representative to collect the "black box. " after that Lexus refused to take my calls or provide any information to me about the "black box. " I had taken possession of the car 34 days when this occurred, it was brand new. This car was equipped with safety features that should have stopped it from driving as it continued over sharp, protruding rocks, went through cactus plants taller than me and over a cliff. The seats have seatbelt sensors, this car knows it is driving with no passengers or a driver. Thankfully my 97 year old, blind and deaf mother-in-law was not in the passenger seat, or my 10 month old grand baby was not in the back seat. This could have been a catastrophe. As it was, a beautiful, new car was totaled and I don't know why.

Rear ended vehicle automatic braking did not activate.
I was driving up a hill, turning on to a dirt road. I had my foot on the brake as I was going down hill, I bounce from the crevice on the dirt road. My car started going faster, I kept my foot on the brake trying to stop, but it did not stop. I avoided hitting a parked car and slammed in to a k-rail. My car would not stop.
My awd check system keeps going on and off every few days. I took it to mavis who gave me codes and stated that there is a TSB found to the issues my vehicle read. I took it to Lexus of brooklyn who basically disregard the recommendations just so I javelin to pay them again to diagnosis whay was already diagnosed.

2008 Lexus Rx 400h faulty abs actuator yes my car is available for inspection upon request . This is a known problem on this vehicle there is 100s of complaints on same issue with the same cars Lexus is neglecting responsibility for this faulty and very dangerous hazard , this part has been faulty for years with out my knowledge and now it’s gotten worse all the lights related to my brake system is on and the brakes seem to have a mind of there own they break real hard at times and times I have to slam on them to break . Yes I have had the car checked and the dealer told me that there was a extended warranty issued on this part but that it’s expired. They also gave me a quote $2,100 just for the part no labor that’s crazy, this is not a extended warranty matter this should be recalled there is thousands of people driving this cars with out knowing that there breaks could give out anytime . I hope that Lexus considers the seriousness of this matter and not wait on till lives are lost do to neglecting. They took effort years ago in extending the warranty on the part but there is thousands of owners like my self that were not aware or notified of this extension , also this is a malfunction that is not noticeable and can happen at anytime especially while you driving .

I was driving about 75mph on the interstate and there were no vehicles or other objects near me when my car braked on its own as if there were a vehicle/object in close proximity to the front of my vehicle.
Just after topping a hill on an interstate highway, both lanes of traffic ahead of us came to a sudden and complete stop. I was in left lane and reacted quickly and applied full braking power and managed to stop a foot or 2 from the rear of the stopped car ahead of us. A few seconds later we were hit from the rear by two cars, one, a black late model camry, which almost stopped and a 2nd, a large, new german-maker grey SUV, which never applied brakes, destroyed the black camry behind us and then destroyed our car, when it hit the left rear and after impact continued pushing the front of the car into the steel cables separating our lane of travel from the oncoming traffic in the other lane of the interstate, until the steel cables stopped us. Neither the electronic collision alert system, the air bags or the emergency braking system activated. The electronic system in our new Lexus then came on and told us that the collision alert/airbag activation system was defective and to contact our dealer for inspection and repair. However, our car was totalled destroyed and taking it to a dealer for "inspection" was neither possible nor practical, but my wife and I walked away without any known injuries. Lexus was advised of the defect, the extent of damage, and told where car was and invited to inspect it. Not sure if Lexus ever inspected it.
