My 2019 Mazda Cx-5 (equipped with the 2. 5l naturally aspirated skyactiv-g engine with cylinder deactivation) has experienced a severe structural failure resulting in a cracked engine cylinder head. This casting defect allows pressurized motor oil to leak continuously from the cylinder head down onto the hot engine block and nearby exhaust components. This has led to the following severe safety risks: engine oil is leaking onto the exhaust manifold and creates an extreme risk of engine compartment fire when the vehicle is in motion; sudden loss of oil pressure or complete structural failure of the cylinder head can cause the engine to stall or seize unexpectedly without warning, completely disabling the vehicle in live traffic and creating an imminent crash hazard; and dropping oil levels and leaking fluids burning against the engine produce strong smoke and chemical odors that enter the cabin, which can cause driver distraction and severely limiting visibility if smoke escapes through the hood. We are also inhaling these odors. Mazda is fully aware of this structural engine defect and has detailed it internally under technical service bulletin TSB #sa-031-21 (as well as related tsbs for oil seepage/leaks on these cylinder heads). Despite acknowledging the manufacturing flaw, Mazda has failed to issue a safety recall for this dangerous fire and stalling hazard, forcing consumers to drive unsafe vehicles or pay thousands of dollars out of pocket for a known defect. The vehicle currently has 62,317 miles. The crack and oil leak were officially diagnosed by the carlsbad Mazda dealership service department on 5/26/26. I am requesting that the NHTSA launch a formal safety defect investigation into Mazda 2. 5l cylinder head cracking to force a safety recall before this defect leads to a fatal fire or highway accident.

My wife was driving our 2016 Mazda Cx-5, and was getting ready to pull into our daughters' driveway. My wife slowed down to a stop before entering the driveway. She gave the vehicle some gas to move forward. She stated that there was a pause, then the vehicle accelerated forward slamming into the garage door and the corner of the garage/house. There was also a Tesla parked inside the garage that was knocked back about a foot and a half. The distance from the edge of driveway and street to the garage door is about 27 feet. This happened at 1:30 pm. The weather conditions were great, sunny and dry. Thankfully, no one was hurt. If my wife was another foot to the right, she would have hit the support post for the upper balcony, and there would have been a good chance of the balcony falling on top of her. Or, if someone was standing in front of my wife when she entered the driveway, they could have seriously been hurt! there were no warning lights on prior to the accident. This happened last Sunday, April 19, 2026. We had the car towed to a local body shop on the same day as the accident. The vehicle was towed away the very next day to our insurance company's salvage lot in bakersfield. I do have all the information, address and phone number, of the lot where our car was towed to. High pressure fuel pump has been replaced twice in less than a year. Car has less than 100,000 miles. Both times the fuel pump went out while driving, causing loss of acceleration and stalling of vehicle. Had to be towed to shop both times.
The fuel pump system needed to be replaced completely, although the mileage is just around 60000 miles.
My car has an issue officially diagnosed by a reputable mechanic that matches the claim of the former TSB sa-058/20 that was changed and clarified through the now existing TSB sa-031/21. My car has a cracked cylinder head. This is the non turbo engine but it does have cylinder deactivation which his believed to be a culprit. The cracking is known to be caused by a manufacturing defect. There is an existing class action lawsuit regarding the turbo versions. There are complexities as to wh this lawsuit is on behalf of the turbo engines, and not the non turbo engines, however the same problems are well documented and widely acknowledged throughout hundreds if not more instances of people online and by Mazda's acknowledgment of the problem through the TSB's. This put me at great safety risk. I was driving north bound right beside semi trucks and busy traffic at speed limits of 70 mph when my engine started smoking. I was able to pull over and get it to a nearby mechanic. They officially diagnosed the vehicle w the cracked cylinder head matching the aforementioned tsbs. Mazda denied my goodwill case, even though it was made without complete information on my situation and car, despite my best attempts to get to the one representing my case with the dealership(I tried to reach him for 2 days with no success). Getting a repair from Mazda dealerships was inconvenient for my location and schedule, but I've always had my vehicle properly serviced. Mazda said this repair would cost near $7500. A mechanic quoted me at least the amount of $5500.
My fuel pump burned out after only 60k miles. . . . Very expensive repair. Mazda said the problem was only with the early 2019 models so mine wasn't included yet my fuel pump did exactly what the ones included in the recall did, but I was left with over $1700 repair bill to replace the fuel pump. Obviously the "improved design" used in the second half of the year isn't really improved, and I've never had to replace a fuel pump on a vehicle with so few miles on it. We were sitting idle waiting for food at a restaurant and the engine just died and we were unable to re-start the vehicle. No issues prior to this, always kept fuel at least 1/4 tank or more full and no engine warning lights until after the engine died.

At stop signs, in a row of 3 or 4 cars, on the 3rd or 4th stop, I apply the brake but it accelerates as I am braking, moving forward 8 or 10 feet. Over the course of 1800 miles, this has occurred 3 or 4 times, narrowly avoiding collisions.
I purchased a 2019 Mazda Cx-5 (as is) on July 20, 2015. On September 11, 2025, as I was traveling on a three lane highway. I attempted to pass another vehicle. As I was attempting to go around the vehicle, my Mazda suddenly began braking and quickly losing acceleration. The speed limit on the highway was 70 mph. I estimated my speed at the time was between 70 and 75 mph as I attempted to pass. Pressing on the accelerator pedal did nothing. I had to try and very carefully steer the car to the right hand side of the road while trying to avoid oncoming traffic approaching from behind. When I finally got the car off the road, this is when I noticed that the car had stalled. The car started right back up and I continued to my appointment. The "radar cruise control" was activated at the time of the incident. On September 15, 2025, I contacted the dealer where the car was purchased and explained what happened. I was asked to bring the car in that same day. The car was supposedly looked over and connected to the service dept. 's computer to see if any error sign appeared. Nothing showed up. I service technician was then asked to ride with me to see if the car would replicated the same stalling as it has previously done. We could not get the car to stall as it had previously done. Before leaving, a salesman asked me if I wanted him to explain the "radar cruise control" to me some more. I agreed. The salesman then proceeded to tell me that "the car did not stall, it was simply braking from the activation of the radar cruise control". Being an experienced driver, I told the salesman that the car had stalled as evidenced by the fact that I had to restart it. The situation was not resolved to my satisfaction, but no other offers were made by the dealer. I requested additional diagnostic work to be done, but the dealer refused to cooperate. I did take the car to another Mazda dealer, but could get no offer but to connect the car to their computer.
Drove 39 miles, around ~55 minutes. Parked the car, realized that I parked a little too far from the spot, car just stalled while rolling. Pressed accelerator, nothing. Turned off the car, tried to start again, it cranked but didn't start. Had to tow it to the nearest mechanic (total of ~2 hrs until my car got picked up and dropped off), mechanic calls me and tells me my car started fine. I'm thinking it could be 5321k recall but not sure as my VIN is not in the recall nor did I get any recall notice. What component or system failed or malfunctioned, and is it available for inspection upon request? - no idea. How was your safety or the safety of others put at risk? - got stranded for hours in the parking lot. Has the problem been reproduced or confirmed by a dealer or independent service center? - no has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? - no were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? - yes, fuel cap like icon and check engine light were on when trying to start the car after it stalled. The car would crank but not start.
Fuel system failure. The vehicle just dies and does not restart, this is a safety issue due to it happening when you are driving on the highway and unable to get out of the way of traffic.

For the 3rd or 4th time since I have own this car I experienced a loss of acceleration when attempting to make quick acceleration while attempting to merge with traffic. Acceleration is fine around town where quick acceleration is not appropriate. But the first attempt to merge with flowing traffic the car will slowly accelerates to about 30 mph and no more regardless as to position of accelerator pedal. On all occasion this bad behavior has this first drive of the day - going to work in the a. M. There is often a warning message indicating the safety radar is not working due to limited visibility which is a very common warning in the a. M.
While driving down highway the car suddenly will stop accelerating. Once this occurs the check engine light will turn on and shortly after the battery light will turn on. The car will not move and the ac will then stop working. The first time the car was brought to dealership (under warranty). They had to replace battery, fuel pump and fuel level indicator. They stated the fuel pump was broken as was the fuel level indicator. They were able to reproduce the problem prior to fixing it the first time. Less than a month after the car was repaired the exact same issue happened. This is a significant safety concern as I was driving on the highway with a [xxx] child in the back seat when the car stopped driving the second time. I was barely able to get over a lane and off the road before the car stopped driving. The ac then stopped working on a day where the feels like temperature was over 100. I had to lay wet cloths over my infant to prevent her from overheating while we waited for aaa and a family member to reach us. The inability to accelerate/sudden stopping of the car while driving on a highway with a speed limit of 70mph posed a significant safety risk to my life, my child’s life and potentially other cars if they weren’t able to stop. I had to wave frantically at the car in the lane next to me so that they wouldn’t hit me. No warning lights came on until after the issue (stopped accelerating) occurred. The dealership (Mazda) now has the car again and will be inspecting it. I have files from the first incident however they are in the car. Car “died” while idling at a traffic light. It started but would not start several hours later and car was towed to mechanic. Diagnostics “determined “after several days that car needed an ecm. Ecm was ordered and replaced, along with the battery. This took several weeks, in the meantime, I had to rent a vehicle. Vehicle picked up on may 29th and driven 250 miles and car sputtered, hesitated, slowing down in heavy traffic at high speeds on highway, putting other drivers and myself in a very unsafe situation. Several days later this happened again while traveling back home, again, at highway speed putting other drivers and myself in high risk situation. (this happened several times during both trips. ) as I was focused on the road, I did not see if any messages were displayed. After returning home, the mechanic was informed via voice mail and again via telephone. The following morning (June 2), the car sputtered, hesitated to accelerate at low speeds and died while driving to repair shop (battery indicator illuminated as I was pulling over). Car has been towed again to repair shop.

When filling up with gas, the gas filler tube would overflow spilling gas out of the filler tube. This happened multiple times. The dealer replicated the issue and found that the back flow sensor, valve and back flow door in the gas tank were broken and faulty. They replaced the gas tank with all of the above components replaced as part of the gas tank.

I was driving on a city street and slowed to turn right onto a driveway at at a private residence. As my car entered the driveway and near the end of the right turn, my car had a sudden unintended uncontrolled acceleration, continued the turn, went over a low hedge of shrubs on the right side of the driveway, continued down a short rocky incline, and landed on a large rock where the car stopped.
Mazda’s Cx-5 engines, particularly in earlier models, have been known to experience issues with hydraulic lash adjusters (hlas), which are responsible for maintaining proper valve clearance. Many owners have reported a persistent tapping or ticking noise, especially during cold starts. This issue is often caused by air bubbles or insufficient oil supply to the hlas, and cold weather exacerbates the problem as thicker oil struggles to circulate until the engine warms up. The hlas have also faced criticism for their sensitivity to oil pressure and quality, making them prone to failure in less-than-ideal conditions. Mazda claims that replacements and redesigns have resolved these issues in newer engine models and has not issued any technical service bulletins (tsbs) for models after 2021. However, I experienced the hla “ticking” noise in my 2022 Cx-5, suggesting the issue has not been fully addressed. Twice now, my 2017 Mazda Cx-5 stalled on the highway with a check engine light popping up--both times was driving around 60-70mph for a couple of hours and had about a quarter tank of gas left. The first time, no code was found, but the second time, after a deeper diagnostic, the dealership found a p0192 code. After reading here and many other places online, based on the code and the car's behavior, it really sounds like the denso low-pressure fuel pump is to blame; that's the source of the recall for 2018 and 2019 Mazda Cx-5s experiencing this same issue [xxx] ). If the 2017 model also uses that fuel pump, why isn't it included in the recall? if this dangerous and erratic car behavior is due to a faulty part, it really should be covered by the recall. While driving on the freeway the check engine light came on. I was able to drive the car home the remaining 20 minutes but there was some noticeable difficulty accelerating. When I went to turn on the car later that day the engine would not turn over and there was an audible rattling/chugging sound and the car itself shook. I was unable to get the car to start and the check engine light remained on. I had aaa check the battery the next day and confirm it was not a battery issue and a diagnostic revealed the p0192 code. Aaa towed it to their shop where it was confirmed by the manager mechanic that the issue was with the high pressure fuel pump and that it was causing an issue with the low pressure fuel sensor as well. The mechanic was unable to determine what caused the pump to fail and has quoted me nearly $2000 to fix it. After some searching online it has become clear that this is an issue many other Mazda owners have faced and that is even a recall for the pump for years and makes from Mazda due to a known issue with denso fuel pumps causing this exact issue. Despite this being known and denso having admitted issue with fuel pumps in cars dating back to 2013, Mazda has not recalled the vast majority of the cars that could be affected. From information I've gathered, it is entirely possible that my vehicle could have just stopped running in the middle of the freeway due to this issue which obviously would have put me in great danger of an accident. To summarize: low pressure fuel sensor and high pressure fuel pump with error code p0192 are cited as the issues. It's possible this issue can cause the car to stop running with no warning, which would be extremely dangerous especially on the freeway. The problem has been reproduced and confirmed by a aaa mechanic shop. The vehicle has been inspected by the aaa mechanic but no others. The check engine light came on but was not clear how extreme the circumstance was.

The radio changes stations by itself, speakers/music goes in and out. The screen freezes and cannot change stations. And also the car lags on taking off sometimes. Often smells like fuel when you turn on the car, assuming a fuel pump issue, not sure. I spoke to the salesman who sold me the car (adam) within less than 30 days of having the vehicle and told me the electrical issue with radio would be covered. Then when I called to have the issue fixed I was told it wasnt going to fixed on their behalf. This car was sold to me with a few issue that I was not aware at the time of purchase.
I was near a red light and applied the brakes, but the car accelerated and ran into the truck stopped in front of me. This happened around may of 2024. I took the car to the elk grove Mazda dealer. They ran diagnostic and road tests but found nothing wrong. The car had the same problem again sometime later inside my garage. The car rammed into the drywall while I tried to stop by pressing the brake pedal. I took the car to the aaa repair shop in August 2024. They spent three days, and they told me they found a problem with the "steering angle sensor. " it was 22 degrees off calibration. They reset the sensor, and the car ran better. However, the car acted again after some driving. It tries to accelerate while I apply the brake pedal near a stop. I took the car back to the aaa shop, but they did not find any problems in a whole week. I still drive this(my only) car, but extra careful, keeping a longer distance between cars in front of me and braking ahead few times ahead of any stop.
Despite a recall on the fuel pump, there continues to be a problem but instead of the “back” fuel pump that was fixed, the “front” fuel pump seems to be having the same issue. When taken to the dealer, the code p0087- fuel rail/system pressure too low shows up. This is causing car to stall, unable to pick up speed or simply pick up too much speed when attempting to accelerate. It’s a safety hazard for a car less than 6 years old.
The cylinder head and timing cover are leaking oil and need to be replaced. It is at the dealer and available for inspection. The car started smoking and lost oil obscuring my vision. The problem appears to be well known by the dealer and they were able to diagnose the issue. The vehicle has only been inspected by the dealer where I purchased it. There were no warning lamps, messages or other symptoms. This vehicle has had regular maintenance per the manufacturers specifications since it was purchased new.
There is a pressure crack that goes almost all the way across my front windshield, there are no chips or dings in the glass. Second you can smell gas fumes near the back of the car but no gas is leaking.

This car that I financed on March 26th 2024 has been back to the dealership over 6 times. My car loses power on the freeway and all main roads. I push the gas pedal and nothing happens so I am forced to pull over and turn the car off and restart it again. It's very dangerous when I'm merging on the freeway and I cannot accelerate and have to pull over in traffic. Sports cars International in lynnwood washington is well aware of the issues I've been having and they have not fixed them. They have allowed me to drive off the lot knowing I am in a car that is dangerous and unsafe, a car that could kill another family or myself. They are unresponsive to call of my phone calls and emails. This car has had nothing but problems since then day I drove it of the lot. Come to find out from their yelp and bbb reviews thus happens to a lot of people that buy cars from them. They are rude to me and I feel like I'm on danger every time I drive this car.
Excessive combustible gasoline vapors in engine compartment and in passenger compartment. Engine, available for inspection. Risk of fire/explosion because of high concentration of combustible vapors/gasoline. Yes -- confirmed by dealer (safford brown Mazda of fairfax (virginia)). No -- notwithstanding being apprised of the problem manufacturer has not inspected no warning signs. Manufacturer has acknowledge gasoline leaking into engine oil. Vapors were exacerbated because of a failed oil enging filler cap (probably cracked), and replacement of the same.
18-20 year same model and trim were recalled for fuel pump. Mine a 2017 has same symptoms died on highway going 75 almost life threatening. Several threads online with 2017 models are stating same case. I believe should be included in recall - very dangerous. Please please escalate so no one gets hurt on the highway!.
Constant ticking sound coming from engine, most apparent while idling but continues even after oil change and engine warm up. Goes away for short periods of time. Always comes back and is very loud. Sometimes engine block shakes and can be felt through the steering wheel.
My car has been getting the p092 code with check engine light, it runs really rough, I have had many parts replaced at my expense when I know the model years 2018 and 2019 have been recalled, I am many others are having the same issues, is it going to take people dying on highways because their car just shuts off. My cat has been having the same issues reported in safety recall 19v497-6744. After googling around, I may have found an answer. Denso, who makes low pressure fuel pumps for a lot of different manufacturers, recalled several million of them a couple of years ago. They determined that there was a manufacturing defect that causes the pump impellers to crack and deform. Then they don't move enough gas to the front of the engine. I guess there is no pressure sensor between the tank and the high pressure fuel pump, so the problem isn't recognized by the car until the sensor in the fuel rail sees it. Several auto makers have issued recalls, including Mazda. But Mazda only went back as far as 2018. Toyota recalled cars as far back as 2013. There are now several lawsuits against various auto makers arguing that denso knew about the problem in 2015, and the recalls should go further back in time. This vehicle has stalled out while driving at least 4-5 times since purchasing it in 2018, although issues started about 2020 or 2021. It usually starts with the radio console either malfunctioning or going dead completely while I’m driving and several minutes later (usually after stopping at a red light but sometimes while actively driving) the car stalls out in the middle of traffic. It starts up after few minutes but it’s a very dangerous situation. It also sometimes hesitates when putting my foot on the gas, as if there’s a fuel injection problem. I already got all the recalls fixed but there’s obviously other significant problems with this vehicle. If it causes me to get injured I will be taking legal action.
The vehicle unexpectedly caught fire and the results could have been catastrophic. Almost new car under warranty. All details are in the uploaded pdf letter and photos. I have contacted Mazda and they claim they do not investigate, only insurance company will investigate. My insurance company will pay me for the total loss but does not plan to investigate. I am writing to express serious concerns regarding the recent service that occurred on November 14th, 2023. Due to the service's failure and negligence, luther brookdale Mazda has jeopardized the safety and well-being of my family members. On [xxx], my 2019 Cx-5 was towed from monticello, minnesota, to the dealership in brooklyn center around 9 pm. While returning from [xxx] dinner in st. Cloud on [xxx] , a sudden warning appeared on the dashboard: "engine coolant temperature high. Stop the vehicle immediately in a safe place. " upon receiving this notification, we pulled over to the shoulder and observed smoke emanating from the engine bay. We promptly turned off the vehicle. Concerned for our safety, we contacted our family to pick us up and had the vehicle towed by aaa. As we awaited the tow truck's arrival, we noticed coolant had sprayed all over the engine under the hood, and the engine cover was missing. Furthermore, the coolant reservoir was below the minimum threshold. This particular issue is the reason I brought the vehicle in for service the week prior, regarding insufficient coolant temperature. It was evident from the state of the engine bay that the dealership failed to follow proper protocol. This lack of oversight likely resulted in damage to the engine and other essential vehicle components, leading to the total loss of the car. I am deeply disappointed by the lack of care and attention to detail in the service provided. This negligence also compromised my family's safety. Moreover, what was meant to be a festive holiday was marred due to the disregard for my property. I have been a loyal customer at luther Mazda brooklyn park for several years, and I am shocked by the incident that has transpired. We have approached the dealer with the issue and they are continue to articulate that there is no engine damage, but with the ongoing class action lawsuit, I am extremely skeptical of their claims. Vehicle engine has stalled multiple times at low speeds. Vehicle will not restart immediately. In order to get vehicle to restart, gas must be revved while pressing start button to get engine to restart. It appears as if fuel is not being delivered to engine. Electrical components remain functional. Vehicle was inspected by dealer and issue could not be recreated. No issues were found. No warning lights displayed or codes found upon inspection. No symptoms of problem; occurrence is sporadic. It has caused traffic and safety hazards due to stalling in roadways.
